Qin Zi said: The syndrome of labor injury (Laoshang) is the syndrome of fever and cough, and phthisis with bone-steaming sensation. Currently, patients are numerous. Examining the *Neijing* (Classic of Internal Medicine), there are only mentions of "deficiency" (Xu), but no mention of "labor"; however, in the section on etiology, there is the statement that diseases arise from deficiency, which can be termed deficiency-labor (Xulao). It was not until the *Jingui Yaolue* (Essential Prescriptions of the Golden Cabinet) that the syndrome of deficiency-labor was elucidated, theories were established, and prescriptions were formulated, thereby making the syndrome of labor injury prominent. When Chao Yuanfang wrote the *Zhubing Yuanhou Lun* (Treatise on the Pathogenesis and Manifestations of All Diseases), he distinguished five types as labor and seven types as injury, thus clarifying the meaning of labor and injury; later, the establishment of six extremes (Liují) and thirty-three steamings (San-shisan Zheng) seemed overly complicated and fragmented. I now carefully define the injuries to essence, qi, and blood, and the labors of the five zang organs, as labor injury. Based on what is stated in the *Yixue Jingyao* (Medical Essentials), contracting cold leading to labor and contracting heat leading to labor, entering from the exterior into the interior, are classified as external contraction (Waigan); while qi deficiency with spirit injury, exhaustion of essence and dryness of blood, originating from within to cause disease, are classified as internal injury (Neishang). I separately establish two categories for external contraction and eight for internal injury, thus completing the general outline and details.
[Syndrome of Labor Injury due to Cold Contraction] At the onset, there is aversion to cold and fever, cough with rebellious qi, stabbing pain in the hypochondrium and ribs, or absence of sweating with body heat, or coolness in the morning and heat in the evening. This is the syndrome of labor injury due to cold contraction or wind-strike.
[Etiology of Labor Injury due to Cold Contraction] The *Xuanzhu* states that individuals with a weak constitution are most susceptible to pathogenic factors. If the pathogen is not expelled and tonifying agents are administered immediately, or if one does not abstain from meat and alcohol, the pathogen receives support from the tonics, remains stagnant, causing fever. Heat damages lung qi, resulting in wheezing and coughing. This is the etiology of labor injury due to cold contraction.
[Pulse of Labor Injury due to Cold Contraction] A floating-tight pulse (Fu Jin Mai) is commonly seen. A floating-tight pulse on the left indicates cold contraction in the blood level; a floating-tight pulse on the right indicates cold contraction in the qi level.
[Treatment of Labor Injury due to Cold Contraction] For a floating-tight pulse on the left, indicating cold contraction in the blood level, treat with Qianghuo Chaihu Tang (Notopterygium and Bupleurum Decoction), adding Chuanxiong (Ligusticum Rhizome) and Shaoyao (Peony Root). For a floating-tight pulse at the right Guan position, indicating cold contraction in the qi level, treat with Gange Fangfeng Tang (Pueraria and Saposhnikovia Decoction), adding Zisu (Perilla Leaf) and Guangpi (Citrus Peel).
Qianghuo Chaihu Tang (Notopterygium and Bupleurum Decoction)
Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Chaihu (Bupleuri Radix), Fangfeng (Saposhnikoviae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Baishaoyao (Paeoniae Radix Alba)
Gange Fangfeng Tang (Pueraria and Saposhnikovia Decoction)
Gange (Puerariae Lobatae Radix), Fangfeng (Saposhnikoviae Radix), Jingjie (Schizonepetae Spica), Chaihu (Bupleuri Radix), Zisu (Perillae Folium), Guangpi (Citri Reticulatae Pericarpium)
