当
Yin qi alone becomes excessive, while Yang qi suddenly declines. Yang is overcome by Yin, resulting in a state of Yin cold both internally and externally, thus forming Yin Toxin Cold Disease (Yindu Shanghan). Some patients present as Yin Toxin immediately upon contracting the disease; others develop Yin Toxin after taking herbal decoctions for five or six days without improvement. This occurs because the disease fundamentally belongs to the Yin category, and excessive intake of cold-natured medicines, or invasion of cold pathogens into the internal organs and external body, causes Yin qi to become even more dominant, preventing the recovery of Yang qi. The symptoms include cold extremities (cold reversal), severe pain in the umbilical and abdominal regions, a sensation as if the body has been beaten, vomiting accompanied by diarrhea, and a pulse that is deep, fine, and rapid. This characterizes Yin Toxin. If Yin Toxin has penetrated deeply and the condition is critical, the six pulses may only be palpable when pressed deeply against the bone, disappearing with light pressure, beating more than eight times per respiration, or even being uncountable. At this stage, methods such as moxibustion with scallions (Cong Yi Fa) and burning moxa (Zhuo Ai Fa) are appropriate.
In the early stages of Cold Disease (Shanghan), within one or two days, Yin Toxin may form; or after taking medicine for six or seven days up to ten days, it transforms into Yin Toxin. Symptoms include heaviness of the body, stiffness of the back, colicky abdominal pain, difficulty in the throat, toxic factors attacking the heart, hardness below the epigastrium, shortness of breath, restlessness, thirst, spontaneous sweating, occasional delirious speech (Zheng Sheng), vomiting with diarrhea, a sensation of being beaten, cyanosis of the lips, blackening of the face, and cold extremities. The pulse is deep, fine, tight, and rapid. Zhang Zhongjing stated: "These are signs of Yin Toxin. It can be treated within five days; by the sixth or seventh day, it is incurable." For Yin Toxin, Gancao Tang (Licorice Decoction) is the primary prescription.
Regarding the onset of Yin Toxin: Yin Toxin originates from deficiency-cold of Kidney Qi, often triggered by sexual activity or catching wind after consuming cold foods. Internally, Yin cold is latent; externally, cold pathogens are contracted. Alternatively, external cold is contracted first, followed by internal latent Yin. With Yin prevailing both inside and out, Yang Qi fails to remain secure, leading to headache, heavy and painful waist, eye pain, bodily fatigue with little fever, cold extremities, continuous cold sweat on the forehead and dorsum of hands, frequent thirst, mental confusion, and a sense of loss. Within two or three days, the patient might be able to walk, feeling not overly burdened. Palpation reveals all six pulses as deep, fine, and rapid; the Chi position is short; the Cun position may be large; the Guan position may be floating-large or deep. If the pulse is forceful when taken but not excessively rapid, it is not a Yin syndrome. If too many cooling medicines are taken, thirst and dryness intensify. In such cases, immediate administration of Yang-restoring and Yin-retreating medicines will bring relief; the treatment focuses on tonifying deficiency and harmonizing Qi. Suitable prescriptions include Zhengyuan San, Tuiyin San, and Wusheng San. Yin syndromes generally should not induce sweating. However, if the healthy Qi is intact and the pulse is large, with fever persisting but the illness unresolved, inducing sweat through medication is permissible.
Yin Toxin manifests as cold hands and feet, stiff waist and back, pounding abdominal pain, sore throat, vomiting and diarrhea, a sensation of being beaten, and a fine pulse nearly imperceptible. This results from deficiency-cold of Kidney Qi, external contraction of wind-cold, or spleen injury due to cold foods. Since the Spleen belongs to Taiyin, and with Yin prevailing internally and externally, Yang Qi cannot be maintained, leading to Yin Toxin. Symptoms include cold extremities, continuous cold sweat on the forehead and hands, frequent thirst, and mental confusion. As Qibo stated: "When Yin prevails, the body feels cold; sweating leads to a constant feeling of chill."
