Predecessors provided numerous names and classifications for *Lao* (chronic consumptive disease). In reality, these are merely exaggerated descriptions. During clinical practice, they instead cause confusion among physicians regarding diagnostic approaches. Generally, external contractions, internal injuries, and the excessive use of the Seven Emotions (joy, anger, worry, thought, sadness, fear, fright) can all lead to this condition. The symptoms manifest as fatigue and poor appetite, or frequent aversion to cold, or frequent fever, or alternating chills and fever. The complexion becomes increasingly haggard day by day, and muscles become increasingly emaciated. This indicates entry into the realm of *Lao*. If coughing persists and hemoptysis (coughing up blood) comes and goes intermittently, it is an established *Lao* syndrome. The treatment principle considers this incurable. These two conditions are categorized separately; refer to relevant sections for details.

Regarding pulse diagnosis: pulses that are extremely large, extremely small, or extremely deficient all indicate *Lao*. However, if the pulse gradually slows, vitality gradually returns; if the pulse gradually becomes rapid (*Shu*), death gradually approaches. If the pulse is rapid and accompanied by tight and wiry qualities, one in ten cannot be saved. If both left and right Guan positions exhibit a wiry pulse, death is near.

Ancients stated that taking cooling herbs for this condition leads to certain death. I believe this is not entirely true. When fire is blazing, removing the firewood is necessary, and there must be expedient measures. Once the fire subsides, ceasing their use causes no harm. Moreover, everyone knows that cooling herbs should not be taken for long periods. It is only the herbs for nourishing Yin and descending fire, as well as those that are neither cool nor warm, that most mislead people. Whenever I encounter families with *Lao* patients, before examining them, if I see books such as *Xue Shi Yi An*, *Jing Yue Quan Shu*, *Yi Fang Ji Jie*, and *Ben Cao Bei Yao* on their desks, and hear that they drink Ophiopogonis Radix (Maidong) as tea, I refuse to examine them. Knowing they are trapped by "medicine demons," I determine they will surely die. I generally dislike cooling herbs, but I list here those cooling formulas that should not be discarded.

For Lung Atrophy (*Fei Wei*), hoarseness, throat obstruction (*Hou Bi*), hemoptysis, and irritability, Zishen Wan (Nourishing Kidney Pill) is suitable.
For urinary retention (*Long Bi*), this formula is also suitable.
For reckless bleeding due to blood heat, surging pulse, high fever, hemoptysis, or epistaxis (nosebleed), Sisheng Wan (Four Generations Pill) is suitable.
For hematemesis, hematochezia, uterine bleeding (*Xue Beng*), painful urinary bleeding (*Xue Lin*) in women, and hematemesis occurring while petechiae and jaundice from Cold Damage have not yet resolved, Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) is suitable.
For bone-steaming tidal fever (feeling heat radiating from the deep bones), worse at night than during the day, heat entering the Blood Chamber in women, or fever during pregnancy, Digupi San (Phellodendri Bark Powder) is suitable.
For afternoon fever and continuous night sweats (*Dao Han*), Danggui Dahuang Tang (Angelica and Rhubarb Decoction) is suitable.
For massive hemoptysis or epistaxis filling basins and buckets, sudden use of bitter-cold or pungent-warm herbs is contraindicated. Immediately use the following formula. After ingestion, allow the patient to sleep deeply; do not wake them. They will recover immediately, regaining health overnight. Dushen Tang (Single Ginseng Decoction) is suitable.
For damp-heat in the Stomach, yellowing of the body, dark urine, mouth ulcers, gum erosion, hematemesis, and epistaxis, Ganlu Yin (Sweet Dew Drink) is suitable.
For cough persisting due to autumn dryness Qi, Xiebai San (Draining White Powder) is suitable.
For feeling chilly and shivering due to autumn dryness Qi, followed by fever after the chill subsides, progressing to cough: Mistakenly using tonics like Ginseng and Costus Root causes lung heat to have nowhere to vent, rushing urgently to the Large Intestine. Food enters but is expelled without digestion; even without food, intestinal waste rushes out with the Qi, causing unceasing diarrhea and dysentery. Use lung-moistening herbs combined with intestinal-moistening herbs so that both source and branches are cleared. One dose stops cold, heat, cough, and diarrhea. This is Yu Jiayan's得意 method. Use Xiebai San minus Rice, adding Scutellariae Radix, Asparagi Tuber, and Apricot Kernel.
For nocturnal emission and seminal loss, and dreams of intercourse with spirits, Fengsui Dan (Sealing Marrow Pill) is suitable.
For afternoon fever, lower back pain, and sore legs, where Liuwei Wan fails, Dabu Yin Wan (Great Nourishing Yin Pill) is suitable.
For rebellious Qi, irritability, vomiting, palpitations, and insomnia, Wendan Tang (Warm Gallbladder Decoction) is suitable.
Add Ejiao (Donkey-hide Gel) and Zao Ren (Semen Ziziphi Spinosae).

