[General Principle]
All swellings and fullness caused by dampness are primarily attributed to the Spleen. (From *Neijing*) The Spleen governs the transformation and transportation of water and grain. When deficient and failing to transport, water-dampness stagnates. Both large meridians and small collaterals become filled with turbid corruption. Body fluids and blood all transform into water. Consequently, swelling occurs in the face, eyes, and four limbs. (From *Huibu*)
[Internal Causes]
The True Water (Yin) and True Fire (Yang) of the human body digest all things to nourish the body. (From *Rumen*) Therefore, water metabolism is governed by the Kidney, while Earth (Spleen) is generated by Fire. Only when Kidney deficiency fails to move water, and Spleen deficiency fails to control water, does Kidney water overflow and soak the Spleen Earth. Thus, the Triple Burner stagnates, meridians are obstructed, water seeps into the skin, and pours into the muscles, causing edema. (From *Xinfa*)
[External Manifestations]
When edema first begins, the eye sockets swell slightly, resembling the appearance after just waking up. The neck pulse throbs, accompanied by occasional coughing. There is coldness between the thighs, swelling of the calves, and abdominal distension. Pressing the abdomen with the hand causes the indentation to rebound immediately upon release, resembling a state wrapped in water. The skin is thin and shiny. (From *Zhenjing*)
[Yang Edema and Yin Edema]
If the whole body is swollen, with yellow-red skin color, irritability, thirst, scanty and difficult urination, constipation, and a deep, rapid pulse, this is Yang Edema. If the whole body is swollen, with blue-white skin color, no thirst, loose stools, and scanty urine without difficulty, this is Yin Edema. (From Zhu Danxi) Yang edema is externally caused by wading in water or exposure to rain, or combined with wind-cold-summerheat pathogens; it usually starts from the upper body, involving the shoulders, back, hands, and face, corresponding to the Three Yang Meridians of the Hand. Yin edema is internally caused by cold water, wine, tea, or combined with fatigue and excessive sexual activity; it usually starts from the lower body, involving the waist, abdomen, shins, and feet, corresponding to the Three Yin Meridians of the Foot. (From *Rumen*)
[Qi Swelling and Water Swelling]
Thick skin with dark complexion, emaciated limbs, chest and abdominal bloating, with swelling developing from top to bottom, mostly belongs to Qi swelling. Thin, tender skin with distinct boundaries of swelling, developing from bottom to top, mostly belongs to Water swelling. Additionally, pressing leaves no depression and rebounds immediately indicates Qi swelling; pressing forms a depression that does not rebound immediately indicates Dampness. (From *Rumen*)
[Wind Swelling and Blood Stasis Swelling]
Wind swelling features wandering pain, rough and numb skin, which is also known as gout with body swelling. Blood stasis swelling features shiny skin with red marks and blood threads, indicating blood transforming into water. (From *Rumen*)
[Fengshui and Shuishui]
Fengshui (Wind Edema) presents with facial swelling and whole-body edema, spontaneous sweating, aversion to wind, floating pulse, heavy body, bone and joint pain, and no thirst; it is suitable for exterior-dispersing treatment. Shuishui (Stone Edema) presents with abdominal fullness but no panting, with a deep pulse; it is suitable for promoting urination. (From *Zhunsheng*)
[Shuifen and Xuefen]
Women's body swelling differs between Shuifen (Water Factor) and Xuefen (Blood Factor). Shuifen involves dampness retention in the Middle Jiao, hardness and enlargement under the heart, disease originating above, with edema appearing before menstruation stops; treatment focuses on the Middle Jiao. Xuefen involves blood knotting at the uterine gate, abdominal distension below the navel, disease originating below, with menstruation stopping before edema appears; treatment focuses on the Lower Jiao. Furthermore, Xuefen disease features hard and painful lower abdomen, intolerable to touch, with clear and long urinary flow, which differs greatly from Spleen deficiency patterns featuring soft large abdomen and obstructed, scanty urinary flow; it is suitable for blood-breaking formulas. If due to pregnancy, there may also be swelling caused by qi obstruction in the water passages, but only qi-smoothing and fetal-anchoring methods should be used; swelling will subside naturally after delivery. (From *Huibu*)
[Interrelation of Panting and Distension]
Panting first, then swelling: this is Lung failure to transform qi, leading to water accumulation as swelling; treatment focuses on the Lung. Swelling first, then panting: this is Spleen failure to transport, leading to water flooding causing panting; treatment focuses on the Spleen. (From *Zazhe*) Treating the Lung requires clearing Metal and descending qi, with promoting water as secondary. Treating the Spleen requires strengthening the Spleen and resolving dampness, with descending qi as an adjunct. (From *Huibu*)
[Lung-Kidney Transmission]
Spleen disease causes Lung Metal to lose nourishment; not only is Lung qi isolated and critical, but turbid qi rises, causing panting and cough. Only when Earth is solid can Lung Metal be clear and肃 (descending), thereby nourishing the source of transformation. Also, Spleen disease causes body fluids to fail to transform; not only is Kidney essence damaged, but damp-heat pours downward, causing dorsal foot swelling. Only when Earth is strong can Kidney water be retained and return to its proper channels. (From *Rumen*)
[Pulse Diagnosis]
A洪大 (Huge and Large) pulse indicates easy treatment. A微细 (Fine and Thready) pulse indicates difficult treatment. A pulse that suddenly appears and disappears indicates death. (From *Maiejing*)
