Li (Left) Spleen deficiency generates dampness; Qi weakness generates phlegm. However, how can middle Qi emptiness lead to the counterflow of Gallbladder Yang causing dizziness? The pulse is slippery, and deep pressure reveals a soggy and soft quality. This is due to Spleen deficiency and Stomach excess: Spleen deficiency causes fluid stagnation transforming into phlegm, while Stomach excess causes Gallbladder Qi counterflow leading to vertigo. A modified formula based on Waitai Fuling Decoction is proposed.
Ginseng Radicula (50g, separately ground and mixed in), Tangerine Peel (75g), Bitter Apricot Kernel Frost (150g), White Silkworm (50g), Clam Shell Powder (100g, water-floated), Stir-fried Wild Atractylodes Macrocephala Rhizome (100g), Calcinated Gastrodia Elata (75g), Poria (250g), Charred Immature Bitter Orange (60g), Tribulus Terrestris (Stir-fried, thorns removed, 100g), Polyporus (50g), Processed Pinellia Ternata (150g), Jian Ze Xie/Alisma Rhizome (75g), Fresh Bamboo Sap and Bamboo Leaves Stir-fried with Ginger Juice (50g).
The above herbs are ground into fine powder. Using decoction from 250g fresh ginger as the binder, pills are made the size of small Sophora japonica seeds. Take 15g every morning and 5g in the afternoon, delivered with Tangerine Red Decoction.
Lin (Left) The Classic states: "Wei Qi (Defensive Qi) warms the muscles, fills the skin, thickens the interstices, and controls the opening and closing." This shows that the circulation of Yang Qi must not be obstructed. Qi deficiency generates phlegm; phlegm-dampness lies internal, obstructing circulation, preventing Wei Qi from distributing. The limbs are often lacking warmth, spirit is difficult to invigorate, and the pulse is deep, wiry, and slippery. It resembles Yang decline, but in reality, it is Yang Qi failing to function within. The strategy is to tonify Qi and resolve phlegm, to diffuse and unblock Yang Qi.
Ginseng Radicula (50g, separately ground and mixed in), Wild Atractylodes Macrocephala Rhizome (100g), Processed Pinellia Ternata (150g), Dipsacus Asper Root (Stir-fried, 75g), Cinnamon Twig (30g), White Cardamom Seed (15g, separately ground and mixed in).

The above herbs are ground into fine powder according to method, and pills are formed using water.
Qin (Left) Generally, latent summerheat syndromes must be accompanied by dampness, because heat steaming moves dampness. When heat is excessive, administering melon seed emetics [Guashui] fails; when heat lingers, administering sweet-cold herbs fails, resulting in Spleen被困 by dampness and Stomach losing its descending function. Spleen被困 leads to limb flaccidity; Stomach failing to descend leads to poor appetite and slight vertigo, oily face, and red eyes. This perfectly matches the principle of "great excess resembling deficiency." Thus, one can achieve the effect of Tangerine Peel and Pinellia replacing Ginseng, and Poria and Atractylodes masking the cloying nature of Donkey-hide Gelatin and Rehmannia. Once weeds are removed, good crops grow naturally. The strategy for regulation can be deduced from this.

The above herbs are ground into powder, and pills are formed using water, the size of Sophora japonica seeds. Delivered with Tangerine Peel and Coix Seed Decoction.
Xue (Left) Distending pain in the epigastrium and hypochondrium, severe pain at times, with a deep and wiry pulse, indicates internal retention of fluid-Qi. Previously, drastic purging methods were used, expelling much water, yet Qi division remains difficult to expand. The epigastrium belongs to the Stomach, and the hypochondrium belongs to the Liver. The Stomach prefers free flow and descent; the Liver prefers smoothness and discharge. If Spleen and Stomach lose their healthy transportation, water-dampness in the Stomach stays, Middle Earth Qi stagnates, Wood cannot vent, leading to rebellious Qi stagnation conflicting with Earth, causing distension, pain, and fullness. Treatment should soothe and harmonize the Liver and Stomach.

