The so-called "all types of dysentery" refers to the fact that dysentery presents without a fixed pattern. Sometimes watery stools and undigested food are discharged, sometimes pus, sometimes blood; the color may be green, yellow, red, or white, changing unpredictably and mixed irregularly.
Or various colors mix together in the discharge. If complicated by heat, it appears yellow or red; if heat is extreme, it turns into pus and blood. If complicated by cold, it appears white; if cold is extreme, it appears green or black. All these changes are due to unregulated diet, imbalance of cold and heat, leading to weakness of Stomach Qi, thereby causing changes in stool color.
The key to treating dysentery lies in: Dysentery originates from accumulation and stagnation. "Accumulation" refers to the stagnation of tangible matter, while "stagnation" refers to the obstruction of Qi mechanism. The tangible matter desires to be expelled, but Qi stagnation hinders its exit, resulting in obvious tenesmus (abdominal urgency), intermittent defecation, occurring more than a hundred times a day and night. Patients urgently seek to stop diarrhea, unaware that this symptom is precisely caused by material accumulation and Qi stagnation. People only see the aforementioned symptoms, regardless of whether the stool is red or white or the pulse is large or small, and uniformly use purgative or draining medications. For material accumulation, they use substances like Radix Crotonis Seed (Badou) and Radix et Rhizoma Rhei (Dahuang); for Qi stagnation, they use Fructus Aurantii (Zhiqiao), Flos Platycodonis (Jiegeng), Pericarpium Citri Reticulatae (Qingpi), and Rhizoma Curcumae (Pengzhu). Addressing both simultaneously will surely achieve success. In between, supplementing with Huanglian Ejiao Wan yields particularly remarkable effects. This is because dysentery often arises when latent summer-heat dampness lingers in the body, compounded by consuming hot wine, wheat noodles, and roasted foods. Ejiao (Donkey-hide Gelatin) is a crucial herb for the Large Intestine; if toxic heat lingers, it can guide it out; if no toxic heat lingers, it can calm the Stomach. If pills are unavailable in the market, one can prepare a decoction using Ejiao, Radix Angelicae Sinensis (Danggui), Pericarpium Citri Reticulatae (Qingpi), Poria Cocos (Chifuling), and Radix Scutellariae (Huanglian), adding Semen Ziziphus Spinosae (Wumei) and thick honey to boil together. This best sweeps away filth. Once the accumulation is removed, the frequency of defecation naturally decreases. Thereafter, regulate with Radix Aucklandiae (Muxiang), Poria Cocos (Fuling), Fructus Amomi Rotundus (Suosha), Semen Alpiniae Katsumadai (Baidoukou), Pericarpium Citri Reticulatae (Chenpi), and Radix Glycyrrhizae (Gancao), and appetite will naturally return.
Prescriptions
Zengsun Jianpi Wan (from *Qianjin Fang*) Treats men with deficiency-wasting, damage to the five Zang and six Fu organs, invasion of cold pathogen, initially presenting as Li Xia (dysentery), which later transforms into five-color dysentery. Stools resemble rotten intestines, dark red/black, with extremely foul odor. The body is emaciated and heavy, the Stomach is cold, food is not digested, and diarrhea is incessant.
Zhongru Fen (Calcitrum, 3 liang) Chishizhi (Halloysitum Rubrum, 3 liang) Fanshi (Alumen) Ganjiang (Semen Zingiberis Recensum) Rongrong (Cibotii Rhizoma) Guixin (Ramulus Cinnamomi) Wuweizi (Fructus Schisandrae) Zexie (Rhizoma Alismatis) Yuanzhi (Radix Polygalae) Jisheng (Taxilli Herba)
Baiziren (Semen Platycladi) Baitouweng (Radix Pulsatillae) Renshen (Radix Ginseng) Tianxiong (Aconiti Lateralis Radix Preparata) Danggui (Radix Angelicae Sinensis) Shihu (Dendrobii Caulis) Shiliupi (Pericarpium Granati) Muli (Concha Ostreae) Longgu (Os Draconis) Gancao (Radix Glycyrrhizae, 2 liang each) Grind all into fine powder, make pills with honey, take 20 pills with wine, gradually increasing to 30-40 pills. This prescription is miraculous for stopping dysentery.
