Li Shizhen stated: Current physicians possess the Soufeng Jiedu Tang (Search for Wind and Detoxify Decoction) to treat Yangmei Chuang (Syphilis/Kaposi's varicelliform eruption), without using Qingfen (Calomel). For deep-seated cases, it takes about a month; for superficial cases, recovery occurs within half a month. For those suffering from tendon spasms, pain, paralysis, and inability to walk due to taking Qingfen-containing medicines, this formula is also effective. The prescription consists of Tufuling (Smilax Glabra Rhizome) 1 liang, Yiren (Coix Seed), Jinyinhua (Lonicerae Japonicae Flos), Fangfeng (Saposhnikoviae Radix), Mugua (Chaenomelis Fructus), Mutong (Akebiae Caulis), Baixianpi (Dictamni Cortex) each 5 fen, and Zaojiazi (Gleditsiae Sinensis Semen) 4 fen. If Qi deficiency exists, add Renshen (Ginseng Radix et Rhizoma) 7 fen; if Blood deficiency exists, add Danggui (Angelicae Sinensis Radix) 7 fen. Use two large bowls of water, decoct and drink, three times a day. Avoid tea, beef, mutton, chicken, goose, fish, meat, distilled liquor, fermented dough, and sexual exertion. This is considered a secret formula. (Bencao Gangmu [Compendium of Materia Medica]. Zhao Xianke’s note: Shi Chen Formula: Danggui 5 qian; pure Jinyinhua, Fangfeng, Jingjie (Schizonepetae Spica), Heshouwu (Polygoni Multiflori Radix), ensuring no contact with iron vessels, crushed with wooden pestles, each 3 qian; Zaojiazi 9 pieces crushed; Tufuling scraped with a porcelain knife to remove skin, crushed with wooden pestle, 4 liang; one pig pancreas. Use six bowls of river water, decoct in a clay vessel until concentrated, take one bowl warm on an empty stomach every morning, noon, and evening, strictly observing dietary restrictions. Even for chronic cases with heavy toxicity, twenty doses will yield results without sequelae. This formula is mild yet miraculous.)
In the county, a maid belonging to Mr. Chen developed Guangchuang (General Syphilis) and sought treatment from Daoist Fang Shang. His prescription used only dried lotus leaves, decocted into a strong tea, consumed daily until satisfied, resulting in cure within six or seven days. Personal trial proved highly effective. Another case involved a county headman who contracted Guangchuang; he transmitted Fang Shang’s formula, which utilized Maidong (Ophiopogonis Radix) and Muzi Caogen (Mother-Root Grass) crushed into juice, mixed with good wine and taken orally, leading to cure. The grass should be sourced from bamboo, preferably without dew. (Li Yi, Jian’an Manbi [Random Notes from Jian'an])
Feng Chuzhan treated a patient surnamed Zhang who had previously suffered from Meichuang (Syphilitic Chancre) and was cured with cooling and detoxifying methods. Shortly after, he suddenly experienced unbearable pain in the skull, cheeks, gums, and nasal bridge, making eating difficult. Believing it was residual toxin causing harm, Feng again applied cooling and detoxifying methods, which gradually led to refusal of food and drink, restlessness, mental fatigue, and faint pulses in all six positions. Feng said: "If it were truly Yang toxin, the pulse would be surging and large. This situation arises from prolonged use of cold medicines, causing dysfunction of Yin and Yang. Even the initial onset of foul sores stems from depletion of essence, blood, and primordial Yang, allowing Yin-cold stagnation to invade." He prescribed a large dose of Shudi (Rehmanniae Radix Praeparata), Maidong, Baizhu (Atractylodis Macrocephalae Rhizoma), Niuxi (Achyranthis Bidentatae Radix), Wuweizi (Schisandrae Fructus), and Fuzi (Aconiti Lateralis Radix Praeparata), along with Bawei Wan (Eight-Ingredient Pill with Rehmannia), administered together for half a month, resulting in recovery. (Zhao Xianke’s note: If excessive cooling medicines caused Yin-cold stagnation and faint pulses, warming methods should be used to nourish Yang. Why use large doses of Shudi, which is cloying and obstructive?)
