Treatment for men, women, and children of all ages presenting with inability to open the eyes, blindness, and dilated pupils.
Use Mahuang Rong (Ephedrae Herba prepared as floss) 8 liang, Bei Wuwei Zi (Schisandrae Chinensis Fructus) 1 jin, and Lao Sheng Jiang (Zingiberis Rhizoma Recens) 1 jin. Take two doses.
Then add Sheng Dang Shen (Codonopsis Radix) 1 jin, and take two doses. If ineffective, add Jiao Zhu (Atractylodis Macrocephalae Rhizoma stir-fried) 1 jin and Sheng Dang Shen 1 jin, and take two doses. If still ineffective, use Shu Di Yi Ba Ji Tang (Rehmanniae Radix Preparata and Morindae Officinalis Radix Decoction) with additional Bei Wuwei Zi for several doses. After symptoms improve, take Buzhong Yiqi Tang (Decoction for Replenishing the Middle and Augmenting the Qi) for three to four doses to consolidate the effect. For wealthy families, taking Dabu Huanjing Wan (Great Tonifying Eye-Recovery Pill) is even better. This pill is made by replacing Dang Shen in the previous formula with Ren Shen (Ginseng Radix) or Gaoli Shen (Korean Ginseng Radix) 1 jin.
In cases where female patients present with dilated pupils, it is often due to Hong Beng (severe menstrual bleeding) or excessive Bai Dai (leukorrhea), indicating a Blood Deficiency syndrome. It is appropriate to use Bei Wuwei Zi 1 jin, taken continuously for two to three or four doses.
Also use Hong Danshen (Salviae Miltiorrhizae Radix et Rhizoma) 0.5 jin and Dang Gui Shen (Angelicae Sinensis Radix body) 0.5 jin, decocted in water and taken orally.
Also use Chao Bai Shao (Paeoniae Radix Alba stir-fried) 4 liang, Danshen 8 liang, Chuan Xiong (Ligustici Chuanxiong Rhizoma) 2 liang, and Quan Dang Gui (Angelicae Sinensis Radix whole) 4 liang, taken for two doses.
Also use Bei Wuwei Zi 4 liang, Hong Danshen 4 liang, Chao Bai Shao 2 liang, Chuan Xiong 2 liang, and Dang Gui 2 liang, decocted in water and taken orally.
If the pupil returns to its normal size but vision remains blurred, it is likely due to anger injuring the Liver.
Also use Zhi Qiao (Aurantii Fructus) 2 liang, Bing Lang (Arecae Semen) 1 liang, Xiang Fu (Cyperii Rhizoma) 2 liang, and Guang Yu Jin (Curcumae Longae Radix) 2 liang (crushed), decocted in water. Separately, use Sha Ren (Amomi Fructus) 1 liang ground into powder, mixed into the decoction before administration. Strictly avoid foods that obstruct Qi movement. After taking four to five doses, continuous flatulence or diarrhea indicates significant efficacy. This signifies turbid Qi descending and clear Qi ascending. Subsequently, administer Buzhong Yiqi Tang continuously for three to seven doses.
On the following day, use Sheng Dang Shen 1 jin, taken for one dose.
Then use Jiao Zhu 8 liang and Sheng Dang Shen 8 liang, taken for one dose until recovery.
For dilated pupils, avoid Qing Xi Zi (Celosiae Semen), Chu Shi Zi (Broussonetiae Fructus), Chong Wei Zi (Leonuri Fructus), Yi Mu Cao (Leoniti Herba), and Yuan Zhi Rou (Polygalae Radix without heart).
Treatment for men and women with constricted pupils, photophobia, and aversion to sunlight, which is caused by Kidney Deficiency.
Use Sheng Di Huang (Rehmanniae Radix) 4 qian, Dang Gui 1.5 qian, Bai Shao 1.5 qian, Chuan Xiong 1.5 qian, Jiu Chao Huang Lian (Coptidis Rhizoma wine-fried) 1.5 qian, Jiu Chao Zhi Mu (Anemarrhenae Rhizoma wine-fried) 3 qian, Jiu Chao Huang Bo (Phellodendri Cortex wine-fried) 3 qian, Jiu Chao Du Huo (Angelicae Pubescentis Radix wine-fried) 3 qian, Yun Ling (Poria) 3 qian, Cao Jue Ming (Cassiae Semen) 3 qian, Gou Qi Zi (Lycii Fructus) 3 qian, Han Shui Shi (Halloysitum Rubrum) 3 qian (calcined and powdered), and Qing Xi Zi 5 qian. Take seven to eight or more than ten doses.
Mix and form pills. Take 3 qian each time, washed down with plain boiled water.
