Shenghui Fang states: Pediatric dysentery with pus and blood is caused by heat-toxin accumulating in the zang-organs. Blood, when subjected to heat, flows and overflows, seeping into the large intestine, where it contends with the fluids between the intestines. Accumulated heat condenses, transforming blood into pus; due to abdominal deficiency, diarrhea occurs, thus forming dysentery with pus and blood.
Ge Shi's Zhouhou Fang treats pediatric toxic dysentery and red-dysentery with stagnation (dysentery), where excrement resembles fish brains, using the Baitouweng Wan (Baitouweng Pill) formula:
Baitouweng (Pulsatillae Radix) (3 fen) Huanglian (Coptidis Rhizoma) (6 fen, ground) Shiliupi (Punicae Pericarpium) (3 fen; if toxic, remove Shiliupi and use Xijiaoxie (Rhinocerotis Cornu Treated Slice) 3 fen instead).
The above three substances are boiled in two sheng of water until eight ge remains. For an infant born forty days old, administer five ge divided into three doses; for older children, increase the dosage accordingly.
Zhouhou Fang nursing mother prescription:
Biandoujing (Vigna umbellata Stem) (1 sheng, roasted until dry, then cut) Renshen (Ginseng Radix et Rhizoma) (3 liang).
Boil the above in three sheng of water to obtain one and a half sheng of decoction. Strain and take the juice to cook millet porridge for the nursing mother to eat; this is beneficial. Usually, cover and keep the breast milk warm, avoiding coldness, which is optimal. Another method: if the nursing mother frequently eats porridge and wishes to nurse the infant, squeeze out a small amount first, then feed; this should be effective.
Zhouhou · Jinxiao Fang treats pediatric dysentery starting at age three, initially presenting with little pus and much blood, changing after four days to much pus and little blood, with more than forty bowel movements day and night. The Zhuzi Wan (Zhuzi Pill) formula is effective upon administration:
Shengdihuang (Rehmanniae Radix) juice (5 small he) Yangshenzhi (Ovis/Capis Ren Medulla) (1 small he).
First warm the kidney fat until hot, divide into three or four doses for immediate effect. The nursing mother must observe dietary restrictions; see the nursing mother formula mentioned earlier.
Qianjin Fang Huangbai Tang (Phellodendri Decoction): Treats pediatric summer injury from sudden cold, where cold suppresses excessive internal heat, allowing heat to enter the stomach, resulting in discharge of red and white mucus resembling fish brains, high fever, headache, body heat, and restlessness in hands and feet. This occurs when Taiyang qi is externally injured by cold, causing heat to enter the stomach; taking this formula is beneficial. If mistakenly treated with purgatives or Wentai Tang (Warming Spleen Decoction), heat will intensify. If purgatives cause frequent passage of red fluid like rotten flesh, or if purgation fails to cure and subsequent use of astringent-hot medicines stops diarrhea but fails to stop flow while doubling high fever, this formula is effective. Alternatively, if warm disease heat is severe and suddenly suppressed by cold, causing heat to enter the abdomen and blood discharge resembling fish brains, this formula is beneficial.
Huangbai (Phellodendri Cortex) Huanglian (Coptidis Rhizoma) Baitouweng (Pulsatillae Radix) [or Bailian (Ampelopsis Radix)] Shengma ( Cimicifugae Rhizoma) Danggui (Angelicae Sinensis Radix) Muli (Concha Ostreae) Shiliupi (Punicae Pericarpium) Huangqin (Scutellariae Radix) Sangjisheng (Taxilli Caulis) Gancao (Glycyrrhizae Radix et Rhizoma) (roasted, each 2 fen) Xijiao (Rhinocerotis Cornu) Aiye (Artemisiae Argyi Folium) (each 1 fen).
Twelve ingredients are chopped. Boil in three sheng of water to obtain one sheng and two ge. For infants aged 100-200 days, administer 3 ge per dose; for those aged over 200 days up to one year, administer 3.5 ge per dose.
Qianjin Fang miscellaneous formulas for pediatric red-white dysentery:
Xiebai (Allii Macrostemonis Bulbus) one bunch, Chi (Sojae Semen Praeparatum) one sheng, boil in three sheng of water to obtain two sheng, divide into three doses. Baiye (Platycladi Cacumen) and Mazimo (Cannabis Fructus) powder, one sheng each, boil in five sheng of water for three boils; for 100-day-old infants, administer 3 ge per dose. Luofahui (Hair Ash) and Lujiaohui (Cervi Cornu Colla Ash) in equal parts, mix with water for 3-year-old children to take 3 qianbi, three times daily.
