Generally speaking, for women, worry is required not only during labor. After childbirth, extreme caution and care are necessary. The most critical and dangerous conditions often arise at this time. Do not assume that because there were no complications during delivery that one can indulge desires and act recklessly without restraint. Although the initial transgression of taboos may be as minute as an autumn hair, the resulting illness can be as vast as Mount Song or Mount Tai. Why? Because diseases after childbirth are more difficult to treat than other illnesses. After a woman gives birth, her five zang organs become weak and emaciated; she should only receive tonification and must not undergo purgation. If she already has an ailment, medication is not needed; if medication is forced upon her, it will further increase her deficiency. In a state of increased deficiency, the path to recovery becomes distant. Therefore, within one hundred days postpartum, a woman must be diligent in her vigilance and fear, avoiding indulgence and contact with taboo factors, and certainly not engaging in sexual intercourse immediately. If such taboos are violated, the body will inevitably become stiff and rigid, resembling opisthotonos (arched back), known as "Chufeng" (Postpartum Wind). This is the manifestation of violating postpartum precautions. If the condition resembles opisthotonos, life becomes as precarious as a candle in the wind. Generally, women suffer from wind pathogens and cold deficiency in the umbilical region, all caused by early engagement in sexual intercourse.
After a woman completes sitting-style delivery, she should first drink one cup of cold urine from an uncircumcised boy. She must not lie down immediately but should sit with eyes closed for a moment before being helped onto the bed to lie supine. Side-lying is prohibited; knees should be flexed, and legs extended only gradually. The head should be propped high on the bed, with thick bedding provided. Surrounding walls should be sealed without gaps to prevent infiltration by "thief wind" (drafts). Simultaneously, someone should frequently apply warm compresses to the area below the navel to ensure lochia does not stagnate. This practice should continue for three days. The parturient should not sleep excessively; if she does, the caregiver should wake her frequently. Old sayings suggest that a woman should not get into bed for three days after delivery. If she waits three days to go to bed, she would have to sleep on the floor, which exposes her to earthy qi, which is inadvisable. Immediately after delivery, grind and administer three fen of vinegar-ink. Another method states: do not consume vinegar-ink, as it may damage the Lung meridian causing coughing. This is indeed an overcautious view. However, vinegar-ink is intended to break up coagulated blood; using overly strong vinegar or excessive amounts could harm the lungs, but moderate use avoids this. Customs vary, and some do not take it. Additionally, within three days postpartum, let the mother smell the smoke of burning charcoal with vinegar or burning dry lacquer. If dry lacquer is unavailable, burn old lacquerware to generate smoke, preventing blood confusion, rebellion, or syncope. In summer, heat bricks outside the room and place them inside with vinegar poured over them. In summer, avoid heating the room or cooking porridge/medicine inside. Shortly after delivery, consume plain white rice porridge initially, avoiding overeating; small, frequent meals are best. Gradually increase the amount daily. Ensure the porridge is cooked properly, avoiding overnight leftovers, and maintain appropriate temperature to prevent stagnation and disease. Continue providing one cup of uncircumcised boy's urine periodically. Prepare urine in advance; in summer, preserve it with mint. Regardless of abdominal pain or existing ailments immediately postpartum, mix half a cup of uncircumcised boy's urine with wine and take warm, five to seven times, which is beneficial. After one "la" (seven days), small amounts of pure wine and salt flavor may be introduced. Another view prohibits alcohol immediately postpartum because wine draws blood rebelliously into the limbs, and the mother's viscera are too weak to withstand its power; hot wine entering the stomach causes dizziness and coma. After seven days, small amounts of wine may be taken, but not excessively. If craving wine or medicinal wine before the month is complete, use one sheng of clean black beans, stir-fried until no smoke rises, soaked in five sheng of non-grain-alcohol wine, plus one liang of high-quality Qianghuo (Notopterygium Rhizome), washed and crushed, steeped together. This is excellent. Use this wine to administer pills or drink sparingly occasionally to dispel wind-cold, nourish qi and blood, discharge lochia, and promote lactation. If the mother is not accustomed to drinking or it is summer, do not force it. After one "la", food may seem tasteless; boil mutton thoroughly or use hen broth, adding slight