Putrid odor, tongue retraction, and testicular retraction (indication of Liver Qi exhaustion) ○ Inability to close the mouth (indication of Spleen Qi exhaustion) ○ Flaccid muscles and everted lips (indication of Stomach Qi exhaustion) ○ Straight hair, withered teeth, and enuresis (indication of Kidney Qi exhaustion) ○
Dry and brittle hair, black complexion, fixed gaze, and closed eyelids unable to open (indication of Yin Qi exhaustion) ○ Sunken eye sockets, tilted ocular axis, and sweat like sticky beads that do not flow (indication of Yang Qi exhaustion) ○ Post-illness wheezing and diarrhea (indication of impending Spleen pulse exhaustion) ○
Fixed eyes unable to move, hands relaxed and hanging down, and upward deviation of the eyes (indication of Taiyang channel Qi exhaustion)
Foaming at the mouth ○ Snoring-like voice ○ Red face ○ Blackish-green face and cyan lips ○ Full philtrum and everted lips ○ Hair and eyebrows standing upright ○ Black flesh under fingernails ○ Palm lines absent, umbilical protrusion, and dorsum pedis edema ○ Cyan face, desire to lie prone, inability to see, and sweat like oil (indication of Liver Qi exhaustion, death in eight days) ○ Eyebrow loss indicates Gallbladder Qi exhaustion ○ Cyan or falling off of hand and foot nails, incessant cursing and madness (indication of Sinew Qi exhaustion, death in eight days) ○ Shoulder breathing, fixed gaze (indication of Heart Qi exhaustion, immediate death) ○ Straight hair like hemp, inability to flex and extend the body, continuous spontaneous sweating (indication of Small Intestine Qi exhaustion, death in six days) ○ Cold mouth, swollen feet, abdominal heat, abdominal distension, and uncontrolled diarrhea (indication of Spleen Qi exhaustion, death in five days) ○ Spinal pain, heavy body, stiffness preventing turning over (indication of Stomach Qi exhaustion, death in five days) ○ Dry ears, swollen dorsal tongue, hematuria, and red diarrhea (indication of Flesh Qi exhaustion, death in nine days) ○ Open mouth with non-returning breath (indication of Lung Qi exhaustion, death in three days) ○ Uncontrolled diarrhea (indication of Large Intestine Qi exhaustion) ○ Withered teeth, black face, yellow eyes, sensation of waist breaking, and spontaneous sweating (indication of Kidney Qi exhaustion, death in four days) ○ Yellow and withered falling teeth (indication of Bone Qi exhaustion).
When the essence of the Five Zang organs is severely depleted and the Shen (Spirit) cannot be retained, hoarseness occurs.
Picking at clothes and bedclothes, delirium (indication of Yangming channel Qi exhaustion),妄语 (wild speech), confusion, mutism, and aphonia; if caused by febrile disease, survival is still possible.
Floating and surging pulse, sweat like oil, unremitting wheezing indicate Lung Qi exhaustion; sticky sweat that does not flow, surging pulse with unremitting wheezing indicate dispersion of True Qi externally.
Yang remains alone while Yin departs, body resembling smoke, fixed gaze, and head shaking indicate Heart Qi exhaustion. The Heart is a Fire organ, so Yang-heat remains alone, presenting the image of smoke from extreme fire scorching.
Cyan color around the lips and mouth, sweating on the four limbs indicate Liver Qi exhaustion. The lips and mouth belong to the Spleen, and cyan belongs to Wood; Wood overacting on Earth is termed "reverse".
Blackish color around the mouth, cold sweat, and yellow body indicate Spleen Qi exhaustion. Water color overacting on Earth, with true Spleen Qi escaping outward.
Incontinence of urine and feces, wild speech, upward deviation of the eyes and fixed gaze indicate Kidney Qi exhaustion. The two yin openings are governed by the Kidneys; fecal incontinence means the gate is not closed, and water essence is exhausted.
If Yin Qi exhausts first and Yang Qi exhausts later, death occurs; the body and face will be green because Yang departs first and Yin exhausts later, thus green remains.
If Yin Qi exhausts first and Yang Qi exhausts later (Note: Text implies Yang remains after Yin departs), death occurs; the body, face, and extremities will be red, axillae warm, and epigastrium hot. This is because Yin departs first and Yang exhausts later, so red appears due to residual Yang not yet fully exhausted.
Inability to ingest food and fluids, bodily numbness, alternating calmness and chaos indicate Stomach Qi exhaustion. The Stomach receives water and grain to nourish muscles.
Exhaustion of the Six Fu organs manifests as cold feet and curled toes; exhaustion of the Five Zang organs manifests as incontinence of urine and feces and numbness of the hands and feet.
Exhaustion of Hand-Taiyin (Lung) Qi leads to dry skin and hair. Taiyin refers to the Lung, which circulates Qi to nourish the skin and hair. Thus, without Qi nourishment, skin and hair become dry and fluids depart. When fluids depart, skin joints are damaged, skin patterns disappear, skin becomes withered, and hair breaks. Loss of fluid nourishment causes hair breakage, indicating hair dies first. Condition worsens on Bing day, death on Ding day, as Fire controls Metal; other organs follow this pattern.
Exhaustion of Hand-Shaoyin (Heart) Qi leads to blocked vessels. Blocked vessels lead to stagnant blood; stagnant blood leads to loss of luster, resulting in a dark face like mud. This indicates blood dies first. Condition worsens on Ren day, death on Gui day.
Exhaustion of Foot-Taiyin (Spleen) Qi leads to failure of vessels to nourish the lips. Lips are the root of muscles; without vessel nourishment, muscles lose smoothness. Loss of muscle smoothness leads to muscle fullness, causing lip eversion. Lip eversion indicates flesh dies first. Condition worsens on Jia day, death on Yi day.
Exhaustion of Foot-Shaoyin (Kidney) Qi leads to withered bones. Shaoyin is the Winter pulse, hidden within and warming the marrow. Without warm marrow, flesh does not adhere to bone. Lack of bone-flesh affinity leads to flaccid and receding flesh, making teeth appear long and dirty due to gum recession. Hair loses luster, indicating bone dies first. Condition worsens on Wu day, death on Si day.
Exhaustion of Foot-Jueyin (Liver) Qi leads to sinew contraction, pulling the testicles and tongue. Jueyin refers to the Liver, which governs sinews. Sinews gather at the genitalia and connect to the base of the tongue. Without vessel nourishment, sinews contract tightly, pulling the testicles and tongue, causing tongue retraction and testicular retraction. This indicates sinews die first. Condition worsens on Geng day, death on Xin day.
Exhaustion of all Three Yin channels leads to dizziness and spinning vision. Dizziness results from loss of Zhi (Will); loss of Will leads to death of the spirit, resulting in blindness. Depletion of Yin causes blindness.
Exhaustion of all Six Yang channels leads to separation of Yin and Yang. Separation of Yin and Yang causes leakage of interstices, leading to terminal sweat emerging like large beads that do not flow. Predicted in the morning, death in the evening; predicted in the evening, death in the morning.
