Wu stated: In all cases of pregnant women suffering from Shanghan (Cold Damage), the treatment principles for the Six Meridians are identical, but the primary objective must be to stabilize the fetus. Any medication that harms the fetus should not be used. For instance, in formulas such as Huoxiang Zhengqi San (Agastache Powder to Regulate the Qi and Transform Dampness), Shimei Xiong Su San (Ten-Ingredient Ligusticum and Perilla Powder), Shen Su Yin (Ginseng and Perilla Drink), and Xiaochaihu Tang (Minor Bupleurum Decoction), which contain Banxia (Pinellia Rhizome), since Banxia can disturb the fetus, it must be removed if used. However, if there is excessive phlegm and vomiting with counterflow qi, Banxia is essential; in this case, Banxia Qu (Pinellia Fermented) should be used. If Banxia Qu is unavailable, soak the raw Banxia in boiling water seven times, remove the skin and umbilical scar, mix with fresh ginger juice, dry it, and then it can be used. Substances such as Chuanwu (Aconitum Carlevari Root), Fuzi (Prepared Aconite Lateralis Radix), Tianxiong (Aconitum Kusnezoffii Root), Cizi (Aconitum Carlevari Lateralis Radix Praeparata), Rougui (Cinnamon Bark), Ganjiang (Dried Ginger), Dahuang (Rhubarb Rhizome), Mangxiao (Mirabilite), Yuanhua (Kadua Flower), Gansui (Euphorbia Kansui Root), Daji (Euphorbia Pekinensis Root), Shuqi (Polygala Root), Shuizhi (Leech), Mengchong (Blue Bottle Fly Larva), Taoren (Peach Seed), Mudanpi (Tree Peony Bark), Ganqi (Dried Lacquer), Daizheshi (Haematitum), Qumai (Dianthus Herb), and Niuxi (Achyranthes Root) are all substances that disturb the fetus. Their use requires careful deliberation. Generally, for pregnant women with Shanghan, decoctions must include Huangqin (Scutellaria Root) and Baizhu (Atractylodes Macrocephala Rhizome), as these two herbs stabilize the fetus. One may decoct these two herbs and administer the liquid, or grind them into fine powder and take two or three qian mixed with warm water, which is also effective. If the pregnant woman has a constitutionally weak physique, Siwu Tang (Four Substance Decoction) should be added to assist, and it should not be omitted. For example, when using Xiaochaihu Tang, remove Banxia, add Baizhu, and combine with Siwu Tang; this can protect the fetus and clear heat, with miraculous efficacy. All other cases should follow this pattern; proper application yields excellent results.
Wan Mizhai stated: The treatment of Shanghan in pregnancy focuses specifically on clearing heat and stabilizing the fetus. Whether inducing sweating or purging, one must treat according to the pulse and symptoms indicating the involvement of the Zang-Fu organs and exterior-interior relationships, without violating fetal qi. Therefore, for exterior patterns, induce sweating primarily with Xiangsu San (Liquorice and Cyperus Powder); for half-exterior half-interior patterns, harmonize primarily with Huanglong Tang (Yellow Dragon Decoction); for interior patterns, purge primarily with Sanhuang Jiedu Tang (Three Yellows Detoxification Decoction). This is a secret transmission in my family, having saved many lives. (The methods of sweating, purging, and harmonizing are indeed established protocols, but prioritizing Xiangsu San, Huanglong Tang, and Sanhuang Jiedu Tang represents an innovation not previously expounded, which is ingenious.) Regarding the ancient formula Liuhe Tang (Six-Harmony Decoction), although its differential treatment is detailed and clear, it is still not as appropriate as this approach. (The ancient Liuhe Tang comprises fifteen formulas ranging from Biaoxu Siwu Tang to Siwu Dahuang Tang, detailed under the modifications of Siwu Tang in the Tiaojing [Regulating Menses] section of Volume I.)
Baizhu San (Atractylodes Powder): Treats Shanghan and febrile diseases. Use this first to stabilize the fetus; it can be taken as soon as headache and fever are felt. Two or three doses will result in recovery. If there is cold extremities and counterflow, indicating a Yin pattern, do not use it.