Shuizhu Muxiang Wan (from *Yuyuan Fang*) Treats Spleen and Stomach affected by dampness, sliding diarrhea of the Fu organs, abdominal pain, day and night without limit, borborygmus with water sounds. Whenever defecation is desired, there is tenesmus and heaviness in the rectum, or passage of yellow-red stool, or pus and blood. It is suitable for all such cases.
Muxiang (Aucklandiae Radix, remove whiskers) Dingxiang (Flos Caryophylli) Hezi Pi (Fructus Chebulae Pericarpium) Danggui (Radix Angelicae Sinensis) Huoxiang Ye (Agastachis Herba) Huanglian (Radix Scutellariae, processed with Ginger Juice, 1 liang each) Houpo (Cortex Magnoliae Officinalis) Qingpi (Pericarpium Citri Reticulatae Viride, remove white pith) Baishao Yao (Radix Paeoniae Alba) Chenpi (Pericarpium Citri Reticulatae, remove white pith) Gancao (Radix Glycyrrhizae, honey-fried) Zhishi (Fructus Aurantii Immaturus, bran-fried) Ganjiang (Semen Zingiberis Recensum, processed, 0.5 liang each) Ruxiang (Olibanum) Roudoukou (Nucis Arecae Condita) Sharen (Fructus Amomi Rotundus, 1.5 liang each) Yumiqiao (Pericarpium Papaveris, honey-water stir-fried until yellow, 6 liang) Grind all into fine powder, make pills with refined honey, size of a bullet. Take one pill per dose, decoct in one large cup of water until reduced to 70%, take slightly warm with dregs before meals.
Zhi Li Xian Fang (Immortal Prescription for Dysentery) Verse says: Eight types of dysentery at death's door are critical, give equal parts of Immortal Prescription to treat you. Three fragrant herbs relieve pain excellently, four white herbs harmonize the middle, do not neglect them.
Dingxiang (Flos Caryophylli) Chenxiang (Lignum Aquilariae Resinatum) Muxiang (Aucklandiae Radix) Baizhu (Rhizoma Atractylodis Macrocephalae) Baizhi (Radix Angelicae Dahuricae) Baifuling (Poria Cocos, white) Baishaoyao (Radix Paeoniae Alba) Gancao (Radix Glycyrrhizae) Zhike (Fructus Aurantii) Hezi (Fructus Chebulae) Diyu (Radix Sanguisorbae) Suan Shiliupi (Pericarpium Punicae Granati) Banxia (Rhizoma Pinelliae) Houpo (Cortex Magnoliae Officinalis) Huanglian (Radix Scutellariae) Chenpi (Pericarpium Citri Reticulatae) Yingsuke (Pericarpium Papaveris, honey-fried) Each 1 qian, divided into five doses. For white dysentery, decoct with fresh ginger; for red dysentery, decoct with Semen Ziziphus Spinosae.
For all dysentery patients, whether new or old, if medication provides temporary relief followed by recurrence, it means that even if new drugs stop the symptoms, the root cause remains. This is due to the imbalance of cold and heat in the intestines and stomach, with the disease root firmly knotted. It is necessary to use decoctions to cleanse and drain the pathological momentum. After dysentery stops, supplement with medication. For those with this condition, follow the posterior Fuzi Liuwei Tang to promote drainage. Afterwards, take Gaoliangjiang Shiwei San to tonify.
Fuzi (Aconiti Lateralis Radix Preparata, processed) Xixin (Herba Asari) Gancao (Radix Glycyrrhizae, honey-fried) Renshen (Radix Ginseng, 2 liang each) Ganjiang (Semen Zingiberis Recensum, 3 liang) Dahuang (Radix et Rhizoma Rhei, 5 liang) Cut into pieces, decoct in seven sheng of water until two sheng four he remain, strain, divide into three warm doses. Take one dose after walking for about ten li. This formula should induce smooth bowel movement; the foul matter discharged in dysentery may resemble fish brain or peach/plum pits. Do not be surprised if it differs from usual. Rest for three to four days, then combine with the posterior Gaoliangjiang Shiwei San for tonification.
