[Method of Qian Yi] For the treatment of chuangzhen (exanthematous diseases such as smallpox and measles), only use medicines with a mild and neutral nature. Do not recklessly use purgatives, nor recklessly employ diaphoretic or attacking methods, to avoid exposure to wind and cold. (Zhang Yuansu stated: "Mild and neutral" does not mean hot-acting agents; examples include Jingjie (Schizonepetae Herba), Bohe (Menthae Haplocalycis Herba), Fangfeng (Saposhnikoviae Radix), Niubangzi (Arctii Fructus), and Gancao (Glycyrrhizae Radix et Rhizoma). The Shuniuzi Tang (Burdock Fruit Decoction) from *Huoren Shu* should be selected in conjunction with Jiegu's Jiedu Fangfeng Tang (Detoxifying Saposhnikovia Decoction). Zhu Danxi stated: Niubangzi (Arctii Fructus), Lianqiao (Forsythiae Fructus), Shanzha (Crataegi Fructus), and Gancao (Glycyrrhizae Radix et Rhizoma) are four herbs that must be used throughout the course of treating variola.) Regarding the case of the Tenth Prince of Muqin Palace suffering from chuangzhen, it was said: Throughout the eruption of chuangzhen, if there are no other complications, purgatives should not be used. One should only use mild medicines, frequently provide milk and food, and avoid exposure to wind and cold. If the rash does not appear after three days, or appears slowly, induce a mild diaphoresis (using Shuniuzi Tang or similar). If the rash appears but is sparse, increase the dosage (e.g., from one decoction per day to two). If it still does not appear after increasing the dosage, induce a strong diaphoresis (using Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Lobatae Radix), Fangfeng (Saposhnikoviae Radix), Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Mahuang (Ephedrae Herba), Guizhi (Cinnamomi Ramulus), etc.). If the rash is sparse after appearing, and the pulse is calm without other symptoms, it indicates the rash was originally sparse, and forcing further diaphoresis is contraindicated. In cases of high fever, promote urination; in cases of slight fever, clear heat and resolve toxicity. (Promoting urination can be done with Sisheng San (Four Saints Powder) or similar.) If the eruption is obstructed, do not induce diaphoresis or purge; instead, use Baolong Wan (Dragon-Holding Pill). If the rash has erupted and the patient can eat, administer Dahuang Wan (Rhubarb Pill) for one or two bowel movements, then stop. High fever requires promoting urination. Slight fever requires clearing heat and resolving toxicity. If the lesions are black, purple, dry, and sunken, use Baixiang Wan (Hundred-Auspicious Pill) to purge; if not black, strictly avoid purging. Symptoms of body heat, irritability, abdominal fullness with wheezing, difficulty in urination and defecation, red face with confusion and severe vomiting indicate the need to promote urination. If there is no improvement, use Xuanfeng San (Dispersing Wind Powder) to purge. If scabs do not dry out by the fifth or seventh day, it is due to internal heat steaming the skin, preventing scab formation. Use Xuanfeng San to guide the heat out, and apply raw Xijiao (Rhinocerotis Cornu) juice to clear heat, ensuring scabbing occurs.
[Method of Li Dongyuan and his disciples] [Zhang Yuansu] discussed: The manifestations of zhen (measles/rash) vary. Sores swelling externally belong to the Shaoyang Sanjiao (Triple Burner) ministerial fire, called "chuang" (sores/ulcers). Small red spots moving within the skin that fail to erupt belong to the Shaoyin Junfire (Monarch Fire), called "zhen" (rash). Generally, if chuangzhen presents with spontaneous vomiting and diarrhea, do not treat recklessly as this often indicates a favorable prognosis, as pathogens are exiting through both upper and lower pathways. Generally, chuangzhen should never be purged from beginning to end. (See previous section on sweating and purging.)
Anli Jiedu Fangfeng Tang (Anli Detoxifying Saposhnikovia Decoction)
Fangfeng (Saposhnikoviae Radix) (1 liang), Digupi (Lycii Cortex), Huangqi (Astragali Radix), Shaoyao (Paeoniae Radix), Jingjie (Schizonepetae Herba), Zhike (Aurantii Fructus Immaturus), Niubangzi (Arctii Fructus) (equal parts).