Ingredients: Baizhu (Atractylodes Macrocephala Rhizome), Huangqin (Scutellaria Root) (equal parts, stir-fried on a new tile).
Preparation and Administration: Cut finely. Take 3 qian per dose, add 3 slices of fresh ginger and 1 jujube, decoct in water, and drink.
Ante-Natal Ejiao San (Donkey-Hide Gelatin Powder): Treats pregnant women suffering from Shanghan or seasonal epidemics. Take this first to stabilize the fetus, then alternate with the main therapeutic agents. (Taking ante-natal stabilizers first is a major principle, but if exterior symptoms are severe, caution is required.)
Ingredients: Ejiao (Donkey-Hide Gelatin, prepared), Baizhu (Atractylodes Macrocephala Rhizome, stir-fried), Sangjisheng (Taxillus Branch), Renshen (Ginseng Root), Baifuling (Poria, equal parts).
Preparation and Administration: Grind into powder. Take 1 qian per dose, mix with rice gruel, and swallow. Take three times daily.
Xiangsu San (Liquorice and Cyperus Powder): For any pregnant woman with Shanghan, regardless of the number of days ill, if chills and headache are present, this formula is indicated.
Ingredients: Xiangfu (Cyperus Rhizome, stir-fried until black), Zisu (Perilla Leaf) (2 qian each), Chenpi (Citrus Reticulata Peel, 1 qian), Gancao (Liquorice Root, 5 fen).
Preparation and Administration: Coarsely crush, add 3 slices of fresh ginger and 5 stalks of scallion white, decoct in water, and drink. For headache, add Chuanxiong (Ligusticum Rhizome) and Baizhi (Angelica Dahurica Root) 1 qian each; this variant is named Xiongzi Xiangsu San.
If the Liver pulse is obtained, external signs include cleanliness obsession, greenish complexion, and irritability; the pulses at all three positions are floating and wiry, with chills but harmonious interior (normal urination and defecation). Add Qianghuo (Notopterygium Rhizome) and Fangfeng (Saposhnikovia Root) 1 qian each to the base formula. This is because the Liver governs wind, indicating the Gallbladder is affected.
If the Heart pulse is obtained, external signs include red complexion, dry mouth, and excessive laughter; the pulses at all three positions are floating and surging, with chills but harmonious interior. Add Huangqin (Scutellaria Root) and Shigao (Gypsum Fibrosum) 1.5 qian each to the base formula. This is because the Heart governs heat, indicating the Small Intestine is affected.
If the Spleen pulse is obtained, signs include yellow complexion, frequent belching, and overthinking; the Chi and Cun pulses are floating and moderate, with chills but harmonious interior. Add Baizhu (Atractylodes Macrocephala Rhizome) and Fangji (Stephania Tetrandra Root) 1.5 qian each to the base formula. This is because the Spleen governs dampness, indicating the Yangming channel is affected.
If the Lung pulse is obtained, external signs include pale complexion, frequent sneezing, sadness, and desire to cry; the Chi and Cun pulses are floating and choppy, with chills but harmonious interior. Add Huangqi (Astragalus Root) and Fangfeng (Saposhnikovia Root) 1 qian each to the base formula. This is because the Lung governs dryness, indicating the Large Intestine is affected.
If the Kidney pulse is obtained, external signs include black complexion and fearfulness; the Chi and Cun pulses are floating and soft, with chills but harmonious interior. Add Pao Fuzi (Prepared Aconite Lateralis Radix) 1 qian to the base formula. This is because the Kidney governs cold, indicating the Bladder channel is affected. (Note: Fuzi is contraindicated as it harms the fetus; Wuzhuyu (Evodia Fruit) can be used for warming instead.) (Note: Although Fuzi is said to harm the fetus, Zhang Zhongjing's Fuzi Tang also treats fetal cold, but only in cases of true cold, so use with discretion.)
Qianghuo Tang (Notopterygium Decoction): Hejian (Liu Wansu) stated: To resolve and benefit from Shanghan, regardless of which meridian is affected. Although it may not completely resolve all pathogens, it prevents adverse transformations and is particularly beneficial for pregnant women.
