After continuous administration of four doses, large amounts of gelatinous phlegm were vomited, and all ailments were completely eliminated. In Xue's Medical Cases, Buzhong Yiqi Tang (Spleen-Raising Qi-Boosting Decoction) and Dihuang Wan (Rehmannia Pill) are often used together for treatment. Even capable physicians like Lu Bu Yuan sometimes imitate this practice. In fact, this is not the correct method of medication. If it is truly Lower Sinking of Clear Yang requiring raising, then the cloying and stagnating nature of Dihuang Wan (Rehmannia Pill) is unsuitable. If it belongs to True Yin Insufficiency requiring nourishing and filling, then the dispersing nature of Sheng Ma (Rhizoma Cimicifugae) and Chai Hu (Radix Bupleuri) should not be administered. This is self-contradictory and lacks discipline. However, treating the upper and lower separately originally has its rules. Tu Jingsi has always had Yin Deficiency, suffering from chronic Phlegm-Cough, Qi rebellion upon movement, and insomnia at night. Frequent use of Yin-nourishing and Yang-subduing drugs led to gradually reduced appetite; slightly consuming rich flavors caused vomiting of sour rot. Meng Ying examined his pulse: Left was wiry and slightly rapid, Right was slippery and wiry. Water overflows frequently, Earth loses its dike control, Liver Wood rises excessively, Lung Metal descends insufficiently. This is mainly due to long-term use of cloying drugs, causing damp-turbidity to accumulate internally, which benefits neither the Lower Jiao nor hinders Middle Jiao transportation. Left strong, Right weak, ascending and descending unregulated. Use Cong Rong (Cistanche Herba), Huang Bo (Phellodendri Cortex), Dang Gui (Angelicae Sinensis Radix), Shao Yao (Paeoniae Radix), Shu Di (Rehmanniae Radix Preparata), Dan Pi (Moutan Cortex), Fu Ling (Poria), Lian Shi (Fructus Meliae Toosendan), Sha Ren (Amomi Fructus) ground into powder, made into pills with Lotus Root Starch. Take in the morning to Warm Kidney Water to Clear Liver. Use Dang Shen (Codonopsis Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Zhi Shi (Fructus Aurantii Immaturus), Chang Pu (Rhizoma Acori Tatarinowii), Ban Xia (Rhizoma Pinelliae Preparatum), Fu Ling (Poria), Ju Pi (Pericarpium Citri Reticulatae), Huang Lian (Coptidis Rhizoma), Ji Li (Tribuli Fructus) sun-dried and ground into powder, made into pills with Bamboo Sap. Take at noon to Cultivate Middle Earth to Eliminate Phlegm. Swallow Weixi Wan (Weixi Pill) at dusk to肃 (purify) the Upper Source to transform turbidity. Treating the Three Jiao separately, all ailments became peaceful. All using pill forms avoids soup medicines aiding Phlegm-Dampness. (Warm Kidney Water to Clear Liver Formula: Dan Cong Rong (Cistanche Herba) 1.5 liang, Chao Chuan Huang Bo (Phellodendri Cortex Preparated) 1.5 liang, Xiang Gui Shen (Angelicae Sinensis Radix) 1 liang, Da Shu Di (Rehmanniae Radix Preparata) 4 liang, Bai Shao (Paeoniae Radix Alba) 2 liang, Dan Pi (Moutan Cortex) 1.5 liang, Yun Ling (Poria) 3 liang, Lian He (Fructus Meliae Toosendan) crushed 4 liang, Sha Ren (Amomi Fructus) powder 2 qian, Lotus Root Starch for pills. Cheng Mou contracted a cold in mid-winter. The physician heard that he ate one raw radish the day before the illness, his stool was loose, and the tongue coating was white. Given that the year was governed by Damp Earth (Wet Earth ruling the lower influence in Five Movements and Six Qi theory), he diagnosed it as Middle Deficiency Cold-Damp disease. Multiple doses of Ginseng, Atractylodes, Aconite, and Cinnamon were administered indiscriminately. Gradually leading to black tongue and coma, the doctor still suspected it was a severe deficiency syndrome. Zhang Jingjiang diagnosed: It is Winter-Warmth disease. Continuous administration of Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) showed no improvement. More than twenty days later, Meng Ying was consulted. Tongue retracted with crimson base, coating black like lacquer, mouth open, penis flaccid, pulse fine, rapid, and wiry; Right pulse feels non-existent upon pressing. Yin fluids completely scorched, Warm-Toxin deeply entrenched. Sweet dew and jade nectar cannot restore the exhausted fluids. Soon, the condition deteriorated fatally. Subsequently, Pan Shengzheng contracted a cold in mid-winter. After fever subsided on the 14th day, he felt no hunger. Various doctors treated