Siwu Tang (Four Substances Decoction) with added Fushen (Poria with Hostwood) and Yuanzhi (Polygala Root), is used to treat postpartum deficiency vexation. Shiquan Dabu Tang (Ten Substance Great Tonifying Decoction) is even more effective.
[General Theory] Blood rushing to the heart due to residual blood occurs because lochia discharge is obstructed after delivery. It is caused by drinking child's urine, discomfort in the epigastrium, lying down too quickly, or consuming incompatible foods. As long as treatment follows the formula, recovery is certain.
To treat residual blood remaining after childbirth, rushing to the heart, causing vexation and abdominal pain:
Chuanxiong Rhizome (Chuanxiong), Raw Prepared Rehmannia Root (Sheng Dihuang), Aurantium Fruit (Zhiqiao), Peony Root (Shaoyao) (equal parts, ground into powder).
Take one cunbi (a traditional measure for powder) with wine, twice daily.
Jiyan Fang (Collected Experiences Formulas) treats postpartum blood and qi vexation. (Same as Qianjin Fang.)
Raw Rehmannia Juice (Sheng Dihuang Zhi), Clear Wine (Qingjiu) (one sheng each).
Mix and boil once, divide into two doses.
Guangji Fang (Guangji Formulas) treats blood and qi vexation: Drinking two sheng of fresh lotus root juice is effective. Bamboo sap (Zhuli) is also acceptable.
To treat residual blood stagnating within seven days postpartum, occasionally rushing to the heart causing dizziness and oppression:
Lotus Leaf (Heye) (1 liang 7 qian 5 fen), Corydalis Rhizome (Yanhusuo) (2 liang), Rehmannia Juice (Dihuang Zhi) (2 he).
Boil the first two ingredients in two sheng of water until reduced to eight he. Add Corydalis Rhizome. Divide into three doses, taken on an empty stomach. Avoid meat for one day.
To treat residual blood attacking the heart postpartum, or continuous bleeding, chest oppression, cyanotic face, and sensation of cold air threatening extinction:
Sheep blood (Yangxue) one zhan, taken in one dose. If symptoms persist, take again for immediate effect.
Jingxiao Fang (Effective Formulas) treats postpartum qi deficiency where cold pathogen combats blood, causing blood and qi stagnation, rushing upward leading to chest and abdominal fullness:
Angelica Sinensis Root (Danggui), Cinnamon Twig (Guixin), Chuanxiong Rhizome (Chuanxiong), Evodia Fruit (Wuzhuyu), Tangerine Peel (Jupi), Fresh Ginger (Shengjiang) (each one liang), Peony Root (Shaoyao) (2 liang).
Coarsely crush the herbs, boil in three sheng of water until reduced to one sheng. Strain and take on an empty stomach.
To treat residual blood rushing to the heart postpartum:
Use aged white plum (Chen Baimai), crushed. Decoct and drink when thirsty.
Jinhuang San (Golden Powder): Treats postpartum evil blood rushing to the heart, causing intermittent irritability.
Corydalis Rhizome (Yanhusuo), Pollen Typhae (Puhuang) (each half liang), Cinnamon Twig (Guixin) (2 qian 5 fen).
Grind into fine powder. Mix with sour plum decoction, cool, and take two qian.
Moyao Wan (Myrrh Pill): Treats postpartum chest and heart irritability, sluggish evil blood flow.
Myrrh (ground), Ginger (Manjiang), Corydalis Rhizome (Yanhusuo), Dry Resin of Toxicodendron (Ganqi, stir-fried), Angelica Sinensis Root (Danggui), Cinnamon Twig (Guixin), Achyranthes Root (Niuxi), Moutan Bark (Mudanpi), Dried Ginger (Ganjiang) (equal parts).
Grind into powder. Use vinegar-boiled wheat paste to make pills the size of paulownia seeds. Take ten to fifteen pills with fermented grain soup.
Buyi (Supplement) treats postpartum residual blood rushing to the heart, causing vexation and abdominal pain:
Use Puhuang (amount unspecified), stir-fry between papers. Take one qian, mixed with east-flowing water decoction. Shixiao San (Formula for Stopping Pain) is also good.
For postpartum shortness of breath threatening extinction and chest vexation, Zhuye Tang (Bamboo Leaf Decoction):
Bamboo Leaves (Zhuye, finely cut), Ophiopogon Tuber (Mendong), Wheat (Xiaomai) (each one sheng), Licorice Root (Gancao) (one liang), Fresh Ginger (Shengjiang) (two liang), Jujube (Dazao) (12 pieces).
Cut all ingredients. Boil in ten dou of water. First boil bamboo leaves and wheat until reduced to eight sheng. Strain, add remaining herbs, boil until reduced to three sheng. Strain and take warm. For deficiency palpitations, add Ginseng Radix et Rhizoma (Renshen) two liang. For lack of strength, add Glutinous Rice (Nuomi) five he.