Ingredients: Qianghuo (Notopterygium Rhizome, 2 qian), Baizhu (Atractylodes Macrocephala Rhizome, 1.5 qian), Fangfeng (Saposhnikovia Root), Chuanxiong (Ligusticum Rhizome), Baizhi (Angelica Dahurica Root), Huangqin (Scutellaria Root), Gancao (Liquorice Root, prepared, 1 qian each), Xixin (Asarum Root, 3 fen).
Preparation and Administration: Coarsely crush, decoct in water, and drink at any time. If there is no sweat, remove Baizhu and use Cangzhu (Atractylodes Lancea Rhizome) instead; adding Zisu (Perilla Leaf) is also acceptable.
Huanglong Tang (Yellow Dragon Decoction): When a pregnant woman suffers from Shanghan for three to five days, presenting with chills and fever, internal restlessness and thirst with desire to drink, and scanty dark urine, this indicates pathogenic factors in the half-exterior half-interior region. This formula is indicated.
Ingredients: Chaihu (Bupleurum Root, 2 qian), Huangqin (Scutellaria Root, 1.5 qian), Renshen (Ginseng Root), Gancao (Liquorice Root, 1 qian each).
Preparation and Administration: Add ginger and jujube, decoct in water, and drink. (Huanglong Tang is essentially Xiaochaihu Tang without Banxia, belonging to the Shaoyang channel medications, but this formula emphasizes harmonization, hence its indication. Its modification methods are even more ingenious.)
If there are alternating chills and fever, no sweat, and dry mouth, add Gegen (Pueraria Root) 2 qian, remove jujube, and add 3 stalks of scallion white.
If headache persists, add Chuanxiong (Ligusticum Rhizome) and Baizhi (Angelica Dahurica Root) 1 qian each, remove jujube, and add 3 stalks of scallion white.
If there is fever with sweat and thirst, add Baizhu (Atractylodes Macrocephala Rhizome) and Gualougen (Trichosanthes Root) 1.5 qian each.
If the pulse is floating, large, and forceful, with high fever and extreme thirst, combine the base formula with Ren Shen Baihu Tang (Ginseng White Tiger Decoction), removing ginger and jujube.
If there is mental restlessness and inability to lie down, add Baifuling (Poria) and Maidong (Ophiopogon Tuber) 1 qian each.
If there is vomiting or hiccups, add Zhi Banxia (Prepared Pinellia Rhizome) and Baifuling (Poria) 1 qian each, remove jujube.
If there is fullness and pain in the chest and diaphragm, add Chao Zhike (Stir-fried Aurantium Fructus), Chao Xiangfu (Stir-fried Cyperus Rhizome), and Chuanxiong (Ligusticum Rhizome) 1 qian each.
If there is constipation, initially add Dahuang (Rhubarb Rhizome) 5 fen; stop once bowel movement occurs. If ineffective, add 1 qian, stopping when bowel movement is achieved.
Sanhuang Jiedu Tang (Three Yellows Detoxification Decoction): Five to six days after a pregnant woman contracts Shanghan, if exterior pathogens have completely resolved and there are no headaches or chills, but only irritability, fever, extreme thirst, dark urine, constipation, or passage of bloody watery stool, and the six pulses are sunken and solid, this indicates the pathogen is in the interior. This formula is indicated.
Ingredients: Huangqin (Scutellaria Root), Huanglian (Coptis Rhizome), Huangbai (Phellodendron Bark), Shanzhi (Gardenia Fruit), Dahuang (Rhubarb Rhizome) (equal parts).
Preparation and Administration: Coarsely crush, decoct in water, and modify according to the pulse and symptoms of the five Zang organs.
If the Liver pulse is obtained, internal signs include irritability and fullness, wasting-thirst, and difficult urination/defecation; the Chi and Cun pulses are sunken, wiry, and forceful, indicating the Liver organ itself is affected. Add Danggui (Angelica Sinensis Root) 1.5 qian, Gancao (Liquorice Root) 5 fen, and double the amount of Shanzhi (Gardenia Fruit).
If the Heart pulse is obtained, internal signs include irritability, heart pain, hot palms, and hiccups; the Chi and Cun pulses are sunken, rapid, and forceful, indicating the Heart organ itself is affected. Add Maidong (Ophiopogon Tuber) 1 qian, Zhuru (Bamboo Sap) one ball, and double the amount of Huanglian (Coptis Rhizome).