This condition involves Kidney Heat; avoid Wu Wei Zi, Zao Ren, Hua Jiao, and Shan Jiao. All herbs containing seeds should be crushed.
Treatment for internal cataracts in men and women with dilated pupils, resulting from the deficiency of the Three Yin meridians. This formula also treats external cataracts with red filaments throughout the eye, photophobia, and aversion to sunlight.
Use Ze Xie (Alismatis Rhizoma) 2 qian, Chao Fen Dan Pi (Moutan Cortex stir-fried) 2 qian, Bei Wu Wei Zi 40 berries, and Gui Wei (Angelicae Sinensis Radix tail) 2 qian.
For internal cataracts, use Gui Shen (Angelicae Sinensis Radix body), Chai Hu (Bupleuri Radix), Huai Shan (Dioscoreae Hypoglaucae Rhizoma), Zao Pi (Corni Fructus), and Shu Di Huang (Rehmanniae Radix Preparata) each 2 qian, Sheng Di Huang 3 qian, and Yun Ling 2 qian.
Decoct in water and take for several doses. If ineffective, take Shu Di Yi Ba Ji Tang. Abdominal rumbling and continuous flatulence indicate turbid Qi descending and clear Qi ascending. Diarrhea is not a concern; instead, take Shu Di Zao Pi Tang (Rehmanniae Radix Preparata and Corni Fructus Decoction). Lower abdominal heat and continuous flatulence are highly effective signs.
Subsequently, use Buzhong Yiqi Tang to conclude treatment.
Shu Di Yi Ba Ji Tang
Shu Di Huang 4 to 8 qian, Yi Zhi Ren (Alpiniae Oxyphyllae Fructus salt-stir-fried) and Zao Pi each 2 to 4 qian.
Treatment for internal cataracts in men and women with inverted pupils (upward deviation of the eyeball). This formula should be taken after exposure to light: Da Shu Di Huang (Great Rehmanniae Radix Preparata) 2 jin, Suo Sha Ren (Amomi Fructus) 2 liang (stir-fried with ginger juice and powdered), mixed into the Shu Di Huang.
Separately, decoct Zhi Qiao in water to wash down the Shu Di Huang. Afterward, take Buzhong Yiqi Tang for two doses to tonify Middle Qi.
When treating internal cataracts and the patient can begin to see people, use this formula: Cun Dong (Ophiopogonis Tuber) 2 liang (heart removed), Yuan Zhi 1 liang (heart removed, stir-fried with ginger juice), and Jian Chang Pu (Acori Tatarinowii Rhizoma) 3 qian. Decoct and take for three to four doses.
Thereafter, take Buzhong Yiqi Tang for several doses to conclude treatment. Strictly abstain from sexual activity for one hundred days.
Another formula uses Yuan Zhi 1 liang, Dang Shen 2 liang, Zhi Cun Dong (Ophiopogonis Tuber honey-roasted, heart removed) 1 liang, and Jian Chang Pu 3 qian (wine-fried), without a guiding herb.
These two formulas calm the spirit, stabilize the will, and tonify the Heart.

Add Ye Ming Sha (Tylopidae Faeces) 2 liang, mix well, steam, and consume.
Treatment for men, women, and children who see distant objects clearly but near objects poorly, or experience cold tears and sticky saliva. This indicates sufficient Qi but deficient Water (Kidney Yin Deficiency).
Use Yuan Shu (Prepared Rehmanniae Radix) 4 liang and Zao Pi 2 liang, decocted in water, taken for two consecutive doses. Remove Zao Pi and add Zhi Qiao 1 liang (stir-fried).
Another formula removes Zhi Qiao and adds Bei Wu Wei Zi 3 liang, taken for three to four consecutive doses until cured.
Treatment for internal cataracts in men, women, and children where the black pupil protrudes, hangs down to the nose, cannot retract, and causes severe pain. Or accompanied by hematochezia (blood in stool).
First, use Da Fa San (Major Dispersing Powder) one dose, specifically this formula: Xi Qiang Huo (Notopterygii Rhizoma et Radix) 2 jin, ground into powder, divided in half, formed into pills the size of wutong seeds with paste. Take 50-60 pills each time, three times a day. Use the remaining half of the Qiang Huo powder, 1 liang each time, decocted in water to swallow the pills, at any time.
If the eyeball does not retract, continue taking it until the disease resolves. Once the eyeball retracts, use Buzhong Yiqi Tang removing Fen Cao (Licorice Root), taken for three to four doses to conclude treatment.
Another formula uses Xi Qiang Huo 4 liang, decocted for fumigation and washing. The patient should wash it themselves to gauge intensity. Manually massage the eyeball back into the orbit. Afterwards, drink the Qiang Huo decoction. One large bowl each time, three times a day, until the eyeball is fully retracted.