him杂乱ly. By late winter, it turned into Dysentery with five colors, leg swelling and bleeding, painful urination, and dry mouth. Meng Ying diagnosed: Left pulse wiry, fine, and rapid; Right pulse wiry, slippery, and empty. Tongue coating yellow, greasy, root scorched, occasional spontaneous sweating. This is Qi and Fluids both exhausted, with Heat-Toxin lingering. It must be due to previous excessive intake of warming and tonifying drugs. Otherwise, why would fever subside on the 14th day but drag on for more than fifty days with such difficult symptoms? Inquiry revealed that this time, tonics were taken only two or three doses. However, in early autumn when he suffered from Malaria, the local doctor said his white tongue and robust body indicated Cold-Damp, so he took dozens of doses of Aconite and Cinnamon, and drank burning wine daily. Meng Ying said: This is the basis for brewing the disease. Treating illness is already difficult, let alone having so much drug toxicity lurking inside, what method can save him? Firmly refused to prescribe. The family insisted on seeking medicine to support them through the New Year. Meng Ying said: This is acceptable. Use Baitouweng Tang (Baitouweng Decoction) plus Yin Hua (Flos Lonicerae Japonicae), Lv Dou (Vigna Radiata Seed), Gui Shen (Angelicae Sinensis Radix), Bai Shao (Paeoniae Radix Alba), Chen Mi (Old Rice), Lu Gen (Rhizoma Imperatae), Lan Ye (Orchid Leaf), Ou (Rhizoma Nelumbinis) as the agent, and use a large dose of Buzhong Yiqi Tang (Spleen-Raising Qi-Boosting Decoction) steamed distillate instead of water to decoct the medicine. After taking, scorched coating gradually receded, stool color normalized, the whole family was overjoyed. Again begged Meng Ying to treat. Alas, the pulse showed no change, so he strongly declined. Also, Mother of Shen Tingsong suffered from Malaria in late autumn. Meng Ying once visited to diagnose: It is transformed from latent Summer-Heat. Moreover, her constitution is Yang-strong with much Phlegm-Fire. Do not fear deficiency and resort to warming and tonifying. Unfortunately, the patient desired quick recovery, sought medical advice widely. Some said it belonged to Three Yin meridians, some called it Child-Mother Malaria, some pointed out as Elderly Fetal Malaria. Crowd noise prevailed, pain could not heal. Dragging to late winter, it also turned into Dysentery. Skin swollen, buttock sores, mouth ulceration, tongue dry. Physicians helpless, again asked Meng Ying to diagnose. Pulse same as Pan Shengzheng, incurable. (Cheng Mou's Right pulse non-existent upon pressing indicates Yin pulse, Qi exhaustion. Medical method for Yang syndrome must use Yin drugs, only if Yang pulse is forceful upon heavy pressing can it be treated. Right pulse non-existent upon pressing means Yang syndrome with Yin pulse, cannot bear Yin drugs, how to treat? Pan Shengzheng's Left pulse wiry, fine, rapid indicates fluid exhaustion, Right wiry, slippery, empty indicates Qi exhaustion.)
Xie Puxiang has a chronically deficient constitution. Early winter, he suffered from cough, reduced phlegm, and decreased appetite. Shi Mou diagnosed: Kidney Qi not grasping, Ming Fire powerless. Repeatedly administering Shenqi Tang (Kidney Qi Decoction) for over a month. Resulted in vomiting, nausea, loose stools, no hunger, no urine. Late winter, Meng Ying diagnosed. Pulse very wiry, coating greasy, tongue red. This is Middle Deficiency with failure of healthy transportation. Misuse of cloying drugs further leaked the pivot mechanism. The harsh nature of Aconite and Cinnamon merely increased Liver overacting. Prescribed Dang Shen (Codonopsis Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling (Poria), Ze Xie (Alismatis Rhizoma), Ju Pi (Pericarpium Citri Reticulatae), Ban Xia (Rhizoma Pinelliae Preparatum), Zhu Ru (Caulis Bambusae in Taeniam Residue), Zhi Zi (Gardeniae Fructus), Yi Yi (Coicis Semen), Ji Li (Tribuli Fructus), Lan Ye (Orchid Leaf), Shi Di (Diospyroi Fructus Calyx). Cultivate Middle, drain Liver, move Water, eliminate Phlegm. Recovered in ten days. (Wiry indicates Liver pulse, wiry implies Spleen failure and Liver prosperity. Lu Dang Shen 3 qian, Chao Bai Zhu 3 qian, Yun Ling 3 qian, Chao Ze Xie 1.5 qian, Zhi Ban Xia 3 qian, Ju Pi 1 qian, Jiang Zhu Ru 3 qian, Hei Zhi Pi 3 qian, Sheng Yi Ren crushed 4 qian, Sheng Bai Ji 3 qian, Jian Lan Ye 3 qian, Gan Shi Di 10 pieces.)
