Swallowing half a dose, he vomited several pieces of purple blood, and the chest and diaphragm instantly felt relaxed. He then drank rice water and gradually advanced to thin congee. Adhering to this regimen for several days, he recovered without further medication. This can be considered a rare case; it is recorded here awaiting instruction.
Xue Lizhai treated a woman who suffered from throat pain after childbirth. After taking heat-clearing agents, her condition worsened. This was due to blood deficiency in the Bladder meridian. The Bladder meridian ascends to the throat and then returns. He prescribed Bazhen Tang (Eight Treasure Decoction) with added Mudanpi (Moutan Cortex), Chaihu (Bupleuri Radix), and Huangbai (Phellodendri Cortex) stir-fried with wine until blackened. She recovered after two doses.
Sun Simiao said: "A sensation of tightness in the throat, as if there were roasted meat stuck there, unable to be spat out or swallowed down, can be cured by holding slices of fresh ginger in the mouth for fifty days." Interpreting this, it should be cold injuring the meridians, causing disharmony of Qi and Blood, which floats to the throat. Women often accumulate cold qi in the blood aspect due to cold invasion, hence this symptom. I used Po Guan Dan (Resurrection Pill) held in the mouth and frequently swallowed. I treated several people with good results. Generally, 'Hou' (throat) implies 'checking'; the throat expels Heaven Qi. 'Yan' (pharynx) implies 'swallowing'; the pharynx expels Earth Qi. Thus, the throat is the passage for respiratory air, while the pharynx is the path for food. If internal knotting of worry and thought, external invasion of wind-cold, and obstruction by phlegm-Qi cause rebellion in the throat, hindering eating and drinking, it may eventually lead to Fan Wei (regurgitation) and Ye Ge (dysphagia) syndromes. (From *Ye Xinglin's Gynecology*.)
Lou Quanshan said: "In the spring of Wuchen year of Hongwu era, many villagers suffered from Hou Bi (sore throat). This was due to the residual pathogenic factors of the previous year's end, specifically the Fire pathogens. I repeatedly used Gan Jie Tang (Licorice and Platycodon Decoction) added with Huanglian (Coptidis Rhizoma), Banxia (Pinelliae Rhizoma), Jiangcan (Bombyx Batryticatus), and Niubangzi (Arctii Fructus) to disperse the pathogen. For those with deficiency, I added Renshen (Ginseng Radix), [missing character], Danggui (Angelicae Sinensis Radix), and Shaoyao (Paeoniae Radix). For those unable to take fluids, I first used Jiedu Xionghuang Wan (Detoxifying Realgar Pill), ground with vinegar and administered. Once the phlegm was expelled, I administered ginger juice. Subsequently using the aforementioned herbs yielded miraculous effects. If treated with sour-cold agents like Danfan (Alumen), they are incurable because the pathogen remains trapped and cannot exit."
Chen Ziming treated a man with Hou Bi (sore throat) making it difficult to swallow fluids, and another man with Chan Hou Feng (entangling throat wind) with excessive phlegm-saliva overflow. Both were given one pill of Shen Xian Zhui Du (Divine Pursuit of Poison Pill) and recovered. (Formula see Insect Section.)
Xue Lizhai treated a man with throat pain and rapid pulse. He prescribed Jing Fang Bai Du San (Schizonepeta and Saposhnikovia Powder to Resolve the Ten Perversities) added with Huangqin (Scutellariae Radix) and Lian (Gardeniae Fructus). After two doses, there was slight improvement. He then removed Huangqin and Lian, and after two more doses, he recovered.
A man suffered from swollen and closed throat with locked jaw, preventing needle insertion. First, Shao Shang (LU11) acupoints on both sides were needled, releasing dark blood, and his mouth opened immediately. Then, the affected area was needled, and he took Qing Yan Li Ge San (Clear Throat and Purge Diaphragm Powder). He recovered after one dose. Generally, expectoration and acupuncture both have dispersing effects, hence their efficacy. Without acupuncture, such cases often prove fatal.
A woman suffered from swollen and painful throat with constipation of both large and small bowels. She took Fang Feng Tong Sheng San (Saposhnikovia and Polygala Powder to Unblock the Intestines), and all symptoms subsided after one dose. She then took Jing Fang Bai Du San for three doses and stabilized. For mild cases, Jing Fang Bai Du Chui Hou San (Powder for Blowing into Throat) is effective; for severe cases, Mi Yao Shi (Secret Key Stone) applied locally and bleeding from the affected site are most effective; otherwise, rescue is unlikely. Needling Shao Shang (LU11) is also an option, but less rapid than bleeding the affected site.