Once stable, use Buzhong Yiqi Tang removing Fen Cao and Chai Hu, adding Di Yu (Sanguisorbae Radix) 5 qian, Dang Shen 1 liang, Jian Qi (Astragali Radix) 1 liang, and Jiao Zhu 1 liang. Take three to four doses to eradicate the root cause.
Treatment for internal cataracts in men, women, and children with blood flooding the pupil (hyphema or vitreous hemorrhage), causing severe pain. If not treated urgently, the eye will swell and rupture. Women often suffer from this. Differentiation must be precise before medication. If the pupil within the black cornea appears red, it is indeed blood flooding the pupil. Treatment must absolutely avoid lifting herbs, as the pupil belongs to the Kidney and the black cornea to the Liver; the primary approach is to soothe and level the Liver and Kidney.
Use Hong Yang Shen (Red American Ginseng) 8 qian (rice-fried), Pao Shen (Codonopsis Radix) 1 liang (raw), Xuan Shen (Scrophulariae Radix) 1 liang, Ku Shen (Sophorae Flavescentis Radix) 1 liang (wine-fried), Huang Lian (Coptidis Rhizoma) 5 qian, and Xiang Fu 3 qian (fried with child's urine).
Grind into powder. Take 5 qian each time, washed down with plain boiled water. This formula also treats internal cataracts caused by accidental ingestion of Danshen pills. If the blood disperses, use the Second Formula.
Second Formula: Xuan Shen, Tian Dong (Asparagi Radix), Zhi Qiao, Qiang Huo, Chuan Xiong, Fang Feng (Wind-Expelling Herb), Zhi He Shou Wu (Polygoni Multiflori Radix Preparata) each 3 qian, Guang Mu Zei (Equiseti Herba) 3 qian (nodes removed), Quan Dang Gui, Cun Dong each 3 qian, Bai Ji Li (Tribuli Fructus) 3 qian (thorns removed), Chuan Niu Xi (Cyathulae Radix) 3 qian (wine-fried), Xing Ren Shuang (Apricot Seed Paste) 3 qian, Sheng Di Huang 5 qian, Xia Ku Cao (Prunellae Spica) 3 qian. Decoct in water and take. Avoid alcohol and sexual activity.
If headache occurs, remove Sheng Di Huang and He Shou Wu, add Ming Tian Ma (Gastrodiae Rhizoma) 5 qian and Gao Ben (Ligustici Wallichii Rhizoma) 5 qian. If pain is mild, no modification is needed; take for three to four doses. Grind another four to five doses into powder, refine with honey into pills, and take alongside the decoction.
Treatment for blood flooding the pupil in men and women.
Use He Huan Shu Ye (Albiziae Cortex Leaf), dried and ground into powder. Combine with one strip of Zhu Lian Tie (pig eyelid tissue), cut into seven sections, and steam together with the powder. Consume one section per day. Cure upon completion. If not cured, prepare another strip and consume; it will surely cure.
Another formula uses Dang Gui Wei (Angelicae Sinensis Radix tail) 5 qian, Chuan Xiong, Bai Ju (Chrysanthemi Flos), Guang Pi (Citri Reticulatae Pericarpium), and Sheng Di Huang each 4 qian, Qiang Huo, Bai Shao, Zhi Zi (Gardeniae Fructus), Yun Ling, Ji Li, and Gan Cao each 3 qian, with Deng Xin Cao (Junci Medulla) as a guide. If constipation is present, add Jiu Jun (Rhei Radix et Rhizoma processed with wine). If not cured, use the Second Formula.
Second Formula: Qiang Huo Teng Feng Tang (Qiang Huo Wind-Expelling Decoction), also known as Ren Shen Bai Du San (Ginseng Detoxification Decoction), modified according to symptoms, taken for three doses. If not cured, use the Third Formula.
Third Formula: Tian Xing Ren Shuang (Sweet Apricot Seed Paste), Fang Feng, Xuan Shen, Bai Ji Li, Xiang Fu, Quan Dang Gui, Niu Xi (wine-fried), Tian Dong, Zhi He Shou Wu, Guang Mu Zei, Chao Zhi Qiao, Xia Ku Cun, Cun Dong (heart removed), Qiang Huo, and Chuan Xiong each 3 qian, Sheng Di Huang 5 qian. Use Dang Shen, Qiang Huo, Du Huo, Qian Hu (Peucedani Radix), Chai Hu, Fu Ling, and Gan Cao each 3 qian, Zhi Qiao, Jie Geng (Platycodonis Radix), and Chuan Xiong each 3 qian.
