Bladder deficiency (three syndromes)
Contraindicate breaking qi, drying, promoting urination.
Suitable to tonify qi, sour-astringent.
Herbs: Ginseng, Schisandra, Cornus (Shanzhuyu), Alpinia oxyphylla (Yizhiren), Rosa laevigata (Jinyingzi).

Urinary incontinence, belongs to qi and blood deficiency.
Contraindicate descending, moistening, dry-hot.
Suitable same as bladder deficiency, add Oyster shell (Muli), Dragon bone (Longgu), Deer antler (Lurong), Mantis egg case (Sangpiaoxiao), Chicken gizzard lining (Jineijin).
Frequent urination unable to endure, add Ophiopogon, Schisandra, Cornus, Asparagus, Phellodendron, Biota seed (Baiziren), Turtle shell (Biejia), Achyranthes (Niuqi), Goji berry (Gouqizi).

Enuresis, belongs to deficiency of the meridian qi, see Small intestine deficiency section. Since bladder deficiency can also cause enuresis, it is listed again here.
Contraindications and indications are all seen in Small intestine deficiency.
Bladder deficiency contraindications and indications are all same as Hernia.

Sanjiao deficiency (two syndromes)
Contraindicate breaking qi, descending, again contraindicate ascending/dispersing, bitter-cold.
Suitable to tonify middle and benefit qi, assisted by acrid-warm.
Herbs: Ginseng, Astragalus, Atractylodes macrocephala, Alpinia oxyphylla, Aquilaria (Chenxiang), Schisandra.

Shortness of breath and abdominal cold, belongs to middle qi deficiency.
Contraindications and indications are all same as Sanjiao deficiency.

Heart excess, i.e., excess fire and excess heat. (five syndromes)
Contraindicate tonifying/astringing, ascending, hot, warm-drying.
Suitable to descend fire and clear heat, bitter-cold to break it, acrid-cold to disperse it, sweet-cold to relieve it, salty-cold to moisten it.
Herbs: Coptis (Huanglian), Rhinoceros horn (Xijiao - note: substitute with Buffalo horn in practice), Gypsum (Shigao), Cinnabar, Moutan bark (Mudanpi), Raw licorice (Shenggancao), Talc (Huashi), Bamboo leaf (Zhuye), Ophiopogon, Child's urine.
For hard stool, add Mirabilite, Rhubarb (Dahuang); for mania, treat similarly.

Delirium, belongs to pathogenic heat in the heart. Tongue ulceration, belongs to heart fire. Irritability/restlessness, belongs to pathogenic heat in the heart and internal flaming of heart fire. (Irritability belongs to heart, restlessness belongs to kidney.) Spontaneous laughter, belongs to heat pathogen in the heart. Mania, belongs to severe pathogenic heat in the heart.
Contraindications and indications above are all same as Heart excess.

Liver excess (five syndromes)
Contraindicate tonifying qi, ascending, sour-astringent, acrid-hot, acrid-warm, drying, other herbs recorded later.
Suitable to clear heat and descend qi, bitter-cold, acrid-cold, sweet-cold, sour-cold.
Herbs: Tangerine peel, Green tangerine peel (Qingpi), Perilla seed, Coptis, Gentian (Longdancao), Raw licorice, Scutellaria, Bupleurum, Bamboo leaf, Indigo naturalis (Qingdai), Red peony root (Chishaoyao).

Irritability/Anger, anger causes qi to rebel upward, severe cases vomiting blood and undigested food diarrhea.
Contraindicate tonifying, ascending, hot-drying, closing qi, other herbs recorded later.
Suitable to descend qi, clear heat, sweet-cold, sour-cold, salty-cold, assisted by acrid-dispersing.
Herbs: Perilla seed, Curcuma aromatica (Yujin), Indigo naturalis, Ophiopogon, Red peony root, Raw licorice, Tangerine peel, Safflower pollen (Puhuang), Angelica sinensis, Amomum (Sharen), Cyperus (Xiangfu), Rehmannia, Child's urine.

Sighing, feeling depressed, hypochondriac pain and vomiting blood, belongs to rebellious liver qi, flourishing liver fire, deficient liver blood.
Contraindications and indications are all same as Irritability/Anger.

Convulsions, belongs to pathogenic heat in the liver, heat generates wind, wind governs tremors and vertigo.
Contraindicated same as Irritability/Anger.
Suitable to clear heat, descend qi, promote urination, moderate the center.

