True Water (Kidney Yin) can moisten and nourish Liver Wood and Heart Fire, allowing them to maintain their generative vitality, thus naturally avoiding the drawback of transforming into fire and generating wind. Currently, leukorrhea occurs frequently, consuming body fluids and essence, causing Liver Wood to lose nourishment. Consequently, Jue Qi (rebellious Qi from the Liver channel) rushes against the Stomach, causing pain in the epigastrium. When constrained, it becomes Qi stagnation; when rising excessively, it becomes Wind. Since Wind is characterized by movement and tremor, the collaterals experience twitching and pulling, even leading to pain. The treatment should nourish Blood and regulate Qi, while simultaneously tonifying Spleen and Kidney. Regarding conception, as long as Qi and Blood are abundant and harmonious, there will naturally be the miraculous workings of Heaven and Earth.

Patient Li (Left pulse): Yin and Yang are the general principles of all things and the parents of change; left and right are the pathways of Yin and Yang. Yin and Yang interpenetrate like a ring without end. Therefore, excess causes disease, and deficiency also causes disease; such is the purport of the classics. I recall that at the first consultation, the patient's legs and thighs were cold and lacked warmth. Subsequently, whenever there was a sensation of heaviness, the collaterals felt uncomfortable. If one says that coldness in the legs and thighs implies Yang deficiency, and deficiency prevents reaching the extremities, the reasoning seems close to truth. However, who would know that severe Yang deficiency must present with pale skin lacking luster, and upon warming and tonifying, spirit would be refreshed and diseases eliminated. Yet this patient's symptoms remained as before, but the complexion was radiant. To tonify Yin using animal-derived substances, Tortoise Shell (Guijia) is the extreme; to tonify Yang using animal-derived substances, Deer Antler (Lurong) is the extreme. However, after taking Deer Antler first, various miscellaneous diseases appeared later, and the pulse was soggy and slippery. Thus, I knew that Yang Qi was not damaged; the apparent damage was actually due to obstruction of Yang Qi flow. The key between tonification and purgation lies in a difference of millimeters determining a thousand miles; the distinction is here. Therefore, without using tonifying substances, the medicine worked promptly. From this, we know that when Yin and Yang interpenetrate, changes generate life, naturally avoiding excess or deficiency. Now that the pathological manifestations have lessened, this is the critical time for follow-up care. The operation of Yang Qi originally encounters no obstruction; the obstruction is caused by Dampness. Below the Damp Earth, Dry Qi acts upon it, so severe Dampness stagnation leads to latent generation of Wind, disharmony of collaterals, and all phenomena arise from this. The current strategy is to transform turbid Phlegm and separate/discharge Damp-Heat. However, excessive separation may harm healthy Qi, so Qi-tonifying herbs are added. We must ensure mutual generation of Yin and Yang, unimpeded traffic, and balance between excess and deficiency; this is the way to nurture the Harmonious Heavenly State. Thus, I propose tonifying Qi, cultivating Spleen, and nourishing Liver as resources for regulation. Upper Jiao: disperse Lung; Middle Jiao: harmonize Stomach; Lower Jiao: separate and drain Bladder. When the Three Jiao flows smoothly, Damp-Phlegm has no place to stand, just as a room filled with mist becomes dry instantly when front and back doors are opened. This is my humble opinion; how do you, experts, view it?

Patient Zhu (Left pulse): Difficulty in flexion and extension behind the knee cap indicates excess in the Yangming channel. Yangming disease fails to bind sinews and bones and facilitate joint movement. Unfortunately, reproductive function is still deficient, with spermatorrhea and insecurity. To consolidate essence inevitably leads to Damp-Heat congestion. However, essence desires consolidation, while Dampness desires discharge; essence and water belong to different paths. Following this intent, use a method of combining astringency and unblocking. Please correct me.

Patient Mou: Only the Right Guan pulse is enlarged. Diet and起居 are normal, but the complexion lacks luster, mouth has foul odor, and thirst is frequent. Bad breath indicates Stomach Heat; foul odor and thirst clearly confirm Stomach Heat. The classics state: The Heart is the root of life and the change of Spirit; its glory manifests in the face. Now the lack of luster in the face suggests Heart channel pathology. Essentially, the Heart governs Blood, and Ying Qi originates from the Middle Jiao. Currently, the Stomach is often scorched by Heat, Stomach fluids cannot self-nourish, and seek relief from water. The Qi of food and water transforms into little Blood; insufficient Blood cannot ascend to glorify the face. The treatment method is to clear Stomach Heat to enrich the source of Blood generation; this cannot be addressed by reckless tonification.

