Summerheat is a Yang pathogen. Upon infection, it enters through the mouth and nose, first obstructing the Qi aspect of the Upper Jiao (Upper Burner), resulting in head distension and epigastric stuffiness. It gradually progresses to facial grime, tongue coating changes, irritability, thirst, and spontaneous sweating. Heat causes Qi leakage.
Alternatively, there may be vomiting, nausea, abdominal pain, diarrhea, cold limbs, fatigue, and listlessness. This is what the *Classic* refers to as "Heat injures Qi." Zhang Zhongjing stated that the pulse for injured summerheat is deficient. The Lung governs Qi; summer fire scorches Metal (Lung), injuring the Lung and causing Qi deficiency. The Heart governs Blood; summerheat enters the Heart first, causing irritability, sweating, and a deficient pulse. This is why *Shengmai San* (Generate the Pulse Powder) from *Qianjin Fang* emphasizes protecting the Lung and clearing the Heart. The *Classic* states: "Due to summerheat, sweating occurs; if irritable, there is panting and thirst; if quiet, there is excessive talking." Generally, summerheat disturbing the Ying (Nutritive) aspect internally causes sweating; pressing upwards on the Lung causes panting; interfering internally with the Heart causes excessive talking. Thus, we know summerheat initially injures the Lung and subsequently transmits to the Pericardium. Jiezhai said: Diseases occurring after the Summer Solstice involving heat are called summerheat. When the Ministerial Fire is in command, humans contract this pathogen, entering via the mouth and teeth, injuring the Pericardium Meridian of Hand-Jueyin. Symptoms include irritability, thirst, fever, headache, and spontaneous sweating. In severe cases, fiery heat controls Metal (Lung), failing to subdue Wood (Liver), leading to convulsions and loss of consciousness, named Shu Feng (Summerwind). Zhang Jianshan said: Clear pathogens strike the upper part, turbid pathogens strike the lower part. Wind, cold, and dampness are all Earth Qi, belonging to turbid pathogens, so they all strike the Foot meridians. Only summerheat is Heaven Qi, belonging to clear pathogens, so it strikes the Hand-Shaoyin Heart Meridian. Summerheat belongs to Fire but combines with wind, cold, dampness, and dryness. Its transmission can become malaria, dysentery, or cholera. Differentiating its depth and variations yields Mao Shu, Shang Shu, Zhong Shu, Zhong He, Shu Feng, Shu Jue, Shu Zhai, and Shi Wen. If contracted but not immediately manifesting, it becomes Fu Shu (Latent Summerheat) in autumn. Symptoms include alternating chills and fever, oppression, worsening in the afternoon, intensifying at dusk, and alleviating with sweating. If treatment is improper, intense heat injures Yin transforming into dryness, while damp stagnation injures Yang transforming into turbidity, leading to coma, internal closure, epigastric fullness, and cold limbs—this is a critical condition. Ye Tianshi followed Liu Hejian's Three Jiao legislation: for the Upper Jiao, use acrid-cool clearing and resolving, such as Zhu Ye (Bamboo Leaf), Lian Qiao (Forsythia), Xing Ren (Apricot Seed), Bo He (Mint), Zhi Pi (Gardenia Peel), Yu Jin (Curcuma Longa Root Rhizome), Sha Shen (Glehnia Root), and Xian He Ye (Fresh Lotus Leaf). For the Middle Jiao, use pungent-bitter unblocking and diffusing, such as Banxia Xiexin Tang (Pinellia Decoction to Drain the Epigastrium). For the Lower Jiao, use warm-moving cold-natured substances, heavy in quality to open the lower orifices, such as Guiling Ganlu Yin (Cassia Twig and Atractylodes Macrocephala Sweet Dew Drink). This is the general principle for treating the Three Jiao. One must further carefully distinguish whether the pathogen is in the Qi aspect or Ying aspect. Treating the Qi aspect has distinctions between cold and warm. For cold, follow Baihu Tang (White Tiger Decoction) and Tianshui San (Heavenly Water Powder); for warm, follow Erchen Tang (Two Aged Substances Decoction) and Zhengqi San (Righteousness Powder). Regulating the Ying aspect has suitability for clearing and tonifying. For clearing, use Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction), adding herbs entering the Heart; for tonifying, use Santang Tang (Three Immortals Decoction), Renshen Guben Wan (Ginseng Root-Solidifying Pill), or Fumai Tang (Restore the Pulse Decoction). For example, for deep-mixed damp-heat, use Cangzhu Shigao Tang (Atractylodes Macrocephala and Gypsum Decoction). For simultaneous burning of Qi and Blood, use Yvnu Jian (Jade Woman Decoction). To open closure and expel filth, use Zhibao Dan (Precious Treasure Pill), Niuhuang Wan (Calculus Bovis Pill), or Xuexue San (Purple Snow Powder). To support deficiency and nourish righteousness, use Shenfu Tang (Ginseng and Aconite Decoction) or Liangyi Gao (Two Powers Paste). Adapting to transformations and examining Yin and Yang ensures proper treatment.