Red, swollen, painful eyes, belongs to blood heat.
Contraindicated same as Liver excess/Irritability/Anger.
Suitable to cool blood and clear heat, sweet-cold, bitter-cold, sour-cold.
Herbs: Rehmannia, Red peony root, Eyebright (Gujingcao), Flowering cherry fruit (Mimenghua), Gentian, Licorice, Chrysanthemum, Schizonepeta (Jingjie), Phellodendron, Rhubarb, Forsythia (Lianqiao), Coptis, Scrophularia (Xuanshen), Gardenia (Shanzhi), Bamboo leaf, Azurite (Kongqing), Malachite (Zengqing), Akebia, Child's urine, Mirabilite, Prunus mume kernel (Ruhei).
In acute cases, puncture the swollen area of the eye socket with a three-edged needle, squeeze out hot blood for immediate relief; otherwise, blood will penetrate the pupil and damage the eye.

Spleen excess, i.e., damp-heat pathogen prevailing. (six syndromes)
Contraindicate moistening, astringing, cloying/greasy, hot, salty, sweet, other herbs recorded later.
Suitable to remove dampness and clear heat, promote urination, acrid-dispersing, wind-drying, bitter-cold.

Abdominal distension due to parasites, originates from spleen, caused by accumulation of damp-heat or internal injury, stagnant blood.
Contraindicate breaking qi, sweet-warm, dry-hot.
Suitable to remove dampness, clear heat, promote urination, eliminate accumulation.
Herbs: Plantago seed, Akebia, Stephania (Fangji), Polyporus, Alisma, Poria, Bird beak fish (Wiaohui - likely a typo for specific herb or animal product, kept as is), Lepidium (Tingli), Hawthorn (Shanzha), Red yeast rice (Hongqu), Bupleurum root (Sanleng), Curcuma zedoaria (Pengzhu), Mulberry root bark (Sangbaipi).

Easy hunger, belongs to pathogenic fire in the spleen.
Contraindicate ascending, acrid-warm, great heat, aromatic-drying.
Aquilaria, Musk, Borneol, Cardamom, Agastache, Amomum (above are aromatic-drying, contraindicated).
Suitable to clear fire and remove heat, generate body fluids, benefit spleen yin, sweet-cold, bitter-cold, sour-cold.

Middle Xiao (Polyphagia), mouth ulcers, sores on lips, belongs to spleen heat.

Suitable sweet-cold, sour-cold, bitter-cold, acrid-cold.
Herbs: Ophiopogon, Licorice, Dark plum (Wumei), Coptis, Phellodendron, Rehmannia, White peony root, Scrophularia, Forsythia, Kudzu root, Gypsum, Gentian, Trichosanthes root (Gualougen), Isatis root (Daqing), Bamboo leaf.

Damp-heat abdominal pain, worse upon pressure.
Contraindicate stifling qi, sour-astringent, warm-hot, drying.
Suitable to promote urination, combined with lifting, bitter-cold.
Herbs: Talc, Akebia, Coptis, Scutellaria, Cimicifuga, Bupleurum, Kudzu root, Saposhnikovia, Plantago seed.
If not adding more, it follows the meaning of "earth stagnation requires plundering" (purging to remove spleen stagnation).

Lung heat (eight syndromes)
Contraindicate astringing/stopping, tonifying qi, ascending, dry-hot, sour, salty.
Suitable to descend qi, moisten, sweet-cold, bitter-cold, assisted by acrid-dispersing.
Herbs: Loquat leaf, Perilla seed, Mulberry root bark, Fritillaria (Beimu), Apricot seed (Xingren), Cynanchum (Baiqian), Asparagus, Peucedanum (Qianhu), Plantago seed, Anemarrhena (Zhimu), Mulberry fungus (Sanghuang), Gypsum, Trichosanthes root, Scutellaria.

Asthma/rapid breathing, belongs to excess heat in lung and rebellious lung qi.
Contraindicated same as Lung excess.
Suitable same, add Platycodon (Jiegeng), Licorice, Trichosanthes seed (Gualouren), Scrophularia, Indigo naturalis.

Heavy voice, qi congestion with thick phlegm, belongs to lung heat.
Contraindications and indications are all same as Lung heat.

Throat eczema, Lung distension, Lung abscess, belong to lung heat.
Contraindicated same as Lung excess.
Suitable to clear heat, resolve phlegm, descend fire, detoxify and dissipate nodules, sweet-cold, bitter-cold, acrid-cold.
Herbs: Mulberry root bark, Mulberry fungus, Scutellaria, Trichosanthes root, Fritillaria, Coix seed (Yiyiren), Barrenwort (Huercao), Houttuynia seed (Jimi), Forsythia, Burdock seed (Niuzi), Licorice, Patrinia (Baijiangcao).