seasoning, to make porridge. Or eat well-cooked porridge with pig trotters, avoiding sow meat and white trotter pork, in moderation. In Jiangsu and Zhejiang regions, it is customary for mothers to eat many boiled eggs for tonification. After three days, warm wheat flour noodles may be eaten in small amounts; eating noodles too early can cause swelling disorders. Overeating risks food stagnation. Within the first month, avoid excessive talking, laughing, fright, anxiety, crying, overthinking, and anger. Forcing oneself out of bed, sitting for long periods, doing needlework, or strenuous labor is harmful. Indulging in raw/cold foods, sticky glutinous rice, greasy vegetables, fish/meat, exposure to wind/cold, undressing for bathing, or washing with cold water may not show immediate effects but severely damages recovery during the confinement period. "Full month" refers to three months, not thirty days. Negligence leads to "Chulao" (Postpartum Labor Sickness), characterized by soreness, heaviness, cold pain in hands, feet, waist, and legs, and a sensation of cold wind blowing through the marrow. Even famous doctors cannot cure this. Avoid staying alone at night due to heart deficiency from blood loss, fearing palpitations. Wash blood-stained clothes but do not expose them to midday sun to avoid evil influences. Do not wash feet, fearing downward attack of qi and blood. Do not scrape the tongue (damaging the heart) or bend forward while brushing teeth, as these cause blood rebellion and syncope. These are standard precautions. After the full month, strictly avoid arbitrary diet, exposure to wind/cold, emotional excesses, vigorous hair combing, loud shouting, manual labor, sexual desire, climbing heights, or straining during urination/defecation. Such disciplined care maintains harmony of qi and blood and restoration of viscera until one hundred days pass. Failure to follow this results in residual postpartum diseases. A hard mass like a rotating plate under the heart in a mother is caused by drinking water and is named "Qi Fen" (Qi Division). Generally, from childhood until menstruation begins, women belong to the Shaoyin category. Once menstruation starts, they are discussed according to Jueyin. After menopause, they belong to the Taiyin meridian. Treating pregnancy and childbirth diseases follows the Jueyin meridian, avoiding the Stomach Qi and the Upper and Middle Jiao, known as the "Three Prohibitions": no sweating, no purgation, no promoting urination. Sweating mimics the syndrome of premature purgation in Cold Damage. Promoting bowel movements makes the pulse rapid and disturbs the Spleen. Promoting urination depletes internal fluids, causing dryness in the Stomach. Medication methods that avoid these three prohibitions allow Ying and Wei to harmonize and fever/chills to cease. For thirst, use Baihu Tang; for qi weakness, use Huangqi Tang; for stabbing blood pain, use Danggui; for abdominal pain, add Shaoyao. Generally, epidemic diseases in postpartum follow modified Chaihu Tang, while miscellaneous conditions follow modified Siwu Tang. Pulse and symptoms must be carefully observed.
[Formulas]
Danggui San (from *Daquan Liangfang*) treats combined qi and blood deficiency postpartum. Caution against heavy tonification to avoid increasing guest heat and causing other diseases. It ensures smooth lochia discharge.
Danggui (Angelicae Sinensis Radix), Shaoyao (Paeoniae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Huangqin (Scutellariae Radix) (each 1 liang), Baizhu (Atractylodis Macrocephalae Rhizoma) (0.5 liang). Grind into fine powder. Take 2 qian, mixed with warm uncircumcised boy's urine or wine. If no abdominal pain, mix with rice water and roast.
Jiajian Siwu Tang (from *Bianchan XuZhi*) treats postpartum blood deficiency with fever, or clarity during the day followed by fever and aversion to cold at night.
Danggui, Chuanxiong, Shengdihuang (Rehmanniae Radix), Chaihu (Bupleuri Radix). Equal parts. Take 3 qian per dose, decoct in one da zhen (approx. 200ml) water until reduced to 6 fen, strain, and drink.
Xiebai Tang (from *Qianjin Fang*) treats postpartual chest烦热 (stifling heat) and rebellious qi.
Xiebai (Allii Macrostemonis Bulbus), Banxia (Pinelliae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Renshen (Ginseng Radix), Zhimu (Anemarrhenae Rhizoma) (each 2 liang), Shigao (Gypsum Fibrosum) (4 liang), Gualougen (Trichosanthis Radix) (3 liang), Maimendong (Ophiopogonis Radix) (0.5 sheng). Coarsely chop. Decoct in 13 sheng water to obtain 4 sheng, strain, divide into five doses. Take three times during the day and twice at night. If heat is severe, add 1 liang each of Shigao and Zhimu. One version excludes Shigao. To treat perceived stifling heat (*Bianchan XuZhi*): Use round white Banxia in any quantity, wash cleanly, soak in boiling water seven times, grind to powder. Insert amount the size of a soybean into the nose to restore consciousness. *Yixue Faming* states: For pre-partum chills and fever, remove Banxia from Xiaochaihu Tang, called Huanglong Tang.
