Wind is the vast energy of heaven and earth. It may strike the Fu organs (Six Bowel Organs), the Zang organs (Five Viscera), the blood vessels, or the meridians. Its nature is agile and changeable, difficult to describe. When wind strikes the Fu organs, symptoms manifest in the four limbs; when it strikes the Zang organs, the condition is severe and life-threatening.
Whenever encountering patients with stroke or Qi stagnation leading to fainting, in an emergency, immediately pinch the Renzhong (GV26) acupoint with the thumb to restore consciousness. If ineffective, use Tongguan San (Opening the Pass Powder) blown into the nasal cavity, while lifting the patient's head. If a sneeze occurs, there is hope for cure. If the jaw is locked and cannot open, apply Huafeng Dan (Wind-Resolving Pill) to rub the teeth; once the mouth opens, administer copious amounts of sesame oil, optionally adding a small amount of Moschus (She Xiang). Alternatively, use Zijin Dan (Purple Gold Pill) taken with wine. One may also use Gouti San (Melon Seed Powder) to induce vomiting, or administer Er Fan (Alum) 2 qian mixed with Ginger Juice. For robust individuals, induce vomiting with oil; for the elderly, induce vomiting by decocting Ginseng Radix et Rhizoma (Ren Shen), Saposhnikoviae Radix (Fang Feng), and Platycodonis Radix (Jie Geng) in shrimp juice. Survival is possible if vomiting occurs.
Tongguan San: Treats trismus (lockjaw) where soup and water cannot enter. Composed of Pinelliae Tuber (Ban Xia), Arisaemae Rhizoma (Tian Nan Xing), and Gleditsiae Sinensis Fructus Spina (Ya Zao), each 5 qian. Grind into an extremely fine powder, take a small amount, and blow into the nose. Cure is possible if sneezing occurs; otherwise, it is untreatable.
Huafeng Dan (Keep this formula at home): Treats stroke phlegm syncope, wind epilepsy with lockjaw, and pediatric convulsions, opisthotonos, fever, and cough. Formula: Fritillariae Bulbus prepared with bile (Dan Xing) 2 qian (if unavailable, prepare with bovine bile); Gastrodiae Rhizoma (Tian Ma) (roasted); Saposhnikoviae Radix (Fang Feng) (remove rhizome); Schizonepetae Herba (Jing Jie Sui); Notopterygii Rhizoma et Radix (Qiang Huo); Angelicae Pubescentis Radix (Du Huo) (remove rhizome); Ginseng Radix et Rhizoma (Ren Shen) (remove rhizome); Asari Radix et Rhizoma (Xi Xin); Chuanxiong Rhizoma (Chuan Xiong), each 1 qian; Aquilariae Lignum Resinatum (Mu Xiang) 0.5 qian. Process with refined honey into pills the size of a Chinese bayberry (Qian Shi), coated with Cinnabar (Zhu Sha). Administer by dissolving in Mint Decoction (Bo He Tang). For cases caused by anger, administer with Perilla Decoction (Zi Su Tang). If lockjaw occurs, rub a small amount on the teeth to open the mouth.
Qingzhou Bai Wan Zi (Qingzhou White Pills): Treats hemiplegia in men and women, excessive wind-phlegm obstruction, vomiting of phlegm foam, and pediatric convulsions. Formula: Pinelliae Tuber (Ban Xia) 7 liang (white variety, soaked); Arisaemae Rhizoma (Tian Nan Xing) (raw use); Typhonii Rhizoma (Bai Fu Zi) each 2 liang; Aconiti Radix Preparata (Chuan Wu Tou) 5 qian (processed, skin and umbilicus removed). Grind into an extremely fine powder, place in a raw silk bag, and wash/swirl in well water. Grind and wash again until all soluble components are extracted. Place in a porcelain basin, sun-dry by day and dew-expose by night. Change water and stir/settle every morning (Spring: 5 days; Summer: 3 days; Autumn: 7 days; Winter: 10 days). Remove water, dry until resembling broken jade, grind finely. Form into mung-bean-sized pills using rice porridge liquid as binder. For hemiplegia due to wind-dampness, take 20-30 pills with warm wine. For pediatric convulsions, take 3-5 pills with Mint Decoction.
Shengsheng Yin (Life-Preserving Drink): Treats various types of stroke, cold-stroke, heat-stroke, damp-stroke, as well as damp-syncope, Qi-syncope, and unconsciousness. Highly effective if taken at onset. Formula: Aucklandiae Radix (Nan Mu Xiang), Arisaemae Rhizoma (Tian Nan Xing) (wrapped in wet paper and roasted), Pinelliae Tuber (Ban Xia) (soaked in boiling water), each 1.5 qian; Atractylodis Rhizoma (Cang Zhu), Glycyrrhizae Radix et Rhizoma (Gan Cao), Asari Radix et Rhizoma (Liao Xi Xin), Acori Tatarinowii Rhizoma (Shi Chang Pu), each 1 qian; Fresh Ginger (Sheng Jiang) 7 slices. Decoct and take. For excessive phlegm, add 2 dried Scorpion (Quan Xie) (honey-fried).