[Mao Shu] Abdominal pain with watery diarrhea indicates invasion of the Stomach and Large Intestine. Use Xiangru Yin (Elsholtzia Decoction). For nausea, indicating phlegm-fluid in the stomach opening, use Xiangru Yin administered with Xiaoshu Wan (Summerheat-Resolving Pill). This is a mild form of summerheat disease.
[Shang Shu] Acquired from stillness, it is a Yin syndrome. Shang Shu means summerheat pathogen injures the flesh level. Cooling oneself in spacious mansions or irregular daily routines lead to aversion to cold, cold limbs, facial grime, irritability, sweating, and a deficient-thin pulse. Use Qing Shu Yi Qi Tang (Clear Summerheat and Boost Qi Decoction). Or, over-exposure to cool/shady places leads to aversion to cold, headache, limb stiffness, skin heat without sweating, and a string-tight pulse. Use Xiaoshu Shiquan San (Summerheat-Resolving Ten Complete Powder). Or, external summerheat combined with internal injury from raw/cold foods leads to abdominal pain, vomiting, and diarrhea. Use Zhengqi San (Righteousness Powder) or Liuhe Tang (Six Harmonies Decoction). Body heat, irritability, thirst, and difficult urination indicate excess dampness with failure of Qi transformation. Use Yiyuan San (Benefit Origin Powder). Irritable heat, headache, and dry thirst indicate damp steaming consuming fluids. Use Maidong Tang (Ophiopogon Decoction). This is a moderately severe form of summerheat disease.
[Zhong Shu] Acquired from movement, it is a Yang syndrome. Zhong Shu means summerheat pathogen injures the Zang-Fu organs. Traveling far in summer traps Yang Qi internally; heat resides in the Kidney, where Water fails to overcome Fire. Symptoms include irritability, thirst, panting, rapid breathing, and sudden syncope. Administer crushed garlic juice mixed with child's urine. After regaining consciousness, treat according to symptoms. Use Maidong Tang or Renshen Baihu Tang (Ginseng White Tiger Decoction). Avoid acrid-warm herbs like Xiangru. Xiangru Yin treats Shang Shu, not Zhong Shu. This is a particularly severe form of summerheat disease.
[Zhong He] Travelers or farmers laboring at noon suddenly experience severe headache, high fever, sweating, muscles feeling like fire, extreme thirst with desire to drink, and lack of energy to move. This is heat injuring Yuan Qi. Use Yiyuan San, Baihu Tang, or Qing Shu Yi Qi Tang. If vomiting/diarrhea present with deep-micro pulse, avoid cold medicines. Use Liuhe Tang or Dashun San (Great Smooth Powder). Deficient pulse with body heat results from Shang Shu; vigorous pulse with body heat results from Zhong He; this distinguishes Shu from He slightly.
[Internal Injury with Summerheat] Sexual exertion in summer months, combined with rich food and fruits, prevents Yang Qi from extending. Pulse is deep-thin or string-tight. Facial grime, no sweating, aversion to cold, cold limbs with stiffness, cholera-like vomiting. Use Lengxiang Yinzi (Cold Fragrance Infusion). If vomiting/diarrhea occur simultaneously with faint pulse nearing extinction, urgently use Jiangshui San (Well Water Powder) to warm. There is also Shu Wei (Summerheat Flaccidity): rich individuals in summer suffer sudden impotence. Do not use only hot or only cold medicines. Use Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity) combined with Shengmai San.
[Shu Feng] Due to summerheat illness, sudden hand/foot convulsions and coma with floating-deficient-rapid pulse. First, dissolve Suhexiang Wan (Liquidambar Pill) in warm water and administer. After waking, use Huanglian Xiangru Yin (Coptis and Elsholtzia Decoction) plus Qianghuo (Notopterygium Rhizome) for great effect. This is due to phlegm-fire stirring wind, obscuring the Pericardium. If vomiting, add Chenpi (Tangerine Peel) and Huoxiang (Patchouli). If difficult urination, add Fuling (Poria), Zhuling (Polyporus), Zexie (Alisma), and Huashi (Talc). If much phlegm, add Shichangpu (Acorus) and Jiangzhi (Ginger Juice). If thirsty, remove Banxia (Pinellia) and add Huafen (Trichosanthes Root). If diarrhea, add Baizhu (Atractylodes Macrocephala). If tendon twitching, add Mugua (Chaenomeles). If abdominal fullness, heavy body, difficulty turning, insensitivity of mouth, facial grime, delirium, and enuresis, this is heat combined with He. Use Baihu Tang.
