The condition of Xiao Ke (Wasting and Thirsting Syndrome) characterized by excessive drinking of water arises from deficiency of the Kidney and heat in the Heart, disharmony of the San Jiao (Triple Burner), resulting in heat above and cold below. This typically occurs in individuals who have a preference for consuming hot wine and roasted meats, or who take drugs such as Ru Shi (Milk Stone) that cause stagnation. Heat toxins accumulate internally without being able to disperse and circulate freely. The Guan pulse becomes obstructed, blood vessels fail to flow, and heat steams the Zang-Fu organs, leading to exhaustion of body fluids. Consequently, there is vexation and heat in the Heart and Lungs, dryness of the throat, causing incessant thirst and subsequent excessive water intake.

Eight-Ingredient Kidney Qi Pill (Bawei Shenqi Wan): There are three types of Xiao Ke. First, thirst with excessive drinking and frequent urination containing oily, bran-like flakes that taste sweet; this is Xiao Ke disease. Second... [text missing, typically refers to Middle Xiao Ke]. Third... [text missing, typically refers to Lower Xiao Ke]... leg swelling and thinning of feet... Three. One drinks alcohol... if not cured, worry about developing large abscesses. Those suffering from thirst for years often die from carbuncles and sores. Many do not know this disease, so ancient and modern texts omit it. ...Soon all become sweet, proving that after eating, flavors transform into marrow, then fat, then flesh and blood. ...The qi of the five zang organs, when moist, flows downward. If the Kidney is deficient and cold... sweetness does not change, color is clear and cold, leading to withered skin. Like a wet nurse's grain qi leaking upward... urine lacks essence internally, leading to frequent urination and emaciation. The Lung is the canopy of the five zang organs. If the lower part is extremely cold, yang qi cannot ascend, causing lung dryness and thirst. In the I Ching, Pi Hexagram has Heaven above and Earth below... failing to ascend, upper and lower do not interact, forming Pi. Like a pot of water heated by fire; the lid becomes moist. Without heat, vapor cannot reach, and the lid stays dry. Sufficient heat... requires warming and tonifying Kidney Qi. Food receiving heat nourishes the upper and aids digestion, preventing dryness. Thus Zhang Zhongjing said, suitable to take Shenqi Wan. ...This disease and Jiaoqi (Foot Weakness) both stem from Kidney deficiency, but Jiaoqi starts in months 2-3, peaks in 5-6, declines in 7-8; Xiao Ke starts in 7-8, peaks in 11-12, declines in 2-3. Why? Jiaoqi is an obstructive disease, Xiao Ke is a dispersing disease. In spring/summer, yang ascends, so obstructive diseases flare while dispersing ones heal; in autumn/winter, yang descends, so dispersing diseases flare while obstructive ones heal. Understanding these two... is the bladder's... regarding Xiao Ke.
