Heat pathogens steaming internally and externally, when encountering wind-cold, repel the internal heat and cannot enter; the heat is blocked by wind-cold and cannot exit. Toxic qi stagnates at the muscle surface, resulting in macule-like spots on the skin. Some may appear cyanotic or purplish. It is appropriate to use Shengdi (Rehmanniae Radix), Zicao (Arnebiae Radix), Lingxiaohua (Campsis Grandiflora Flos), Danpi (Moutan Cortex), Xuanshen (Scrophulariae Radix), Lianqiao (Forsythiae Fructus), Huangqin (Scutellariae Radix), Honghua (Carthami Flos), Quanxie (Buthus), Shigao (Gypsum Fibrosum), Chuanlian (Coptidis Rhizoma). If constipation occurs, add Shengjun (Rhei Radix et Rhizoma).
Continuous eruption without receding indicates excessive heat toxicity. If it fails to erupt thoroughly at once, heavy exterior-releasing methods should be employed. After the rash subsides, Huaban Jiedu Tang (Macula-Resolving and Toxin-Clearing Decoction) should be used (composition: Xuanshen, Zhimu, Shengma, Zhizi [Zhi Shi], Renzhonghuang, Lianqiao, Shigao). If constipation persists, add Shengjun, Shengdi, Honghua, and Zicao.
Bleeding from the gums indicates blazing stomach fire. Tianma (Gastrodiae Rhizoma), Zhike (Aurantii Fructus), Huafen (Puerariae Radix), Maidong (Ophiopogonis Radix), Shengdihuang (Rehmanniae Radix Recens), and Huanglian (Coptidis Rhizoma) are suitable, with heavy doses of Shigao and Shengtiaoxin (Huangqin). This results from accumulated heat and toxic fire entering the stomach. Qingwei Yin (Stomach-Clearing Drink) and Liangge San (Cooling Diaphragm Powder) are appropriate (composition: Bohe, Shanzhi, Lianqiao, Jiegeng, Huangqin, Shengdi, Gancao, Chuanlian, Shengma, Danpi, Shigao).
Swelling and pain in the teeth and cheeks with salivation flowing from the mouth corners (suitable for using Chuanxiong, Fangfeng, Shigao, Huanglian, Xuanshen, Huangqin, Gancao, Niubangzi) require avoiding yang-tonifying agents, otherwise pain will persist continuously.
After the rash, red macules appear with itching. One to two days after the rash, red macules itch all over the body as they spread out. Scratching increases the itchiness; the appearance resembles fine millet seeds or cloud-like patches erupting suddenly. This is caused by consuming spicy and greasy foods without restraint after the rash subsides, leading to external contraction of wind-heat and cold-dampness. Urgently apply Sanhua Ban Tang (Three-Flower Macula Decoction) added with Fangfeng and Huanglian to dispel wind-heat and prevent diarrhea/dysentery. If diarrhea/dysentery persists for a long time, this indicates residual toxins shifting to the large intestine. Do not recklessly administer astringents, which may cause internal toxins to attack upward, turning into vomiting, inability to eat due to locked mouth (Jinkou), and untreatable conditions. If slippery diarrhea continues unceasingly, or if fresh blood is passed, or if stools resemble dripping roof water or turbid liquid, urgently take Sanhuang Tang (Three-Yellow Decoction) added with Binglang (Arecae Semen), Zhike (Aurantii Fructus), and Liu Yi San (Six-to-One Powder); treatment offers hope for recovery.