Ganzhuru Tang (Sweet Bamboo Sheath Decoction): Treats postpartum internal deficiency, vexation, heat, and shortness of breath.
Sweet Bamboo Sheath (Ganzhuru) (one sheng), Ginseng Radix et Rhizoma (Renshen), Poria (Fuling), Licorice Root (Gancao) (each one liang), Scutellaria Root (Huangqin) (three liang).
Coarsely crush, boil in six sheng of water until reduced to two sheng. Strain and divide into three doses, three times daily.
Renshen Danggui Tang (Ginseng and Angelica Decoction): Treats excessive blood loss postpartum. Blood deficiency leads to yin deficiency; yin deficiency generates internal heat, causing heart vexation, shortness of breath, spontaneous sweating, and headache.
Prepared Rehmannia Root (Shudihuang), Ginseng Radix et Rhizoma (Renshen, remove rhizome), Angelica Sinensis Root Body (Dangguishen, remove rhizome), Cinnamon Bark (Rougui, remove coarse bark), Ophiopogon Tuber (Mendong, remove heart) (each two qian).
White Peony Root (Baishaoyao, stir-fried, two qian 5 fen). For severe blood heat, add Raw Rehmannia Root (Shengdihuang) two qian.
Boil in two zhong of water with one he of Coix Seed (Jingmi) and ten bamboo leaves until reduced to one zhong. Take well after meals.
Xiebai Tang (Allium Macrostemon Decoction): Treats postpartual chest heat and rebellious qi.
Allium Macrostemon Bulb (Xiebai), Pinellia Tuber (Banxia), Licorice Root (Gancao), Ginseng Radix et Rhizoma (Renshen) (each one liang), Trichosanthes Root (Gualougen) (two liang), Ophiopogon Tuber (Mendong) (half sheng).
Coarsely crush, boil in thirteen sheng of water until reduced to four sheng. Strain and divide into five doses, three times during the day and twice at night. For severe heat, add Anemarrhena Rhizome (Zhimu) one liang.
Yunqi treated postpartum deficiency vexation with insomnia using Shaoyao Zhichi Tang (Peony and Fermented Soybean Decoction).
Peony Root (Shaoyao), Angelica Sinensis Root (Danggui), Gardenia Fruit (Zhizi) (each five qian), Fermented Soybean (Xiangchi) (half he).
Prepare and take according to Zhizi Chi Tang method. Postpartum cold damage presents similarly to complications after purging. (Although this formula originates from Yunqi, it is less stable than Zhang Zhongjing's Suanzaoren Tang.)
Zhang Zhongjing's Erwu Huangqin Tang (Two-Ingredient Scutellaria Decoction): When a woman in the puerperal period exposes herself to wind, suffering from vexation and heat in the four limbs and headache, give Xiaochaihu Tang (Minor Bupleurum Decoction). If there is no headache but only vexation, this decoction governs. (See Xiaochaihu Tang in the Shaoyang Disease section of Shanghan Lun.)
Scutellaria Root (Huangqin) (one liang), Sophora Root (Kushen) (two liang), Dry Rehmannia Root (Gan Dihuang) (four liang).
Boil these three ingredients in eight sheng of water until reduced to two sheng. Take one sheng warm. Often causes expulsion of worms via vomiting or diarrhea.
When women are deficient during childbirth, experiencing vexation, restlessness, nausea, and vomiting, to calm the middle and benefit qi, Zhupi Dawan (Large Bamboo Skin Pill) governs.
Fresh Bamboo Sheath (Sheng Zhuru), Gypsum (Shigao) (each two fen), Cinnamon Twig (Guizhi), Anemarrhena Rhizome (Baiwei) (each one fen), Licorice Root (Gancao) (seven fen).
Grind the five ingredients into powder. Mix with date pulp to make pills the size of a bullet. Take one pill with water, three times during the day and twice at night. Increase Baiwei if there is heat. Add Semen Platycladi (Baishi) one fen if there is vexation and panting.
[Chen] Usually, various deficiency vexations and heat are treated with Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) or Wendan Tang (Warm Gallbladder Decoction). However, one must not ignore that postpartum conditions differ from ordinary cases. Herbs like Gypsum should not be used lightly; doing so will cause death.
Empirical Formula: Treats postpartum irritability.
One piece of Yuyuliang (Halloysitum Rubrum), shaped like a sour filling. Bury half of it in the ground, compacting the soil tightly around it. Use one cheng of charcoal and one jin of hair-top fire. Remove the fire when two-thirds have been consumed. Cover with wet sandy soil overnight. Then retrieve, discard the outer layer, and use only the inner part. Grind finely, wash with water, clarify five to seven times, dry on paper, and grind thousands of times more. Take two qianbi mixed with Licorice decoction. One dose brings immediate relief.
