[General Principles] The general principle for treating dysentery should follow the *Neijing* (Classic of Internal Medicine), referencing the famous theories of various schools, to achieve perfection. It may be caused by the invasion of the six excess pathogens (Six Excesses) from outside, or damage to the interior from dietary accumulation, or occasionally by the prevalence of seasonal epidemic qi. All can lead to dysentery. This disease generally occurs in late summer and early autumn because there is an abundance of summer-heat and dampness pathogens. Injury to the Qi level results in white stools; injury to the Blood level results in red stools. Mixed red and white indicates both Qi and Blood are affected. Charcoal-yellow color indicates food stagnation; mung bean juice color indicates dampness. Pure blood without feces indicates extreme deep heat-toxin; mixed discharge of scattered blood indicates internal injury; presence of five colors indicates all five Zang organs are diseased. Greenish-blue color indicates wind-pathogen and excess Liver Qi. Treatment must distinguish between internal injury and external contraction, deficiency and excess, cold and heat. In the initial stage, if accompanied by headache, fever, and soreness/pain in the four limbs, it is appropriate to use exterior-releasing methods to induce slight sweating; do not abruptly administer purgatives for interior excess. If there is dry mouth, yellow tongue coating, tenesmus (urgent sensation with heavy feeling in the rectum), or defecation dozens of times a day, then purging the interior cannot be delayed. Heat in hands and feet with ability to eat indicates excess; cold hands and feet with inability to eat indicates deficiency. Ancients emphasized regulating Blood and moving Qi as the main approach: regulating Blood allows pus-blood stools to heal naturally; moving Qi eliminates tenesmus. Within three to five days of onset, one should employ "treat obstruction with unblocking" (using purgatives for obstructive symptoms); do not immediately use astringents to forcibly stop diarrhea, otherwise other diseases will arise. Only when the condition persists for a long time, righteous Qi is deficient, and diarrhea does not stop, should tonifying and astringent formulas be used. Furthermore, one must regulate diet, be cautious in daily life, and avoid all greasy foods, wheat products, and fruits. If old stagnation is not cleared and new accumulation increases, how can the intestines and stomach become clear? How can Yuan Qi recover? Gradually progressing to epigastric fullness and aversion to food, forming Anorexic Dysentery (Jinkou Li), often leading to untreatable cases. For dysentery that has persisted for some time, purgatives are unsuitable due to Stomach deficiency. Heat-stagnation accounts for 60-70%, while deficiency-cold also exists. Deficiency requires tonification; cold requires warming. Elderly and weak persons should not use purgatives.
[Main Formula] Huanglian (Coptidis Rhizoma, stir-fried with wine, 1.5 qian), Huangqin (Scutellariae Radix, stir-fried with wine, 2.5 qian), Baishao (Paeoniae Radix Alba, stir-fried with wine, 2 qian), Muxiang (Aucklandiae Radix), Zhike (Aurantii Fructus Immaturus, stir-fried, each 1 qian), Binglang (Arecae Semen, 0.5 qian), Zhigancao (Glycyrrhizae Radix et Rhizoma Preparata cum Melle, 0.3 qian). Add Danggui (Angelicae Sinensis Radix, 1.5 qian) and Suasha (Amomi Fructus Rotundus, 1 qian) for abdominal pain; add more Muxiang and Baishao (each 0.5 qian). Add Huashi (Talcum, stir-fried, 1.5 qian) for tenesmus and astringent/difficult urination. For white dysentery, add Baizhu (Atractylodis Macrocephalae Rhizoma, stir-fried), Baifuling (Poria, stir-fried), Huashi, and Chenpi (Citri Reticulatae Pericarpium, each 1 qian). For irritability and closure/urgency, add Dahuang (Rhei Radix et Rhizoma, 5 qian). For concurrent food stagnation, add Shanzha (Crataegi Fructus) and Zhishi (Aurantii Fructus Immaturus, each 1 qian). For red dysentery, add Danggui, Chuanxiong (Ligustici Chuanxiong Rhizoma), and Taoren (Persicae Semen, each 1.5 qian). If initially using purgation, add Dahuang (5 qian). For mixed red and white, add Danggui, Chuanxiong, Taoren (each 1.5 qian) to regulate Blood; add Huashi, Chenpi, Cangzhu (Atractylodis Rhizoma, stir-fried, each 1.5 qian) to regulate Qi. If food stagnation exists, also add Shanzha and Zhishi. For mixed red-black and reddish-difficult urination, add Mutong (Akebiae Caulis), Zexie (Alismatis Rhizoma), Fuling (Poria, each 1 qian), and Heishanzhi (Gardeniae Fructus Nigra, 0.5 qian) to separate and drain turbidity.
