The second daughter-in-law of the Chen family contracted a seasonal epidemic pathogen (Shigan, seasonal flu-like illness) in March. She experienced rebellious Qi (Qi moving upward abnormally) preventing sleep. Mengying palpated her pulse as very slippery and rapid, diagnosing it as pregnancy. He advised that medication must account for this condition. After taking several doses, she recovered, and subsequently gave birth to a son.
Mengying further stated: "Yesterday, I treated Madam Wei, wife of Ziheng, who was also pregnant. Other physicians treated her for deficiency consumption (Xu Sun). Although her pulse was weak, thready, and rapid, the slippery quality remained evident. Her elder sister-in-law, Ms. Guan, died unmarried; her condition was not untreatable but was mismanaged by medication. During her illness, she read my medical cases and was deeply convinced. She said, 'If you had not come tonight, I would not have risen again.' The renowned doctors previously consulted merely stated that deficiency syndromes require tonification, without addressing the root cause of the deficiency. Their prescriptions clustered around tonic herbs, with Rehmanniae Radix (Dihuang) taking precedence. Even when Ginseng Radix (Renshen) or Astragali Radix (Huangqi) were used, they were listed later. Even if their medication did not violate principles, they had not grasped the doctrine that 'Yang generates Yin, and Qi is the commander of Blood,' let alone other aspects. By mid-autumn, news arrived that Madam Wei had delivered."
Shen Yunshu suffered from hematochezia (blood in stool) for over thirty years. His body was emaciated, waist painful, with coughing, phlegm, and rebellious Qi, seemingly suitable for warming and tonifying. Mengying palpated his pulse as wiry and rapid, observed a yellow tongue coating, and asked about short, red urine, concluding it was hemorrhoidal bleeding. It was likely due to the misuse of warming and tonifying methods. If he agreed to take my medicine, he could recover in ten days. The prescription used Weijing Decoction combined with Baitouweng Tang, adding Loquat Leaf (Pipaye), Inula Flower (Xuanfuhua), Platycladi Cacumen (Baiboye), and Lotus Root (Ou). This method combines lung-astringing and phlegm-dispelling with liver-clearing and blood-cooling. (Fresh Reed Rhizome [Lugen] 2 liang, raw Winter Melon Seed [Dongguazi] 4 qian, raw Peach Kernel [Taoren] ground 1 qian, wine-fried Baitouweng 1.5 qian, Melia Toosendan [Lianke] pounded and decocted first 3 qian, wine-fried Phellodendri Cortex [Huangbai] 1.5 qian, ginger-processed Loquat Leaf wrapped 3 qian, Inula Flower wrapped and decocted first 3 qian, wine-fried Platycladi Cacumen 1.5 qian, whole Lotus Root cut into pieces 2 liang boiled for decoction water. Wiry and rapid pulse indicates Yin deficiency with heat.) For postpartum conditions, unblocking stasis is paramount. However, some cases cannot be judged by common rules. Zhou Hetings' wife, after recent childbirth, suffered from sweating upon movement, inability to open her eyes, and extreme anxiety, with no lochia discharge. Her pulse showed a deficient, wiry, and spacious-large quality. Mengying administered Sanjia Yingshi Danshen Hupo Gancao Xiaomai Douyi Decoction. She felt relief after one cup and recovered after several doses. When questioned on how he knew it was not stasis, Mengying replied: "This is a Yin-deficient constitution. After childbirth, Ying fluids and blood are largely depleted, causing Wind-Yang to rise. Although there is no lochia, the chest and abdomen are comfortable. How could one mistakenly treat it as stasis rushing upward and recklessly administer blood-breaking drugs?" (The symptoms of sweating upon movement indicate rising Wind-Yang; absence of lochia shows it is clearly not an excess syndrome. Oyster Shell [Muli] pounded 6 liang, Blood Turtle Carapace [Biejia] pounded 4 liang, Blood Tortoise Plastron [Guiban] pounded 2 liang, Purple Quartz [Yingshi] pounded 5 qian, these four decocted first for eight zhong, wine-fried Salvia Miltiorrhiza [Danshen] 3 qian, Amber [Hupo] ground and taken with liquid 4 fen, Raw Licorice Root [Fangcaogen] 3 qian, Northern Wheat [Beixiaomai] 4 qian, Soybean Coat [Douyi] 5 qian. The formula's principle focuses on subduing Yang, extinguishing wind, and calming fright.)