[Shu Jue] Summerheat syncope with cold extremities. Use Erxiang San (Two Fragrances Powder) or Renshen Qianghuo San (Ginseng and Notopterygium Powder) combined with Xiangru Yin. If heat is deep and cold is deep, with irritability, coma, constipation, red urine, and deep-slippery-rapid pulse, modify Da/Xiao Chengqi Tang (Major/Minor Order the Qi Decoctions). For deep-mixed damp-heat, use Cangzhu Baihu Tang plus Huashi. For cold syncope with micro pulse, modify Lizhong Tang (Regulate the Middle Decoction) or Sini Tang (Reverse Cold Decoction).
[Shu Zhai] Summerheat robbing Yin, causing coughing/vomiting blood. Use Liuwei Tang (Six Substances Decoction) plus Ejiao (Donkey-Hide Gelatin), Maidong (Ophiopogon), Mudanpi (Tree Peony Bark), or Xingren (Apricot Seed), Xigua Cuiyi (Watermelon Rind), Zhuye (Bamboo Leaf), Xian Shihu (Dendrobium).
[Zhu Xia] Summer headaches, weak legs, poor appetite, emaciation, fatigue, lethargy, five-palm heat. Mostly due to Yuan Qi deficiency. Use Buzhong Yiqi Tang (Tonify the Middle and Boost Qi Decoction) removing Shengma (Cimicifuga), adding Banxia, Baishao (White Peony Root), Yimi (Coix Seed), Wuweizi (Schisandra), and Shashen. For Lung/Stomach Yin deficiency, use Shengmai San or add Yuzhu (Polygonatum Odoratum), Qizi (Goji Berry), Bian Dou (Lablab Bean), Shihu (Dendrobium)—sweet/moistening substances.
[Shi Wen] Person injured by dampness, then struck by summerheat. Cold reverse legs, chest/head heaviness, delirium, profuse sweating. Pulse: Yang aspect soggy-weak, Yin aspect small-urgent. Absolutely do not induce sweating; sweating leads to death. Use Cangzhu Baihu Tang.
Also, cold bathing causes summer-damp interaction, whole-body pain, spontaneous sweating, fever. Use Wuling San (Five-Poria Powder) plus Qianghuo. If laboring under summerheat and bathing cold while sweating, body numbness like needle pricks, occasional red swelling or blisters. Use Liuhe Tang plus Cangzhu, Jingjie (Schizonepeta), Fangfeng (Saposhnikovia).
[Shu Xie] Summerheat injuring intestines/stomach, or carrying food/dampness. Irritability, thirst, red urine, abdominal pain, paralytic watery diarrhea. Modify Guiling Ganlu Yin. For chronic diarrhea, use Yulong Wan (Jade Dragon Pill).
Summerheat injuring Heart/Spleen, vomiting/diarrhea, edema, cholera with tendon twitching. Use Liuhe Tang. Excessive drinking due to summerheat causing water-summerheat mixing, upper vomiting/lower diarrhea. Use Jieshu Sanbai San (Resolve Summerheat Three Whites Powder). Summerheat injuring with vomiting/diarrhea plus irritability. Use Xiangpo Yinzi (Elsholtzia and Magnolia Infusion). Cold-damp accumulation in Spleen/Stomach causing vomiting/diarrhea. Use Dashun San. If summerheat injures externally and yin-cold internally, forced from both sides. Use Lianli Tang (Linked Licorice Decoction), Guiling Wan (Cassia Twig Pill), Suopi Yin (Constrict Spleen Drink).
[Shu Yang] Summer month red/swollen crown, sore throat, or leg/foot swelling inches long, unable to walk, day/night fever. Use Baidu San (Defeat Toxin Powder). Once heat resolves, swelling subsides.
Summer sores with blisters caused by damp-heat entering meridians/subcutaneous tissue. Use Huanglian Jiedu Tang, Xiangru Yin.