A man suffered from swollen and painful throat with a rapid and excess pulse. His pain ceased after one dose of Liang Ge San (Cool the Diaphragm Powder). His swelling subsided after two doses of Jing Fang Bai Du San added with Niubangzi (Arctii Fructus). He was then treated with Jing Fang Bai Du San, followed by Gancao (Licorice Root), Jiegeng (Platycodon Root), Jingjie (Schizonepeta Spike), Fangfeng (Saposhnikovia Root), and Xuanshen (Scrophulariae Radix) for four doses, leading to recovery.
A man suffered from swollen and closed throat with excessive phlegm-saliva. Bofan (Alumen) was blown into the throat, expelling about a bowl of phlegm. He then took Qing Yan Li Ge Yin (Clear Throat and Purge Diaphragm Decoction) for four doses and stabilized.
A man suffered from swollen and painful throat, unable to swallow medicine. The affected area was needled, releasing a little purple blood, bringing slight relief. He held Po Guan Dan (Resurrection Pill) in his mouth and took Qing Yan Qing Du San (Clear Throat and Clear Toxin Powder), recovering thereafter.
A man suffered from dry and painful throat. He took Si Wu Tang (Four Substance Decoction) added with Zhimu (Anemarrhenae Rhizoma), Huangbai (Phellodendri Cortex), and Xuanshen (Scrophulariae Radix). After four doses, there was slight improvement. He then took Gu Ben Wan (Root-Securing Pill) for one dose, and it did not recur.
A man suffered from throat pain worsening in the afternoon with a rapid but forceless pulse. He took Si Wu Tang added with Zhimu, Huangbai, Jingjie, and Fangfeng. After four doses, he recovered. The formula was then modified by removing Jingjie and Fangfeng, adding Xuanshen, Gancao, and Jiegeng. After several doses, he fully recovered.
A frail person suffered from throat pain worsening with cool medicines or exertion. This was deficient-heat in the Middle Jiao. He took Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) added with Huangqin and Huanglian. After four doses, he recovered. The herbs were then removed, and after several more doses, it did not recur. For afternoon pain, removing Huangqin and Huanglian and adding Zhimu, Huangbai, and Xuanshen is also effective.
A man suffered from swollen and painful Ru E (goiter/tonsillar abscess) with a floating and rapid pulse, before suppuration. Malicious blood was removed by needling, and he took Jing Fang Bai Du San. It resolved after two doses.
Another man suffered from swollen and painful Ru E, unable to eat. The lesion was white, indicating suppuration had formed. Needling released pus, and he stabilized.
A man suffered from Ai Yong (abscess of the pharynx) with swelling and pain. The pulse was floating and rapid, becoming deep and solid. He took Fang Feng Tong Sheng San once, resulting in one bowel movement, and the condition retreated significantly. He then took Jing Fang Bai Du San, resolving after two doses.
A man suffered from swollen and painful throat and wished to be needled to drain toxicity. Fearing needles, he only wanted medicine, but by the time it was cooked, he could no longer swallow. Urgently, the affected area was needled to release toxic blood, and he took Qing Yan Xiao Du Yao (Clear Throat and Eliminate Toxin Medicine), recovering thereafter.
A patient whose Qi had ceased but heart remained warm was urgently needled at the affected site. Releasing dark blood revived him. Individuals like Bao Fuqing and Qiao Weiwei, who had this condition historically, always recovered upon bleeding. Generally, these are caused by Fire pathogens acting rapidly. One must distinguish urgency and whether suppuration has formed. In cases of swelling, closure, and obstruction, death can occur instantly. Mi Yao Shi should be blown into the affected site to expel phlegm; once Qi flows, the patient revives. If closure persists after expectoration, it is due to malicious blood or pus-toxin, requiring immediate needling; otherwise, rescue is impossible. Ancients said: Treating Fire in Hou Bi is like fighting a fire; delay is not an option. Also: "View Hou Bi as quickly as a running horse," truly so! Although there are many formulas for throat diseases, only acupuncture has the power to revive the dead.