Xu Jimie's wife suffered from malaria in mid-autumn. Treating it herself as cold-dampness worsened the condition. Her nephew Zhiqing believed it involved wind-summerheat and administered clearing and resolving formulas repeatedly without improvement. Mengying diagnosed a wiry, slippery, and surging pulse. The patient was corpulent and sweaty, with a yellow tongue coating, bloody stools, vomiting, thirst, delirium, insomnia all night, and a desire to lie on the floor. This indicated abundant internal latent phlegm disturbed by summerheat affecting the Yangming channel. He prescribed a large dose of Gypsum Fibrosum [Shigao], Anemarrhenae Rhizoma [Zhimu], Rhinoceros Horn [Xijiao] (now substituted), Scrophulariae Radix [Yuanshen], Dendrobii Caulis [Shihu], Lonicerae Flos [Yinhua], Scutellariae Radix [Huangqin], Trichosanthis Radix [Huafen], Eustomatis Folium [Lanye], and Bamboo Sap [Zhuli]. After three doses, the condition stabilized, followed by multiple doses for recovery. (Condition: Wind-summerheat with phlegm, stirring up Wind-Yang. Raw Gypsum [Shigao] decocted first 1.6 liang, sliced Rhinoceros Horn [Xijiao] decocted first 4 qian, Scrophulariae Radix slices soaked, liquid taken, dregs discarded 1 liang, wine-fried Anemarrhenae Rhizoma 4 qian, Dendrobii Caulis decocted first 1 liang, Jimo Lonicerae Flos 1.5 liang, wine-fried Scutellariae Radix 1.5 qian, Nanhua Trichosanthis Radix 5 qian, Jianlan Leaves added later 3 qian, Ginger-processed Bamboo Sap 2 large wine cups mixed in.)
Zhuang Zhijie, aged seventy, suffered from intermittent malaria. He shivered during chills and spoke deliriously during fever. Mengying observed a wiry, rapid, and hurried pulse, black tongue coating, and dry mouth. This indicated chronic hot phlegm with latent summerheat. He prescribed Anemarrhenae Rhizoma, Trichosanthis Radix, Scrophulariae Radix, Dendrobii Caulis, Scutellariae Radix, Bambusae Folium [Zhuru], Forsythiae Fructus [Lianqiao], Sea Cucumber [Haizhe], Radish [Lufu], and Nelumbinis Plumula [Lianzixin]. After a few sips, he improved. In mid-winter, he caught a wind pathogen, resulting in phlegmy cough, headache, anorexia, and chills. Self-medicating with Qianghuo, Perillae Folium, and Schizonepetae Spica caused worsening symptoms, including thirst and anuria. Mengying's pulse diagnosis matched autumn but was floating, indicating Wind-Warmth. Having taken warming dispersants, the heat intensified with wind. The crimson tongue with no moisture required clearing and transforming. He used Mulberry Leaf [Sangye], Loquat Leaf, Gardenia [Zhizi], Anemarrhenae Rhizoma, Winter Melon Seed, Scrophulariae Radix, Chrysanthemi Flos [Juhua], Trichosanthis Radix, Fritillariae Bulbus [Beimu], and Pear Juice [Lizhi]. Symptoms reduced immediately, and he recovered in ten days. (Previous condition: Heat excess with Yin deficiency, treating by purging heat and saving Yin. Later condition: Warming dispersants depleted fluids, treating by clearing and transforming cooling the lungs. Similar appearance, different reality. Former formula: Wine-fried Anemarrhenae Rhizoma 4 qian, Nanhua Trichosanthis Radix 5 qian, Scrophulariae Radix slices soaked and decocted, dregs discarded 1 liang, Dendrobii Caulis decocted first 1 liang, wine-fried Scutellariae Radix 1.5 qian, Ginger-processed Bambusae Folium 3 qian, Forsythiae Fructus shell 3 qian, light Sea Cucumber decocted first 2 liang, Radish cut 8 qian, Nelumbinis Plumula 1 qian. Latter formula: Winter Mulberry Leaf 4 qian, fresh Loquat Leaf brushed and wrapped 3 qian, Black Gardenia Peel 3 qian, wine-fried Anemarrhenae Rhizoma 3 qian, raw Winter Melon Seed 4 qian, Scrophulariae Radix slices soaked and taken, dregs discarded 8 qian, Nanhua Trichosanthis Radix 4 qian, Sichuan Fritillaria pounded 5 qian, whole Northern Pear 1 liang, juice extracted and taken.)
