When skin and flesh are damaged and subsequently invaded by external wind pathogens entering the meridians, gradually transmitting inward, the manifestations include alternating chills and fever (Hanre Jiaozuo), trismus (Koujin) with teeth grinding, opisthotonos (Jiaogong Fanzhang), and salivation (Koutu Xianmo). If the pathogen enters the Yin level, the body becomes cool with spontaneous sweating (Shihan), and the wound flattens and sinks back to its original state, indicating that the toxin is retracting inward. At this stage, Wanling Dan should be administered to induce sweating (Fahan), allowing the wind pathogen to exit from the exterior. Subsequently, Yuzhen San should be applied topically to the affected area; the production of pus indicates efficacy. If, after sweating, the aforementioned symptoms persist, the wound remains flat, consciousness fluctuates between clarity and coma, symptoms come and go intermittently, trismus prevents opening the mouth, and speech is difficult or absent, these are ultimately signs of a fatal prognosis.
Wanling Dan
Cangzhu (Atractylodis Rhizoma) 8 liang, Quanxie (Scorpio), Shihu (Dendrobii Caulis), Tianmaju (Gastrodiae Rhizoma), Danggui (Angelicae Sinensis Radix), Zhigancao (Prepared Glycyrrhizae Radix et Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Jingjie (Schizonepetae Herba), Beixixin (Asari Radix et Rhizoma), Fangfeng (Saposhnikoviae Radix), Mahuang (Ephedrae Herba), Chuanwu (Aconiti Radix Preparata, soaked in water and peeled), Caowu (Aconiti Kusnezoffii Radix Preparata, same processing), Heshouwu (Polygoni Multiflori Radix) each 1 liang, Xionghuang (Realgar) 6 qian.
Grind into fine powder and form pills with honey. One liang of powder can be made into four, six, or nine pills, divided into three grades for use according to constitution and urgency. Coat externally with Zhusha (Cinnabar) and store in sealed jars. For administration, decoct nine stalks of Congbai (Allii Fistulosi Bulbus) in water, dissolve one pill, and take orally. Cover with blankets to induce sweating for cure. If sweating is delayed after ingestion, administer Congtang (scallion soup) again to promote it; profuse sweating resembling a shower will occur. After a short while, gradually remove the blankets, and the sweat will naturally cease. This formula is suitable for all cases of swelling, toxicity, and wind-pathogens where dispersing and diffusing are indicated.
Yuzhen San
Nanting (Arisaematis Rhizoma), Fangfeng (Saposhnikoviae Radix), Baizhi (Angelicae Dahuricae Radix), Tianma (Gastrodiae Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Baifuzi (Typhonii Rhizoma).
Grind equal parts into powder and apply topically to the affected area. In cases of severe trauma approaching death where slight warmth remains at the heart, administer 2 qian mixed with hot wine and Tongbian (child's urine); after two consecutive doses, life can be restored immediately. For tetanus presenting with trismus, opisthotonos, and even teeth grinding with tongue retraction, mix 3 qian with Tongbian and administer orally for immediate recovery. (From *Mifang Jiyuan*)
Another method: Use Qilao (larva of *Oryctes rhinoceros*, also known as Digechong). Grasp the larva firmly until it exudes fluid from its mouth, then apply this fluid directly to the wound. The patient will feel numbness and sweating, and survival is assured. Previously, a child suffered from tetanus due to a scalp laceration; treated according to this method, they recovered instantly.
