经曰∶天枢隐隐痛者大肠疽;其上肉微起者大肠痈。(天枢穴,又名大肠募,大肠募在脐旁开二寸。)

The Classic states: Dull pain at the Tianshu point indicates Large Intestine Carbuncle (Dachang Ju); slight elevation of the flesh at this location indicates Large Intestine Abscess (Dachang Yong). (Tianshu [ST25], also known as the Front-Mu point of the Large Intestine, is located 2 cun lateral to the umbilicus.)
Chen Yuan Gong said: Severe abdominal pain where the abdomen cannot be pressed, with the right leg flexed and unable to extend, is often mistaken by people for excessive internal heat and food stagnation. Who would know it is an abscess of the Large Intestine! In all cases of abdominal pain accompanied by inability to extend the legs, it is an intestinal abscess; in Large Intestine Abscess, the leg is particularly unable to extend. However, Large Intestine Abscess invariably arises from fire. When fire is excessive and does not dissipate, it condenses and forms an abscess. The excess of fire stems fundamentally from a deficiency of water. When water declines, fire flares up; without water to control it, fire nurtures toxicity. The treatment method need not target the fire directly; by strengthening water to control fire, the toxic factors will naturally dissipate. Qingchang Yin (Clear the Intestines Drink): Honeysuckle Flower (Jinyinhua) 3 liang, Angelicae Sinensis Radix (Danggui) 2 liang, Sanguisorbae Radix (Diyu), Ophiopogonis Radix (Maidong), Scrophulariae Radix (Yuanshen) each 1 liang, Coicis Semen (Yiren) 5 qian, Raw Licorice Root (Sheng Gancao) 3 qian, Scutellariae Radix (Huangqin) 2 qian. Decoct in water and take orally. After one dose, pain subsides slightly; after two doses, the leg can extend; after another two doses, the toxin is completely cleared. (This formula invigorates blood and resolves toxicity; although it drains fire, it essentially nourishes yin. The two complement each other, yielding miraculous effects. If one does not nourish yin to moisten the intestines but only attacks toxicity to drain fire, the Large Intestine will be damaged first, making it unable to withstand the scorching fire-toxin. No wonder treatment fails. *Bingjian*)
He further said: In Large Intestine Abscess, the right leg does not extend, abdominal pain is severe, pus and blood are discharged, and the anus feels like being cut by a knife; this indicates ulceration. Those who can eat survive; those who cannot eat die. However, among those who cannot eat, some cases are not due to blazing fire-toxin, and one should not abandon treatment simply because the patient cannot eat. For all carbuncles and sores, the presence of Stomach Qi is auspicious. Regardless of whether the toxin is yin or yang, if Stomach Qi is depleted, it is generally incurable. Therefore, for yin carbuncles, supporting Stomach Qi must be the primary priority. Adding a small amount of herbs to resolve pus and detoxify ensures that righteous qi is not injured, and fire-toxin disperses easily. Now, if the Large Intestine abscess has ruptured and there is no desire to eat, Stomach Qi is exhausted, representing a critical condition. Failing to urgently tonify the Stomach and instead only treating the abscess leads to death. Kaiwei Jiuwang Tang (Open the Stomach and Save from Collapse Decoction): Ginseng Radix et Rhizoma (Renshen), Dioscoreae Rhizoma (Shanyao), Coicis Semen (Yiren), Scrophulariae Radix (Yuanshen), Atractodis Macrocephalae Rhizoma (Baizhu) each 1 liang; Honeysuckle Flower (Jinyinhua) 2 liang; Raw Licorice Root (Sheng Gancao) 3 qian; Caprinae Sanguis (Shanyangxue) 1 qian, ground into powder. Decoct in water, mix with the powder, and take orally. After one dose, Stomach opens; after two doses, pus decreases; after three doses, pain stops; after four doses, complete recovery. (This formula saves the Stomach while resolving pus and detoxifying within it. Its brilliance lies in Honeysuckle Flower treating toxicity while still being a yin-nourishing herb, acting as the general who saves lives in sore diseases, a commander of utmost benevolence and courage. Assisted by Ginseng and Atractylodes, its ability to disperse toxins is divine. Goat Blood stops bleeding, quenches thirst, and promotes qi circulation, guiding other herbs directly into the abscess to disperse it. Harmonized together, with proper management, support, and attack, how could it fail? Naturally, Stomach Qi opens, transforming essence and transmitting it to the Large Intestine. If Stomach Qi is not yet damaged, the effect is even more pronounced. Do not doubt and refrain from using it, thereby wasting human life.)
He again said: In Large Intestine Abscess, there is severe lower abdominal pain, dribbling urination, mental fatigue, loss of taste, sallow complexion, weakness in the four limbs, spontaneous sweating, night sweats, and insomnia. People assume it is caused by excessive fire generating an abscess, but who knows it is actually water deficiency failing to moisten the intestines! The Large Intestine's ability to conduct relies entirely on the infusion of Kidney Water. Now, due to drunkenness, overeating, and sexual indiscretion, vital energy is excessively damaged, leading to massive depletion of essence. This causes fire to move and water to dry up. Combined with the intake of cold and raw foods, qi and blood become discordant, dampness stirs, phlegm generates, intestinal and gastric obstruction occurs, transformation and transportation cease, qi and blood stagnate, and an abscess forms. Thus, prior to the abscess forming, there was already a deficiency of Kidney Water. After the abscess ulcerates, further loss of water exacerbates the deficiency. If treated as fire-toxin, it rarely avoids becoming a fatal condition. One must greatly tonify Kidney Water and supplement Spleen and Stomach Qi. Then, the Spleen and Stomach's transformation of essence to generate water becomes easier, and the intestines, once receiving abundant moisture, will naturally recover. The abscess resolves without direct treatment, and as qi and blood become sufficient, muscles regenerate. Jia Wei Liuwei Dihuang Wan (Modified Six-Ingredient Pill with Rehmannia) to treat this: Prepared Rehmannia Root (Shudi) 2 liang, Dioscoreae Rhizoma (Shanyao), Corni Fruit (Shanzhuyu) each 8 qian; Moutan Cortex (Danpi) 6 qian, Alisma Rhizoma (Zexie) 1 qian, Poria (Baifuling) 3 qian; Ginseng Radix et Rhizoma (Renshen), Ophiopogonis Radix (Maidong) each 1 liang; Astragali Radix (Huangqi) 5 qian. Decoct in water and take orally. After several doses, pain stops and spirit strengthens. (Note: Liuwei Dihuang Wan tonifies Kidney Water; adding Ginseng, Astragalus, and Ophiopogon tonifies Spleen and Stomach Earth. When Earth is robust, Lung Metal flourishes naturally. The Lung and Large Intestine are exterior-interior partners, and the Lung is the mother of the Kidney. Mother and child support each other, and exterior-interior aspects care for one another, thus achieving miraculous results.)
Wang Kentang said: Large Intestine Abscess often presents with a sensation of heaviness and swelling during defecation. In the early stage, there is fever, aversion to wind, and spontaneous sweating. The skin is rough and scaly like fish scales, and pressing reveals acute pain within the abdomen.