Furthermore, regarding the gradual deepening of Yin Toxin: Accumulated Yin cold at the lower part causes slight Yang to dissipate at the upper part. Thus, symptoms are heavy, with cold extremities, worsening abdominal pain, throat obstruction, or fullness and hardness below the heart, restlessness, thirst, continuous cold sweat, occasional manic speech, and cyanosis of nails and face. The six pulses are deep and fine, beating seven times or more per respiration. For such cases, moxibustion of 300-400 moxa cones should be applied rapidly to the Qihai (CV6) or Guanyuan (CV4) acupoints, until the extremities become warm. Concurrently administer Jin Ye Dan, Lai Su Dan, Yu Nu San, Huan Yang San, and Tuiyin San.
Regarding the sinking and prostration of Yin Toxin: These symptoms are similar to those of gradual deepening but more severe. The six pulses are felt only when pressed deeply against the bone, vanishing with light touch, beating more than eight times per respiration, or uncountable. At this stage, medicinal herbs are difficult to effect. Only moxibustion using cones half the size of a jujube, totaling over 300 cones, should be used. If the extremities do not become warm, the case is untreatable. If warmth returns temporarily, continue assisting with sulfur and hot medicines. If Yin dissipates and Yang returns...
Poem:
Yin Toxin Cold Disease brings body heaviness,
Back stiffness and eye pain unbearable.
Lower abdomen urgent pain, mouth cyanotic and black,
Toxic vapors rush to the heart, uncontrollable.
Extremities cold, only desiring vomit,
Throat obstructed, pulse deep and fine.
If moxibustion is quickly applied below the navel,
By the sixth day, joy may be seen.
Yin Toxin feels like being struck,
Illness deep, sweating, thirst...
When Yin Qi alone is excessive and Yang Qi suddenly ceases, it is called Yin Toxin. The body is cold with sweat, and the pulse is deep, fine, and rapid. If the pulse is deep but not rapid, it did not form suddenly at the onset. Some cases transform after several days of medication, likely due to extreme damage to Kidney Qi, spleen injury from raw/cold foods, internal latent Yin, and external cold contraction. Conversely, if Yang Qi alone prevails and Yin Qi suddenly ceases, it is called Yang Toxin. The body is hot without sweat, and the pulse is wiry, flooding, hurried, and rapid. Some form suddenly at onset, likely due to excessive alcohol and wheat, excessive cinnabar causing dryness, and extreme heat in the intestines and stomach. Or, a Yang-type illness mistakenly treated with warm medicines, exacerbating heat into pathogenic factors. With Yang prevailing internally and externally, Yin Qi cannot be maintained. Both Yin and Yang toxins present with headache and mild diarrhea. Suppressing Yin requires hot medicines; suppressing Yang requires cold medicines, following established principles. However, these two toxins ascend with rebellious Qi, lodge in the chest, causing severe pain and hardness below the heart, which standard methods cannot resolve. Urgent measures must be taken to drain them. Draining Yin Toxin restores Yang; draining Yang Toxin restores Yin. Through the ascending and descending of Yin and Yang, and the circulation of Ying and Wei, resolution occurs naturally through profuse sweating. If hardness below the heart persists for six or seven days, survival is impossible. External symptom treatments are detailed in subsequent chapters.
External signs of Yin Toxin: Cyanosis of eyes, face, lips, and nails; open mouth with shortness of breath; throat obstruction; cold extremities; little fever; pain heavy as if beaten; chills; headache and eye pain; heavy waist and stiff back; toxic vapors attacking the heart; hardness and oppression below the heart; colicky abdominal pain; cold hardness of external genitalia and below the navel; continuous cold sweat on forehead and hands; vomiting and diarrhea; thirst; dryness due to extreme Yin; mental confusion; coherent yet abnormal speech; solemn voice; black tongue coating. To restore Yang in Yin diseases, if medication penetration is insufficient, fire therapy must be borrowed, but not forced. Ginger and Galangal both disperse cold pathogens and promote Yang Qi; they are essential drugs. For Yin Toxin with deep, fine pulse, cold body, and sweat, these can be used. Prescriptions for Yin Toxin include Gancao Tang, Baizhu Fuzi San, Zhengyang San, Rougui San, Huiyang Dan, Fanyin Dan, Tianxiong San, Zhengyuan San, Tuiyin San, and Jin Ye Dan, selected as appropriate. This restores Yang and resolves through sweating. A physician proficient in this will achieve cure. Master Chen Beishan's formula verse: Treats Yin Toxin with knotting below the heart, extreme pain on pressure, constipation of urine and stool, unresponsive to daily medication. If exhaled air is slightly warm, it can still be treated. Take ten kernels of Croton seed (Badou), grind into paste, mix with one qian of flour, form a cake, place firmly on the center of the navel. Apply small moxa cones, moxibustion 5-7 cones. When abdominal borborygmus is heard, bowel movement will occur after some time. Next, take a bundle of green onion whites, tie tightly, slice into cakes, heat until warm, apply below the navel. Use an iron to heat above it, replacing as needed. Gradually feel body temperature rise. Then take Wuji San (Five Accumulations Powder) 2 qian, Fuzi powder 1 qian, 7 slices of ginger, 2 jujubes, a little salt, decoct in one large cup of water until 0.7 remains. Take warm. Continue for 2-3 doses until sweat appears and recovery occurs. Another method: Crush one garlic clove, form a cake, heat, place on navel, moxibustion 10 cones. Urine and stool will pass. For deep Yin Toxin, moxibustion Qihai and Guanyuan until extremities are warm, assisted by previous medicines. If six pulses are deep against the bone and condition is critical, use the "moxa first, then scallion" method at the navel. Formula details are previously described. In treating Yin Toxin, when Yang Qi briefly returns, irritability often arises; do not mistakenly administer cooling medicines. If dryness is severe, use Fanyin Dan and similar formulas.
