The etiology and pathogenesis of heat syndromes in the five meridians vary. When hot qi steams upward and rushes to the Heart, it causes delirious speech,妄言 (delusional talk), and chaotic spirit/mind; Xijiao (Rhinoceros Horn) and Huanglian (Coptis Rhizome) can stabilize the spirit. When heat steams the Liver, it causes convulsions and spasms resembling wind-stroke; treatment should employ Tianma (Gastrodia Rhizome), Qianghuo (Notopterygium Rhizome), and Dansheng (Arisaema Cum Bile). When heat steams the Spleen, it causes abdominal distension, thirst with desire to drink, eyelid edema, and constipation; one should administer Zhike (Aurantii Fructus), Shanzhi (Gardeniae Fructus), and Dahuang (Rhei Radix et Rhizoma). When heat steams the Lung, causing wheezing, coughing, and nasal discharge, use Dailing (Campanulae Radix), Beimu (Fritillariae Bulbus), and Sangbai (Mori Cortex). When heat steams the Kidney, use Xuanshen (Scrophulariae Radix) and Zhimu (Anemarrhenae Rhizoma).
Eruptions caused by plague-toxin arise when children contract wind-pathogens, which hide and accumulate between the muscles. When Yang qi activates, the pathogens surface on the skin. Throughout the year, repeated exposure to abnormal qi leads to eruptions emerging from the axillae, which are the pathways of Qi. When plague pathogens exit along these airways, it is lung plague-toxin causing eruptions; treatment should primarily utilize Qingfei Yin (Clear the Lungs Decoction).
Post-eruption pi-accumulation occurs after measles have receded. Due to unregulated diet and lack of caution regarding fright, long-standing accumulation transforms into disease, wrapped by phlegm and qi, thus forming pi-disease. This results in a yellowish complexion, emaciated muscles, a thin neck, weak legs, an abdomen distended like a drum, body heat, and mouth ulcers. Treatment should employ Piqi Wan (Pi-Qi Pill) and Hezi Wan (Hezi Pill) to eliminate the condition.
Generalized erosion post-eruption occurs after the rash has subsided. Excessive water intake and irregular diet injure the Spleen and Stomach, affecting the muscles, back, and hypochondrium, where small blisters emerge. These blisters are bright and clear, known as "crystal blisters." Within a day, they transform into pustules resembling boiling water, then ulcerate into erosions. The method requires treating with Weiling Wan (Weiling Pill) internally, and applying Baiyun San (White Cloud Powder) externally.
Subsequent shagan (measles-related gangrene) arises from deficiency-heat in the Spleen and Stomach flaming upward, steaming the mouth. Saliva flows as clear water, constituting mouth ulcers. If mouth ulcers persist for a long time, they bleed and exude fluid; blood accumulates and suppurates, becoming black, rotting, and foul-smelling, which is defined as shagan. Treatment should apply Bingjin San and HT San externally.
After the rash recedes, if the scrotum becomes spontaneously red and swollen, bright like a lantern, with unbearable pain, this is residual toxin and evil heat flowing into the Bladder. It should be treated with Daochi San (Guide Out Redness Powder).
Malaria following eruptions occurs because the Lung and Spleen are delicate. Bathing too early allows damp-heat and water qi to lodge in the skin. Wei qi fails to guard, allowing evil qi to invade secretly. Yin and Yang contend, alternating between excess and deficiency. When Yin prevails over Yang, there is cold inside and out; when Yang prevails over Yin, there is heat inside and out. Treat with Jienue Dan (Antimalarial Elixir). (Formula see *Jingjian Lu*.)
