Q: What is Guangchuang (Syphilis)?
A: This condition arises from damp-heat in the Liver and Kidney meridians. It may result from excessive sexual activity damaging the Kidney meridian, leading to susceptibility to turbid pathogens. Alternatively, it may stem from accumulated toxins from chancre (Xiagan) that persist and flare up. Also known as Fanhuachuang (Flower-turning Sore), where flesh protrudes outward, resembling wax or cotton, hence also called Mianhuachuang (Cotton Sore); this indicates intense pathogenic toxin. Smaller lesions are called Guangdou, or resemble red-rooted pustules, indicating shallower toxin. For this condition, one should first consume toxic substances to vent the toxin, then administer Tongsheng San. Tufuling (Smilax Glabra Rhizome) must be taken concurrently. Once the toxin subsides, use Bazhen Tang or Shiquan Dabu Tang to tonify Qi and Blood. Strict adherence to dietary and lifestyle taboos is required for full recovery. Sexual abstinence is strictly prohibited; violation renders medication ineffective and risks chronic toxicity. Avoid vinegar, as its astringent nature traps toxins, leading to Guangxuan (Syphilitic Tinea). Avoid pork intestines, which can stir latent fire, prolonging the illness. Avoid Calomel (Qingfen) and cold water, which cause subsequent bone/joint pain and wind-type nodules, potentially appearing years later. Symptoms include hard, pale flesh, itching or pain, often at joints, head/face, throat/nose, or meridian intersections. Ulcerated lesions weep pus profusely, which is alarming. Mild cases manifest as Guangxuan (Thousand-layer Tinea), often on palms and soles. Residual toxin, also called Qi Du or Shi Du, causes wandering bone/joint pain or intermittent soreness; treatment depends on depth. Initially use Xiaofeng Baidu San, followed by tonics. For severe sores or deep nodules, fumigation is essential, followed by Tongsheng San to clear fire-toxin, then Bazhen Tang or Dabu Tang. Weak patients should avoid fumigation to prevent overwhelming heat.
Xue Ji states: This disease stems from deficient Yuan Qi allowing pathogenic invasion, or transmission. Located in Liver/Kidney meridians, it often appears on the lower body. Some experience joint pain before sores; others vice versa. Initial floating-rapid pulse indicates pathogen in Spleen/Lung meridians; treat initially with Jingfang Baidu San. Wiry-rapid pulse indicates Liver/Gallbladder involvement; treat initially with Longdan Xiegan Tang. Deep-rapid pulse indicates internal organ involvement; treat initially with Neishu Huanglian Tang, then HuANJI Xiaodu San as main formula. If sores are protruding, red, and painful, heat-toxin is intense. Mild pain indicates toxin subsiding. White sores without scabs indicate Yang deficiency. Red sores without scabs indicate Yin/Blood deficiency. Itching with floating-deficient pulse indicates Qi failure to nourish; itching with floating-rapid pulse indicates Blood failure to nourish. Itching between buttocks/back or neck indicates Bladder Yin deficiency. Itching in genitals or inner thighs indicates Liver Blood deficiency. Itchy, heavy scrotum indicates Liver Yin deficiency with damp-heat. Frequent, scanty, red urine indicates Liver Yin deficiency. Frequent, scanty, white urine indicates Spleen/Lung Qi deficiency. Facial itching or redness after scratching indicates external pathogen conflict. Eyebrow itching or hair loss indicates Liver/Gallbladder blood dryness. Poor appetite, dry mouth craving hot drinks indicates Stomach Qi deficiency. Indigestion, loose stools indicate Spleen Qi deficiency. Morning or night diarrhea indicates Spleen/Kidney deficiency. Improper treatment may cause eye damage, glans ulceration, or limb contracture. Use Jiwei Luhui Wan to clear Liver fire, Liuwei Dihuang Wan to nourish Kidney water, Juanbi Jiedu Yin to nourish Blood and dispel pathogens. Misuse of Calomel makes treatment difficult. For excess dampness, drain dampness first. For exterior excess, release exterior first. For interior excess, purge interior first. For both excess, release exterior and purge interior. For exterior deficiency, tonify Qi. For interior deficiency, tonify Blood. For both deficiencies, tonify Qi and Blood.