Grind into powder. Decoct 4-5 qian with water and take orally. If the patient can eat and has solid stool, use Danggui Wan (Angelica Sinensis Pill) to mildly promote bowel movement.
Danggui Wan (Angelica Sinensis Pill) and Zao Bian Baixiang Wan (Date-Changed Hundred-Auspicious Pill) (also treat constipation).
[Li Dongyuan] Xiaodu Jiuku Tang (Disinfecting and Saving from Distress Decoction) treats complete chuang symptoms, aiming to digest them so they do not erupt; if already erupted, it prevents further growth in sparse cases. This formula was established in November and adjusted according to the seasons; only those who understand natural transformations and herb properties can apply it effectively.
Mahuanggen (Ephedrae Radix et Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Shengma (Cimicifugae Rhizoma), Huangbai (Phellodendri Cortex) (wine-fried, 5 fen each), Chaihu (Bupleuri Radix), Chuanxiong (Chuanxiong Rhizoma), Xixin (Asari Radix et Rhizoma), Bencao (Ligustici Rhizoma), Gegen (Puerariae Lobatae Radix), Huangqin (Scutellariae Radix) (wine-fried), Cangzhu (Atractylodis Rhizoma) (2 fen each), Huanglian (Coptidis Rhizoma), Dangguishen (Radix Angelicae Sinensis) (3 fen each), Sumu (Sappan Lignum), Baizhu (Atractylodis Macrocephalae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma) (raw), Juapi (Citri Reticulatae Pericarpium) (1 fen each), Wuzhuyu (Evodiae Fructus) (0.5 fen), Honghua (Carthami Flos) (a little), Lianqiao (Forsythiae Fructus) (0.5 qian, reduce for early eruption, increase for heavy eruption), Shengdihuang (Rehmanniae Radix) (5 fen).
Cut into pieces. Take 5 qian per dose, decoct in two large bowls of water until one bowl remains, strain, and drink while slightly warm. Zhen arises due to internal injury and is caused by the reversal of Ying (Nutritive) Qi. Strictly prohibit harsh drugs like Qianniuzi (Ipomoeae Semen) and Badou (Crotonis Fructus). Use Banxia (Pinelliae Rhizoma), Zhishi (Aurantii Fructus Immaturus), Dahuang (Rhei Radix et Rhizoma), Yizhiren (Alpiniae Oxyphyllae Fructus) to stop vomiting and diarrhea. If ear tips are cold, yawning, startle during sleep, sneezing, and eyes feel gritty, eruption is imminent. Use:
Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Lobatae Radix), Baishaoyao (Paeoniae Radix Alba), Gancao (Glycyrrhizae Radix et Rhizoma), Dangguishen (Radix Angelicae Sinensis), Lianqiao (Forsythiae Fructus) (equal parts).
Cut into pieces. Decoct in water and take orally. This is the standard method, adjusted subsequently based on symptoms:
If lung pus forms, showing wheezing/cough first, or high qi with panting, add small amounts of Renshen (Ginseng Radix et Rhizoma) and Huangqin (Scutellariae Radix) to drain latent fire and tonify primal qi.
If heart red spots form, showing dry throat, palpitations, body heat, muscle swelling, and wiry-hong pulse first, add small amounts of Huanglian (Coptidis Rhizoma). If Mingmen (Gate of Life) causes nettle-rash, showing bone pain, body heat, and immobility due to pain first, add small amounts of Shengdi (Rehmanniae Radix) and Huangbai (Phellodendri Cortex). All chuangzhen are yin-pattern sores, caused by internal injury from milk/food, spleen/stomach deficiency, and reversed Ying Qi. Although fire burns internally, yin covers externally. Thus, Qian Yi created Siwu Shengma Tang (Four Substances and Cimicifuga Decoction) to disperse it. If transmission patterns occur, follow the adjustment rules.