A man suffered from throat pain with phlegm-saliva obstructing upwards. The intention was to treat with Jing Fang Bai Du San added with Lianqiao (Forsythiae Fructus), Shanzhi (Gardeniae Fructus), Xuanshen, and Niubangzi. However, he self-medicated with sweet-cold Fire-reducing agents, which aggravated fever and made the throat more swollen and painful. Urgently, Shao Shang (LU11) points were needled. The original formula was resumed with Mahuang (Ephedrae Herba) to induce sweating. All symptoms receded. Only a purple spot in the throat remained painful, indicating impending suppuration. Mahuang was removed from the formula. After one dose, suppuration broke, and he recovered. For early or mild throat pain, Jing Fang Bai Du San is suitable for dispersion. For late or severe cases, Shao Shang (LU11) must be needled. If blood stasis is formed, the affected site must be pricked; sometimes Shao Shang is used. If the throat is clear but not fully resolved, suppuration will likely form, but removal brings peace. If constipation exists, even with light bleeding, Fang Feng Tong Sheng San should be used to purge. Sweet-cold agents are contraindicated unless there is deficiency.
A woman felt a lump in the throat that could not be swallowed or spat out, accompanied by fatigue and fever. Si Qi Tang (Four Substances Decoction) was used first to clear the throat, followed by Xiao Yao San (Free and Easy Wanderer Powder). Another woman had similar symptoms plus chest/diaphragm discomfort, abdominal distension, poor appetite, and restless sleep. Fen Xin Qi Yin (Decoction to Separate Heart Qi) cured her.
Ye Su, a government official with Yin deficiency, suddenly developed throat swelling, chills, fever, headache, stiff neck, staring eyes, and involuntary urination due to anger. These are Liver Fire symptoms. The Liver governs sinews; Fire causes swelling. Excessive Fire dries Blood, causing sinew spasms and neck stiffness. Yin collapse and flaccidity lead to urinary incontinence. The affected site was needled to release toxic blood. Si Wu Tang, Chaihu, Shanzhi, Xuanshen, and Gancao were used to revive him. Liu Wei Wan (Six Ingredient Pill with Rehmannia) material was then used to nourish Liver Blood and Kidney Water, curing all symptoms.
The Prefect Ye suffered from swollen and painful throat, difficult phlegm, hot hands and feet, and craving for cold drinks. Qing Yan Li Ge Tang (Clear Throat and Purge Diaphragm Decoction) failed after two doses. Needling Shao Shang (LU11) provided slight relief. Phlegm exited the nose like glue, and purple blood from the affected site brought slight relief. After five to seven days, foul pus was coughed up, and he recovered.
Gu Keming, a righteous gentleman, suffered from throat pain worsening at night, indicating Liver-Kidney Yin deficiency heat. Si Wu Tang added with wine-blackened Huangbai, Zhimu, Maidong (Ophiopogonis Radix), and Wuweizi (Schisandrae Fructus) cured him. Later, due to overwork, his throat became swollen and closed. Bleeding the affected site and taking Jiegeng Tang (Platycodon Decoction) to expel phlegm resolved it. In mid-summer, he had dry cough, hoarseness, thirst, fever, and afternoon fever. Zishen Wan (Nourish Kidney Pill) and Ji Jian Ba Wei Wan (Modified Eight Ingredient Pill) were taken intermittently for over three months. Fortunately, being young and careful, he recovered.
Li Chaoyong, a Tongbu Official, suffered from swollen throat and mouth/tongue ulcers, indicating Upper Jiao Wind-Heat. Jing Fang Bai Du San for two doses relieved throat pain. Xuanshen Shengma Tang (Scrophularia and Cimicifuga Decoction) cured the mouth ulcers.
Huang Beipan, a local official, suffered from stuffy throat pain, craving cold drinks, and constipation, indicating excess Heat in Upper/Lower and Exterior/Interior. Fang Feng Tong Sheng San cured him instantly.
Hu Chengfu, a local official, suffered from throat pain, indicating deficient Heat in Kidney and Bladder meridians. Si Wu Tang added with Zhimu, Huangbai, and Xuanshen showed slight improvement after four doses. Gu Ben Wan prevented recurrence after one dose.
Lu Yizhai, a Bureau Director, suffered from swollen/painful throat, [missing character] eyes, and red urine, indicating Damp-Heat in the Bladder. Si Ling San (Four Poria Powder) added with Zhimu, Huangbai, Huanglian, Yinchen (Artemisiae Scopariae Herba), and Fangji (Stephaniae Tetrandrae Radix) cured him instantly. Liu Wei Di Huang Wan (Six Ingredient Pill with Rehmannia) completed the cure.
