A patient presented with swelling, pain, fever, thirst, and sweating. Xue Ji stated: This is injury to Yin blood. First, use Bian stone (a surgical tool for draining pus or blood) to remove foul matter to unblock congestion, then administer Siwu Tang (Four Substances Decoction) [Danggui, Chuanxiong, Baishao, Shudihuang] added with Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), Danpi (Moutan Cortex), and Gusui Bu (Drynariae Rhizoma) to clear Liver fire, resulting in recovery.
Another patient had a wound where the tissue was rubbed apart, yet swelling and pain persisted. Xue Ji stated: This is stasis of blood retained internally; urgent application of Bian stone is indicated. The patient did not comply. Xue Ji applied a paste made from fresh radish juice mixed with Zhizi powder externally, covered the broken skin with Danggui Ointment, and administered internal blood-activating agents, leading to improvement. Years later, itching and pain recurred whenever the weather was cloudy, revealing the error of not having used Bian stone initially.
A third patient presented with blackish swelling of the buttocks and thighs without skin rupture, accompanied by distending pain and a heavy sensation. Others believed there was no internal stasis and recommended cooling topical applications for pain relief. Xue Ji palpated the Chi pulse as choppy and knotted, concluding that due to the patient's obesity and thick adipose tissue, the stasis was deeply accumulated. Puncture would lead to immediate cure; otherwise, internal ulceration with risks of tendon rotting and bone damage would occur. Xue Ji inserted the needle four cun deep, draining several sheng of dark blood, after which swelling and pain ceased. On that day, the patient developed fever, aversion to cold, irritability, thirst, and headache, signs of deficiency of both Qi and Blood. Recovery was achieved using Shiquan Dabu Tang (Ten Thousand Benefits Great Tonifying Decoction).
A man suffered a sprain in his right thigh, resulting in localized swelling and pain. Xue Ji advised urgent puncture to remove stagnant blood and tonify Yuan Qi to prevent future complications. The patient refused, opting instead for external application of Dahuang (Rhei Radix et Rhizoma) and other herbs, and internal administration of Liqi Yin (Flowing Qi Decoction). Later, copious amounts of foul pus (several bowls) were discharged continuously. Upon re-evaluation, the leg appeared emaciated while the pulse was large, accompanied by thirst and fever. Xue Ji declined treatment, and the patient subsequently died.
Wang Rudao, a friend of Xue Ji, suffered a sprain at the Huantiao point (GB30), presenting with stasis-induced swelling, pain, fever, and thirst. Xue Ji immediately used Bian stone to remove stasis. Knowing the patient had underlying deficient fire in the Lower Jiao, he prescribed Bazhen Tang (Eight Treasures Decoction) [Renshen, Baizhu, Fuling, Gancao, Danggui, Chuanxiong, Baishao, Shudihuang] added with Huangbai (Phellodendri Cortex), Zhimu (Anemarrhenae Rhizoma), Niuxi (Achyranthis Bidentatae Radix), and Gusui Bu, which resolved symptoms within four doses. Subsequently, Shiquan Dabu Tang was used with minor additions of Huangbai, Zhimu, Maimen Dong (Ophiopogonis Radix), and Wuweizi (Schisandrae Fructus); after more than thirty doses, the wound healed.
A woman injured her arm, causing bleeding, bone pain, fever, thirst, irritability, and dizziness, with worsening symptoms in the afternoon (Ribu). This was diagnosed as Yin deficiency with internal heat. She was treated with Bazhen Tang added with Danpi, Maimen Dong, Wuweizi, Gusui Bu, and Rougui (Cinnamomi Cortex), along with Dihuang Wan (Rehmannia Pill), resulting in complete recovery. Later, Rougui was removed, and Niuxi and Xuanduan (Dipsaci Radix) were added; after more than twenty doses, the sore healed.
