(Attachment: Zhongshu [Heatstroke], Shufeng [Summerwind], Zhuxia [Summer Malaise])
The Classics state: Heat illness occurring after the Summer Solstice is termed Shu (Summerheat). It also states: A weak pulse accompanied by body heat results from injury by Summerheat. Generally, during the Summer season, Ministerial Fire commands and intense heat prevails; contracting pathogenic factors leads to disease, which differs in Yin/Yang and Motion/Stillness. For instance, laborers who work and run under scorching sun and intense heat contract this through "motion," representing Yang syndrome of Zhonger (Heatstroke). Symptoms must include dizziness, irritability, aversion to heat, dry teeth and tongue, burning skin upon touch, extreme thirst with desire to drink, profuse sweating, and lethargy due to Qi deficiency. This is caused by Summerheat-Heat externally damaging Yuan Qi (Original Qi). Coolant formulas are suitable, such as Yiyuan San (Beneficial Origin Powder) or Huanglian Xiangru Yin (Coptis and Elsholtzia Decoction); for deficiency patterns, Qingshu Yiqi Tang (Clear Summerheat and Boost Qi Decoction) or Shengmai San (Generate Pulse Powder) may be used. Conversely, wealthy and idle individuals who seek coolness in deep halls and large mansions contract this through "stillness," representing Yin syndrome of Shangshu (Injury by Summerheat). Symptoms must include headache and bone pain, aversion to cold, bodily stiffness and tension, vexation and chest fullness, fever without sweating. This is caused by Yin-Cold blocking Yang Qi. Acrid-warm formulas are suitable, such as Huoxiang Zhengqi San (Patchouli and Regulatory Qi Powder) (see Vol. 2 end), Suopi Yin (Spleet-Suppressing Drink), or Lizhong Tang (Regulate the Middle Decoction) (formula see Cold Injury chapter) plus Fuzi (Prepared Aconite Root) for severe cases. Zhonger presents with a vacuous and large pulse; Shangshu presents with a string-tight pulse.
Main Formula (Treats Shangshu Yin-Cold): Ganjiang (Dried Ginger Rhizome), Houpo (Magnolia Bark), Cangzhu (Atractylodis Rhizoma), Fuling (Poria), Chenpi (Citrus Reticulatae Pericarpium), Zhike (Aurantii Fructus Immaturus), Qianghuo (Notopterygii Rhizoma et Radix), Baizhi (Angelicae Dahuricae Radix) (each 1 qian), Gancao (Licorice Root) (0.5 qian). Add fresh ginger and green onion, decoct and take orally to induce sweating.
For headache, add Chuanxiong (Ligusticum Chuanxiong Rhizome) (1 qian) and Xixin (Asari Radix et Rhizoma) (0.5 qian); for food stagnation, add Shanzha (Crataegi Fructus) and Maiya (Hordei Fructus Germinatus).
Shiwei Xiangru Yin (Ten-Ingredient Elsholtzia Decoction): Treats Zhonger with thirst, vomiting or diarrhea, and all Summerheat syndromes.
Xiangru (Elsholtziae Herba) (2 qian), Houpo (Magnolia Bark), Biandou (Lablab Bean Seed), Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria), Chenpi (Citrus Reticulatae Pericarpium), Huangqin (Scutellariae Radix), Mugua (Chaenomelis Fructus) (each 1 qian), Gancao (Licorice Root) (0.5 qian).
Decoct with water and take cold. If heat is severe, add Huanglian (Coptidis Rhizoma); if thirst is severe, add Shigao (Gypsum Fibrosum) and Zhimu (Anemarrhenae Rhizoma).
Yiyuan San (Beneficial Origin Powder) (also known as Liuyi San [Six-to-One Powder] or Tianshui San [Heavenly Water Powder]): Treats Zhongshu with body heat, vexation/thirst, diarrhea, difficult urination, leukorrhea, and dysentery. It is a sacred medicine that dries dampness and removes heat, promotes urination and strengthens the Spleen, transforms food and moves stagnation, expels blood stasis, and clears fire pathogens. It is indispensable for long journeys in summer. If urine is red and scanty, combine with Wuling San (Five-Ling Powder) (formula see Lin Syndrome chapter).
Huashi (Talcum) (water-flowered, 6 liang), Gancao powder (Licorice Root powder) (1 liang), ground uniformly. Take 2 qian each time, at any time, mixed with Dengxin Tang (Junci Medullae Decoction). For heat and thirst, use Xinji Shui (freshly drawn cold water).
Qingshu Yiqi Tang (Clear Summerheat and Boost Qi Decoction): Treats Zhongshu with sweating, panting and shortness of breath, and reluctance to speak due to movement.
Cangzhu (Atractylodis Rhizoma), Huangqi (Astragali Radix), Shengma (Cimicifugae Rhizoma) (each 1 qian), Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Chenpi (Citrus Reticulatae Pericarpium), Maidong (Ophiopogonis Radix), Shenqu (Massa Medicata Fermentata), Zexie (Alismatis Rhizoma) (each 0.5 qian), Huangbai (Phellodendri Cortex), Danggui (Angelicae Sinensis Radix), Qingpi (Aurantii Fructus Immaturus), Gegen (Puerariae Lobatae Radix), Gancao (Licorice Root) (each 0.3 qian), Wuweizi (Schisandrae Chinensis Fructus) (9 grains). Decoct with water and take orally.
Suopi Yin (Spleet-Suppressing Drink): Resolves latent Summerheat, eliminates vexation and thirst, dissipates Summerheat toxicity, stops vomiting/diarrhea, and treats cholera.
Gegen (Puerariae Lobatae Radix), Baibian Dou (Lablab Bean Seed) (stir-fried), Gancao (Licorice Root) (each 1 qian), Wumei Rou (Mume Semen), Sharen (Amomi Fructus), Caoguo (Tsaoko Fructus) (each 0.5 qian). Decoct with water and take cold.
Shengmai San (Generate Pulse Powder): Generates fluids and boosts Qi, moistens the Lungs and nourishes the Heart. Suitable for summer when Yin is internal and Yang is external, presenting with vexation and thirst.
Renshen (Ginseng Radix et Rhizoma) (1 qian), Maimendong (Ophiopogonis Radix) (2 qian), Wuweizi (Schisandrae Chinensis Fructus) (9 grains). Decoct with water and take at any time.
Simple Prescription: Treats Zhongshu. Take soil from cart wheels, mix with water, let settle, and drink the clear liquid; or dig a pit, fill with water, stir, let settle, and drink the clear water.
Another prescription: Crush garlic and hot soil, mix with water, remove dregs, and take orally. Or boil Shuiliao (Polygonum Hydropiper) juice and administer.
Another prescription: Si Gua Ye (Luffae Folium) (one leaf), Bai Yanmei (Salted Plum) (one whole, including seed), grind finely, and swallow with freshly drawn cold water. Also treats Zhongshu and cholera.
Method: For those who faint due to exposure to Summerheat, drinking water immediately causes death. Move them to a cool place, scoop up hot road soil to form a nest on the umbilicus, pour urine from multiple people into the nest; then administer Tongbian (Child's Urine), followed by Di Jiang Shui (Earth Slurry Water) mentioned above, and fan slowly until they recover.
