The syndrome belongs to wandering fire and accumulated heat in the Upper Jiao (Upper Burner). The Lung Metal is steamed by this heat, disturbing the pure Qi and impairing the functions of transformation and production. Consequently, symptoms such as cough, blood-tinged sputum, alternating chills and fever, spontaneous sweating, frequent urination, turbid saliva in the mouth, and excessive secretion of phlegm-drool occur. A rapid and deficient pulse at the Cun position indicates this condition. Additionally, excessive sweating, severe vomiting, or inappropriate diuresis for Xiao Ke (Wasting-Thirst), or harsh purgation for deficiency-induced constipation, can severely deplete body fluids and daily damage True Yin. This leads to Lung dryness and also results in Fei Wei (Lung Flaccidity). If coughing is accompanied by aversion to wind and distension/fullness in the chest and hypochondrium, one should guard against developing this ailment.
Main Formula: Bei Mu (Fritillariae Bulbus), Zhi Mu (Anemarrhenae Rhizoma), Zi Wan (Asteris Radix), E Jiao (Colla Corii Asini), Huang Qi (Astragali Radix), Fu Ling (Poria), Ren Shen (Ginseng Radix et Rhizoma) (each 1 qian); Jie Geng (Platycodonis Radix), Mai Dong (Ophiopogonis Tuber), Dong Hua Hua (Farfarae Flos), Bo He (Menthae Haplocalycis Herba) (each 0.7 qian); Gan Cao (Licorice Root), Wu Wei Zi (Schisandrae Fructus) (12 grains); Ginger slice (1 piece). Decoct in water, dissolve E Jiao, and take. For alternating chills and fever, add Chai Hu (Bupleuri Radix) and Huang Qin (Scutellariae Radix). For increased phlegm, add Ju Hong (Citri Reticulatae Pericarpium Rubrum) and Zhi Ban Xia (Pinelliae Rhizoma Preparatum). For Blood deficiency, add Dang Gui (Angelicae Sinensis Radix) and Bai Shao (Paeoniae Radix Alba). For dry throat and thirst, decoct separately: Sheng Jiang (Zingiberis Rhizoma Recens) (5 qian), Ren Shen (2 qian), Gan Cao (3 qian), Da Zao (Jujubae Fructus) (3 pieces). For excessive drool without thirst, use Gui Zhi (Cinnamomi Ramulus), Gan Cao (2 qian), Da Zao (6 pieces), Zao Jiao (Gleditsiae Spina) (2 strips, roasted, skin and fibers removed), decoct in water and divide into two doses.
Bao Fei Yin (Lung-Protecting Drink): Treats Fei Wei with cough, dull pain in the chest, and persistent dry cough.
Formula: Ren Shen, Mai Dong, Yi Yi Ren (Coicis Semen), Bai Bu (Stemonae Radix), Huang Qi, Sang Bai Pi (Mori Cortex), Wu Wei Zi, Dang Gui, Shao Yao (Paeoniae Radix, wine-fried), Pian Huang Qin (Scutellariae Radix), Bai He (Lilii Bulbus) (equal parts). Add ginger, decoct in water, and take.
The Classics state: Dian Ji (Mania-Epilepsy) and Jue Kuang (Convulsive Madness) arise from long-term reversal of Qi. It is also said: When Yang Qi is excessively high, it leads to Kuang Dian (Madness-Epilepsy). When Yang is entirely above and Yin rises from below, resulting in deficiency below and excess above, Dian Kuang occurs. It is further stated: Heavy Yin leads to Dian (Epilepsy/Depressive Psychosis), while Heavy Yang leads to Kuang (Mania). It is also said: When Yin and Yang contend repeatedly, the patient desires to climb high and sing, discards clothes, and runs about. Regarding the onset of Kuang disease: initial symptoms include little sleep, lack of hunger, self-perception of worthiness, wisdom, and nobility; speech lacks discrimination between good and bad, ignoring kinship and distance; this is a disorder of the Shen (Spirit). The cause of contracting the disease lies in extreme anger, fear, joy, or unfulfilled aspirations; or loss/gain, or fright, which disturbs the person's Shen. The Heart is the residence of the Spirit; when the Spirit departs, the residence becomes empty, allowing body fluids to enter and transform into Phlegm that confuses the Heart Orifices. Phlegm stagnates and transforms into Fire; since Fire nature flares upward, the patient climbs high and shouts loudly. Alternatively, grievance cannot be expressed, anger cannot be vented; Liver Wood and Gallbladder Fire flare up to disturb the Heart; Heart Fire surges, the Spirit residence loses control, and long-term Qi reversal causes the disease. A solid, large, firm, and slippery pulse indicates survival; a wiry, rapid, and deficient pulse indicates death. Treatment should focus on descending Fire, clearing Phlegm, calming the Heart, and tranquilizing the Spirit.
Adjunctive intake: Gun Tan Wan (Rolling Phlegm Pill), Huo Tan Wan (Expelling Phlegm Pill) (see Phlegm Section), Zao Ming Dan (Life-Creating Pill) (see Pediatric Fright Section).
Main Formula: Huang Lian (Coptidis Rhizoma), Huang Qin, Zhi Zi (Gardeniae Fructus, stir-baked), Lian Qiao (Forsythiae Fructus), Fu Shen (Poria with Hostwood), Yuan Zhi (Polygalae Radix), Zao Ren (Ziziphi Spinosomi Semen, stir-baked, crushed), Shi Chang Pu (Acori Tatarinowii Rhizoma), Dang Gui, Sheng Di Huang (Rehmanniae Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma) (equal parts); Gan Cao (half dose). Add Deng Xin Cao (Junci Medullia) and Zhu Ye (Bambusae Folium), decoct in water, mix with Chen Sha (Cinnabar), and take. For deficiency, add Ren Shen and Bai Zhu (Atractylodis Macrocephalae Rhizoma). For Excess Heat with severe constipation, add Da Huang (Rhei Radix et Rhizoma) or Fang Feng Tong Sheng San (Wind-Avoiding Universal Powder) (see Wind Section). For increased phlegm, add Ju Hong, Nan Xing (Arisaematis Cum Bile), Ban Xia, Tian Ma (Gastrodiae Rhizoma), Bai Fu Zi (Typhonii Rhizoma), Hai Shi (Spongia), Gua Lou (Trichosanthis Fructus), Zhu Li (Bambusae Sap), and remove Yuan Zhi, Zao Ren, Dang Gui, Sheng Di Huang.

Formula: Wei Jin (Shu aromatic herb, 6 liang), Ming Fan (Alumen, 3 liang).
Usage: Grind to fine powder, bind with thin paste to form pills the size of tung tree seeds. Take 50 pills per dose with white boiled water.
Simple Prescription: For Yang Kuang with blazing Fire and thirst, melt Lead, mix with equal parts Shui Yin, stir and grind finely. Take 0.3 qian, mixed in one bowl of water. During severe mania, giving water to drink can calm the patient.
For this syndrome, investigate the cause of illness and accommodate the patient's wishes. Mania caused by extreme joy should be suppressed with fear. Consuming "Xini" (likely a specific local remedy or food item mentioned in original context) clears Phlegm and Fire, which is highly beneficial. Avoid fresh pork, mutton, strong flavors, alcohol, and foods that generate Fire or move Qi aggressively.