Case 1: Patient with whole-body sores, rapid left wrist pulse. Used Jingfang Baidu San to resolve exterior symptoms, then Xianfang Huoming Yin for six doses to improve sores, combined with Bixie Tang for over a month until cure.
Case 2: Woman with pain, fever, constipation, thirst, deep-solid pulse. Used Neishu Huanglian Tang for two doses to resolve interior symptoms, then Longdan Xiegan Tang for several doses to reduce toxin, intermittently taking Bixie Tang for over a month until cure.
Case 3: Patient with genital chancre unresponsive to other drugs, spreading to whole body with swelling resembling flower-turning, tendon spasm, bone pain worse at night, caused by Liver/Kidney damp-heat. First administered Daoshui Wan five doses, then Longdan Xiegan Tang for several doses, followed by damp-draining and Spleen-strengthening herbs. Applied Shenyi Gao externally to draw pus, used Garlic-moxibustion to extract toxin, resulting in cure.
For exterior excess, use Jingfang Baidu San. For interior excess, use Fangfeng Tongsheng San. For Qi deficiency, use Sijunzi Tang. For Blood deficiency, use Siwu Tang plus symptom-specific herbs. Misuse of Calomel traps toxin internally, causing recurrence. Overuse of Fangfeng Tongsheng San depletes Qi/Blood, often leading to untreatable conditions.
Case 4: Patient with chills, fever, thirst, constipation, solid pulses on both hands. Used Fangfeng Tongsheng San to resolve symptoms, then Jingfang Baidu San combined with Longdan Xiegan Tang for cure.
Case 5: Patient with swelling and pain. First used Longdan Xiegan Tang and Daoshui Wan (four doses each) for slight improvement, then Xiao Chaihu Tang plus Huangbai and Cangzhu for over fifty doses to stabilize.
Case 6: Patient with swollen/ulcerated glans, red/scanty urination, wiry-rapid Liver pulse. Used Xiao Chaihu Tang plus Mutong, Qingpi, Longdancao for four doses, then Longdan Xiegan Tang for several doses for cure.
Case 7: Child with glans infection spreading to lower abdomen, pain, fever. Used Xiao Chaihu Tang with Luhui Wan, applied Shenyi Gao, cured in over a month.
Case 8: Patient with leg lump post-cure, ulcerating slowly. Applied mashed garlic, moxa, Shenyi Gao, Heiwanzi, and interior-supporting herbs, cured in two months.
Case 9: Woman fumigated with Calomel under blankets, causing skin collapse and pus discharge. Used powder of Shihuang, Huangbai, and Lvdou Fen spread on bed, took Jinyinhua San, cured in over a month.
Case 10: Patient cured except for hard back lump, ulcerating after a year, weeping for three years, with Qi/Blood deficiency and poor appetite. Used Liujunzi Tang plus Danggui and Huoxiang for thirty doses, plus Bixie Tang, cured in over two months.
Case 11: Patient with intense disease and Spleen/Stomach Qi/Blood deficiency, cured with previous formulas.
Case 12: Woman cured except for palm-sized ulcers on legs/armpits, tendon spasm, bone pain for three years. Unresponsive to drugs, evening fever, poor appetite. Used Bixie Tang combined with Xiaoyao San, doubled Baizhu and Fuling for several doses to stop fever and improve appetite, applied Shenyi Gao, then Bazhen Tang plus Niu Xi, Duzhong, Mugua for thirty doses for cure.