[Patient Name Omitted] Weili (tailbone area) is where the Du Meridian passes, the key area of the two yin openings. After strangury and turbid urine stopped, damp-heat remained internal, causing soreness at Weili. Various medicines failed. Long-standing illness not cured, even administering Ginseng and Deer Antler did not dispel disease. Dampness pressing down causes Qi sinking, lower abdomen feels distended and painful. Excess dampness overwhelms Spleen transportation, stool is loose and diarrheal. Upon using Qi-promoting and Middle-regulating, bitter-warm combined transformation methods, effects were immediate like drum responding to beat, all ailments vanished. Now just recovered, seemingly needing to tonify True Yin and regulate Du Meridian. However, Yin-tonifying drugs are often stagnant/cloying. Blind experimentation would cause damp-heat to re-congest, loss outweighing gain. Now proposing to combine Middle-regulating and Qi-promoting, bitter-warm transformation, with tonifying yet non-stagnant items. Please advise.

Guo (Left) Heart pain piercing to the back has established methods to follow. Unfortunately, the patient has chronic throat issues, so acrid-hot medicines cannot pass through. Therefore, attacking and draining phlegm-water to expand Yang Qi in the chest, using acrid-slippery and lubricating herbs to unblock Yang Qi in the chest, then administering acrid-warm and great-hot substances... the throat suffers no harm. This clearly shows Yin pathogenic factors rebelling upward. Throat pain is indeed Yin excess forcing Yang upward. If it were Yin deficiency with Fire flaming up, there would certainly not be such contradiction internally. Proceeding with established methods from *Jinkui Yaolue* seems appropriate.

Madam Shen Yunqing Pulse is generally constant, left section wiry. Around front and back of ears, phlegm nodules overlap, triggered by labor, feeling like pain or distension upon onset. Fortunately, palpation is not hard, root still floats. Limbs occasionally feel burning, palpitations and unrest, Qi division distending, vertigo and dizziness, sore waist, leukorrhea. Front and back of ear belong to Shaoyang sector. Liver governs growth and development. If frustrated, it cannot flourish. Qi stagnation generates Fire; Fire stagnation generates Phlegm. Liver Fire carrying Phlegm rises, while Gallbladder Meridian Qi tends to descend. Unable to rise, unable to descend, it spreads horizontally into Shaoyang Meridian, condensing without dispersing, forming palpable nodules. Wood and Fire flourishing scorch Ying Yin, causing Yin division daily deficit, Qi division daily excess. Symptoms like Qi distension, limb heat, rising fire all stem from this. Yang Qi shaking causes heart palpitations and vertigo. Wood stagnation不调, Spleen clear Qi sinks, fluids leak out, hence continuous leukorrhea. Fluid depletion, Kidney deficiency, waist mansion soreness. Strategy: Vent Wood, nourish Ying, harmonize Yin, calm Spirit, combined with dissipating phlegm nodules. Please advise Yunqing.

Li (Left) Usually expectorates grey-black thick phlegm, gradually leading to hemoptysis. Repeatedly administered clearing/nourishing upper/middle jiao to calm floating heat, then added acrid-warm, and bleeding stopped. Now all stable, black phlegm rarely seen. Pulse deep and fine, right Cun and Guan slippery and large. Indeed damp-phlegm excess internal, cold-water Qi forcing heat upward, damaging Lung and Stomach collaterals. Otherwise, how could blood disease be treated with acrid-warm? And why would acrid-warm be effective? Since medicine worked before, method should be followed.

Pan (Left) Phlegm abundant, thick and sticky, even pinkish. Throat pain. Repeatedly administered clearing/transforming, phlegm became thinner, pink receded. Phlegm is sticky/turbid, only occurring with excess dampness and fluid stagnation. Subsequently advancing Yin-nourishing agents, diet normal. Shows damp transforms then phlegm消, Qi moves then damp transforms. Yin deficiency cannot transform Qi; Qi not moving damp, phlegm generates internally. Mr. Zhang Jiebin said Prepared Rehmannia is the holy medicine for resolving phlegm, this is what he meant. Otherwise, after clearing/transforming, following with Yin-nourishing, one of the two would fail. Now peaceful, should maintain Yin-nourishing and Qi-transforming method.