[Fu Shu] Manifesting in autumn as latent summerheat. Initially inhaled via mouth/nose, then binding internally at the Mouyuan (Diaphragmatic Origin). Triggered by new coolness, symptoms include headache, epigastric stuffiness, progressing to dry lips/teeth, internal blazing fire, consuming Yin fluids. Internal heat/oppression, sudden nasal cold air obstruction. Summerheat miasma steams into sticky saliva, suppresses into cold reversal, alleviates with sweating. However, some have one reversal then heat, resolving with sweat. Some have two/three/four reversals then heat, with only head sweating. Some have discordant four extremities, warm body but increasing restlessness. Pathogen lies deep in Mouyuan, heat binds internally, exterior appears cold—Zhang Zhongjing's "heat deep, cold deep." Using bitter-cold direct descent goes to intestines/stomach, ignoring diaphragmatic binding. Use Shanzhi (Gardenia), Shigao (Gypsum), Tiandong (Asparagus Root), Dan Zhuyeqian (Light Bamboo Leaf), Zhike (Bitter Orange), Gualou (Trichosanthes Fruit), Yujin Zhi (Curcuma Juice). If initially Qi aspect blockage in Hand-Taiyin Lung, treat Upper Jiao. Use Xingren, Beimu (Fritillaria), Bohe, Yimi, Huashi, Tongcao (Akebia Stem), Banxia, JuPi (Tangerine Peel), Houpo (Magnolia Bark). If spreading from Qi to Blood, mental dullness, retracted tongue, black nose/red lips, pathogen counterflows to Danzhong/Pericardium. Entering Ying collaterals, consciousness dims, internal closure/external collapse, rapid crisis. Urgently use Shichangpu, Xijiao Jian (Rhinoceros Horn Tip) ground juice, Lianqiao (Forsythia), Yuan Shen (Scrophularia), Xian Shengdi (Rehmannia), Jinyin Hua Lu (Honeysuckle Flower Dew). Or aromatic pathogen-expelling/orifice-opening: Huoxiang, Peilan (Plagiorchis), Yujin, Niuhuang Wan, Zhibao Dan, Xuexue San. For hot-phlegm sticky: Zhuli (Bamboo Sap), Beimu, Juhong (Red Tangerine Peel), Gualou, Huafen, Shashen, Gancao (Licorice), Zhe Li Zhi (Pear Juice). All selectable. Most contraindicated: choppy-rapid pulse, righteous deficiency/pathogen sinking, cold extremities, coma, fluid exhaustion. If short urine/spontaneous diarrhea, tidal fever unresolved, affecting Middle/Lower Jiao, optionally use Liuyi San (Six-to-One Powder) or Siling San (Four-Poria Powder). Ultimately, clearing Upper/Middle Jiao is key.
Treating summerheat with profuse sweating injures Middle Qi first. Even with diaphragmatic fullness/tidal fever, absolutely avoid purgatives because formless heat cannot be scattered by drugs. Even with heavy forehead pain, absolutely avoid sweating agents because exterior herbs ascend, supporting phlegm/food/turbid Qi in the diaphragm.
Xu Xueshi treated damp-heat disease: chest/neck sweating, cold feet, delirium. Pulse: Guan front soggy, Guan back rapid. This is first summerheat, then dampness, interacting. Soggy-weak in Yang aspect indicates dampness battling summerheat. Small-urgent in Yin aspect indicates summerheat steaming dampness. Xu defined Guan front as Yang, Guan back as Yin. Ji defined Floating as Yang, Deep as Yin. This is named Shi Wen. First use Baihu Renshen Tang. Then Baihu Cangzhu Tang. Headache recedes, feet warm, sweating stops, cured in three days.
Sun Zhao treated damp-heat: moist body, cold feet, abdominal fullness, unconsciousness, small-weak-urgent pulse. Use Wuling San combined with Baihu Tang. After ten-plus doses, slight recovery. Then Qingzao Tang (Clear Dryness Decoction) for regulation and safety. For Yin disease cold reversal, calves/arms cold. Here calves cold, arms not, so not descending cold rising up. It is Yang-micro cold reversal, requiring heat-resolving herbs.
Zhang Shiwang said: Zhong He uses Baihu Tang to treat heat injuring Qi; Renshen Baihu Tang treats concurrent injury of formless Qi. Zhong Shu uses Shengmai San for summerheat injuring formless Qi. Qing Shu Yi Qi Tang treats summerheat injuring Qi with wind-heat complicating, invading meridians via deficiency. Shang Shu uses Shiwei Xiangru Yin for wind-heat-damp jointly injuring form/Qi. Predominantly exterior transforms Xiangru Yin into Xiaoshu Shiquan; predominantly interior transforms into Liuhe Tang. These are the three tripod methods of summer. Xiaoshu Wan treats upper excess dampness flooding causing fullness. Yiyuan San treats lower excess heat blocking causing difficult urination. Dashun San treats fruit injuring Spleen. Lengxiang Yinzi treats cold food injuring Stomach. Laifu Dan treats Yin Qi knotting below. Wuling San treats Yang Qi blocked inside.