If the Spleen pulse is obtained, internal signs include abdominal distension and fullness, delirium; the pulse is sunken, tight, and forceful, indicating the Spleen organ itself is affected. Add Chao Zhishi (Stir-fried Aurantium Fructus Immaturus) and Jiangzhi Chunpi (Magnolia Bark stir-fried with ginger juice) 1.5 qian each, and double the amount of Dahuang (Rhubarb Rhizome).
If the Lung pulse is obtained, internal signs include wheezing, cough, and chest fullness; the Chi and Cun pulses are sunken, choppy, and forceful, indicating the Lung organ itself is affected. Add Chao Tingli (Descurainia Seed) 1 qian, Jiegeng (Platycodon Root) 5 fen, and double the amount of Huanglian (Coptis Rhizome).
If the Kidney pulse is obtained, signs include diarrhea with tenesmus, cold and counterflow in the lower legs; the Chi and Cun pulses are sunken and stone-like, indicating the Kidney organ itself is affected. Add Paoganjiang (Prepared Dried Ginger) 5 fen, Shudihuang (Rehmannia Root Preparata) 1.5 qian, and double the amount of Huangbai (Phellodendron Bark). (Note: Since there is already cold counterflow, and the main formula uses Three Yellows, even with added Ganjiang, it may ultimately be inappropriate; use with discretion.)
Huangqi Jiemi Tang (Astragalus Muscle-Releasing Decoction): Treats pregnant women with Shanghan wind-strike and spontaneous sweating. (For wind-strike with spontaneous sweating, if headache and pathogenic evil are strong, use this formula with caution.)
Ingredients: Renshen (Ginseng Root), Huangqi (Astragalus Root), Danggui (Angelica Sinensis Root), Chuanxiong (Ligusticum Rhizome), Gancao (Liquorice Root, prepared) (0.5 liang each), Shaoyao (Peony Root) (6 qian).
Preparation and Administration: Coarsely crush. Take 8 qian per dose, decoct in water, and drink. Adding Cangzhu (Atractylodes Lancea Rhizome) and Shengdihuang (Rehmannia Root) is also acceptable.
Guizhi Shaoyao Danggui Tang (Cinnamon Twig, Peony, and Angelica Decoction): Treats pregnant women with Shanghan presenting with floating pulse, heavy head, and cutting pain in the abdomen. This formula is indicated.
Ingredients: Guizhi (Cinnamon Twig, note: Cinnamon can cause abortion, so use fire-roasted), Shaoyao (Peony Root), Danggui (Angelica Sinensis Root) (1 liang each).
Preparation and Administration: Cut finely. Take 1 liang per dose, decoct in water, and drink.
Shaoyao Tang (Peony Decoction): Treats pregnant women with Shanghan presenting with spontaneous diarrhea, abdominal pain, inability to eat or drink, and sunken pulse, indicating a Taiyin disease. This formula is indicated. (Suitable for interior deficiency.)
Ingredients: Shaoyao (Peony Root), Baizhu (Atractylodes Macrocephala Rhizome), Fuling (Poria), Gancao (Liquorice Root) (1 liang each).
Preparation and Administration: Coarsely crush. Take 1 liang per dose, decoct in water, and drink.
Jiajian Danggui Liuhuang Tang (Modified Angelica and Six Yellows Decoction): Treats pregnant women whose sweating continues excessively after induction of sweating for Shanghan, damaging fetal qi.
Ingredients: Dangguishen (Body of Angelica Sinensis Root), Huangqi (Astragalus Root, prepared), Shengdihuang (Rehmannia Root), Huangqin (Scutellaria Root), Baizhi (Angelica Dahurica Root), Ejiaozhu (Fried Donkey-Hide Gelatin), Zhigancao (Prepared Liquorice Root) (equal parts).
Preparation and Administration: Use a pinch of Xiaomai (Floating Wheat), decoct in water, remove the wheat, add 5 qian of the herbal mixture, decoct until 70% remains, and take warm. (The method of pre-decoction is ingenious.)