[World Physicians] Rash and variola originate in the Spleen; use Chen Shi's Ren Shen Qing Ge San (Ginseng Clearing Diaphragm Powder). (Rash appearing faintly on skin like hemp seeds is called Mazizi.) Water pox (varicella) often arises from cold damage heat-toxin; use Shengma San (Cimicifuga Powder) and Qianghuo San (Notopterygium Powder).
[Zhu Danxi on Qi/Blood Deficiency/Excess] Zhu Danxi differentiated pediatric variola/rash by Qi/Blood deficiency/excess, using Sijunzi Tang (Four Gentlemen Decoction), Siwu Tang (Four Substances Decoction), wine-fried Huangqin (Scutellariae Radix), and Huanglian (Coptidis Rhizoma). For Qi deficiency, add Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) to detoxifying herbs. For Blood deficiency, add Siwu Tang (Four Substances Decoction) to detoxifying herbs. Lou Quanshan noted that "detoxifying herbs" refer to wine-fried Huangqin and Huanglian. Zhang Yuansu stated: Detoxification uses Sanhuang Tang (Three Yellow Decoction) or Jinhua Wan (Golden Flower Pill).
Differentiate variola by person's clarity/turbidity and assess courage/fear via physique. Purple variola indicates Blood Heat, treated primarily by cooling blood. White variola indicates Qi Deficiency, treated primarily by tonifying Qi. Black depression in the center with white exterior and slow eruption requires tonifying Qi with slight blood-cooling herbs.
[Wan Quan] The main treatments for variola are: Resolving the Exterior, Harmonizing the Middle, and Resolving Toxicity. Resolving the Exterior involves dispersing to expel pathogens outward, preventing retention inside, using Fangfeng (Saposhnikoviae Radix), Baizhi (Angelicae Dahuricae Radix), Jingjiesui (Schizonepetae Spica), Shengma (Cimicifugae Rhizoma), Gegen (Puerariae Lobatae Radix), Chaihu (Bupleuri Radix), Guizhi (Cinnamomi Ramulus). Harmonizing the Middle focuses on Spleen/Stomach, assisting Blood/Qi to keep interior qi solid, preventing blood/qi depletion, nourishing the rash to maturity, preventing itching/collapse, using Huangqi (Astragali Radix), Renshen (Ginseng Radix et Rhizoma), Baishaoyao (Paeoniae Radix Alba), Danggui (Angelicae Sinensis Radix), Muxiang (Aucklandiae Radix), Chenpi (Citri Reticulatae Pericarpium). Resolving Toxicity clears fire, cools blood, and regulates qi to control toxins, using Shandougen (Millettiae Speciosae Radix), Dalizi (Arctii Fructus), Zicao (Leonuri Herba), Lianqiao (Forsythiae Fructus), Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Zhizi (Gardeniae Fructus).
[Weng Zhongren on Qi Deficiency, Blood Heat, and Heat Stagnation] (Differentiation detailed in Deficiency/Excess section) For Qi Deficiency: Initial fever with cold extremities, alternating hot/cold, mental fatigue, soft/pale muscles, reduced appetite, limb weakness, quiet sleep, clear/d regular bowels. Before spots appear, use Shenqi Yin (Ginseng-Astragalus Decoction) with mild dispersants like Zisu (Perillae Folium), Fangfeng (Saposhnikoviae Radix), Baizhi (Angelicae Dahuricae Radix). After spots appear, use Shenqi Yin with mild agents like Chuanxiong (Chuanxiong Rhizoma), Jiegeng (Platycodonis Radix). After four days, heavily use Shenqi Yin adjusted to symptoms. After seven-eight days when pus is sufficient, use Baoying Baibu Tang (Infant Protection Hundred-Tonics Decoction) to nourish Qi/Blood. For late-stage collapse/black scabs, use Muxiang Yigong San (Aucklandia Meritorious Powder) to conclude treatment.
