Qi Gu (Bifurcated Bone), referring to the junction where the ends of the two Fu Gu (Webbed Bone, i.e., lower costal margin) meet, is located directly above the Ji Wei Gu (Jiwei Bone, i.e., xiphoid process). Internally, it is close to the Heart (Zang organ governing spirit and blood circulation), making it highly susceptible to injury from contact or impact. Whether due to falls, blows, or being struck by a horse, blood will inevitably stagnate and form stasis, resulting in severe pain. In mild cases, the injury is confined above the diaphragm; in severe cases, it affects the Heart, leading to coma with closed eyes, loss of consciousness, locked jaw, phlegm-rapid breathing, flaring nostrils, prolonged unconsciousness, or post-awakening mental confusion. This indicates that blood stasis has become hard and coagulated, obstructing flow, which is difficult to treat and often fatal. If consciousness remains clear without confusion, but there is persistent stasis-related pain, chest fullness, shortness of breath, muteness, and the ability to consume some food when awake, it is advisable to take Jiajian Suzi Taoren Tang (Modified Perilla Seed and Peach Kernel Decoction) with Zhi Qiao (Aucklandiae Radix) in the morning, and Shuxue Wan (Blood-Dispersing Pill) in the evening. Externally, apply Wanling Gao (Ten-Thousand Spirits Plaster) and fumigate/iron the area with Chao Re Dingtong San (Heated Pain-Relieving Powder). Only then can recovery be expected. Furthermore, any bifurcating bones throughout the body are collectively termed Qi Gu; students of orthopedics should be aware of this.

Bi Xin Gu (Shielding Heart Bone), also known as Ji Wei Gu (Xiphoid Process), consists of brittle bone located between the Qi Gu at the lower chest. Traumatic injuries from falls, blows, or impacts cause unceasing pain, qi sensation radiating through both hypochondria, abdominal distension and pain, inability to straighten the waist, and a tendency to press the chest with both hands. Treatment involves internal administration of Bali San (Eight-Centigram Powder), external washing with Ai Cu Tang (Mugwort and Vinegar Decoction), application of Wanling Gao (Ten-Thousand Spirits Plaster), and drinking light yellow wine for thirst. Avoid tea, raw/cold foods, and bran rice porridge.

Fu Gu (Webbed Bone), referring to the lateral costal margins below the chest. The upper and lower pairs are prone to injury on both sides. Above this region, protection by the elbow and arm makes injury difficult. For injuries in the lower part near the abdomen, manual manipulation is easier because the ribs are accessible at the edge, allowing fractured ribs to be reduced. Patients typically exhibit head-down posture, hunched back, painful groaning, preference for lateral recumbency over supine position, severe visceral pain upon standing, dizziness, faintness, and reduced appetite. Treatment involves internal administration of Zhenggu Zijin Dan (Orthopedic Purple Gold Pill), washing with Baxian Xiaoyao Tang (Eight Immortals Freedom-Deciding Decoction), and application of Wanling Gao (Ten-Thousand Spirits Plaster) along with blood-dispersing agents for cure. However, if the second rib above is fractured and displaced, causing depression that cannot rise, treatment is difficult due to its location above the diaphragm. Even with aggressive intervention, complete restoration is unlikely. Blood stasis remaining above the diaphragm, if not resolved by medication, may form a cyst. Mild cysts contain serous fluid (yellow water), while hard ones consist of blood clots, leading to chronic, stubborn diseases.