Sun Weishen's wife had chronic Yin deficiency and Liver stagnation, suffering from pain, distension, poor appetite, evening fever, emaciation, sore throat, and laryngeal herpes (Houxuan), unable to conceive for nine years. Previously, her menstruation was delayed, and people thought she would die. Mengying found her pulse not thin and skin moist, promising strategies for longevity with illness. Menstruation resumed, symptoms eased, and she could work, though appetite remained low. In autumn, menstruation had been absent since March, and old symptoms worsened. Pulse was wiry, slippery, and stronger on the left. Mengying exclaimed, "Not only will she live longer, but she dreams of bears (indicating pregnancy)." In late winter, she indeed gave birth to a son.
Weng Jiashun fell from a ladder last autumn, initially uninjured. Ten days later, sudden chills and fever developed with knee swelling and pain. Surgeon Wang treated it; after ulceration, it failed to heal. Physician Xu identified it as collateral injury, showing gradual improvement. However, year-long tonification led to muscle wasting, dark complexion, and limping, despite normal appetite. Every half-month, severe pain, swelling, chills, and fever occurred, confining her to bed for days, costing much money but worsening the condition. In winter, she developed throat erosion, gum decay, red eyes, and hoarseness. Mengying found a slippery, rapid pulse, crimson tongue, constipation, bad breath, and scant urine, indicating damp-heat steaming. This resulted from surgeons using only warming and supporting methods, followed by warming tonics after ulceration, ignoring the patient's constitution and pathology, trapping damp-heat, phlegm, and fire. The disease sought outlets, causing frequent chills and fever. Physicians mistook this for deficiency, tonifying for over a year, leading to exhaustion of both person and wealth. Mengying prescribed Scrophulariae Radix, Phellodendri Cortex, Anemarrhenae Rhizoma, Glycyrrhizae Radix, Lonicerae Flos, Trichosanthis Radix, Mung Bean [Lvdou], Gardenia, Sea Cucumber, and Eleocharis Tuber [Biqi] in large doses, applied Xilei Powder externally, and instructed daily consumption of pear, sugarcane, Gracilaria [Qilincai], and persimmon cakes. After fifty days, all symptoms vanished, and she regained strength and mobility. (When healthy Qi is strong, pathogenic factors are also strong; this case is treatable as shown by slippery/rapid pulse and bad breath. Scrophulariae Radix slices soaked, liquid taken, dregs discarded 1 liang, wine-fried Phellodendri Cortex 1.5 qian, wine-fried Anemarrhenae Rhizoma 3 qian, Jimo Lonicerae Flos 8 qian, Raw Licorice Root 3 qian, Nanhua Trichosanthis Radix 5 qian, Mung Bean pounded 1 liang, Black Gardenia Peel 3 qian, light Sea Cucumber decocted first 2 liang, whole Eleocharis Tuber 2 liang.)
