Nang Yong (Scrotal Abscess) refers to an abscess swelling in the scrotum. This condition is governed by the Foot Jueyin Liver Meridian and arises from Yin deficiency of the Liver and Kidney meridians, with Damp-Heat sinking downward. In the initial stage, presenting with pain and swelling, and dark, scanty, or painful urination, Longdan Xiegan Tang (Gentiana Root Decoction to Drain the Liver) should be used to clear heat, drain dampness, and detoxify, or herbs such as Huangqin (Scutellariae Radix), Lianzi (Coptidis Rhizoma), Huangbai (Phellodendri Cortex), Shanzhi (Gardeniae Fructus), Yiren (Coicis Semen), Mutong (Akebiae Caulis), Gancao (Glycyrrhizae Radix et Rhizoma), and Danggui (Angelicae Sinensis Radix) may be selected. If pus has formed but urination is difficult, this indicates obstruction and closure by Heat-Toxin; Tuoli Xiaodu San (Decoction to Support the Interior and Eliminate Toxins) should be used first, followed by needle drainage. If pus has been discharged but pain and swelling persist, it signifies that Heat-Toxin has not yet resolved; Qinggan Yying Tang (Decoction to Clear the Liver and Nourish the Blood) should be employed. This syndrome results from deficiency of the anterior yin passages and impediment of Damp-Heat; therefore, herbs for draining dampness and nourishing Yin are indispensable. If after ulceration the pus is thin or profuse, or healing is delayed, Shiquan Dabu Tang (Decoction for Tenfold Great Tonification) added with Shanyurou (Corni Fructus), Danpi (Moutan Cortex), and Zexie (Alismatis Rhizoma) must be used to tonify. If deficiency is not treated with tonification, vigorous individuals often develop chronic stubborn diseases, while frail elderly individuals often succumb to death.

Another condition is Tuo Nang (Scrotal Sloughing), which begins with alternating chills and fever, redness of the scrotum, swelling of the testicles, wet cracking of the skin, turning black after two days, and intermittent putrefaction every other day. Within a few days, the entire scrotum sloughs off, leaving the testicles exposed and hanging, appearing critical but actually not fatal; this is caused by Damp-Heat flowing downward. Apply Zhenzhu San (Pearl Powder) externally, cover with Baiyu Gao (White Jade Ointment), and internally administer Siling San (Four-Powder Decoction) or Bixie Tang (Dioscorea Hypoglauca Decoction). There is also a condition called Shui Han (Hydrocele), characterized by pain and swelling with shiny skin color, no heat or redness, and accumulated fluid inside. It is suitable for puncture with a needle to drain the fluid, after which peace is restored; internally, Wuling San (Five-Ingredient Powder with Poria) or similar diuretic herbs for draining dampness should be taken.

Xuan Yong (Hanging Abscess) occurs behind the scrotum and in front of the anus, also known as Haidi Lou (Bottom-of-the-Sea Fistula), and is the most difficult to heal. Patients suffering from this are extremely deficient, with the condition arising from deficiency of the Three Foot-Yin Meridians and accumulation of Damp-Heat. Initially, it resembles a lotus seed; over time, it gradually enlarges to the size of a plum or peach, becoming red, swollen, and painful. After ulceration, mild cases form fistulas, while severe cases exhaust Qi and Blood, often transforming into Lao Zhai (Consumption Disease). Treatment methods: For initial pain and swelling with dark, scanty urination, indicating Liver Meridian Damp-Heat, Longdan Xiegan Tang is the primary prescription. If accompanied by swelling and fever, clear the Liver and eliminate toxins using Xiao Chaihu Tang (Minor Bupleurum Decoction) with Banxia (Pinelliae Rhizoma) and Renshen (Ginseng Radix) removed, and Cheqian (Plantaginis Seed), Huangbai, Xiong (Ligustici Rhizoma et Radix), Guizhi (Cinnamomi Ramulus - *Note: Context implies Danggui/Angelica based on typical formulas, but text says Xiong/Guizhi? Text says "Xiong, Gui". Assuming Xiong = Chuanxiong/Costus Root, Gui = Danggui/Chinese Angelica*), and Gancao added. For already ulcerated cases, use Bazhen Tang (Eight-Treasure Decoction) with processed Gancao, Chaishu Shao (Bupleuri Caudex tip), wine-fried Huangbai, and Zhimu (Anemarrhenae Rhizoma). One must strictly avoid excessive use of cold-natured herbs to prevent damaging Stomach Qi. Only Zhi Gancao (Processed Licorice Root), named Guolao San (State Elder Powder), does not damage Qi and Blood nor disturb the Zang-Fu organs, and its efficacy is rapid; it is most suitable for use.
