Water diseases of the five zang organs present different syndromes. These include Pi Shui (Skin Water), Zheng Shui (Correct Water), Shi Shui (Stone Water), and Feng Shui (Wind Water). The internal causes are primarily related to Qi stagnation, Dampness, and Food retention.
Employ Qi-moving and accumulation-reducing methods: For severe cases, use Ta Zhong Wan (Tread Swelling Pill); for mild cases, use Da Ju Pi Tang (Large Tangerine Peel Decoction) or Shen Jin San (Divine Metal Powder).
Employ purgative methods: Use San Sheng San (Three Sages Powder), composed of Zhi Shi (Immature Bitter Orange), Luo Bo Zi (Radish Seed), and Qian Niu (Morning Glory Seed), ground into powder; administer according to the patient's constitution and size. If there is excess Qi syndrome, select from San Hua Shen You San (Three Flowers Divine Protection Powder), Zhou Che Wan (Boat Cart Pill), Yu Gong San (Yu Gong Powder), or use Ku Hu Wan (Bitter Gourd Pill).
Employ methods to unblock the mechanisms: Use Shen Qi Wan (Kidney Qi Pill), ensuring Fu Ling (Poria) and Di Huang (Rehmannia Root) are used in conjunction. Coat with Ping Wei San (Calm the Stomach Powder). If the body is slightly solid/excess, use this after purgation to cleanse the Fu organs of the bladder (referring to promoting urination).
Employ methods to strengthen Earth (Spleen): When Spleen deficiency fails to control water, causing water to infiltrate and move recklessly, treatment should focus on drying dampness in the Spleen and tonifying deficiency. Purgatives must absolutely be avoided. Use Dui Jin Yin Zi (Metal-Contending Drink) or Pi Ji Wan (Spleen Accumulation Pill).
Employ draining and leaching methods: Use Sang Bai Pi (Mulberry Root Bark), Ting Li Zi (Periploca Seed), Da Ju Pi Tang (Large Tangerine Peel Decoction), Wu Ling San (Five Ingredient Powder with Poria), etc.
Employ steaming methods: Use medicinal decoctions and sit in the steam according to specific techniques.
Employ Miao Ci (Mysterious Pricking/Mirrored Needling): For those with full pulses and excess Qi, points such as Weicang (BL47), Hegu (LI4), Shimen (CV5), Shuigou (GV26), Zusanli (ST36), Fuliou (KI7), Ququan (LR8), and Siman (KI14) may be used.
Employ digestion-promoting methods: For edema resembling water caused by dietary injury to the Taiyin channel due to food retention, differentiate between cold and heat patterns and use Fen Xiao Wan (Separating and Reducing Pill).
Employ opening the ghost gate method: This refers to inducing sweating.
Employ digestion-promoting methods: For Qi distension, clear heat and move Qi using Zhong Man Fen Xiao Wan (Middle Fullness Separating and Reducing Pill). For Cold distension, use Zhong Man Fen Xiao Tang (Middle Fullness Separating and Reducing Decoction).
Employ middle-tonifying and dampness-resolving methods: This indicates severe Spleen deficiency. Use large doses of Ren Shen (Ginseng Root), Bai Zhu (Atractylodes Macrocephala Rhizome), Chen Pi (Aged Tangerine Peel), Fu Ling (Poria), and Cang Zhu (Atractylodes Rhizome).
Employ blood-invigorating methods: For cases involving Blood stasis/accumulation.
Employ cleansing the Fu organ of the bladder method: For patients with astringent/difficult urination, directly employ draining and leaching methods.
Employ purgative methods: For patients with excess pulses and robust constitutions, after purgation, immediately employ recovery/regulating methods.
Employ Qi-moving methods: Hou Po (Magnolia Bark) treats distension because its pungent flavor disperses. Another method: take one large Toad, place it inside a pig's stomach, cook until done, remove the Toad, and have the patient consume the entire contents in one day, resulting in immediate cure; this is miraculous.
Employ yin-nourishing methods: Without Yin, transformation cannot occur, leading to distension. Therefore, add herbs like Huang Qin (Scutellaria Root) and Lian Zi (Lotus Plumule) or similar bitter-cold herbs to the prescription.
Employ acupuncture: Points include Shangwan (CV13), Zusanli (ST36), Zhangmen (LR13), Yinggu (KI10), Guanyuan (CV4), Qimen (LR14), Xingjian (LR2), Pishu (BL20), Xuanzhong (GB39), and Chengman (ST21).