Sun Wenyuan treated Wu Dongxing, who suffered from lower back pain after malaria subsided, accompanied by white turbidity in urine, cough, significant muscle wasting, and resistance to various treatments. The pain was severe, nearly fatal. Pulse diagnosis revealed left string-taut and fine, right slippery and large, both rapid (six beats per breath), with thirst and red eyes. Sun deduced that past Meichuang toxins remained latent in the collaterals, triggered by Qi and Blood deficiency post-malaria, causing sudden severe pain. He prescribed Danggui, Baishao (Paeoniae Radix Alba), Gancao (Glycyrrhizae Radix et Rhizoma), Niuxi, Gouteng (Uncariae Ramulus cum Uncis), Yiren, Mutong, and Baixianpi, decocted using 4 liang of Tufuling as the water base. After ingestion, pain ceased, cough reduced by half, and pulse slowed. Several doses yielded great effect, and spirit gradually recovered. On the Winter Solstice, after drinking alcohol and engaging in sexual activity, the next day he experienced waist binding sensation, dorsal foot pain, left eye redness, scanty urination, heat sensation in soles, and rising heat from hips causing unbearable pain. Sun repeated the previous formula adding Shihu (Dendrobii Caulis), Honghua (Carthami Flos), and Shengdi (Rehmanniae Radix), Huangbo (Phellodendri Cortex), regulating for three days with slight improvement. Coinciding with heavy snow and extreme cold, a female physician, mistaking constipation for heat, administered 1.5 qian of Xuanmingfen (Mirabilite) overnight. Bowels remained blocked, waist pain intensified, legs contracted, breathing became weak, face turned blue-grey, and all pulses vanished due to pain. Sun recognized this was caused by Xuanmingfen, stating: "Although symptoms appear hot, the disease lies in collaterals, sinews, bones, and marrow. Purging the intestines and stomach is useless. Leg contraction and waist pain result from extreme cold freezing the collaterals. Treatment should first warm and disperse cold pathogens to relieve pain, then address the root cause." He prescribed Guixin (Cinnamomi Cortex), Duzhong (Eucommiae Cortex), Zhigancao (Prepared Licorice), Cangzhu (Atractylodis Rhizoma), PoguZhi (Psoraleae Fructus), and Wujia Pi (Acanthopanacis Cortex). Two doses stabilized pain, limbs softened, and appetite returned. He advised: "The disease is in the collaterals; wait for spring warmth to eradicate it completely, do not rush." Ignoring this, the patient took Yin-nourishing and fire-clearing herbs for two months with no success. Seeking further treatment, Sun administered Wei Shen San (Baked Kidney Powder), causing five or six loose stools (still purging the intestines). Limbs became cold, and others criticized the purging method. Sun said: "The condition has improved; why worry?" Subsequent Spleen-regulating herbs restored vitality, waist and hips softened, and walking resumed. Finally, 2 qian of Weilingxian (Clematidis Radix et Rhizoma) powder baked inside a pig kidney was eaten, causing one or two more bowel movements. Once damp-heat from syphilis cleared, Sun switched to Yiren, Danggui, Sheng/Shudi, Baishao, Niuxi, Huangbo, Danshen (Salviae Miltiorrhizae Radix), and Guiban (Testudinis Carapax et Plastrum) for final regulation and complete recovery. (Zhao Xianke’s note: Wei Shen San uses Kansui or Qian Niu Zi to unblock collaterals, unlike Xuanmingfen which directly attacks Yangming. Subsequently baking Weilingxian shows exquisite intent.)
Yu Dongfu commented: "This case demonstrates recurring conditions where Master Sun adapted to complex pathogenesis. The record contains seven distinct interventions. Only by documenting the entire course can we discern what worked and what didn't, establishing a model for future practice. Omitting half the history obscures origins and outcomes, making evaluation impossible. This prompts reflection on Linzheng Zhinan [Guide to Clinical Practice].”
