The syndrome of acute convulsion (Ji Jing) is generated by heat. When heat invades the Heart, phlegm obstructs the Lungs; extreme heat generates wind, and wind movement induces惊 (convulsion). In severe cases, symptoms such as spasms (Chuo Nuo), stiff falling (Jiang Pu - lying supine or prone), upward deviation of the eyes (Fan Mu), and the gradual appearance of the eight signs (Ba Hou - typical manifestations of pediatric convulsion) occur. Before a child's convulsion is about to manifest, their spirit is invariably unsettled, appearing vague and fearful of people, looking left then right, observing above then below. At this stage, it is imperative to clear heat, descend phlegm, and calm/nourish the heart spirit to prevent the condition from worsening. If there is only body extension, spasms, and upward gazing, but the jaw is not tightly clenched, there is no phlegm rattle in the throat, and no saliva foam in the mouth, this is not convulsion. This may be due to accompanying fright, food stagnation, cold damage (Shang Han), smallpox/rubella (Dou Zhen), or heat accumulation in the San Jiao (Triple Burner), causing dysfunction of the five zang organs. Pathogenic factors flow into the meridians, where heat affects the sinews and vessels, also causing spasms. Treatment should involve purging methods according to the severity. The pulse is floating, rapid, full, and tight; the vein pattern on the tiger's mouth (Hu Kou) shows equal parts blue and purple. The treatment should primarily utilize Li Jing Wan (Detoxicate Convulsion Pill), Dao Chi San (Guide Out Red Powder) (see Fire section for formula), and An Shen Wan (Calm Spirit Pill). (These can be used in conjunction with previous formulas).
Main Formula (Li Jing Wan): Long Dan Cao (Gentianae Radix), Qing Dai (Indigo Naturalis, used by painters), Fang Feng (Saposhnikoviae Radix) (each 3 qian), Gou Teng (Uncariae Ramulus cum Uncis) (2 qian), Huang Lian (Coptidis Rhizoma) (1 qian), Niu Huang (Calculus Bovis), She Xiang (Moschus), Bing Pian (Borneolum Syntheticum) (each 2 fen). Grind Qing Dai, Niu Huang, She Xiang, and Bing Pian into fine powder. Mix this powder evenly with the other four ingredients. Use Shen Qu (Medicated Leaven) paste to form pills the size of millet seeds. Take 30 pills each time, delivered with a decoction of Jin Yin Hua (Lonicerae Japonicae Flos) and Bo He (Menthae Haplocalycis Herba).
An Shen Wan: Treats pediatric palpitations, thirst and oppression, red face and flushed cheeks, dry mouth, and solid pulse.
Formula: Ya Xiao (Natrii Sulfas), Shan Yao (Dioscoreae Rhizoma), Mai Men Dong (Ophiopogonis Tuber), Fu Shen (Poria with Hostwood), Gan Cao (Glycyrrhizae Radix et Rhizoma) (each 5 fen), Zhu Sha (Cinnabaris) (water-ground, 1 qian).
Preparation and Dosage: Grind all ingredients into fine powder. Form pills the size of a chicken head using honey. Take half a pill each time, delivered with sugar water.
Zhen Jing Zao Ming Dan: Treats fetal fright, acute and chronic convulsions. Epilepsy with loss of consciousness, upward staring eyes, phlegm obstruction due to convulsion, startle during sleep, restless night sleep, incessant crying, Ke Wu (external malevolent qi invasion), and internal injuries. It treats all kinds of fright diseases from infancy to adulthood, including strange shapes/symptoms that cannot be named, as well as adults suffering from fright, worry, and overwork leading to insomnia, palpitations, vagueness, fear, mania, postpartum aphonia in women, and confusion with inappropriate laughter or crying. (This is a miraculous medicine.)
Formula: She Han Shi (Hematitum) (lightly roasted until red-hot, quenched in rice vinegar; repeat roasting and quenching seven times, grind fine, water-grind, settle, remove water, dry, and grind again ten thousand times. 4 liang), Dai Zhe Shi (Haematitum) (ground, as above), Chen Sha (Cinnabaris) (water-ground), Qing Meng Shi (Calcareous Schist treated with Niter) (niter-treated, calcined to golden color, water-ground, re-ground), Nan Xing (Arisaematis Rhizoma prepared with ox bile), Fu Shen (Poria with Hostwood) (each 5 qian), Jiang Can (Bombyx Batryticatus) (washed, stir-fried), Chan Tui (Cicadae Periostracum) (cleaned of soil), Bai Fu Zi (Typhonii Rhizoma), Shi Jun Zi (Quisqualis Fructus), Tian Ma (Gastrodiae Rhizoma) (each ground to powder, 3 qian), Niu Huang (Calculus Bovis) (from Shaanxi, 7 fen), She Xiang (Moschus) (5 fen), Bing Pian (Borneolum Syntheticum) (3 fen).
Preparation and Dosage: Grind uniformly, mix with refined honey to form pills, coat with gold foil. Adults take 2 qian, children take 1 qian, infants take 3-5 fen. Dissolve in a decoction of Deng Xin Cao (Junci Medullae) and Bo He (Menthae Haplocalycis Herba). A decoction of Jin Yin Hua (Lonicerae Japonicae Flos) is particularly good for delivery.
Bao Long Wan: Treats conditions caused by wind-cold, summerheat, fever, Bian Zheng (physiological changes in infant development), phlegm obstruction due to convulsion, wheezing, and cough. It is suitable for conditioning after convulsions, for any slight discomfort from infancy to adulthood, and is effective for adult wind-phlegm syndromes.
Formula: Nan Xing (Arisaematis Rhizoma prepared with ox bile in the twelfth lunar month, multiple preparations are better. 4 liang), Tian Zhu Huang (Bambusae Concretio Silicea) (separately ground, 1 liang), Zhu Sha (Cinnabaris) (water-ground), Xiong Huang (Realgar) (sublimed/ground. Each 5 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma) (7 qian), She Xiang (Moschus) (5 fen, do not add if smallpox has not erupted), Niu Huang (Calculus Bovis) (from Shaanxi, Qian Ceng - likely referring to quality/layering), Hu Po (Electrum) (genuine. Each 5 fen, especially effective.).
Preparation and Dosage: Mix uniformly. Use snow water from the twelfth lunar month or well water drawn at dawn. Separately decoct Gan Cao (Glycyrrhizae Radix et Rhizoma) juice. Mix to form pills the size of a Chinese chestnut. Take one pill each time, dissolve in Bo He (Menthae Haplocalycis Herba) tea before sleep. Increase dosage for urgent conditions. For wind-cold, deliver with a decoction of Sheng Jiang (Zingiberis Rhizoma), Cong Bai (Allii Fistulosi Bulbus), and Zi Su Ye (Perillae Folium) to induce sweating. Adults take 3-5 pills each time. For wind-phlegm, dissolve in Zhu Li (Bambusae Succus) mixed with a few spoons of Sheng Jiang (Zingiberis Rhizoma) juice. Ginger soup is also acceptable.