For Blood Heat: Initial intense fever, red cheeks/face, dry/brittle hair, irritability/thirst, crying day/night, restless sleep, preference for cool places, dark/scanty urine. Before eruption, Shengma Gegen Tang (Cimicifuga-Kudzu Decoction) or Shengma Liuqi Yin (Cimicifuga Moving-Qi Decoction) are acceptable, but Shishen Jiedu Tang (Ten-Gods Detoxifying Decoction) is safest. Three-four days after appearance, when heat subsides and pus forms, adjust Taiyi Baohe Tang (Supreme Harmony Protecting-Decoction). After eight-nine days of sufficient pus, use Baoying Baibu Tang. For purple-black dry/sunken lesions between days seven-eight, use Duoming Dazao (Life-Saving Great Formation), Tanxiao Bojin (Laughing-Bragging Gold), Yizi Jin (One-Character Gold), or Baixiang (Hundred-Auspicious), Niuli (Bull-Lee), Zhuwei (Pig-Tail), Dushen (Unique Spirit) formulas cautiously. After diarrhea with black/dry-red lesions, use Muxiang Yigong San. This is a secret ancestral method, highly effective.
For Heat-Toxin Stagnation: Intense fever, red face, dry skin/hair, coarse breathing/panting, abdominal distension/irritability, delirium, restless sleep, constipation, dark urine, facial/eye swelling, crying/anger. Before spots, take Shengma Gegen Tang once, followed by Qianghuo San Yu Tang (Notopterygium Dispersing-Stagnation Decoction). Within three days of appearance, when heat subsides and pus forms, take Yiyuan Touji San (Origin-Penetrating Muscle Powder) adjusted. After sufficient pus, use Yingtong Baibu Tang. For purple-black dry/sunken lesions after six-seven days, use Duoming Dazao, Tanxiao Bojin, Yizi Jin, Baixiang, Niuli, Zhuwei, Dushen cautiously. After diarrhea with Muxiang Yigong pattern, use Muxiang Yigong San.
Treating weak variola has two methods: For Qi Deficiency, tonify Qi; since Qi deficiency leads to cold, also warm. Adjust warmth/tonification by severity. Treating Blood Heat Stagnation has five methods: Excessive exterior heat causes dryness; excessive exterior cooling causes ice-trapping; excessive interior heat causes constipation; excessive interior cooling causes diarrhea; excessive Qi stagnation causes distension/panting; suppressed heat-toxin causes distension/madness; overwhelming toxin burdens interior/exterior, hard for infants to bear. Thus, treatment lies in: Calming Exterior, Harmonizing Middle, Equalizing Qi, Penetrating Muscle, Resolving Toxicity. Calm exterior to prevent dryness/freezing. Harmonize middle to prevent constipation/diarrhea. Equalize Qi to prevent stagnation/panting. Penetrate muscle to allow heat-toxin to reach surface. Resolve toxin to distribute internally/externally. If these five are maintained, even dense Blood Heat Stagnation is manageable.
Shenqi Yin (i.e., Baoyuan Tang) specifically treats Primal Qi deficiency, mental fatigue, soft muscles, blue-white face, poor appetite, quiet but listless sleep, regardless of eruption status.
Renshen (Ginseng Radix et Rhizoma) (1 qian), Huangqi (Astragali Radix) (2 qian), Gancao (Glycyrrhizae Radix et Rhizoma) (5 fen, raw for initial heat, roasted after eruption).
Yang Renzhai called it Baoyuan Tang (Primal-Origin Decoction) for tonifying primal Qi, established for Qi deficiency. Later physicians used it indiscriminately for Blood Heat/Toxin Stagnation, treating excess with excess, which is erroneous.
Decoct with one cup of water and one slice of ginger until 4-5 fen remain, take anytime.
Shenqi Yin Adjustment and Contraindications
For confirmed Qi deficiency, use this formula as the mainstay throughout; do not change. For mixed symptoms arising from Qi deficiency, adjust this formula moderately. Avoid excessive dispersing or bitter-cold agents.

Shishen Jiedu Tang (Ten-Gods Detoxifying Decoction) specifically treats intense fever, red face, dry hair, red spots before day three, thirst, restlessness, dark urine (Heat Excess).