Qi stagnation belonging to the Lung is due to Lung dryness. Various phlegm, wheezing, and vomiting belonging to the Upper Jiao are also due to Lung dryness. Qingzao Jiufei Tang (Clearing Dryness and Saving the Lung Decoction) is suitable.

Although the above formulas are considered cooling, they nurture vital energy. They are indispensable yet temporary expedients for treating *Lao*.

The character *Lao* contains the radical for Fire; there is no *Lao* without fever. Physicians warn against cold/cooling, considering nourishing Yin the safest method. However, they do not realize this condition is often caused by Yin Excess, and nourishing Yin exacerbates the disease. People say: "Yin deficiency causes fire movement." I alone say: "Yin excess causes fire movement." Why? The Heart and Lung are superior, the position of Yang. When chest Yang is distributed, like the sun and moon rising, small fires lose their light. Where is the fever? Only when Yin Qi of the Lower Jiao becomes excessive, invading the Yang position. Dampness of the Foot-Taiyin Spleen moves to form fluid retention, harassing the Hand-Taiyin Lung, causing persistent cough. Cold Qi of the Foot-Shaoyin Kidney moves to form Yin blood, harassing the Hand-Shaoyin Heart, causing unceasing hemoptysis. These two symptoms are the outline of consumptive *Lao*; is this not caused by Yin excess? Furthermore, the Heart and Lung are like the vast sky. When Lower Jiao Yin Qi rushes upward, dark clouds cover everything; day looks like night. Not only candles shine, but fireflies from rotting grass also glow. Is this not evidence of Yin excess causing fire movement? Besides, the Dragon-Thunder Fire in the body differs from other meridians' fire. It requires four sides covered in Yin clouds to surge; under the scorching sun, Thunder Dragons lie dormant. The following formulas are excellent methods for reducing fever. Students must study them carefully.

First is Zhang Zhongjing's method, using Xiaojianzhong Tang (Minor Construct the Middle Decoction) as the main formula. Cinnamon Twig and Ginger in the formula disseminate chest Yang, thereby eliminating Yin fire. Later generations lacked the insight of ancients, fearing the heat of Ginger and Cinnamon, using only warm-tonic herbs. Li Dongyuan said: Ginseng and Licorice are good medicines for draining fire. He also said: Sweet-warm reduces great heat. Though viewing ancient formulas slightly lower, they still follow ancestral rules.
Dispersing Lung Yang clears heavenly Qi, preventing earthly dampness from steaming into clouds, stopping Dragon-Thunder fire. This is a major method for reducing fever. Eight formulas:
Baoyuan Tang (Protecting Origin Decoction), Buzhong Yiqi Tang (Tonifying the Middle and Augmenting Qi Decoction), Danggui Buxue Tang (Angelica Tonifying Blood Decoction), Sijunzi Tang (Four Gentlemen Decoction), Liujunzi Tang (Six Gentlemen Decoction), Wuwei Yigong San (Five Agents Extraordinary Power Powder), Xiangsha Liujunzi Tang (Aucklandia and Amomum Six Gentlemen Decoction), Guipi Tang (Restore the Spleen Decoction).
These eight formulas tonify the Foot and Hand Taiyin meridians. They are highly effective for reducing fever, improving appetite, resolving phlegm, and stopping bleeding. Except for cough caused by fluid retention, where Ginseng (sweet-bitter, Yin nature, moist) aids dampness and is unsuitable. Thus, Zhang Zhongjing removes Ginseng for cough, adding Dried Ginger and Schisandra, or adding Asarum. Adding cold/warm/dry/damp herbs as appropriate is exquisite. Note: For Lung dryness/heat, Ginseng is essential.