Patient Rong (Left pulse): Pain in the right shoulder and arm. Previously stated as deficiency of Yangming collaterals, with Wind-Phlegm entering collaterals. Transforming Phlegm and unblocking collaterals yielded results like a drum responding to a beat. The plan for regulation seems to adhere to established methods without need for new chapters. Who would know otherwise? Disease has root and branch; the branch can be treated symptomatically, such as transforming Phlegm and unblocking collaterals. Once the branch recedes, attention should turn to the root. For instance, asthma occurred several times, and now movement causes Qi rebellion. If it were merely Phlegm-induced asthma, besides Phlegm pathogen, there would be no other cause. Then after an asthma attack, there should always be expectorated Phlegm daily, how could it vanish without a trace? The air we exhale comes from Heart and Lung; inhaled air enters Kidney and Liver. Thus, Lung above governs exhalation, Kidney below governs inhalation. Only lower deficiency leads to Kidney deficiency, failing to receive Lung Qi descending. Qi reaches midway and returns, which is why asthma occurs. Hence, it stops after onset and does not easily recur. Although lower parts are empty and lack stimulation, they can still retain; only movement triggers inability to retain. Liver is the child of Kidney; Water deficiency fails to nourish Wood, Wood dryness generates Wind, muscle twitching, and occasional dizziness are all signs of hyperactive Wind-Wood. Thus, once the branch recedes, one must address the root. The proposal is to tonify Kidney Yin without being cloying, assist Kidney Qi without being harsh/dry, and still incorporate methods to dispel fluid retention to treat the upper.

Patient Mou: Suffering from chronic asthma, Lung Qi fails to descend, Spleen Earth fails to transport and rotate. Initially generating Phlegm and accumulating Fluid Retention, subsequently Qi mechanism stagnates, unable to circulate and drain, leading to Shaofold Hernia and Masses, palpable mass below umbilicus, distension and fullness in lower abdomen, hard upon pressure. Both sides of lower abdomen belong to Liver, center belongs to Chong Mai. Classics state Chong Mai disease: men suffer internal seven hernias, women suffer leukorrhea and masses. Essentially due to irregular Chong Qi, collaterals are blocked, forming accumulations and collections, hence arising. Should disperse and unblock collaterals. Do not think winter pastes/pills require tonification to nourish health and expel disease; if pathogen is not removed, what reliance can Healthy Qi have?

Patient Qian (Left pulse): Qi rebels when lying down, bowel movements resolve only after twenty days, limbs occasionally become cold, sometimes heat/burning in chest feels like threads rushing, loosening after vomiting sour water, leg/thigh tendon pulling, frequent spermatorrhea. Pulse deep and wiry, coating white. This is due to latent Phlegm-Fluid inside, obstructing Lung Qi upwards causing Qi rebellion, confining Spleen transport downwards causing Spleen constraint, blocking the flow of Yang Qi. Stomach Earth loses descent, Gallbladder Wood floats up, confusing the Essence Gate. Mere rigid tonification is useless. Propose tonifying Qi, dispelling Fluid, separating/differentiating, and draining turbidity.

Patient Ge (Right pulse): Unblocked means no pain, pain means unblocked, this is the constant principle. Pain in Zhongwan clearly indicates Liver Wood attacking Stomach Earth, accompanied by dizziness/vertigo and palpitations below the heart. Master Zhang Zhongjing said: Palpitations below the heart indicate Water Qi. Thus knowing Stomach lacks descending function, turbid pathogens gather. Soothing and harmonizing Liver and Stomach especially requires expelling turbid pathogens as the main focus.

Patient Yu (Left pulse): After malaria, sudden emission/semen leakage occurred, followed by chaotic dreaming, soft body and weakness. Excess Heart leads to frightening/worrying dreams, Deficient Heart leads to chaotic dreams. Current pulse is floating and slippery. After administering clearing agents, emission decreased. This is likely due to excess Spleen Damp disturbing the Essence Gate, floating Qi of Heart and Gallbladder, thus presenting symptoms resembling Heart Deficiency.