Dangguiwei (Angelicae Sinensis Radix), Shengdihuang (Rehmanniae Radix), Honghua (Carthami Flos), Mudanpi (Moutan Cortex), Chishaoyao (Paeoniae Radix Rubra), Jiegeng (Platycodonis Radix), Mutong (Akebiae Caulis), Dafupi (Trogopterori Fructus), Lianqiao (Forsythiae Fructus), Chuanxiong (Chuanxiong Rhizoma).
This formula treats Blood Heat variola by cooling/moving blood, assisted by Jiegeng/Chuanxiong for lifting/dispersing, guided by Dafupi/Mutong to drain heat-toxin, supported by Lianqiao/Mudanpi for detoxification. It distributes heat-toxin internally/externally. Ancient use of Huanglian Jiedu Tang (Coptis Detoxifying Decoction) risks freezing heat-toxin with sudden cold, causing obstruction/internal rupture. This formula is safer. If Huanglian/Huangbo (Phellodendri Cortex) are necessary, wine-fry them to moderate coldness and assist ascending action for detoxification.
Shishen Jiedu Tang Adjustments

For Blood Heat variola with overwhelming toxin, use strong lifting/dispersing to reach surface, plus draining to eliminate heat internally, aided by cooling/detoxifying/blood-moving herbs. This allows resolution even if dense, facilitating pus formation. This achieves Calming Exterior, Harmonizing Middle, Resolving Toxicity perfectly. As Danxi said: "Clear heat, level excess." From fever to spot appearance (before day three), when toxin is not fully at surface and Blood Heat signs are present, use this formula. Do NOT add tonics like Renshen, Huangqi, Baizhu, Fuling to heat patterns pre-pus, as this treats excess with excess, causing distension, wheezing, delirium, sore throat, mouth ulcers. For vomiting/diarrhea, strictly avoid Banxia, Dingxiang, Guizhi, Ganjiang, Muxiang, Huoxiang (Agastaches Herba), Hezi, Rouguo, as this adds heat to heat, causing dryness, bleeding, constipation. Longgu and Kufan block Qi channels, closing the door to eruption, causing distension/wheezing. Once blood pustules form and Qi/Blood settle, with white crown on head indicating pus formation, change formula.
Qianghuo San Yu Tang (Notopterygium Dispersing-Stagnation Decoction) specifically treats Excess Heat Stagnation unable to reach surface, coarse breathing, distension, irritability, delirium, restlessness, constipation, erect hair, swollen face, angry eyes. Also treats slow eruption due to external Wind-Cold.
Fangfeng (Saposhnikoviae Radix), Qianghuo (Notopterygii Rhizoma et Radix), Baizhi (Angelicae Dahuricae Radix), Jingjie (Schizonepetae Herba), Jiegeng (Platycodonis Radix), Digupi (Lycii Cortex), Chuanxiong (Chuanxiong Rhizoma), Lianqiao (Forsythiae Fructus), Gancao (Glycyrrhizae Radix et Rhizoma), Zicao (Leonuri Herba), Dafupi (Trogopterori Fructus), Niubangzi (Arctii Fructus).
Grind into coarse powder. Decoct one cup of water with 14 strands of Dengxin until 6 fen remain, take warm.
At initial fever/spot appearance, toxin is strong, suppressed by external Wind-Cold, or pores are closed, blocking meridians. Clear Qi cannot guide toxin to surface, causing internal heat-stagnation leading to distension, wheezing, constipation, mania, convulsions, bleeding, dry skin, swollen face/eyes, restlessness. This Heat-Toxin Stagnation requires immediate dispersing/lifting agents assisted by harmonizing/penetrating methods. Qianghuo, Baizhi, Fangfeng lift/disperse/detoxify; Jiegeng lifts/equalizes Qi; Jingjie, Lianqiao, Niubangzi clear stagnant heat; Digupi clears heat in bones/tendons and regulates organs; Zicao smooths skin/pores; Dafupi guides heat downward. Using this to treat heat...