Gaoliangjiang (Rhizoma Alpiniae Officinarum) Xixin (Herba Asari) Huangqi (Radix Astragali) Baizhu (Rhizoma Atractylodis Macrocephalae) Kushen (Radix Sophorae Flavescentis, 5 liang each) Dingxiang (Flos Caryophylli, 2 liang) Renshen (Radix Ginseng) Ganjiang (Semen Zingiberis Recensum, 4 liang each) Doukou Zi (Fructus Cardamomi, 2-3...) Zhike (Fructus Aurantii, remove core) Wumei (Fructus Mume, 1 liang each) Dingpi (Cortex Caryophylli) Muxiang (Aucklandiae Radix, specifically for pain relief, 5 qian each) Chenpi (Pericarpium Citri Reticulatae, 1 liang, use red part to generate blood, omit if red dysentery)
Make into pills. For five-color dysentery, take with decoction of spring tea and Chenpi. If in powder form, take with one spoonful of honey, spring tea, and Wumei decoction.
Jiajian Muxiang Zhusan (from *Wei Shi Fang*) Treats all types of dysentery with miraculous effect.
Muxiang (Aucklandiae Radix) Gancao (Radix Glycyrrhizae) Danggui (Radix Angelicae Sinensis) Roudoukou (Nucis Arecae Condita) Renshen (Radix Ginseng) Guanzi Gui (Ramulus Cinnamomi Cassiae) Shaoyao (Radix Paeoniae) Hezi (Fructus Chebulae) Wumei (Fructus Mume, remove pit) Ejiao (Donkey-hide Gelatin, fried with clam powder) Baifuling (Poria Cocos, white, 5 qian each) Yingsuke (Pericarpium Papaveris, 1.5 liang, remove calyx, honey-fried) Cut into coarse pieces. Take 4 qian per dose, with 1.5 cups of water, 3 slices of fresh ginger, 2 red dates, decoct together, strain, take on an empty stomach.
Da Duaxia Wan (Great Stop-Down Pill) Treats sliding dysentery with loss of control, emaciation, inability to eat, thin pulse, cold skin, shortness of breath, inability to speak, occasional spontaneous sweating. This is due to consumption of Spleen and Stomach deficiency. Consumption leads to Qi depletion. Since grain Qi does not enter the Stomach, the Stomach lacks master Qi to nourish, causing physical and Qi exhaustion. The fluids of the five Zang are not retained, called "Five Deficiencies." This is difficult to treat; survival is possible if the patient can eat. Also treats cold retention in Fu organs, umbilical and abdominal pain.
Fuzi (Aconiti Lateralis Radix Preparata, processed) Roudoukou (Nucis Arecae Condita, bread-baked) Muli (Concha Ostreae, weighed, 1 liang each) Xixin (Herba Asari, remove soil, 7.5 qian) Ganjiang (Semen Zingiberis Recensum, processed) Bailonggu (Os Draconis, ground) Chishizhi (Halloysitum Rubrum, ground) Suan Shiliupi (Pericarpium Punicae Granati, boiled in vinegar until dry, baked, 1.5 liang each) Gaoliangjiang (Rhizoma Alpiniae Officinarum, remove rhizome, 1.5 liang) Baifan (Alumen, calcined) Hezi (Fructus Chebulae, remove pit, 1 liang each) Grind into powder, make pills with vinegar paste, size of paulownia seeds. Take 30 pills per dose, down with millet soup. Another method uses plain water; if Baifan and Hezi are unavailable, it is also acceptable.
Jiajian Pingwei San (Modified Pingwei Powder)
Baizhu (Rhizoma Atractylodis Macrocephalae) Houpo (Cortex Magnoliae Officinalis) Chenpi (Pericarpium Citri Reticulatae, 1 liang each) Gancao (Radix Glycyrrhizae, 7 qian) Binglang (Semen Arecae) Muxiang (Aucklandiae Radix, 1.5 liang each) Taoren (Semen Persicae) Huanglian (Radix Scutellariae) Renshen (Radix Ginseng)
Ejiao (Donkey-hide Gelatin) Baifuling (Poria Cocos, white, 0.5 liang each) Grind into fine powder, decoct together with Pingwei San. Add more Taoren if bleeding increases; add Huanglian if heat diarrhea occurs; add Fuling if urination is obstructed; add Binglang if Qi does not descend and tenesmus persists; add Ejiao if pus is present; add Shaoyao and Gancao if abdominal pain occurs; add more Baizhu if dampness increases; add Dahuang if pulse is surging. The Stomach Qi is the foundation in all seasons. Prolonged bloody diarrhea damages Spleen and Stomach, blood fails to flow to limbs, enters the Stomach causing bloody dysentery.