Wang Wenyuan, a scholar, suffered from swollen/painful throat and mouth ulcers worsening with labor, indicating Spleen-Lung Qi deficiency with Bladder Heat. Jiang Shichuan, Vice Commissioner, consulted in winter of Gui Mao year. Right Cun pulse was floating, rapid, and forceful; he had oral ulcers. I said: Fire transmitted to Lung. Eat bland food, rest carefully. In autumn of Jia Chen year, Chi pulse was rapid and forceless. I said: Lung Metal fails to generate Kidney Water. Rest to nourish the source. He said: Now my ears and throat itch painfully, limbs feel hot. Rootless Fire will surely kill me. He told Liu Guxia: Xue Lizhai doubts my illness. In early spring of Yi Si year, Guxia told Xue: Mr. Jiang's illness matches your prediction. We examined him together; throat was swollen and ulcerated, pulse more flooding. Using Fire-purging drugs accelerated his death.
Wu Shangshe from Yunjian, over fifty, suffered from swollen/painful throat. Bleeding was performed. Spirit was clear, but Chi pulse was flooding, rapid, and irregular, feeling minute and almost absent upon pressure. I said: Pain with form belongs to Yang. Severe tonification of Yin is needed. You injured Yin Blood instead; you will die. He indeed died. This case was Kidney Qi deficiency with rootless Fire attacking upwards. Only Ji Jian Ba Wei Wan could save him by returning Fire to its source.
Ma Yuanyi treated Ren Caizhi with chronic throat pain for three years. Pulse was deficient with choppy quality, indicating Middle Qi injury from labor and stagnation. Deficiency leads to excess Fire. Fire rises, injuring Lung. Lung loses Spleen's generating Qi and is scorched by vigorous Fire, depleting fluids unable to nourish the root. Lower Jiao Yin Fire cannot hibernate. Vigorous and deficient Fire combine to cause prolonged illness. Treatment: First resolve upper stagnation-heat, then guide floating Fire downward. Though chronic, it can be cured. Ziwan (Asternatis Radix), Gegen (Puerariae Radix), Xingren (Armeniacae Semen), Suzi (Perillae Fructus), Qianhu (Peucedani Radix), Jiegeng, and Gancao for two doses made the pulse permeable. Then Renshen, Shihu (Dendrobii Caulis), Zhi Gancao, Banxia Qu (Pinellia Fermented), Juhong (Citri Reticulatae Pericarpium Rubrum), Huanglian, and Rougui (Cinnamomi Ramulus) for four doses eliminated throat pain. Renshen Qi Wei Wan completed the cure.
Shi's wife, obese and prone to labor/stagnation, felt throat itching like insects crawling for five-six years. Pulses were floating-deficient and deep-choppy, indicating Yangming Qi-Blood malnourishment with Fire stirring Wind. Yangming meridian starts at nose, crosses [missing character], descends outside nose, circles lips, follows cheekbone to ear. Branch goes from Daying to Renying, follows throat to Quepen. Now Blood deficiency and Wind blazing mean meridians are not nourished by Blood but invaded by Wind. Wind dominance causes dryness; Wind movement causes motion. Treatment should focus on Blood, not Wind. When Blood is sufficient, Wind extinguishes itself. Sheng Dihuang (Rehmanniae Radix), Zhi Shouwu (Polygoni Multiflori Caulis Praeparatus), and Tiandong (Asparagi Radix) as Sovereign herbs nourish Yangming Blood; Qinjiao (Magnoliae Officinalis Cortex? No, Gentianae Macrophyllae Radix), Baijili (Tribuli Fructus), and Gaju (Chrysanthemi Flos) as Ministers clear Yangming Wind; Lu Gen (Phragmitis Rhizoma) juice and Sugarcane syrup as Assistants moisten dryness and nourish Yin. Two months of regulation cured her.

Li Xixing recalled the Wude era when he garrisoned Luzhou. Xu Ren showed me Zhen Quan's newly compiled *Ming Tang*. At that time, Prefect Cheng Junchuo suddenly had cheeks/jaw swollen as large as a measuring cup, throat blocked, unable to take water/grains for three days. I requested Zhen Quan to save him. Needling the tip of the right hand's second finger, within the time of a meal, breath returned. Next day, eating/drinking was normal. (From *Qianjin Yi*). Note: *Tong Ren* says Shao Shang is on the medial side of the thumb nail corner. Cheng Junchuo's symptoms differed slightly, but efficacy was identical. Li Yun says prick finger tip; *Tong Ren* says thumb tip. Uncertain which is correct. If Shao Shang was pricked, it should be the thumb. I leave both records for experts. (From *Zisheng Jing*.)