Zhen Yuanchu's wife suffered from mania-like illness. Seems related to Seven Emotions, but not entirely so. Mrs. Chen suffered from this for over a month. Shaman and doctors changed frequently, costs were huge, old disease increased daily. Meng Ying palpated her pulse: wiry and rapid. She could eat and defecate normally. Qi often rushes upward, abdomen occasionally painful and distended. Asked about menstruation, she said illness started after period, followed by abundant leukorrhea. Meng Ying said: Cause is as described, daytime clear, nighttime delirious speech,酷似 (closely resembling) Heat entering Blood Chamber syndrome. Directly treat from Blood Stasis. Prescribed Tao Ren (Persicae Semen), Hong Hua (Carthami Flos), Xi Jiao (Rhinoceri Cornu), Chang Pu (Rhizoma Acori Tatarinowii), Dan Xing (Star Anise/Artemisiae Argyi Folium - *Note: Context suggests Dan Xing/Nan Xing i.e., Star Phlegm*), Xuan Fu (Inulae Flos), Zhe Shi (Hematitum), Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma), Hu Po (Electrum), Cong Bai (Allii Fistulosi Bulbus). Two doses, Blood Stasis indeed moved, spirit became clear and wise. Then removed Tao Ren and Hong Hua, added Dang Gui (Angelicae Sinensis Radix) and Yuan Shen (Scrophulariae Radix), several doses, recovered. (Can eat and defecate, so disease not in Qi. Qi often rushes upward, abdomen occasionally painful and distended, indicates Stasis stopping transforming into Heat, fanning Liver Yang. Leukorrhea indicates Yin fluid insufficient to transform into blood, flowing down timely. Raw Tao Ren ground 1.5 qian, original Hong Hua 6 fen, Bang Xi Jiao decocted first 4 qian, Shi Chang Pu added next 1 qian, Chen Dan Xing stewed and mixed to take 1 qian, Xuan Fu wrapped and decocted first 3 qian, Sheng Zhe Shi crushed and decocted first 8 qian, Zi Dan Shen 3 qian, Xi Mao Po (Electrum) flushed 8 fen, Lian Xu Cong Bai 2 fen. Then remove Tao Ren and Hong Hua, add Xiang Quan Gui 2 qian, Yuan Shen slices soaked decocted strained 8 qian.)