Jiawei Huangqin Tang (Modified Scutellaria Decoction): Treats pregnant women who suffer from persistent diarrhea with accompanying heat after purging for Shanghan, damaging fetal qi.
Ingredients: Huangqin (Scutellaria Root, 2 qian), Baishaoyao (White Peony Root), Baizhu (Atractylodes Macrocephala Rhizome), Baifuling (Poria), Zhigancao (Prepared Liquorice Root), Ejiao (Donkey-Hide Gelatin) (1 qian each).
Preparation and Administration: Use 1.5 cups of water, add Ejiao later, decoct until 80% remains, and drink.
Jiawei Zhuye Tang (Modified Bamboo Leaf Decoction): Treats pregnant women whose heat persists after sweating and purging, indicating deficiency. This formula is indicated.
Ingredients: Renshen (Ginseng Root), Maidong (Ophiopogon Tuber), Shengdihuang (Rehmannia Root), Ejiao (Donkey-Hide Gelatin), Zhigancao (Prepared Liquorice Root) (1 qian each).
Preparation and Administration: Add 12 leaves of Zhuye (Bamboo Leaf) and 1 he of Jingmi (Rice Grain), decoct, and drink. (This is a variation of Zhuye Shigao Tang, replacing Zhimu (Anemarrhena Rhizome) and Shigao (Gypsum Fibrosum) with Ejiao and Shengdihuang, which is very ingenious.)
Huanglong Siwu Tang (Yellow Dragon Four-Substance Decoction): Treats pregnant women who experience fever after recovering from Shanghan. This formula is indicated.
Ingredients: Chaihu (Bupleurum Root, 2 qian), Huangqin (Scutellaria Root, 1.5 qian), Renshen (Ginseng Root), Gancao (Liquorice Root), Danggui (Angelica Sinensis Root), Chuanxiong (Ligusticum Rhizome), Shaoyao (Peony Root), Dihuang (Rehmannia Root) (1 qian each).
Preparation and Administration: Coarsely crush, decoct in water, and drink. If caused by food stagnation, add Zhishi (Aurantium Fructus Immaturus) to the base formula.
Jiajian Siwu Tang (Modified Four-Substance Decoction): Treats pregnant women with Shanghan where extreme heat causes macules resembling brocade patterns.
Ingredients: Danggui (Angelica Sinensis Root), Baishaoyao (White Peony Root), Shengdihuang (Rehmannia Root), Huangqin (Scutellaria Root) (equal parts).
Preparation and Administration: Coarsely crush. Take 8 qian per dose, decoct in water, and drink.
Zhizi Daqing Tang (Gardenia and Qing Dai Decoction): Treats pregnant women with Shanghan where macules turn black.

Preparation and Administration: Coarsely crush. Take 5 qian per dose, use 1.5 cups of water, add 3 cun of finely chopped scallion white, decoct, and drink.
Another Formula: Treats pregnant women with Shanghan where macules suddenly turn black, urine resembles blood, and the fetus is about to fall.
Ingredients: Zhizi (Gardenia Fruit), Shengma (Cimicifuga Rhizome) (4 liang each), Qingdai (Indigo Naturalis, 2 liang), Shengdihuang (Rehmannia Root, 20 roots), Shigao (Gypsum Fibrosum, 0.5 jin), Congbai (Scallion White, chopped, 1 sheng), Huangqin (Scutellaria Root, 3 liang).
Preparation and Administration: Decoct in water, divide into three doses. Avoid hot foods. Also, apply mud from the bottom of a well to the epigastrium; replace when dry.
Hu Tai Fa (Method to Protect the Fetus): Treats protection of the fetus during Shanghan and febrile diseases.
Usage: Use Bailiaozi (Stemonae Rhizoma) in any quantity, grind into powder, mix with egg white, spread on paper like the size of a bowl, and apply to the area below the navel where the fetus resides. Moisten with water when dry.
Hu Tai Fa (Method to Protect the Fetus): Treats all heat conditions in pregnant women, both internal and external manifestations. (This method is ingenious.)
Usage: Grind Fulonggan (Dragon's House Earth) into powder, mix with mud from the bottom of a well, apply to the epigastrium, to prevent injury to the fetus.
