[Da Lun] Women with a deficient constitution may contract wind-pathogenic qi, which travels with the blood or overflows into the skin. This causes sudden pulling pain that migrates without a fixed location, hence the name "Zou Zhu" (Migrating Numbness/Pain). It is treated with Jia Jian Xiao Xu Ming Tang (Modified Minor Continuation Life Decoction).
[Xue Ji's Commentary] Li Dongyuan stated: If a person feels heavy in the body and experiences Zou Zhu pain, this is due to the struggle between damp-heat, or wind-heat stagnation failing to extend, adhering to the physical form. This syndrome often results from irregular diet and lifestyle, or improper management of the Seven Emotions and labor, leading to Spleen-Stomach deficiency. The interstices (Cou Li) become loose, allowing external pathogens to invade, resulting in internal heat, afternoon fever, spontaneous sweating, night sweats, irregular menstruation, and poor appetite. Treatment methods: For swelling and pain caused by damp-heat, use Qing Zao Tang (Clear Dryness Decoction; see Miscellaneous Diseases - Heatstroke). If phlegm is present, assist with Er Chen Tang (Two Aged Decoction; see Miscellaneous Diseases - Phlegm-Rheum). For pain caused by Liver Fire, use Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder). For pain caused by Spleen Stagnation, use Jia Wei Gui Pi Tang (Modified Restore the Spleen Decoction). For pain caused by Blood Deficiency, use Si Wu Tang (Four Substance Decoction). For pain caused by Qi Deficiency, use Si Jun Zi Tang (Four Gentlemen Decoction). For combined Qi and Blood Deficiency, use Ba Zhen Tang (Eight Treasures Decoction). Add Qiang Huo (Notopterygii Rhizoma et Radix) and Chuan Xiong (Ligustici Chuanxiong Rhizoma) to all these formulas. For early menstruation with pain, use Jia Wei Xiao Yao San. For dizziness, fatigue, and pain, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). Generally, if pressing aggravates the pain, it indicates Excess of Pathogenic Qi; if pressing relieves the pain, it indicates Deficiency of Yuan Qi (Primal Qi). Pain after labor also indicates Yuan Qi Deficiency. Pain due to improper diet indicates Spleen Qi Deficiency. Pain after anger indicates Exuberant Liver Fire. If symptoms are lighter during the day and heavier at night, it is a disease of the Blood aspect.
Case 1: A woman suffered from Li Jie (Joint Ankylosis) with fever and thirst, reduced appetite, and delayed menstruation. Her pulse was洪大 (Huge and Large) when lifted and微细 (Fine and Thin) when pressed. After four doses of Fu Zi Ba Wu Tang (Aconite Eight-Ingredient Decoction), the pain ceased. After Jia Wei Xiao Yao San, her Yuan Qi recovered. After Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia), her menstruation regulated.
Case 2: A obese woman with prior heat and early menstruation developed Tong Feng (Gout-like pain). She had slight lower limb swelling, frequent urination during severe pain, heaviness, and a slow pulse. This was Wind-Dampness with Blood Deficiency and Heat. Two doses of Qiang Huo Sheng Shi Tang (Notopterygium Overcoming Dampness Decoction) gradually healed the swelling and pain. Several doses of Qing Zao Tang cleared the urine. Jia Wei Xiao Yao San resolved the internal heat. Later, food stagnation caused recurrent fever and pain with facial edema; Liu Jun Zi Tang (Six Gentlemen Decoction) added Chai Hu (Bupleuri Radix) and Sheng Ma (Cimicifugae Rhizoma) cured her. Anger caused lower abdominal fullness, chills, fever, and vomiting; adding Shan Zhi (Gardeniae Fructus) and Mu Xiang (Aucklandiae Radix) to the previous formula settled her. Only lower abdominal bearing-down sensation remained, indicating Spleen Qi Sinking, cured by Bu Zhong Yi Qi Tang. Later, labor and anger caused vomiting of phlegm, whole-body swelling/pain, and menstrual chills/fever, indicating Liver Wood insulting Spleen Earth. Liu Jun Zi Tang added Chai Hu and Shan Zhi resolved the symptoms, followed by Bu Zhong Yi Qi Tang for stability.
Case 3: A woman with poor appetite, aversion to wind/cold, Tong Feng, vomiting, chills/fever, and a string-tight pulse. Fu Zi Ba Wu Tang cured the limb pain; Du Huo Ji Sheng Tang (Radix Saposhnikoviae and Taxillus Decoction) cured the back pain. Knee swelling/pain was cured by Da Fang Feng Tang (Major Saposhnikovia Decoction); Gui Pi Tang and Xiao Yao San restored her Yuan Qi.