Dou (Left) Shaoyin Meridian connects to tongue base, follows throat. Throat has "Stone Swan" (likely referring to tonsillar stones or severe inflammation), occasional fever. Hand Shaoyin Heart is Fire; Foot Shaoyin Kidney is Water. Only when Fire flourishes does Water deplete further; only when Water depletes does Fire flourish further. They cause each other. Youthful innocence not sufficient, symptoms as described. Doubtless Yang excess, Yin deficiency. Tonify deficiency, drain excess, unchangeable principle.

Zhang (Left) Stomach pain long-standing, once vomited saliva congealed with stasis. Stomach is Yangming, Yangming is land of abundant Qi and Blood. Retained fluid obstruction, pain implies blockage. Extreme formless Qi stagnation causes formed Blood to also obstruct, congealing into stasis. Accumulation extreme breaks through, breaking through means flow, flow means no pain. Concern is: although Stomach mansion flows, past stagnation in collaterals surely not fully resolved. This is trickle stream, spark fire. Now using pill medicine to warm/unblock Stomach Yang and cultivate Spleen Earth, hoping Earth Qi rotates, so stagnation in collaterals secretly dissolves.

Chen (Left) Right pulse slightly slippery, Left Guan pulse uniquely wiry. Wiry indicates Wind-Wood, sign of hyperactivity. Described no other ailments, only occasional vertigo. Yin deficiency Wood flourishing, Wood dry generating Wind, also causes vertigo. If Yin deficiency, how can there be no accompanying symptoms besides vertigo? Our organs are Yin, fu-organs are Yang. Zang ascends, Fu descends. Yin substance uses Yang function, called Yang within Yin. Yang substance uses Yin function, called Yin within Yang. Thus Li (Fire) hollow inside, Kan (Water) full inside. Liver is Yi Wood, Gallbladder is Jia Wood. Liver is Zang, Gallbladder is Fu. One Zang one Fu, exterior-interior correspond. With Spleen/Stomach ascend/descend together. Liver combines with Spleen to ascend, generating Heart Blood. Gallbladder combines with Stomach to descend, growing Mingmen Fire. Now right pulse slightly slippery, Stomach has damp-phlegm, Stomach lacks descending flow. Stomach descending insufficient, how can Gallbladder Qi alone descend? Thus Liver/Spleen ascending excess, Gallbladder/Stomach descending insufficient. Excess and insufficiency are beginnings of bias. Yang biased upward, causing vertigo/dizziness, quite appropriate. If strengthening Water to nourish Liver Wood and subdue Yang Qi, only for Yin deficiency Yang hyperactivity, having Yin to control Yang effect. If Gallbladder/Stomach descending little, receiving Yin-softening items, Stomach mansion becomes more blocked, Gallbladder harder to descend, thus Liver rises higher. To calm Liver, must descend Gallbladder. To descend Gallbladder, must descend Stomach. Narrow view, unsure if appropriate.
Processed Pinellia Ternata (150g), Tangerine Peel (75g), Fen Dan Pi/Moutan Cortex (50g), Immature Bitter Orange (75g), Poria (200g), Chu Ju Hua/Chrysanthemum Morifolium Flower (75g), Clam Shell Powder (200g, floated), Hei Shan Zhi/Gardenia Fruit (75g), Dou Yi/Soybean Coat (150g), Mulberry Leaf (75g).

Wu (Left) Suddenly fainted (Jue), afterwards intermittent symptoms, eating again causes bloating/discomfort, pulse wiry-slippery. This is Phlegm-Qi internal stagnation, Jia Wood floating upward, causing temporary Yin-Yang closure, Stomach losing descending authority, Wood pathogen shaking. Should use tonifying-unblocking, combined with resolving phlegm and extinguishing Liver.

Sun (Right) Stomach pain and vomiting long-standing, repeatedly treated from Liver-Stomach perspective, stops and recurs. Years, no major effect. Pulse deep-wiry. Although Liver is fierce, no endless rigid-sharp logic. Although Stomass invaded, no permanent discord logic. Due to hidden phlegm-stasis in Stomach, although attacked once and expelled, sticky-coagulated parts remain internal. Strong medicines definitely not suitable for chronic disease. Propose Weiling San method, hiding strength within mildness, to observe later.