Treatment for internal cataracts in men and women with green fluid flooding the pupil (green glaucoma-like symptoms), affecting either both eyes or one eye over two days. Use this formula: Shu Di Huang 3 jin, Huai Zao Ren (Ziziphi Spinosae Semen) 1.5 jin. Ensure thorough cleaning. Stir-fry with pig heart blood until fragrant and grind into powder. Place Shu Di Huang on top in a steamer to absorb the Zao Ren powder before consumption.
Alternatively, decoct Shu Di Huang in water and mix with Zao Ren powder for consumption. Take twice daily, each time consuming 8 liang of Shu Di Huang and 4 liang of Zao Ren. If insomnia occurs, use stir-fried Zao Ren to ensure sleep.
After finishing this course, take Buzhong Yiqi Tang with Long Yan (Longan Aril) as a guide, or alternatively use raw Zhi Qiao 2 liang as a guide. Take continuously for four to five doses.
This condition often presents with dilated pupils and greenish eyeballs. Use Bei Wu Wei Zi 1 jin, taken for two consecutive doses.
If the color does not return to black, it indicates upward rushing of deficient Qi. Use Sheng Dang Shen 1-2 jin, decocted and consumed to replenish Qi. If light perception returns, take Qianjin Ci Zhu Wan (Thousand Golden Magnetite and Cinnabar Pill) in the morning and Shi Hu Ye Guang Wan (Dendrobium Night Vision Pill) in the evening.

Key Principles for Treating the Root of Eye Diseases
For men with purulent tearing and all internal cataract deficiency eye diseases, there is invariably spermatorrhea. Treat the spermatorrhea first, then treat the eye disease. Otherwise, effectiveness is difficult to achieve.
For women with eye diseases that come and go, or blue membranes/clouds obscuring the vision, there is invariably severe leukorrhea. Treat the leukorrhea first, then treat the eye disease, for effectiveness. Otherwise, it is difficult to cure.
For treating male spermatorrhea and female leukorrhea, first take Da Fa San for one to two doses to disperse old cold completely. Then use Shu Di Yi Ba Ji Tang, or Shu Di Zao Pi Tang, or Liu Wei Di Huang Tang. All three formulas should additionally include Hong Qi (Goji Berry) and Qian Shi (Euryales Semen) each 3 qian for better results.
Contraindicated Medications and Processing Methods for Eye Diseases
When treating eye diseases, Gui (Cinnamon Twig) must be strictly avoided. Reviewing all formulas in this book reveals no use of Gui.
For eye diseases with green discoloration, avoid Huang Jing Zi (Vitex Negundo Fruit).
For dilated pupils, avoid Chu Shi Zi, Qing Xi Zi, Chong Wei Zi, Yi Mu Cao, and Yuan Zhi Rou.
For constricted pupils, avoid Bei Wu Wei Zi, Zao Ren, Hua Jiao, Shan Jiao, Hu Jiao (Black Pepper), and Huo Cao Jiao.
For external cataracts, avoid tonics, cooling herbs, raw/cold foods, wind/cold/dampness, and strong liquor. External cataracts are Cold patterns.
For internal cataracts, avoid astringent Qi herbs and Qi-stagnating substances.
When eating raw Fu Zi (Aconite), avoid sexual activity, pork, mutton, chicken, dog, fish, and all raw/cold foods. Scallions and garlic must especially be avoided; violation leads to death.
When eating Tian Xiong (Aconite Lateralis Radix Preparata) up to 1 liang, take it cold. Taking it hot may cause deficient Fire to attack upward, resulting in throat swelling and pain.
All herbs containing seeds should be crushed; Man Jing Zi should be ground.
When using Sheng Jiang, crush it with the skin; Old Ginger is preferable.
When using Shu Di Huang, always first take Da Fa San with heavy Mahuang Rong to disperse old cold completely, then use Shu Di Huang, as it tends to stagnate cold.
When using Ming Tian Ma and Jiang Can (Bombyx Batryticatus), stir-fry with ginger juice; Quan Chong (Whole Scorpion) and Chong Tui (Snake Slough), wash with wine; Bei Xi Xin, remove leaves completely; Yi Zhi Ren, remove shell and stir-fry with salt water; Ba Ji (Morinda Officinalis), remove heart; Chong Tui, remove wings/legs and wash mud off with wine; Cun Dong, remove heart; Mu Zei, remove nodes. Other herbs should be processed as required. (Note: Snake slough washed with wine, insect slough wings/legs removed).
When taking Danshen, avoid sour and salty flavors.
When taking Di Huang, avoid Lai Fu Zi (Radish Seed), scallions, garlic, copper, and iron.
My commentary: There are many more contraindicated and processed herbs for eye diseases not listed in the original text. Their application depends on the practitioner's discretion in clinical practice.