Expectoration of pus, blood, and bloody sputum, coughing up blood, belongs to severe excess fire-heat in the liver, this is called "excess when righteous and pathogenic qi struggle".
Contraindicated same as Lung excess.
Suitable to clear heat and descend qi, cool blood, resolve phlegm.
Herbs: Loquat leaf, Mulberry root bark, Child's urine, Perilla seed, Cut grass (Jiancao), Safflower pollen, Ophiopogon, Asparagus, Stemona (Baibu), Mulberry fungus, Lily bulb (Baihe), Licorice, Rehmannia, Coix seed, Fritillaria, Bletilla (Baiji), Platycodon, Aster (Ziwan), White peony root, Winter bud (Kuandonghua).

Upper Xiao, belongs to excess fire in the lung and upper jiao heat.
Contraindicated same as Lung excess.
Suitable to descend qi, clear heat, tonify lung, generate fluids, sweet-cold, bitter-cold, sour-cold, acrid-cold.
Herbs: Perilla seed, Platycodon, Stemona, Lily bulb, Ophiopogon, Loquat leaf, Scutellaria, Glehnia (Shashen), Coptis, Kudzu root, Mulberry root bark, Asparagus, Anemarrhena, Scrophularia, Gypsum, Licorice, Trichosanthes root, Schisandra, Reed rhizome, Winter melon (Donggua), Human milk, White peony root, Bamboo leaf.

Kidney has no excess, thus no purging method.

Mingmen excess (two syndromes)
Contraindicate tonifying qi, warming, heating.
Suitable bitter-cold, sweet-cold, salty-cold.
Herbs: Asparagus, Ophiopogon, Phellodendron, Anemarrhena, Scrophularia, Akebia, Moutan bark, Plantago seed, Alisma.

Persistent erection without desire, belongs to Mingmen fire excess, solitary yang without yin, this syndrome is mostly untreatable.
Contraindicated same as Mingmen excess.
Suitable same, add Schisandra, Child's urine, Rehmannia.

Urethral涩pain, belongs to Mingmen excess fire.
Contraindicated same as Mingmen excess.
Suitable to clear heat, open orifices, sweet-cold, bitter-cold, salty-cold, assisted by light-draining.
Herbs: Plantago seed, Phellodendron, Anemarrhena, Scutellaria, Achyranthes, Rehmannia, Asparagus, Licorice, Child's urine, Poria, Akebia, Ophiopogon.

Small intestine excess (one syndrome)
Contraindicate astringing/stopping, tonifying qi.
Suitable unblocking/draining, light-draining, bitter-cold, sweet-cold, salty-cold.
Herbs: Plantago seed, Poria, Akebia, Phellodendron, Anemarrhena, Raw licorice, Ophiopogon, Scutellaria, Coptis, Achyranthes, Child's urine, Rehmannia.

Difficult urination and redness, or涩pain and hematuria.
Contraindications and indications are all same as Small intestine excess.

Gallbladder excess (two syndromes)
Contraindicate sweating, vomiting, purging.
Suitable harmonizing, acrid-cold, sweet-cold, bitter-cold, acrid-warm.
Herbs: Bupleurum, Scutellaria, Pinellia (Banxia), Fresh ginger, Licorice, Gentian, Tangerine peel.

Bitter taste, deafness, hypochondriac pain, alternating chills and fever.
Contraindicated same as Gallbladder excess.
Suitable use Zhang Zhongjing's Xiao Chai Hu Tang (Minor Bupleurum Decoction), modify according to accompanying syndromes.

Ozena (Nasal sinusitis), belongs to gallbladder transferring heat to the brain.
Contraindicate acrid-warm, dry-hot.
Suitable to clear heat, tonify brain, sweet-cold, sweet-neutral, assisted by acrid-cold.
Herbs: Asparagus, Glehnia, Mint (Bohe), Bupleurum, Magnolia flower (Xinyi), Astragalus complanatus (Shayuanjili), Chrysanthemum, Stone date (Shizao), Scutellaria, Scrophularia, Anemarrhena, Rehmannia.

