Hematemesis caused by cold damage (Shanghan) occurs when heat resides in the Yang meridians. Pathogenic factors are abundant at the exterior. If sweating should have been induced but was not, heat toxins penetrate deeply and knot within the five Zang organs, thereby forming blood stasis and heat. This steams upward to the Upper Jiao. Heat travels with Qi and merges into the Stomach. When the Stomach receives blood, it becomes distended and full, and Qi rebels upward, thus causing hematemesis.
The song states: "All because pathogenic heat strikes the Three Yang meridians; physicians failed to measure [the situation] when they should have induced sweating. Heat toxins penetrate deeply and remain in the five Zang organs, thus causing hematemesis, which is alarming." This refers to the failure to induce sweating when indicated, allowing heat toxins to penetrate deeply, thus causing hematemesis. In cases of severe blood stasis, use Didang Wan (Madder and Dogwood Pill). For mild cases, use Tao Ren Chengqi Tang (Peach Kernel and Rhubarb Decoction); concurrently administer Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction). According to the Treatise on Syndrome Differentiation and Treatment, use Sanhuang Xiexin Tang (Three Yellows Heart-Draining Decoction) or Bai Pi Di Xue San (Cypress Leaf and Earth-Blood Powder). After drastic purgation, if the Cun pulse is deep and slow, the Chi pulse is absent, there is throat obstruction, and one spits pus and blood, use Mahuang Shengma Tang (Ephedra and Asarum Decoction). If vomiting occurs after taking Guizhi Tang (Cinnamon Twig Decoction), pus and blood will subsequently be vomited; treat hematemesis with Xijiao Dihuang Tang.
When various Yang meridians are affected by heat pathogens and the exterior is not released, the fault lies with the physician. Consequently, heat toxins flow into the Zang organs, and Sanhuang Tang (Three Yellows Decoction) is the primary treatment. Whenever Yang meridians are affected by heat, and the pathogen is at the exterior, if sweating is neglected, heat toxins enter the Zang organs, accumulate and form stasis internally, eventually leading to hematemesis. Generally, if cold damage fails to induce sweating, pathogenic heat transforms into evil blood. If the accumulated toxin is not eliminated, it can also steam and rot the blood. Symptoms include eyes closed and red, mental confusion with short speech, palpitations with chest pain and oppression, dizziness and fainting, desire to rinse the mouth without swallowing, restlessness, vomiting, panting,惊狂 (frightened mania), delirium, epistaxis or hematemesis, cold spine and feet, bone heat with open skin, cold limbs, excessive sweating, stubborn phlegm, distension and urgency in the chest, hypochondrium, and lower abdomen, black stool with slight diarrhea, frequent urination with incontinence. These are all signs of blood stasis syndrome. Both men and women possess this vascular pathology; women suffering from cold damage see it particularly often, as they contract the disease during the onset or cessation of menstruation, or while ill, their menses happen to arrive. Pain from blood stasis is generally heavier at night and lighter during the day, or clear during the day and delirious at dusk. Blood belongs to Yin, aligning with its category [night/Yin]. The aforementioned symptoms need not all be present; if even a fraction appears, one should proceed with treating it as a blood syndrome. Select Xijiao Dihuang Tang, Xiaochaihu Tang (Minor Bupleurum Decoction), Tao Ren Chengqi Tang, or Sanhuang Tang. Whatever the severity, for blood binding, Didang Tang/Wan is primary, assisted by Chuanxiong (Sichuan Lovage Rhizome). It is favorable until black stools are completely expelled. Nevertheless, hemoptysis in cold damage is especially difficult to rescue, let alone hematemesis. Hematemesis is never a minor ailment. In the initial stage, attack methods may still be used. However, if bleeding comes continuously or recurs after dispersal treatments, it is absolutely incurable. Detailed formulas are found in the Preface. A general remedy: take one small cup of radish juice, add fresh well water, decoct Maohua (Lily Flower Bud), and administer. For blood heat, use Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction). According to the Treatise on Syndrome Differentiation and Treatment, use Dixue San (Earth-Blood Powder), Baipi Tang (Cypress Bark Decoction), or Sanhuang Xiexin Tang. After drastic purgation, if the Cun pulse is deep and slow, the Chi pulse is absent, there is throat obstruction, and one spits pus and blood, use Mahuang Shengma Tang. There are two syndromes here: one is Yang Du (Yang Toxin), the other is Shaoyin syndrome.