[Dysentery Onset] Collect flowers and leaves of Honghua (Polygoni Multiflori Caulis/Vine near water), dry in sun, stir-fry, and grind into powder. Take 3 qian per dose. For red dysentery, take with honey water; for white dysentery, take with sugar water. Shanzha Rou (Crataegi Fructus, stir-fried to charcoal, ground powder, 3 qian), Binglang (Arecae Semen, baked and ground powder, 1 qian), Chen Xicha (Aged fine tea, ground, 2 qian). For red dysentery, take with honey water; for white dysentery, add Juhong (Citri Reticulatae Pericarpium Rubrum, 0.5 fen) and Muxiang (0.6 fen), grind together, and take with sugar water for immediate effect. Qianjin Fang uses Xiebai (Allii Macrostemonis Bulbus, one handful), finely cut and boiled into porridge to stop dysentery. Hui Xiancaizi (Chenopodium Album Seed, dried in shade with leaves, ground powder), take 3 qian per dose with sugar water.
[White Dysentery] For continuous discharge of white pus, use Baifan (Triticum aestivum flour, stir-fried and sifted). Mix into rice porridge just before it is done and eat. Even if defecating over a hundred times that day, it will be effective. For discharge resembling fish jelly, use White Duck Blood (Anas platyrhynchos domestica blood) mixed with good liquor, taken hot.
[Cold Dysentery with Abdominal Pain] Roudoukou (Myristicae Semen, 1 liang, peeled), wrapped in dough mixed with vinegar, roasted, and ground to powder. Take 1 qian per dose with rice drink. Effective even if unable to eat. For cold dysentery caused by eating crab, wash fresh Oujie (Nymphaeae Nelumbinis Rhizomatis Node), crush, boil juice, and take hot. For white dysentery resembling frozen glue or nasal mucus, warm herbs such as Ganjiang (Zingiberis Rhizoma), Suosha (Amomi Fructus Rotundus), Houpo (Magnoliae Officinalis Cortex), and Rougui (Cinnamomi Cortex) should be used to warm.
[Heat Dysentery] Discharge of pure bright red blood, hot/astringent painful urination, slippery/excess-strong pulse, aversion to heat and preference for cold, irritability/urgency/multitude of vexation. YiYuan San (Formula seen in Summer-heat Chapter), take 3 qian per dose, mixed with newly drawn well water, until improvement. Cheqiancao (Plantaginis Herba, take seeds and leaves), crush juice, one cup, mixed with half cup white honey, take orally. Or use Dahuang (Rhei Radix et Rhizoma, 1 liang, soaked in wine for one day, remove wine), decoct in water evenly, divide into two doses, or add to decoction for oral administration.