Chen Xiaoyan's daughter-in-law missed one period, then menstruated the next month. Mengying diagnosed pregnancy based on a small but weak Chi pulse with gentle onset and termination. It proved correct. Tu Zhongru's younger sister-in-law had slightly delayed menstruation. Mengying accidentally diagnosed pregnancy, which was confirmed. At three months, she suffered threatened miscarriage (Tai Lou). Another physician failed to preserve the fetus, resulting in abortion. Post-abortion lochia flowed, but she had chills, fever, headache, spontaneous sweating, and a sensation of cold originating from the heart. Physicians gave warming-transforming medicines, worsening the condition. Eight days later, Mengying found a deep, solid, and rapid pulse with a purple-dark tongue coating, diagnosing blood stasis. He prescribed Peach Kernel [Tao Ren], Polygonati Odorati Rhizoma [Zelan], Hawthorn Fruit [Shanzha], Leonuri Fructus [Chongwei], Inula Flower, Safflower [Honghua], Salvia Miltiorrhiza, Akebia Caulis [Tongcao], Amber, Arcaea Concha [Gekiao], and Luffa Sponge [Sigualuo]. Severe abdominal pain followed, expelling a piece of stasis resembling lung tissue. Symptoms improved next day, and lactation began. The formula was repeated minus Tongcao, plus Malta [Maita]. Abdominal pain recurred, expelling more stasis clots, and symptoms disappeared. Asked why he treated stasis despite flowing lochia and no pain, Mengying explained: "Normal delivery is like a ripe melon falling; blood nourishing the fetus flows freely. Stasis causes pain. Abortion is like squeezing an unripe abscess; roots remain. Fetal blood gathers but doesn't disperse; shallow parts exit, deep parts stay. With a blood-abundant constitution and warming-upward medicines, the natural exit is blocked. Using meridian-unblocking and Qi-moving methods guided the stasis downward, opening the exit. This requires specific pulse signs; not all abortions present this way." (Formula: Raw Peach Kernel ground 3 qian, Zelan Leaf 3 qian, Charred Hawthorn pounded 1.5 qian, Leonuri Fructus pounded and decocted first 4 qian, Original Safflower 6 fen, Salvia Miltiorrhiza 3 qian, Inula Flower wrapped and decocted first 3 qian, Akebia Caulis 1 liang, Arcaea Concha pounded 1 liang, Luffa Sponge 5 qian, these four boiled for clear soup to decoct other herbs, Amber ground and taken with liquid 8 fen. Later, remove Tongcao, add Raw Malta 8 qian.)
Suo Rongting's sister had seasonal malaria. Physician Gu cleared heat and resolved pathogens, improving appetite and sleep. Suddenly, she developed rebellious Qi, cold limbs, fatigue, drowsiness, deafness, slurred speech, anorexia, but normal bowel movements without pain. Knowing her history of blood loss and chronic Qi deficiency, Gu switched to Ginseng and Aconite, worsening the condition. Mengying found a wiry-slow pulse and yellow-greasy coating, diagnosing early pregnancy with Qi obstruction and phlegm condensation blocking pivot mechanisms. Treatment required venting and resolving phlegm using Acori Tatarinowii Rhizoma [Shichangpu], Immature Bitter Orange [Zhishi], Inula Flower, Pinelliae Rhizoma [Banxia], Coptidis Rhizoma [Huanglian], Poria [Fuling], Tangerine Peel [Chenpi], Scallion White [Congbai], Sea Cucumber, and Bamboo Sap. Immediate effect, fully recovered after three doses. Subsequently, Suo Shengxian's wife had malaria progressing to epigastric fullness, vomiting, hiccup, cold nose, sweating, anorexia, insomnia, and intermittent pulse. Physicians feared for her life. Mengying diagnosed phlegm, using the same formula minus Scallion and Bamboo Sap, adding Allii Macrostemonis Bulbus [Xiebai] and Trichosanthis Semen [Gualouren], Bambusae Folium. Effective. (Phlegm-resolving and Qi-moving formula: Acori Tatarinowii Rhizoma added later 2 qian, Stir-fried Immature Bitter Orange 1 qian, Inula Flower wrapped and decocted first 3 qian, Processed Pinelliae Rhizoma 2 qian, Ginger-processed Coptidis Rhizoma 8 fen, Cloud Poria 3 qian, Tangerine Peel 1 qian, Fresh Scallion White added later 1.5 qian, Light Sea Cucumber decocted first 1 liang, Ginger-processed Bamboo Sap 1 wine cup mixed. For Suo's wife, remove Scallion and Bamboo Sap, add fresh Allii Macrostemonis Bulbus pounded, added later 3 qian, Ginger-processed Trichosanthis Semen ground 3 qian.)
