The pathogenic heat qi resides within the zang-fu organs, steaming the Liver (Gan, liver), and attacking the eyes. Once the heat toxin is vigorous, it merges into the blood vessels, accumulating and knotting without dispersing, condensing to form eye polyps (fleshy growths on the eyelids or bulbar conjunctiva).
Eye polyps arise from dysfunction of the Spleen (Pi, spleen) and Liver, allowing wind-heat pathogens to invade. Subsequently, there may be itching or pain, redness, swelling, ecchymosis, and ulceration. If stagnant heat persists for a long time, the polyps grow larger; if they approach the pupil, they obstruct vision.
The *Longmu Lun* states: "Polyp invading the cornea, an external obstruction." At the initial onset of this eye disease, there is either itching or pain, followed by chronic redness and ulceration for many years. Wind pathogen stagnation in the Lung (Fei, lung) causes attacks without fixed timing.
The verse says: "The roots of polyps are of two types; one must distinguish their origins. Some arise after years of redness and ulceration, caused by wind rushing from the Lung leading to redness. Others involve itching or pain without location, where the eyelid flesh and gelatinous membranes invade the external obstruction." This occurs at the initial onset, due to latent heat in the chest and Stomach (Wei, stomach), with wind from the diaphragm rushing into the eyes, causing flesh to grow on the eyelids. Initially, it is small like a hemp seed.
The verse says: "Eyelid skin and flesh are smooth with gelatinous membranes, gradually growing large like plums or peaches. Puncture the interior with a needle, discharge pus, then regrow; take medicine again to cut off the root cause, perhaps resulting from this affliction."