Yin Syndromes: Taiyin, Shaoyin, and Jueyin all belong to Yin syndromes. Taiyin is the Spleen; Shaoyin is the Kidney; Jueyin is the Liver. What is the Taiyin Syndrome? The Taiyin Spleen meridian governs chest and diaphragmatic distension. The *Jia Yi Jing* states: Pathogens arising from Yang come from wind, rain, cold, and heat; pathogens striking Yin come from diet, dwelling, and emotional fluctuations. Also: Treacherous winds and empty pathogens affect Yang; irregular diet and improper living habits affect Yin. Affected Yang enters the Fu organs; affected Yin enters the Zang organs. Entering the Six Fu organs causes intermittent fever and upper respiratory distress; entering the Five Zang organs causes bloating and obstruction, lower diarrhea, and long-term intestinal dysentery. What is the Shaoyin Syndrome? The Shaoyin Kidney meridian governs a faint and fine pulse, vexation but desire to sleep, or spontaneous diarrhea with thirst. Question: Why does the classic mention Shaoyin disease on the first or second day? It means that during the epidemic, the pores were closed by cold, allowing entry directly into the Yin meridians, bypassing the Three Yang meridians. Although Cold Disease involves Three Yin and Three Yang, it generally starts in Yang (Taiyang) or Yin (Shaoyin). These two meridians are exterior-interior pairs and are most commonly affected. Yangming and Taiyin are less common; Shaoyang and Jueyin (Liver/Gallbladder) are even rarer. For illnesses lasting one to twelve or thirteen days, if Taiyang symptoms persist, treat Taiyang. If Shaoyin symptoms appear immediately, attack Shaoyin directly, without waiting for transmission from Taiyang. Cold pathogens entering Taiyang cause fever and aversion to cold; entering Shaoyin cause only aversion to cold without fever. For mild cold in the Three Yin meridians, use Lizhong Tang; for slight cold reversal or cold-induced diarrhea, use Ganjiang Tang; for slight coldness in finger tips, termed "Qing," do not use Sini Tang yet, as the disease is mild. Use Lizhong or Ganjiang types. For severe cases, use Sini Tang. For absent pulse, use Tongmai Sini Tang. What is the Jueyin Syndrome? The Jueyin Liver meridian governs thirst, rising Qi, heat and pain in the heart, hunger without desire to eat, vomiting roundworms upon eating, and intractable diarrhea if purged. If Yin Qi alone is excessive and Yang Qi suddenly ceases, it is Yin Toxin. Symptoms include cold extremities, pounding abdominal pain, sensation of being beaten, deep and rapid pulse, vomiting or diarrhea. Urgently moxibustion below the navel and administer acrid-hot medicines to restore Yang and resolve through sweating. Ancients said: Acrid and sweet disperse and promote Yang, referring to Guizhi, Gancao, Ganjiang, Fuzi, etc., which can restore Yang Qi. Mildly use acrid-sweet; severely use acrid-bitter. Extreme Yin causes restlessness; Yin syndrome mimics Yang. Students must differentiate by pulse.