Treats blood/qi rebellion causing chest vexation and fullness (from *Zhouhou Fang*).
Use 1 jin of fresh bamboo skin, decoct in 3 sheng water to obtain 1.5 sheng, divide into two doses.
Zaojia San treats postpartum unconsciousness.
Chuanlianpi (Toosendan Cortex), Zaojiaoshupi (Gleditsiae Sinensis Cortex) (each 1 jin), Zaojiaohé (Gleditsiae Sinensis Seed) (1 he), Shilian (Nelumbinis Semen) (1 he, remove plumule). Coarsely grind. Decoct soup while hot. Use for fumigation. Wash the private parts thoroughly in a clean room, lay mats at the washing site. After washing, dry with silk cloth, then take Yulu Tongzhen Wan, two pills with hot wine, then lie supine.
Xionggui Tang (also called Lixiao San, from *Baiyi Xuanfang*) treats sudden elongation of breasts postpartum, thin as intestines, hanging below the lower abdomen, unbearable pain, critical condition, named "Ruxuan" (Breast Suspension). Also treats lochia retention, abdominal pain, or excessive bleeding causing dizziness and collapse. Treats fetal movement pain and bleeding during pregnancy.
Chuanxiong, Danggui (each 2 jin). Take 0.5 jin each of Chuanxiong and Danggui, coarsely chop, decoct strongly in water in a clay pot. Drink warm at any time. Take remaining 1.5 jin each, cut into large pieces, slowly burn in an incense burner to produce smoke. Place under a table facing the patient. Patient lowers head, leans on table, inhaling smoke through mouth/nose and applying to affected breast until one dose is used. Reassess condition. If not fully recovered, repeat. After two doses, if breasts retract but don't return to normal, rub one castor bean (Bimazi) with cold water and apply to vertex, wash off shortly after, then full recovery. This is Master Tongzhen's secret formula. Add Shasaren if needed. Add Shaoyao for stabbing abdominal pain. Add Wumei and Maimendong for dry mouth/thirst. Add Ganjiang and Baishaoyao for chills. Add Fuling and Shengjiang for fluid retention in heart with mild nausea. Add Renshen and Zhuye for deficiency vexation insomnia. Add Shudihuang, Juapi, and Xingren for constipation. Add Cheqianzi for difficult urination. Add Houpo for abdominal/flank distension. Add Xiangfu for metrorrhagia. Add Ziwan, Banxia, and Shengjiang for cough with phlegm. Add Niuxi for low back/leg pain. Add Yanhusuo for epigastric pain. Add Mudanpi for retained blood with heavy/painful waist/abdomen.
Treating women who took too early labor-inducing drugs, exerting excessive force before natural expulsion, injuring membranes, resulting in a jade-like ribbon (3-4 chi long) protruding from the vaginal canal postpartum, pulling heart/abdominal pain unbearably. Moving it slightly causes agony. A case at Longxing Tiezhuguan involved a woman with this symptom. Physicians used Shixiao San, which relieved pain slightly but didn't detach the flesh ribbon. A Daoist asked about the case, noting Shixiao San failed to remove the root. He bought 3 jin old ginger, washed clean without peeling, pounded in stone mortar, mixed with 2 jin clear oil, baked in pot until oil dried and charred. Used a piece of cooked silk (~5 chi long), folded, held by a sturdy woman to gently lift the flesh ribbon, coil it, insert back into vaginal canal, wrap in silk bag, apply moistened oil-ginger paste to the area, fumigate. When ginger cooled, reheat with iron. Maintain ginger scent for a day and night. Ribbon shrunk significantly. Repeat method. Two days later, ribbon fully retracted, pain ceased. Then administer Shixiao San and Xionggui Tang for tonification. Daoist said this is Lady Wei's secret recipe for strange diseases. Never let the ribbon break in two, or it becomes incurable.
Danggui San (from *Daquan Liangfang*) treats postpartum deficiency, tonifying blood.