Gao Shiquan's daughter-in-law had small bones and fragile flesh, inherently deficient. Winter fatigue led to anorexia, insomnia, insecurity, night sweats, and tinnitus. Physician Feng continuously administered Yin-nourishing medicines, worsening the condition. Mengying observed hyperactive Left Cun pulse and wiry-slippery bilateral Guan pulses with yellow-greasy coating, diagnosing internal blazing of Heart-Liver Fire and failing descent of Stomach Qi. While Yin deficiency was the root, mere filling was inappropriate. Regulating ascent and descent was necessary before tonification. He prescribed Salt-processed Coptidis Rhizoma, Acori Tatarinowii Rhizoma, Scrophulariae Radix, Salvia Miltiorrhiza, Gardenia, Dendrobii Caulis, Wheat [Xiaomai], Anemarrhenae Rhizoma, Ophiopogonis Tuber [Maidong], Bamboo Leaf [Zhuye], and Nelumbinis Plumula. Immediate response. Follow-up with Ligustri Lucidi Fructus [Nvzhenzi], Ecliptae Herba [Hanliancao], Oyster Shell, Tortoise Plastron, and Rehmanniae Radix for recovery. Classics state: "No good prognosis for wheezing; wheezing with profuse sweating is critical." (Regulating ascent/descent and calming Heart/Liver formula: Salt-processed Coptidis Rhizoma 1 qian, Acori Tatarinowii Rhizoma added later 2 qian, Scrophulariae Radix slices 1 liang, Flower Maidong 4 qian, these two soaked, liquid taken, dregs discarded, Salvia Miltiorrhiza 3 qian, Black Gardenia Peel 1.5 qian, Dendrobii Caulis decocted first 1 liang, Northern Wheat 4 qian, wine-fried Anemarrhenae Rhizoma 3 qian, fresh Bamboo Leaf 2 qian, Nelumbinis Plumula 1 qian. Effectiveness lies in bitter-pungent combination. For Yin deficiency with rising fire, sudden bitter-cold purging or sweet-cold moistening fails to rapidly descend floating fire. If internal Qi mechanism does not descend, medicine is ineffective. The principle is modifying heavy doses to light administration, allowing Yin medicines to float to Upper Jiao Qi level and then descend, calming fire and gathering Qi, enabling Yin medicines to work. Later, remove Coptidis, Acori, Gardenia, Dendrobii, Wheat, Bamboo Leaf, Nelumbinis Plumula, Ophiopogonis; add Ligustri Lucidi Fructus pounded 5 qian, Ecliptae Herba 4 qian, Oyster Shell pounded 4 liang, Blood Tortoise Plastron pounded 2 liang, these two decocted first for eight zhong, Large Raw Rehmanniae Radix 8 qian soaked and decocted, dregs discarded.)
Pan Kentang's wife suddenly developed asthma in mid-winter, worsening with treatment. Mengying diagnosed: "Both Qi Kou pulses are governed by Lung channel. Now Lung is obstructed by phlegm, Qi circulation blocked. Though deficiency and urgency appear, it may not be true deficiency. She is young, eats well, acute onset, greasy coating, thick phlegm. Despite cold feet, red face, anorexia, insomnia, sweating, these are merely phlegm blocking pivot mechanisms, ascending without descending." He prescribed Gypsum, Scutellariae Radix, Anemarrhenae Rhizoma, Trichosanthis Radix, Inula Flower, Magnetitum [Zhashi], Trichosanthis Semen, Akebia Caulis, Sea Cucumber, Bamboo Sap, Radish Juice, and Pear Juice. Improvement after one dose, stability after two. Removed Gypsum and Magnetitum, added Scrophulariae Radix and Apricot Kernel [Xingren], stable after ten days. Once phlegm-cough cleared, used Glehniae Radix [Shashen], Rehmanniae Radix, and Ophiopogonis Tuber for Yin nourishment. (Raw Gypsum 1.2 liang, Raw Magnetitum pounded 5 qian, decocted first for six zhong, wine-fried Anemarrhenae Rhizoma 4 qian, Ginger-juice processed Trichosanthis Radix 4 qian, Inula Flower wrapped and decocted first 3 qian, Ginger-processed Trichosanthis Semen ground 4 qian, Fang Akebia Caulis 3 qian, Light Sea Cucumber decocted first 1 liang, Ginger-processed Bamboo Sap 2 wine cups mixed, whole Radish and whole Northern Pear each 1 liang juiced and mixed. Later, remove Gypsum and Magnetitum, add Scrophulariae Radix slices 5 qian soaked, liquid taken, dregs discarded, Bitter Apricot Kernel 1.5 qian. Once phlegm-cough cleared, remove Scutellariae Radix, Trichosanthis Radix, Inula Flower, Akebia Caulis, Bamboo Sap, Radish Juice, add Glehniae Radix 8 qian, Large Raw Rehmanniae Radix 8 qian, Flower Maidong 4 qian, these two soaked and decocted, dregs discarded.)