Case 13: Post-syphilis patient with ulcers on legs/arm, weeping pus, poor intake/sleep. Used Bazhen Tang plus Fushen and Suanzaoren, daily garlic application and moxa, followed by plasters for slight relief. Then Douchi moxa and Shiquan Dabu Tang for cure.
For swelling/hardness/pain, garlic moxa and Zhonghe Gao with internal tonics are effective.
Case 14: Scholar with prior symptoms: glans itching/discharge, then small sores/dry pustules on scrotum/thighs/abdomen/flanks/arms. Mistakenly used wind-dispelling drugs, causing fatigue, chills/fever, reduced appetite, loose stools. Left Chi pulse surging-rapid, Left Guan wiry-rapid. Right Guan floating-slow, slightly wiry on pressure. Diagnosis: Liver/Gallbladder channel issue. Left Guan wiry, Left Chi floating-rapid indicates Kidney Water deficiency transmitting empty heat to Liver. Right Guan floating-slow indicates weak Spleen/Stomach Qi. Pressure wiriness indicates Liver Wood overacting on Spleen Earth. Used Liuwei Dihuang Wan and Buzhong Yiqi Tang as main, assisted by Huanji Xiaodu San for cure.
Case 15: Scholar with red/painful sores, constipation. Took wind-dispelling/blood-cooling drugs, causing tongue pain, dry mouth, floating-rapid pulse. Indicates pathogen gone but Yin deficiency. Used Liuwei Wan formula plus Zhizi and Danggui for four doses, pulse/symptoms receded. Then Bazhen Tang plus Zhizi and Danpi, sores turned white, finally Sijunzi Tang plus Danggui for cure.
Case 16: Patient taking toxin-purging drugs developed protruding/red/painful sores, fatigue, chills/fever, floating-deficient pulse. Indicates recurrent Qi injury with excess pathogen. Used Buzhong Yiqi Tang for two doses for cure.
Case 17: Scholar mistakenly used wind-dispelling/toxin-clearing drugs, injuring Yuan Qi. Due to overwork, experienced internal heat, dry mouth, gum pain, surging-rapid-deficient Right Guan pulse. Indicates Spleen/Stomach injury with stirring fire. Used Qingwei San for cure.
Official Liu Huafu suffered for months, used Calomel/Cinnabar, developed inch-sized nodules on face/arms/buttocks, ulcerated with weakened form/Qi, chills/fever, dry mouth/tongue/lips, dribbling urine, phlegm rising, poor appetite. Indicates Spleen/Stomach injury, weak organs, stirring empty fire. Used Liujunzi Tang for twenty doses, then Buzhong Yiqi Tang plus Shanzhuyu, Shanyao, Maidong, Wuweizi to restore Stomach Qi and resolve symptoms. Only unclear urine/phlegm remained, cured with Jiawei Bawei Wan.
Case 18: Scholar with facial itching, granular/plaque-like sores/discharge, surging-rapid Spleen/Lung pulses. Indicates Wind pathogen injury. Used Jingfang Baidu San plus Bixie and Gouteng for several doses for gradual improvement. Dry mouth/internal heat treated with Siwu Tang plus Zhizi, Gouteng, Jinyinhua, Gancaojie for cure. Subsequent whole-body itching/internal heat/dry mouth resolved with assistance of Liuwei Wan.
Case 19: Merchant developing sores after labor/alcohol, took wind-dispelling/blood-cooling agents. Developed peach-nut sized nodules on face/chest/back/arms/flanks, uneven, itchy/painful, weeping pus, severe Qi/Blood deficiency, alternating chills/fever, thirst, phlegm obstruction. Indicates damp-heat congestion with deficient Yuan Qi unable to recover. Applied Danggui Gao externally; internally used Buyin Bazhen plus Bixie (5 qian), Huanji Xiaodu San plus Gegen and Gouteng (1 qian each) for over twenty doses to gradually retreat symptoms. Continued main formula with regulating agents for over two months, restoring Qi/Blood and curing sores.
