[Supplementary Discussion]
After the recovery from Shiqi disease (exogenous warm-heat disease), sudden redness, swelling, and pain of the eyes occur, or nebulae (membranous opacities) may form. This is due to the residual heat of the Heart and Lung organs attacking upward into the eyes. The eyes receive the essence of the Five Zang organs and are not exclusively governed by the Liver. The heat of the Heart and Lung can also invade them. Therefore, after Shiqi disease, if the residual toxins in the Zang-Fu organs have not been fully cleared, they fumigate and manifest in the eyes. Or, because the body is weak and has not yet recovered, one should mostly rely on rest and recuperation to return to normal. Otherwise, using decoctions for aggressive purging will be of no benefit.
The *Longmu Lun* states: Ophthalmic sequelae after Shanghan (cold damage) and Rebing (febrile diseases). At the initial onset, this may be caused by rising early with Shanghan, or after Rebing, when the Qi of the Zang organs is not yet fully restored, and the residual heat in the Six Fu organs has not dissipated. The constitution is weak and easily damaged; consuming too much food that causes stagnation, contains toxins, or is hot-natured leads to eye diseases. Symptoms may include seeing black spots floating in vision, dilation of the pupil, fluctuating vision, redness, swelling, and tearing. It is appropriate to use a lancet to drain stasis blood and administer Longdan Wan (Gentian Pill), Shengxi Yinzi (Raw Rhinoceros Horn Decoction), and Xiegan Tang (Drain Liver Decoction). One must strictly avoid using eye drops, as it may damage the eyes.
The verse says: Febrile diseases and Shanghan often leave behind deficiency after recovery; due to dietary stagnation and heat affecting both eyes. Eye pain begins with dimness within one day, and soon spreads to involve both eyes. Redness and swelling require lancing the inner eyelid; Shengxi Yinzi is most effective in dispelling the pathogen. Taking half a dose of Jueming Wan at this time prevents white nebulae from becoming more obstructed. Why is it unsuitable to use eye drops? Fear of generating sores and damaging the anterior structures, taking years to resolve.
It also states: External obstruction red eye following Tianxing (epidemic) disease. When the eyes first suffer, there is sudden redness, swelling, pain, and tearing. If a patient is affected, the severity varies, usually starting in one eye and then spreading to affect both. It is crucial to lance and wash away stasis blood, followed by administering Xiegan Tang, washing the eyes with Xiyan Tang, and applying Naojian Jian (Borneol Ointment) for cure.
The verse says: Sudden redness, swelling, and pain together define Tianxing Chiyan (Epidemic Red Eye). Epidemic heat qi transmits through contact; constitution determines severity. Xiegan Yinzi should be taken; lancing blood in the eyelids restores clarity. Washing with Qinpi Tang and swallowing pills is advised; if not cured, chronic cases may see recurring nebulae. Avoid toxic foods and manage with care; hook-cauterization is contraindicated to prevent injury to the eyeball. Forcing analgesic drops damages the Shen Guang (spiritual light of vision), which is alarming. What is most hidden in treatment? Moderating with cooling herbs brings balance.