Fan Lianju suffered from illness. Severe after ten days. Meng Ying viewed: Bowels moved, heat subsided but not cleared, Qi rebellious, no hunger, hiccup, spontaneous sweating. Pulse shape void and large, tongue purple without coating. Image of Upper Jiao Heat lingering, Lower Body Yin Deficiency. Prescribed Xi Yang Shen (Radix Notoginseng), Xuan Fu (Inulae Flos), Zhu Ru (Caulis Bambusae in Taeniam Residue), Pi Ye (Folium Eriobotryae), Shi Hu (Herba Dendrobii), Shi Di (Diospyroi Fructus Calyx), Mu Li (Concha Ostreae), Gui Ban (Plastrum Testudinis), Dou Dou (Lablab Seed), Niu Xi (Achyranthis Bidentatae Radix). Two doses, tongue moist, knew hunger, hiccup and sweating stopped. Removed Dou Dou, Xuan Fu, Shi Di, added Shu Di (Rehmanniae Radix Preparata) 1 liang soaked decocted strained, Hu Tao Rou (Walnut Kernel) 3 qian, Dang Gui Shen (Angelicae Sinensis Radix Tip) 2 qian. Administered and recovered. His wife had greasy coating, sour mouth, tinnitus, insomnia, no hunger, mental fatigue, epigastric pain, head shaking. Pulse void and wiry, pressing choppy and weak. Used Dang Gui (Angelicae Sinensis Radix), Bai Shao (Paeoniae Radix Alba), Gou Qi (Lycii Fructus), Mu Gua (Chaenomelis Fructus), Lian Shi (Fructus Meliae Toosendan), Ban Xia (Rhizoma Pinelliae Preparatum), Shi Hu (Herba Dendrobii), Fu Shen (Poria with Hostwood), Zhu Ru (Caulis Bambusae in Taeniam Residue), Lan Ye (Orchid Leaf), Bai Dou Kou (Amomi Fructus Rotundus) as method to Nourish Ying, regulate Qi, harmonize Stomach, soften Liver. Several sips, recovered. His daughter had high fever, constipation, both bowels and urine blocked. Yellow coating, great thirst, bloating unbearable. Pulse wiry, slippery, rapid, excess. This is Fu Organ Syndrome. Administered Taoheshengji Tang (Peach Kernel and Magnolia Bark Decoction) plus Hai Dai (Thallus Laminariae/Jiaojuncao), Lu Fu (Radish) for two doses, cured. Han Zulin was seventy years old, ate and drank heavily, but liked taking medicine. Doctors thought he was old, thus used Aconite, Cinnamon, Ginseng, Deer Antler etc., hoping for longevity. Unexpectedly, evening of the 8th day still ate well, woke up at third watch, suddenly shivered and trembled. Soon limbs convulsed. Died the next day. Meng Ying said: This old man has Yang-exuberant constitution, excessive rich food, Phlegm-Fire increases daily. Reaching septuagenarian, True Yin depletes daily. Long-term use of such Fire-assisting and Yin-scorching drugs leads to Wind arising from Fire, roots pulled out instantly. Similar to Pediatric Acute Convulsive Wind syndrome. (Fan Lianju Upper Jiao Heat lingering Lower Body Yin Deficiency Formula: Xi Yang Shen 3 qian, Xuan Fu wrapped decocted first 3 qian, Jiang Zhu Ru 3 qian, Fresh Pi Ye brushed wrapped 3 qian, Dou Dou 2 qian, Zhi Niu Xi 8 fen, Shi Hu 1 liang, Mu Li crushed 4 liang, Zhi Gui Ban 2 liang. Ancient saying: Fat and white people often have Qi Deficiency. Another saying: Phlegm-Fluids must be harmonized with warm medicines. Self-administered resolving/dispersing drugs ineffective. Meng Ying diagnosed: Left Guan wiry, Cun slippery like pearls, Chi fine and dry, tongue tip very crimson. True Yin originally deficient, Water not nurturing Wood, Wind-Yang internally blazing, colliding fluids forming Phlegm. Claiming Qi Deficiency, hastily took Dang Shen (Codonopsis Radix), Gou Qi (Lycii Fructus), Dang Gui (Angelicae Sinensis Radix) etc. After swallowing, felt fire rising, Qi rebellious, gradually disjointed speech, frequent urination, spontaneous sweating, pulse void, hurried, irregular. Meng Ying immediately administered San Jia (Three Shells), Er Zhi (Two Solstices), Yuan Shen (Scrophulariae Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma), Xiao Mai (Tritici Fructus), Zhu Ye (Folium Bambusae), Lian Xin (Plumula Nelumbinis). To harmonize Heart-Liver Yang, suppress Dragon-Leopard Fire. One dose, calm. Subsequently worked hard, relapsed with contraction, still given light-clear method. Two doses later, due to anger and worry, Liver Yang rebelled again, malarial redness and facial redness, left ear occasionally deaf, sleepless at night, irritable spirit. Next day sudden profuse sweating, wetting clothes and bedding. Pulse extremely wiry, rapid, fine, still Yin Deficiency Yang Floating, do not mistake for Yang Deficiency, recklessly administer Aconite and Cinnamon. First ordered fumigation with charcoal vinegar, dusting with oyster powder. Then灌 (pour) large dose of Er Zhi (Two Solstices), Er Dong (Two Winters), San Jia (Three Shells), Yuan Shen (Scrophulariae Radix), Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma), Ren Shen (Ginseng Radix), Huang Lian (Coptidis Rhizoma), Tong Xiao (Child's Urine) and recovered.