Using straight Bai Jiangcan (Silkworm), fried to powder, mixed with fresh Ginger Juice, taken as 1 qian bi, was very effective. Ge Yanhui, Supervisor in Minzhong, suffered from Hou Bi. Wuba Magistrate used this formula, bringing instant relief. Another method: Take 2 qian dissolved, if not cleared, wait half an hour, take again, clearing stubborn phlegm. Take a piece of Dahuang, slow-fire roasted until ash-like, sliced thickly. Hold a piece size of two fingers in mouth, swallowing juice, replacing every meal-time. If patient cannot speak/swallow, prop them up leaning back/sitting. Administer medicine via pen-tube into nostril (Male Left, Female Right). After medicine, sit upright. Phlegm will be expelled shortly. If not propped up, fear expulsion through nose. After vomiting, hold remaining Dahuang as before. (From *Bai Bai Fang*.)
For acute Hou Bi with mouth unable to open: Wrap one Badou (Crotonis Fructus) in yellow wax paper, size of nostril, cut middle, insert into nostril urgently. Air rushes into throat, breaking the blockage. Remove when relief is felt. Hao Governor Wang Yafu's Formula: Shell Badou, crush, wrap in cotton, insert into left/right nostril. Expels evil matter. Small sore may appear later, harmless. (Same source.)
For critical throat pain: Pulling hair from vertex forcefully opens passage. If no hair, pinch vertex skin. Dr. Liu obtained this formula but hadn't tested it. Suddenly a soldier suffered throat pain, unable to speak. Removed headgear; he had bald patch from alcoholism. Pinching vertex skin forcefully relieved him. (From *Shi Zhai Fang*.)
Miu Zhongchun treated Chan Hou Feng (Hou Bi) with a verified opening formula: Mingfan (Alumen) 3 qian, Badou (shelled) 7 grains. Melt Alumen into Badou, burn until Alumen is dry, remove Badou, grind fine, blow into throat. Hot saliva flows out, opening ensues. (From *Guang Biji*.)
Ma Mingju treated Ni Zhongzhao with Hou Xuan (Throat Lichen). Local doctors tried everything, gradually corroding tissue. Food had to be soft/flour-based, swallowed with head tilted back, weeping constantly. Ma said: This is not Wind-Fire toxin. Did you have Meichuang (Syphilis) in youth? No. Parents? Yes, cured three years before I got it. Ma said: Must be accidental ingestion of Sheng Yao (Calomel/Sublimated Mercury). Those with this sore absorb cold/clear Lightfen (Calomel) toxin; toxin erupts near healing. Sheng Yao toxin erupts after healing. Children inherit deep toxin, extending to grandchildren/nephews. If not treated as结了毒 (Condensed Toxin), death is certain. Gan Jie Tang as Sovereign, add Shandougen (Millettiae Speciosi Caulis), Longdancao (Gentianae Radix), Shayang (Belamcandae Rhizoma). Each dose use Tubalingfu (Smilacis Glabrae Rhizoma) 0.5 jin, decoct strongly, send down with 2 fen Niuhuang (Bovis Calculus). Cured in half month without blowing medicine. Later admitted parents took pills to cure, then had mouth ulcers—indeed Calomel. Modern doctors use it casually, unaware of deep toxicity. Hence this clarification. (From *Guang Biji*.)

Jin Suo Yao (Golden Lock Key) expelled 5-6 bowls of thick phlegm. Qing Yan Li Ge Tang reduced swelling/pain after one dose. Reducing Xiaoshi (Mirabilite) and Dahuang, continued treatment, and she stabilized. (From *Waike Zhengzong*.)
Xiao Wanyu treated Mrs. Wang, who loved spicy food. In late summer, stomach disease caused throat pain. Pulse was flooding, slippery, slightly rapid. Gan Jie Tang added with Huangqin, Huanglian, Gualou (Trichosanthis Fructus), and Xuanshen cured her in two doses.

At dawn, throat pain, rebellious Qi, alternating chills/fever. Previously slight throat pain, now swollen/painful. Swollen uvula like fat man's thumb. Diagnosed Heart/Kidney deficient Fire combined with post-menstrual Wind, Wind-Fire dual closure. Dispersing exterior would increase deficiency; Cold/Cool herbs would trap Wind-Fire. Qianhu, Suzi, Lianqiao, Xuanshen, Chishao (Paeoniae Radix Rubra), Zhebei (Fritillariae Thunbergii Bulbus), Gancao, Jiegeng decocted and taken, cured instantly. (Same source.)