Stomach excess (six syndromes)
Contraindicate ascending, tonifying/astringing, acrid-warm, dry-hot, moistening.
Suitable purging; if pathogen is not knotted, suitable to clear heat and disperse, bitter-cold, acrid-cold, sweet-cold.
Herbs: Rhubarb, Immature bitter orange, Anemarrhena, Gypsum, Kudzu root, Bamboo leaf, Isatis root (Daqing), Minor Isatis (Xiaoqing), Indigo naturalis, Ophiopogon, Licorice.

Delirium and mania, skin eruptions, discarding clothes and running, climbing high and singing, belongs to excess pathogenic heat in the stomach.
Contraindicated same as Stomach excess.
Suitable same.
If stool is knotted, add Mirabilite to urgently purge. For eruptions, add Burdock seed, Scrophularia, Trichosanthes root, use Gypsum as chief herb; if stool is knotted, also add Rhubarb to purge.

Gastric discomfort and acid regurgitation, bad breath or bland taste, frequent desire to eat, belongs to stomach fire.
Contraindicated same as Stomach excess.
Suitable to clear heat and descend fire, bitter-cold, sweet-cold, acrid-cold.
Herbs: Coptis, Indigo naturalis, Forsythia, Ophiopogon, Dendrobium, Reed rhizome juice, Bamboo leaf, Gypsum.

Vomiting, belonging to stomach fire, must have red face, short red urine, or涩, dry stool, bitter or dry mouth/thirst.

Large intestine excess (four syndromes)
Contraindicate tonifying/astringing, dry-hot.
Suitable moistening purging, bitter-cold, acrid-cold.
Herbs: Hemp seed, Peach kernel, Coptis, Scutellaria, Sophora flower (Huaihua), Rehmannia, Rhubarb, Gypsum, Anemarrhena, Aurantium (Zhike).

Hard stool and constipation
Contraindicated same as Large intestine excess.
Suitable same, add Mirabilite, Pig bile, Areca nut (Binglang), Prunus japonica, Rock sugar (Shimi).

Visceral toxin and intestinal wind bleeding, belongs to large intestine damp-heat.
Contraindicate purging, dry-hot.
Suitable to clear heat, cool blood, combined with lifting, sweet-cold, bitter-cold.
Herbs: Sophora flower, Sanguisorba (Diyu), Coptis, Scutellaria, Rehmannia, White peony root, Schizonepeta, Saposhnikovia, Licorice, Red yeast rice, Platycladus seed (Cebaiye), Pulsatilla (Baitouweng), Safflower pollen, Egg (Jizi), Kudzu root.

Intestinal abscess, belongs to large intestine excess fire.
Contraindicated same as Intestinal wind bleeding.
Suitable purging, bitter-cold, resolving cold (likely typo for resolving toxicity/pus).
Herbs: Rhubarb, Angelica dahurica (Baizhi), Bletilla, Acanthopanax (Bailian), Baiyaosub, Honeysuckle vine (Renwendeng), Forsythia, Licorice, Coptis, Astragalus, Rehmannia, Tianmingjing, Alum (Mingfan), Beeswax (Huangla), Raw honey (above three made into pills).

Bladder excess (one syndrome)
Contraindicate dry-hot, astringing.
Suitable moistening, light-draining.
Herbs: Anemarrhena, Phellodendron, Akebia, Dianthus (Qumai), Plantago seed, Talc, Poria, Polyporus, Alisma.

Retention of urine, belongs to bladder excess heat.
Contraindicate breaking qi, dispersing, dry-hot; if due to insufficient body fluids, also contraindicate promoting urination.
Suitable same as Bladder excess, assisted by lifting, Cimicifuga, Bupleurum.

Sanjiao excess (three syndromes)
Contraindicate tonifying/astringing, ascending, dry-hot.
Suitable descending, clearing heat, regulating qi, sweet-cold, bitter-cold, salty-cold.
Herbs: Perilla seed, Ophiopogon, Anemarrhena, Phellodendron, Scrophularia, Gardenia, Scutellaria, Coptis, Child's urine.

Throat obstruction (Houbi), i.e., winding throat wind, belongs to flourishing Shaoyang ministerial fire and Shaoyin sovereign fire. The classic says: "One yin and one yang knotting causes throat obstruction." One yin is Shaoyin sovereign fire. One yang is Shaoyang ministerial fire. Contraindicated same as Sanjiao excess.
Suitable acrid-dispersing, assisted by bitter-cold, salty-cold; acute cases have three methods: acupuncture, blowing powder, vomiting.
Herbs: Burdock seed, Belamcanda (Shegan), Coptis, Phellodendron, Sophora root (Shandougen), Ophiopogon, Raw rhinoceros horn, Anemarrhena, Scrophularia, Child's urine, Smilax bulbil (Shancigu), Bitter platycodon (Kujiegeng), Euphorbia (Xuisui), Perilla seed, Fritillaria, Licorice.
Acute treatment: use Chalcanthite (Danfan), Glauber's salt (Puxiao), Calculus Bovis, grind to powder, mix well, blow into throat. Another method: use 3 qian Alum, 7去皮 Bay beans, calcine with alum until alum is dry, discard bay beans, take alum powder, blow into throat, hot saliva flows out and relief occurs.