Young unmarried women often suffer depression; dry cough stems from constrained fire. Wang Qiting's sister, past twenty, suddenly developed incessant dry cough, sleeplessness, unresponsive to medicine. Mengying diagnosed overflowing pulses bilaterally, wiry-thin on left, thirst, no tongue coating. True Yin long deficient, Wind-Yang usurping, Chong vessel cough unceasing, risk of collapse. Prescribed Oyster Shell, Tortoise Plastron, Soft-shelled Turtle Carapace [Biejia], Purple Quartz, Cistanche Caulis [Rongrong], Poria, Prepared Rehmanniae Radix [Shudi], Angelicae Sinensis Radix [Guishen], Achyranthes Bidentatae Radix [Niuxi], Cordyceps [Chongcao], and Walnut Meat [Hutaorou]. Medicine just brewed, she nearly collapsed; forced feeding induced sleep. Next evening, chills and spasms recurred; still fed previous medicine. Third night, only night sweats. Four doses eliminated symptoms, restoring sleep and appetite. Delaying such Yang-subduing and rebellion-suppressing formulas by one or two days could lead to unknown changes. Treating as constraint with opening/draining agents would be worse. Thus, pattern differentiation is paramount. (Chong vessel connects to Liver; Chong cough is Liver Yang attacking Lung. Large Prepared Rehmanniae Radix 1 liang soaked, liquid used to decoct first Oyster Shell pounded 6 liang, Blood Soft-shelled Turtle Carapace pounded 4 liang, Blood Tortoise Plastron pounded 1 liang, Purple Quartz 3 qian, these four decocted first for six hours, liquid used to decoct light Cistanche Caulis 1.5 qian, Cloud Poria 3 qian, Angelicae Sinensis Radix 2 qian, Processed Achyranthes Bidentatae Radix 5 fen, Cordyceps 1.5 qian, Walnut Meat with skin ground 3 qian.)
Yuyi Cao states wind-cold can present with Dai Yang syndrome, referring to the elderly. However, deceptive cases exist. Huang Dingru's mother, very old, caught wind-cold in late winter. Phlegm-cough, rebellious Qi, forehead sweat, malar flush, epigastric fullness, anorexia, restlessness. Mengying diagnosed wiry-rapid-hurried Left pulse, slippery-surging-right pulse, full tongue coating. Winter-Warmth with phlegm obstructing Lung, governing function impaired, Liver Yang driving upward. Similar to Luo Qianfu's case of phlegm-fire mimicking lone Yang. Prescribed Xiao Xianxiong Tang plus Allii Macrostemonis Bulbus, Inula Flower, Magnetitum, Trichosanthis Radix, Sea Cucumber, Eleocharis Tuber, Bamboo Sap. Phlegm moved, bowels opened, recovered in days. Subsequently, Chen Shunting's father, over sixty, had phlegm-cough and rebellious Qi, relieved only by Ginger Soup. Physicians deemed true deficiency-cold, administering warming tonics, leading to sore throat, anorexia, gray-spiky coating, constipation, anuria. Mengying diagnosed double-wiry pulse, empty on pressure, no Stomach Qi. "Thirst for Ginger Soup indicates phlegm obstructing Clear Yang, not cold. Repeated harsh medicines exhausted Stomach Yin; medicine is useless." (Dai Yang syndrome: True cold false heat, facial flush, restlessness, cured by strong Yang tonification. This case: Right pulse slippery-surging, Winter-Warmth with phlegm obstructing Lung, governing function impaired, Liver Yang reversing upward. Dispelling phlegm-heat calms Wind-Yang. Ginger-juice processed Coptidis Rhizoma 2 qian, Ginger-processed Trichosanthis Semen ground 3 qian, Processed Pinelliae Rhizoma 1 qian, fresh Allii Macrostemonis Bulbus pounded 1.5 qian, Inula Flower wrapped and decocted first 3 qian, Raw Magnetitum pounded and decocted first 8 qian, Ginger-processed Trichosanthis Radix 4 qian, Light Sea Cucumber decocted first 2 liang, whole Eleocharis Tuber pounded 1 liang, Ginger-processed Bamboo Sap 1 large wine cup mixed.)