Dispersing Heart Yang spreads the Light of Li (Fire), extinguishing small fires. Another major method for reducing fever. Ten formulas:
Fuzi Lizhong Tang (Aconite Regulating the Middle Decoction), Jinxiao Zhufu Tang (Near-Efficiency Atractylodes and Aconite Decoction), Renshen Yangrong Tang (Ginseng Nourishing Grace Decoction), Shengyu Tang (Sage Replenishing Decoction), Zhengyuan Dan (Upright Origin Pill), Erjia Longgu Tang (Double Added Dragon Bone Decoction), Xihe Dan (Black Tin Pill), Zhufu Tang (Atractylodes and Aconite Decoction), Fuzi Tang (Aconite Decoction), Shenfu Tang (Ginseng and Aconite Decoction).
These ten formulas treat the Foot and Hand Shaoyin meridians. Verification is similar to above. They circulate essence/Qi, support primal Qi, and supplement Fire to attack Water. For hematemesis, Lizhong Tang decocted equally as per ancient methods is miraculous. Or add Aucklandia and Angelica as per *Ren Zhai Zhizhi*. The miracle lies in blood warming and circulating meridians; Dried Ginger and Licorice work together. Alternatively, use Roasted Licorice 4 qian, Charred Dried Ginger 2 qian, Schisandra 2 qian. Miraculous.
To guide fire back to its source, use Babai Wan (Eight Ingredients Pill). Since Xue Lizhai and Zhang Jingyue, it has been kept as a secret treasure. Actually, it treats symptoms, not root, and should not be taken long-term. I often see prolonged use of Yin-nourishing agents worsening fever. Later doctors review previous cases, saying hot herbs are forbidden, but doubling Rehmannia to neutralize toxicity guides fire back. Initial doses of Babadihuang Tang seem miraculous, but subsequent doses fail. After three to four doses, deficiency worsens, fever burns intensely. Families doubt Cinnamon/Aconite errors or blame processing/origin, viewing them as poison, relying wrongly on Yin-nourishing herbs. No one understood why. I clarify: Yin Qi occupying Yang position allows evil fire to steal movement. Suddenly receiving Cinnamon/Aconite to support chest Yang makes evil fire retreat. Hence initial efficacy. Failure at 3-4 doses is habituation. Worsening at 5-6 doses occurs because the small amount of Yang herbs cannot counteract the large amount of Yin herbs (Rehmannia). Yin herbs are soft/slow; Yang herbs are hard/fast. The stomach is like a furnace. Soft/slow lingers; hard/fast arrives first. Initially, only Yang support is seen, ignoring Yin nourishment. Later, lingering old Yin combines with new Yin, countering the fast-acting Yang which has already dissipated. Daily Yang intake is like fire burning out quickly; Yin intake is like water accumulating slowly. Tiny Yang cannot fight accumulated Yin, increasing heat. Dual views in the world cause error.
Babadihuang Tang (Eight Ingredients Rehmannia Decoction), Quanzhen Yiqi Tang (Complete True Single Energy Decoction), Shimai Dihuang Wan (Ten Ingredients Rehmannia Pill).

After *Lao* recovery, conditioning is essential. Pills are superior to decoctions. Suitable:
Babadihuang Wan (Eight Ingredients Rehmannia Pill), Tianwang Buxin Dan (Heavenly Emperor Nourishing Heart Pill), Guilu Erxian Gao (Turtle and Deer Two Immortals Paste), Huanshao Dan (Returning Youth Pill), Quanlu Wan (Whole Deer Pill), Jiawei Hujian Wan (Modified Tiger Knee Lock Pill).