Zhu Huaizhu, imprudent in his youth while traveling, contracted Yangmei Chuang. Fearful of discovery, he sought rapid cure via fumigation and washing, which failed. He then applied herbal powders topically and took three doses of decoction daily for two months, but the toxin invaded internally. Lower body manifested anal prolapse and mushroom-like ulcers; upper body showed lung congestion, severe asthma, chest distension preventing supine position, internal heat with aversion to cold, delirium, scrotal nodules with swelling and pain, and spreading ulcers on feet. Pulses were surging, large, and rapid. Zhu diagnosed: "Severe disease, but curable in a young adult. However, there is stagnant blood and toxic matter in the chest. It must be expelled before administering medicine, otherwise confusion may arise." Another doctor was consulted, resulting in severe asthma and coughing up bowl-fulls of purple-black bloody pus, foul-smelling. This was caused by fumigation forcing toxin into lungs, blocking orifices. Asthma and cough required expectoration before clearing. Zhu prescribed Mudanpi (Moutan Cortex), Sangbaipi (Mori Cortex), Baixianpi, Mutong, Qianhu (Peucedani Radix), Zhike (Aurantii Fructus), Jiegeng (Platycodonis Radix), Gancao, Yiren, Xingren (Armeniacae Semen Amarum), and Tinglizi (Lepidii Fructus). Post-ingestion, urgent frequent bowel movements occurred (>20 times), expelling much black-purple purulent blood. Asthma and cough eased, red phlegm lightened. Within 2-3 doses, lying down became possible. Formula changed to Baishao, Danggui, Baixianpi, Beimu (Fritillariae Bulbus), Huanglian (Coptidis Rhizoma), Yinhua, Zaojiao (Gleditsiae Fructus), Yiren, Maidong, Mutong. Two doses reduced foot ulcer swelling; scrotum discharged two bowls of pus/blood; radial pulses normalized. Adding Danshen for two more doses formed scabs, falling off after three days, revealing testicles covered by thin white membrane. Hongfen Shuang (Red Mercury Compound) with flesh-generating herbs applied three times daily dried discharge. Final regulation used Renshen, Danggui, Baishao, Baizhi (Angelicae Dahuricae Radix), Gancao, Baixianpi, Zaojiao, Yiren, Heshouwu for one month until cured. Daily diet included Tufuling and pork simmered together, seasoned normally, soup drunk as tea, ensuring full recovery. A daughter was born the following year.
Tan Gongliang suffered from Jiedu (Syphilitic Gumma). Doctors used Wubao Dan (Five-Treasure Pill) for three years without effect. Zhongchun stated: "Wubao Dan is incomplete; without Hongqian (Menstrual Blood) spiritual medicine, it cannot succeed." Lacking Hongqian, Songzhi (Rosin), Qianfen (Lead Carbonate), and Mazhi (Sesame Oil) were mixed externally, yielding immediate relief. Gongliang initially used Ruxiang Gao (Frankincense Paste) gifted by Qiao Bo, which quickly stopped pain and generated flesh. Using the latter two ingredients proved even more effective, demonstrating that efficacy does not depend on precious materials. (Guang Biji [Broad Records])
Lu Yangyu treated Zhu Shaochuan, who presented with chills, fever, cough, and bone/joint pain due to wind-cold invasion. Dispersing herbs resolved chills/fever but pain and cough persisted. Qianghuo (Notopterygii Radix et Rhizoma) for pain, Qianhu/Xingren for cough failed. Assuming Yin deficiency due to sexual excess, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) was given for a month, worsening thirst, fever, and pain. Pulse was deep, fine, string-taut, rapid, and forceful. Lu asked about past Meichuang. Patient admitted prior infection. Lu diagnosed: "This is residual toxin. Pain indicates toxin in collaterals/marrow; cough/thirst indicates toxic fire flaming upward." He prescribed Blood-nourishing and detoxifying herbs, doubling Tufuling. Reduction seen in few doses, complete cure in ten.
Xue Lizhai treated a man with widespread Meichuang, left hand pulse floating and rapid. Jingfang Baidu San (Jingjie and Fangfeng Decoction to Overcome Pathogenic Factors) resolved exterior symptoms. Xianfang Huoming Yin (Immortal Recipe for Resolving Life-Threatening Sores) for six doses showed gradual improvement. Combined with Bixie Tang (Smilacis Glabrae Decoction), cured in a month.
A boy suffered external genital lesions extending to the lower abdomen, painful and febrile. Xiaochaihu Tang (Minor Bupleurum Decoction) with Lubin Wan (Aloe Pill) and Shenyi Gao (Miraculous Ointment) patch cured in a month.
A scholar presented with itching and oozing on genitals, followed by small scrofula-like nodules on scrotum, inner thighs, abdomen, ribs, arms, some dry, some purulent. Mistakenly treated with wind-dispelling herbs, leading to limb fatigue, chills/fever, reduced appetite, loose stools, floating-stringy pulse, floating-rapid尺 (Kidney) pulses. Diagnosed as Kidney Water deficiency with Heat, Liver Wood overacting on Spleen Earth. Liuwei Dihuang Wan and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) as main formulas, assisted by Huanji Xiaodu San (Transform Flesh and Detoxify Powder), resulted in cure.
Another patient, after taking toxic-purgative herbs, developed convex, red, painful ulcers, limb fatigue, chills/fever, floating-empty pulse. Diagnosed as Yuan Qi (Primal Qi) injury with Excess Evil. Buzhong Yiqi Tang for two doses cured him.