Wuyao Shunqi San (Aucklandia Qi-Regulating Powder): Treats pain, numbness, paralysis, slurred speech, muscle spasms, difficulty walking, weak waist and knees caused by wind-Qi invasion; also treats women's blood-Qi disorders, elderly cold-Qi disorders, chest/diaphragm distension/stabbing pain, borborygmus, vomiting, and diarrhea. Treating wind requires regulating Qi first; when Qi flows smoothly, phlegm dissipates. This formula treats Qi stagnation and Qi depression. Formula: Litseae Fructus (Wu Yao), Ephedrae Herba (Ma Huang) (remove joints), Citri Reticulatae Pericarpium (Chen Pi) (remove white pith), each 2 qian; Chuanxiong Rhizoma (Chuan Xiong), Angelicae Dahuricae Radix (Bai Zhi), Bombyx Batryticatus (Jiang Can) (stir-fried), Aurantii Fructus Immaturus (Zhi Ke) (remove pulp, bran-fried), Platycodonis Radix (Jie Geng) (remove rhizome), each 1 qian; Zingiberis Rhizoma Recens (Gan Jiang) (soaked) 0.5 qian; Glycyrrhizae Radix et Rhizoma (Gan Cao) (honey-fried) 0.3 fen. Decoct with Fresh Ginger (Sheng Jiang) and Jujubae Fructus (Da Zao). For aversion to cold and high fever, add Allii Macrostemonis Bulbus (Cong Bai); for generalized itching, add Menthae Haplocalycis Herba (Bo He); for limb spasms, add Aquilariae Lignum Resinatum (Mu Xiang) and Dendrobii Caulis (Shi Hu); for dampness, add Atractylodis Rhizoma (Cang Zhu), Atractylodis Macrocephalae Rhizoma (Bai Zhu), and Arecae Semen (Bing Lang); for foot edema, add Achyranthis Bidentatae Radix (Niu Xi), Acanthopanacis Cortex (Wu Jia Pi), and Angelicae Pubescentis Radix (Du Huo); for generalized pain, add Angelicae Sinensis Radix (Dang Gui), Cinnamomi Cortex (Rou Gui), Boswelliae Resina (Ru Xiang), and Myrrhae Resina (Mo Yao); if accompanied by Huang Qi syndrome, add Astragali Membranacei Radix (Huang Qi) and double Gan Jiang; for chest distension, add Aurantii Fructus (Zhi Shi) and Curcumae Radix (E Zhu); for dizziness, add Asari Radix et Rhizoma (Xi Xin) and Fine Tea (Xi Cha); for inability to move legs, add Notopterygii Rhizoma et Radix (Qiang Huo), Saposhnikoviae Radix (Fang Feng), and Moschus (She Xiang); for acute heart/abdominal pain, add Foeniculi Fructus (Xiao Hui Xiang); for inability to lift limbs/head/neck, add Dipsaci Radix (Chuan Xu Duan) and Syringae Ramulus et Folium (Wei Ling Xian); for scrotal edema, combine with Wuji San (Five Accumulations Powder); for cold-bi of four limbs, add Aconiti Lateralis Radix Preparata (Chuan Fu Zi), Cinnamomi Cortex (Guan Gui), and Gentianae Macrophyllae Radix (Qin Jiao); for chronic paralysis, remove Ma Huang and Gan Jiang, add Gastrodiae Rhizoma (Tian Ma), Fang Feng, Qiang Huo, Ban Xia, Tian Nan Xing, Mu Xiang, and Dang Gui; for painful numbness, add Aconiti Radix (Tian Xiong), Xi Xin, and Fang Feng; for women's wind-blood disorder, add Fang Feng, Jing Jie, and Bo He; for arm pain, add Qiang Huo, Fang Feng, Cang Zhu, Perillae Folium (Zi Su), and Thin Cinnamon Bark (Bo Gui); for Qi-stagnation lumbar pain, add Persicae Semen (Tao Ren) and decoct with wine; for back/heart pain, combine with Xingqi Xiangsu San, add Cang Zhu, Ban Xia, and Poria (Fu Ling); for facial deviation, add黄连 (Huang Lian) stir-fried with ginger, Qiang Huo, Fang Feng, Jing Jie, Bamboo Sap (Zhu Li), and Ginger Juice; for extreme painful numbness, combine with Sanwuqi San; for day-night pain (lighter by day, heavier by night), combine with Shenmi Zuojing Tang; for inability to walk for 2-3 years, combine with Duhuo Jisheng Tang.
