Hongjiao Bi Luan (Gouty Arthritis with Numbness and Cramping) is caused by the concurrent invasion of Wind, Cold, and Dampness pathogens into the muscle intervals. When the True Qi (Zhen Qi) fails to circulate throughout the body, Bi Syndrome (Obstruction Syndrome) forms. Among these, if Wind pathogen predominates, it manifests as Xing Bi (Wandering Bi), characterized by pain that wanders without a fixed location; if Cold pathogen predominates, it manifests as Tong Bi (Painful Bi), characterized by severe pain; if Dampness pathogen predominates, it manifests as Zhuo Bi (Fixed Bi), characterized by heaviness and numbness of the limbs, coldness, anhidrosis, and moist skin. If the Bi syndrome moves only up and down the body but not left and right, it is called Zhou Bi (Circulating Bi); if the pathology lies within the muscles, manifesting alternately above and below, with lesions on the left corresponding to the right and vice versa, it is called Pian Bi (Hemilateral Bi).
If Yin Qi is excessive and Yang Qi is deficient in the Bi syndrome, it leads to aversion to cold in the body; if Yang Qi is excessive and Yin Qi is deficient, the Bi syndrome manifests as heat. In various Bi syndromes, if Wind pathogen predominates, it is easy to cure; if the pathogen is in the skin, it is also easy to cure; however, if the pathogen penetrates deeply into the sinews and bones, causing cramping and pain, it is difficult to cure. Prolonged Bi syndrome penetrating deep causes stagnation of Ying Wei (Nutritive and Defensive Qi) and obstruction of the meridians, leading to insensitivity to pain and itch. If the Wind pathogen cannot be eliminated, the feet feel as if immersed in ice, accompanied by vexatious pain in the abdomen, thighs, and calves, or vomiting, mental restlessness, a sensation of emptiness in the heart, frequent sweating, dizziness, sadness, fear, shortness of breath, and low mood. These are its symptomatic manifestations.
Furthermore, Wind-Dampness pathogens adhering to the four limbs cause them to be unable to hold or move freely; if the pathogens enter the Zang organs, they lead to aphonia, flaccidity of the four limbs, and loss of voluntary control over the mouth and tongue. This disease often starts from the feet, likely because the local terrain is low and damp, with little frost or snow in winter. The vapor rising from mountains and rivers keeps the ground warm, causing the pores (Cou Li) to open, allowing Wind-Dampness to invade. In spring, the area often has drizzle or sudden cold snaps, causing Wind-Dampness to bind deeply and fail to dissipate, resulting in weak legs. There are also cases of sudden chills followed by jaundice, which is due to the internal binding of Wind-Dampness. Or, one may first encounter Heat pathogen, then Cold pathogen, where Cold binds with Heat, Heat enters the Zang organs, and Cold remains in the exterior. This condition tends to cause dry heaving and vomiting. Winter warmth with little frost or snow is called "Shi Xing Shi Qi" (Seasonal Dampness Epidemic).
Wind Du (Toxicity) hides in the muscles. In spring and summer, sudden cold and dampness weaken it, causing flaccidity and weakness of the four limbs. In individuals with robust constitutions and excess heat, high fever and limb paralysis occur, with a pulse that should be Floating, Large, and Tight. In individuals with deficiency and cold, the pulse should be Slow, Weak, and Thready as the manifestation.
When encountering Du (Toxicity), one should first attack the toxicity. After the toxicity subsides, treat the deficiency. If tonifying deficiency while toxicity is present, the toxicity will become more severe. The key is to prevent deficiency in advance; do not treat it like a person with Excess and Fullness.