Dong Yuan stated: "After middle age, descending functions prevail, Clear Yang easily sinks, lifting is appropriate." Zhao Juzhai, over sixty, strained himself running, developing prolapsed anus and hemorrhoids, anuria. Physicians planned Buzhong Yiqi Tang and Shenqi Wan. Mengying palpated slippery-rapid pulse, greasy-stagnant coating. Chronic alcoholism, internal damp-heat accumulation, strain caused pathogen sinking. Forcing back anal prolusion blocked Bladder Qi, causing difficult urination. Not Kidney Fire failure nor Clear Yang sinking. Formula: Plantaginis Semen [Cheqianzi], Akebia Caulis, Linderae Radix [Wuyao], Cyperi Rhizoma [Yanhusu], Gardenia, Citrus Aurantium [Juhe], Meliae Semen [Jinlingzi], Alismatis Rhizoma [Zexie], Semina Lygodii [Haijinsha] to regulate Bladder Qi transformation and leach water. Urination gradually resumed. Switched to Saposhnikoviae Radix [Fangfeng], Sanguisorbae Radix [Diyu], Moutan Cortex [Danpi], Lonicerae Flos, Schizonepetae Spica, Sophorae Flos [Huairui], Dendrobii Caulis, Coptidis Rhizoma, Angelicae Sinensis Radix to clear Blood-level heat and guide dampness. Hemorrhoids flattened. Without pattern differentiation, using lifting/warming tonics would block Qi further, causing turbid water to ascend, especially dangerous in the elderly. (Plantaginis Semen pounded and decocted first 4 qian, Fang Akebia Caulis 3 qian, Tai Linderae Radix 1.5 qian, Yuanhusu 1 qian, Black Gardenia Peel 3 qian, Citrus Aurantium pounded and decocted first 1 qian, Meliae Semen pounded and decocted first 3 qian, Raw Alismatis Rhizoma 3 qian, Semina Lygodii wrapped and decocted first 4 qian. Switched to Qing Saposhnikoviae Radix 1.5 qian, wine-fried Sanguisorbae Radix Charcoal 2 qian, Raw Schizonepetae Spica 2 qian, wine-fried Moutan Cortex 1 qian, Jimo Lonicerae Flos 1 liang, wine-fried Sophorae Flos 1.5 qian, Dendrobii Caulis decocted first 5 qian, wine-decocted Coptidis Rhizoma 8 fen, Angelicae Sinensis Radix Tail 1 qian.)
Xu Zhiqing recovered in late autumn after Mengying cleared latent summerheat. His maternal aunt, knowledgeable in medicine, forcibly administered Xiaochaihu Tang, worsening the condition. Mengying prescribed Wendan Tang minus Licorice, plus Raw Gypsum, Scutellariae Radix, Anemarrhenae Rhizoma, Trichosanthis Radix, Radish, stabilizing him. Relapse due to early labor, cleared by Physician Zhao. Late winter relocation fatigue caused recurrent malaria, facial/foot edema, asthma-cough, irritability. Left pulse wiry-strong-rapid, right slippery-large irregular, yellow-greasy coating, thirst, polyuria. Lung-Stomach phlegm-heat excess, Liver-Gallbladder Wind-Yang usurping. Fear of deficiency and reckless tonification would fail.