[Blood Dysentery] Jiucai (Allii Tuberosi Herba, with roots), crush juice, one cup, mix with one cup liquor, take warm for best effect. Hetiao (Nelumbinis Folii Petiolus) decocted in water is also effective. Chuqianshu (Toosendan Fructus/Celastrus root bark), Qufan (Alumen, each 1 liang), Huaimi (Sophorae Flavescentis Flos, 2 liang), Zhigancao (Glycyrrhizae Radix et Rhizoma Preparata cum Melle, 5 qian). All ground to powder, made into pills with water. Take 3 qian per dose with rice drink, twice daily. For blood dysentery where various medicines fail and condition is critical, take Chuqian Gen Baipi (Root bark of Celastrus/Ochrosia, 5 qian), two cups of liquor, decoct to one cup, take orally for immediate effect, frequently verified. For pure blood discharge, Mashiren (Cannabis Fructus) juice cooked with Lüdou (Phaseoli Radiati Semen) and eaten is effective. For inability to eat and unbearable pain, use Yanshusuo (Cyperi Rhizoma, stir-fried to powder, 3 qian), mixed with rice drink to stop immediately, or take warm with light liquor. For clear blood discharge and stabbing abdominal pain, Chunken Baipi (Broussonetiae Papyriferae Cortex/Root bark), slice and stir-fry, grind to powder, make pills with rice vinegar, size of tung tree seeds, take 30-50 pills on empty stomach with rice drink.
[Wine Dysentery] Dysentery with blood and abdominal pain, or stool like fish brain/five colors. Lao Sigua (Lagenariae Spongiosa Fructus, one old loofah), burn with peel to ash, grind. Take 2 qian with liquor on empty stomach, until improved.
[Regardless of Red/White Chronic Dysentery] Bai Biandou Hua (Lablab Purpureus Flower), take number equal to patient's age in years, add Bingtang (Rock Sugar, 3 qian), Chen Xicha (Aged fine tea, 3 qian). Decoct in one bowl of water to 60% volume, take. Wait one meal interval, then use Hong Biandou Hua (Red Lablab Flower) decocted similarly. Frequently verified.
[Chronic Dysentery Not Healing] Sangshu Zhuchong Shi (Silkworm excrement from mulberry tree), stir-fry until dry, grind fine, sift, take with sugar water for immediate effect.
[Anal Pain] For chronic dysentery with unbearable anal pain, wrap stir-fried Yan Yan (Salt) in cloth and apply warm compress. Or use Zhishi (Aurantii Fructus Immaturus) heated and applied. Smoke Shuai Ai (Artemisiae Argyi Folium), Huangla (Beeswax), Hezi (Terminalia Chebulae Fructus) and fumigate the area.
[Rectal Prolapse after Dysentery] Shizhi (Halloysitum Rubrum), Fulonggan (Furnacis Terra Cinerata/Zao Xin Tu), grind fine and apply. Wubeizi (Chinese Gall, 3 qian) ground and inserted; Baifan (Alumen, one piece), decoct in half bowl of water to wash. For large intestine deficiency-cold prolapse, use Woniu (Helix Aspersa/Snail, 1 liang, baked dry and burned to ash), mixed with Zhuzhi (Porci Adeps/Pig fat), apply to area for immediate retraction, most effective.
[Qingshu Wan] Treats dysentery regardless of red/white, day/night, unlimited frequency, tenesmus, etc. Use Zhuling (Polypori Porria, stir-fried with wine), Chishao (Paeoniae Radix Rubra), Guangpi (Citri Reticulatae Pericarpium), Wuyao (Linderae Radix), Yanhusuo (Cyperi Rhizoma, stir-fried with light vinegar soup), Sheng Gancao (Glycyrrhizae Radix et Rhizoma, sliced and sun-dried, ground powder, each 2 liang), Gegen (Puerariae Lobatae Radix, 3 liang), Huangqin (Scutellariae Radix, 3 liang stir-fried with wine), Zexie (Alismatis Rhizoma, 4 liang stir-fried with salt water), Guangmuxiang (Aucklandiae Radix, 1 liang sliced and ground). All made into fine powder. Use Qinghao (Artemisiae Annuae Herba, 3 liang), Ziye (Perillae Folium, 2 liang) decocted into thick soup, sprinkle onto powder, make pills with water method. Coat with Baiyunling (Poria, 4 liang ground powder), Huashi (Talcum, 8 liang water-flowered and ground). Take 3 qian per dose, swallow with soup decocted from Bai Biandou Hua (20 flowers) and Xian Heye (Fresh Lotus Leaf, 3 qian). Or decoct Chen Niangan Luobocai Gan (Dried radish greens from previous year) soup for better effect. After one or two doses, frequently tested for immediate effect. Avoid greasy/meaty foods, wheat, eggs, raw/cold foods for several days. Prepare this medicine for others after intense heat. Do not use for white dysentery resembling fish jelly or watery diarrhea.