Xiao Xuming Tang (Minor Continuation Life Decoction) (Use only for true Fu organ strike): Treats sudden stroke with excessive phlegm-saliva obstruction and exterior Six Meridian syndromes suitable for releasing the exterior. Formula: Ma Huang (remove joints), Apricot Kernel (Xing Ren) (soak, peel), Cinnamomi Cortex (Guan Gui), Paeoniae Radix (Yue Shao Yao - likely Bai Shao), Chuanxiong Rhizoma (Chuan Xiong), Stephaniae Tetrandrae Radix (Fang Ji), Scutellariae Radix (Huang Qin), Ginseng Radix et Rhizoma (Ren Shen), Glycyrrhizae Radix et Rhizoma (Gan Cao) (honey-fried), each 2 qian 4 li. *Neijing* states: "When pores open, one shivers with cold; when closed, one feels hot and stifled." Knowing it is sudden cold-strike, initially use Xuming Tang with modifications.
- Wind striking Taiyang meridian, no sweat, aversion to cold: Double Ma Huang, Xing Ren, Fang Feng.
- Strike Taiyang (deficiency), sweating, aversion to wind: Double Gui Zhi, Xing Ren, Shao Yao.
- Strike Yangming, body heat, sweating, no aversion to wind: Double Gui Zhi, Huang Qin, add Ge Gen.

If stroke does not fit these four meridians, with mixed six syndromes involving Shaoyang/Jueyin, or joint spasms/numbness: Double Qiang Huo and Lonicerae Japonicae Flos (Lian Qiao).
General rule: Spring/Summer, add Shi Gao, Anemarrhenae Rhizoma (Zhi Mu), Huang Qin. Autumn/Winter, add Guan Gui, Fu Zi, Shao Yao. For heat, replace Hei Fu Zi with Bai Fu Zi. For tendon spasm/slurred speech/chord pulse, add Coicis Semen (Yi Yi Ren). For tendon spasm, add Ren Shen, remove Huang Qin/Shao Yao to avoid worsening cold. After slight improvement, add Dang Gui. For irritability/constipation, remove Fu Zi/Gui Zhi, double Shao Yao, add Zhu Li. For constipation >3 days/chest discomfort, add Zhi Shi, Da Huang. For slurred speech/trembling hands/feet, add Shi Chang Pu, Zhu Li. For thirst, add Ophiopogonis Radix (Mai Dong), Trichosanthis Radix (Tian Hua Fen), Ge Gen. For heat/thirst, add Qin Jiao. For body pain, add Qiang Huo (also for convulsions). For irritability/fright, add Rhinocerotis Cornu (Xi Jiao). For anger, add Capricornis Cornu (Ling Yang Jiao). Formula: Magnoliae Officinalis Cortex (Nan Hou) (fried with ginger juice), Qiang Huo, Rheum Palmatum Radix et Rhizoma (Da Huang), Aurantii Fructus Immaturus (Zhi Shi), equal parts. Decoct with ginger. Take again if no effect, aiming for mild laxation. Once internal/external pathogens are cleared, take Yufeng Tang to harmonize the middle. For blood vessel/Fu strikes, strictly avoid Long Nao (Borneolum Syntheticum), She Xiang, and Niuhuang (Calculus Bovis), as Moschus enters the Spleen, Niuhuang enters the Liver, and Borneol enters the Bone, potentially drawing wind inward.

Feng Fei (Limbs unresponsive, slight sensation); 3. Feng Yi (Unconsciousness); 4. Feng Bi (Numbness). *Jinkui Yaolue* Stroke Chapter: Pulse floating and tight at Cun. Tight is Cold, Floating is Deficiency. Cold/Deficiency clash, pathogen in skin -> facial deviation/incoordination. Pathogen in Luo -> skin numbness. Pathogen in Jing -> immediate weakness. Pathogen enters Fu -> unaware of people. Pathogen enters Zang -> tongue stiff, drooling. Thus, Pian Ku etc. are treated with Da/Xiao Xuming, Paifeng Tangs to disperse. Hejian (Liu Wansu) said: Fire flares up, Kidney Water deficient, cannot control. Heat stagnates, Spirit confused, sinews/bones useless. Also caused by excess of Five Emotions (Joy, Anger, Worry, Sorrow, Fear), sudden strike, primarily Heat. Dongyuan (Li Dongyuan) said: Stroke is not external wind. People over 40, Qi injured...