Red and hot head and face, belongs to rising fire in upper jiao.
Contraindicated same as Sanjiao excess.
Suitable descending, clearing heat, sweet-moderating, assisted by sour-astringent.
Herbs: Perilla seed, Asparagus, Ophiopogon, Scrophularia, Mint, Loquat leaf, Pear, Persimmon, Sugarcane, Child's urine, Schisandra, Trichosanthes root, Peony.

Red and white wandering wind, belongs to blood heat, heat generates wind, hence prone to wandering, commonly known as Fire Dan. Children often suffer from this, adults occasionally do too.
Contraindicated same as Sanjiao excess.
Suitable to clear heat, cool blood, combined with moving blood, acrid-cold, sweet-cold, bitter-cold, salty-cold.
Herbs: Coptis, Phellodendron, Safflower pollen, Rehmannia, Raw licorice, Moutan bark, Forsythia, Scrophularia, Achyranthes, Safflower (Honglanhua), Burdock seed, Red peony root, Indigo juice, Ramie root (Zhugen), Child's urine, Moutan bark.
Suitable combined external treatment: scarification to release hot blood, and use Pillwort (Qigucao), Shenhuaocao, mash and apply, heals easily.

Various Malaria, more heat.
Contraindicate acrid-hot.
Suitable to clear heat.

Various Dysentery
Contraindicate breaking, closing qi, astringing, drying, warm-hot, salty-cold, slippery/greasy.
Suitable to clear heat and eliminate accumulation, open stomach qi, lift, promote urination.
Herbs: Coptis, Scutellaria, White peony, Red yeast rice, Hawthorn, Broad-leaved tangerine peel (Guangjuhong), Cimicifuga, Kudzu root, Talc, Lotus seed, Licorice, White hyacinth bean, Dark plum.
If stomach is weak, add 3-2 qian Ginseng, 40 Lotus seeds, 2 qian Tangerine peel, 2 qian Cimicifuga. If abdominal pain, decoct 4 qian Coptis, 3 qian White peony, 1.5 qian Honey-fried licorice, 1 qian Phellodendron, 0.7 qian Cimicifuga. If tenesmus, add above herbs plus 2 qian Angelica sinensis. If severe heaviness in anus, add 1.5 qian Areca nut, 1.5 qian Aurantium, 7 spoons Cyperus juice. If thirsty, remove Cyperus and double Talc. If urine is red,涩, short, or difficult, also double it. If red discharge is heavy, double Dark plum, Hawthorn, Red yeast rice; if white discharge is heavy, add 0.7 qian Evodia. If nausea/vomiting, i.e., Throat-lock dysentery, mostly use Ginseng, Lotus seed, Hyacinth bean, White peony, assisted by 0.7 qian Green Cimicifuga. For chronic dysentery not stopping, add 1 qian Myristica, 3 qian Ginseng, 1.5 qian Amomum, 2 qian Poria.

Diarrhea
Contraindicate moistening, breaking qi, purging, bitter-cold, slippery.
Suitable to calm stomach (tonify spleen), lift, promote urination.
Herbs: Ginseng, Poria, Lotus seed, Atractylodes macrocephala, Cimicifuga, Plantago seed, Tangerine peel, Agastache, Quince, Kudzu root, Honey-fried licorice, White radish (Bailai fu), Hyacinth bean.
For deficiency-cold, add Myristica, Poria, Psoralea, Evodia. For deficiency-heat, remove Atractylodes macrocephala, add Sichuan Coptis, double Peony and Lotus seed.
For summer-dampness disease, resulting in short red urine or thirst, double Ginger-Coptis as chief, assisted by Kudzu root, Cimicifuga. Due to wind-cold invasion, use two Atractodises, Evodia, Amomum, Chenpi, Dried ginger, Perilla. If due to food stagnation, combine with digestion-promoting, Hawthorn, Malt, Medicated leaven (Shenqu), Chenpi, Myristica.