[Jinkou Li / Anorexic Dysentery] Ancient formula uses Shilian Rou (Nelumbinis Plumula, 2 qian ground), taken with aged rice drink. If true Shilian is unavailable, cook Ou Zhi (Nymphaeae Nelumbinis Rhizomatis Juice) until cooked, add little sugar, take frequently. Concurrently consume multi-year aged rice thin congee to regulate Stomach appetite, must be effective. Danxi Formula: Renshen (Ginseng Radix et Rhizoma, 2 qian), Huanglian (Coptidis Rhizoma, 1 qian stir-fried with Ginger Juice). Decoct strongly, sip slowly throughout the day. If vomiting, take again. Once one sip goes down, appetite opens. Also guide heat downward: use one Tianluo (Vivipara Vivipara Snail), crush, add little Shexiang (Moschus), apply to umbilicus for immediate effect. Dasuan (Allii Sativi Bulbus) crushed and paste applied to Yongquan (Kidney 1, sole of foot) (also treats children). Shuiwa (Rana temporaria/Frog, one), crush with entrails, bake hot on new tile, add Shexiang (0.3 fen), apply to umbilicus; once Qi flows, able to eat. Jin Liyu (Goldfish, one tail, 1-2 jin, cleaned), cook with Congbai (Allii Fistulosi Bulbus), Jiang (Ginger), and Hujiao (Piperis Longi Fructus, 3-5 fen). Let patient smell it, eat whatever they desire for immediate effect. Wu Mei (Mume Fructus, 1 jin) cracked, placed in basin, soak with boiling water (more than 10 bowls) until cooked. Let patient sit on basin, let steam rush up to anus, wash when warm. Patient falls asleep, help to bed, decoct aged rice porridge soup to wait. If thinking of eating, eat half bowl, drink again shortly after, not too much. Halve dose for children. For those where all medicines fail, use Shuixianzi (Fly larvae/Maggot from cesspit), wash clean, bake dry on tile to powder. Take 1-2 spoons per dose with rice drink, able to think of eating. For any Jinkou Li, if accompanied by headache, heart vexation, warm hands/feet, and no excessive use of cooling medicines, observe pulse/symptoms: if Spleen/Stomach not weak, it is toxin/qi rushing up to Heart/Lungs causing vomiting/anorexia. Use Baidu San (Decoction for Resolving Toxins: Fuling, Chuanxiong, Chaihu, Qianhu, Jiegeng, Qianghuo, Duhuo, Zhike, Sheng Gancao), equal parts, take 4 qian per dose, add 100 grains of Chencangmi (Aged stored rice), 3 slices of Shengjiang, one date, decoct in water, take warm. If pulse/Qi is weak, or epigastrium/abdomen distended, hands/feet cold, initially no vomiting but caused by excessive use of Poppy (Yangsuyu), Wu Mei, bitter/astringent/cool medicines leading to vomiting upon smelling food: this is Spleen/Stomach deficiency. Use Shanyao (Dioscoreae Rhizoma, half raw, half stir-fried), ground to powder, take with rice drink. If severe deficiency-cold and weak pulse, must treat with Lizhong Tang (Decoction for Regulating the Middle: Renshen, Chuaizhu, Paoganjiang, Zhigancao), each 1 qian, decoct in water and take.
[Thirsty Dysentery Not Stopping] Yangfei (Ovis Aries Lung, one whole), make soup with Yanchi (Fermented Soybean Salt), eat. Cure in three times.
