The pulse at the cunkou position on the right hand, specifically the yin aspect prior to the qi kou, is solid (Shi), indicating involvement of the Hand Taiyin Lung Meridian. When the Lung is excess, heat is generated; when heat is abundant, Yang Qi becomes hyperactive. The patient suffers from lung distension, sweating profusely as if covered in dew, upward reversal of Qi with panting, a sensation of obstruction in the throat resembling the urge to vomit, fullness and distension in the chest that radiates to the shoulders. This condition is termed Lung Excess-Heat.
Bian Que stated: Lung Excess-Heat causes panting and reversal of Qi, chest oppression requiring仰息 (breathing with the body leaning back), and difficulty in the throat. This occurs because heat Qi invades the Hand Taiyin Meridian, resulting in Lung Excess disease. In severe cases, the lips turn red, the nostrils flare, there is excessive drinking without relief, sores appear on the back, and pain extends to the inner thighs, knees, and shins.
When the pulse at the qi kou position on the right hand shows both Yin and Yang aspects as solid, it indicates that both the Hand Taiyin Lung Meridian and the Yang meridians are in an excess state. Symptoms include headache and dizziness,惊狂 (fright and mania), and painful obstruction of the throat (Hou Bi).
