When pathogenic factors of Shanghan (Cold Damage) invade the Yin meridians, the disease is located in the interior. The treatment method should be purgation. For any cases presenting with abdominal fullness and constipation, or dry mouth and tongue with thirst, or tidal fever and delirium.
Purgative Method
Heat Syndrome
If... syndrome
Da Chai Hu Tang (Major Bupleurum Decoction) is used for purgation. If the pulse is deep and short, with tongue retraction and testicular contraction, this indicates that toxic vapors have entered the zang organs; Cheng Qi Tang (Ordering the Qi Decoction) should be used for purgation. If there are cold extremities, deafness, and testicular contraction, with a deep and wiry pulse, this indicates simultaneous involvement of the Shaoyang (Lesser Yang) and Jueyin (Terminal Yin) meridians. Furthermore, if Jueyin disease presents with diarrhea and delirium, but the pulse is not fine and minute, Xiao Cheng Qi Tang (Minor Ordering the Qi Decoction) is appropriate.
Tiao Wei Cheng Qi Tang (Regulate the Stomach Ordering the Qi Decoction) (from *Shengji Zonglu*) treats Yangming disease. In cases where neither vomiting nor purgation has been applied, and the patient suffers from mental restlessness, Tiao Wei Cheng Qi Tang may be administered.
In Taiyang (Greater Yang) disease persisting for three days, if sweating fails to resolve the condition and there is steaming heat, Tiao Wei Cheng Qi Tang may be given. After vomiting in Shanghan, if abdominal fullness occurs, this formula can be used. For cases treated with sweating and harmonizing methods, where chills are absent but fever persists with a steaming sensation, or after prolonged illness, with a warm sensation in the epigastrium suggesting nausea, chest pain, loose stools, abdominal fullness, and slight depression/irritability, these result from extreme vomiting and purging. Or after prolonged illness, delirium during sleep requires purgation. However, using highly toxic pills containing Silver Powder (Fen Shui), Croton Seed (Ba Dou), etc., which are drying and hot, damages True Yin and exacerbates pathogenic heat, leading to unceasing diarrhea with accompanying heat. Also, if exterior heat is present and purgation is applied too early, pathogens enter due to deficiency without forming Jie Xiong (Chest Binding), resulting only in unceasing heat diarrhea, epigastric fullness and urgency, or pain, irritability, thirst, dry throat, and a slippery, rapid, and excess pulse. For all cases of abdominal fullness and solid pain, irritability, thirst, delirium, red urine, hard stool, and [pulse]...