[General Treatment Principles]
The general principle is to regulate the Middle Jiao and strengthen the Spleen. When Spleen qi is solid, it naturally ascends and descends, transporting and circulating, so water-dampness eliminates itself. This treats the root cause. (From Zhu Danxi)
[Six Methods of Differentiated Treatment]
The method for treating water disease is to act according to natural principles, following the exterior-interior, cold-heat, and upper-lower dynamics, guiding the pathogen along its trend. Thus, sweating, purging, draining, clearing, drying, and warming are appropriate. Among these six methods, changes should not be rigidly adhered to. (From *Huibu*)
[Treatment Divided by Yin and Yang]
Yang Edema: Suitable for acrid-cold herbs to dissipate nodules and move qi, and bitter-cold herbs to drain fire and dry dampness. Yin Edema: Suitable for bitter-warm herbs to dry the Spleen and overcome dampness, and acrid-hot herbs to guide qi and support Yang. (From *Rumen*)
[Treatment Divided by Sweating and Draining]
Body heat: suitable for sweating. No body heat: suitable for promoting urination. Muscle pain: suitable for sweating. Red and difficult urination: suitable for promoting urination. Swelling above the waist: suitable for sweating. Swelling below the waist: suitable for promoting urination. This is called "opening the ghost door" (sweating) and "cleaning the clean mansion" (promoting urination), eliminating from both upper and lower. (From *Huibu*)
[Clearing Damp-Heat]
Dampness is the qi of Earth, and Earth is the child of Fire; thus, dampness often generates heat, and heat often becomes dampness. Mother and child influence each other, representing a change in qi. When damp-heat is excessive, fire attacks the Spleen causing swelling, it is suitable to clear Heart fire and descend Lung Metal, ensuring Liver Wood is controlled and the Spleen is free from thief pathogens. With normal ascending-descending circulation of clear and turbid, damp-heat transforms, and the drainage channels open. The败浊 (decayed/turbid) qi, the clear part returns to become qi, blood, and body fluids; the turbid part becomes sweat above and urine below, gradually exiting. (From Zhu Danxi)
[Warming Cold-Dampness]
Although water is controlled by the Spleen, it is actually governed by the Kidney. The Kidney is originally a Water organ, housing Yuan Yang (Primordial Yang). If Mingmen Fire declines, it cannot control yin-cold or warm-nourish Spleen Earth, so yin does not follow yang, and essence transforms into water. Thus, some edema is due to fire decline. External symptoms include limb swelling, cold hands and feet, indigestion, loose stools, and cold breath. This is true Yang collapse with Spleen, Lung, and Kidney deficiency. (From Xue Ji *Lizhai*) The method should warm the Middle Jiao and warm the Lower Jiao, allowing Shaohuo (Ministerial Fire) to generate qi, steaming Spleen Earth upward. When Yuan Yang recovers and yin shadows dissipate, the Triple Burner receives orders, the Juedu (waterways) function properly, and water passages naturally unblock. (From *Bidu*)
[Nourishing Yin Deficiency]
The Kidney is the gate of the Stomach. If the gate is not smooth, water accumulates causing disease. Thus, some edema is due to Yin deficiency. Lung Metal fails to descend, causing swelling. Symptoms include large abdomen, swollen umbilicus, low back pain, stiff feet, short and difficult urination, cough with phlegm, inability to lie flat, red face, thirst, yet appetite remains, and stool is dry. This is water rising with Dragon, and Ministerial Fire overflowing water. It is suitable to nourish Yin and tonify the Kidney, while protecting the Lung and transforming qi. (From *Zhunsheng*)
[Attacking Excess Pathogens]
Swelling caused by external anger and internal dietary injury: Why? Because Liver is often in excess; anger makes it flourish further, injuring the Spleen, making Spleen more deficient. Dietary injury leads to non-transportation and dampness generation. Excessive damp-heat,郁极 (stagnating to extreme), erupts, reaching the head above, flowing to the feet below, filling the front and back of the body in the middle. Swelling like a gourd, firm like stone, cold like ice, unable to sit or lie down, is hardest to treat. Although promoting urination is indicated, the bladder inside and scrotum outside are tightly connected, blocking the path, with no passage. How can the disease leave? One must open the large intestine to expel water, supplementing as one purges, so pathogens go and healthy qi is undamaged, followed by gradual regulation, hoping for survival. (From Zhao Xianke *Yuyi Cao*)
[Warning Against Over-Draining]
Treating dampness by promoting urination is a common method, but sticking to this for deficiency syndromes often leads to death. Because Spleen qi is deficient and failing, further draining makes it more deficient. Although immediate effect is achieved, healthy qi and Yin are damaged. (From Zhu Danxi)
[Contraindications for Edema]
At the onset of edema, when the pathogen is sharp, one must strictly avoid sweet-warm herbs that aid dampness and cause fullness. Acupuncture is especially forbidden; violating this leads to fatal fluid loss. Alcohol and sex should be abstained from, salt and soy sauce avoided, and anger restrained, to preserve Taihe (Great Harmony). Otherwise, it is incurable. (From *Rumen*)
[Prognosis of Fatal Edema Cases]
Abdominal distension first, then spreading to limbs: treatable. Limb swelling first, then returning to abdomen: difficult to treat. Black lips, scorched ears, fullness in the philtrum, flat and hard back flesh, red swelling like crimson, many green veins on abdomen, scrotum without wrinkles, inability to digest five grains, and slippery diarrhea are all critical. Dark black face indicates Liver extinction. Palm without lines indicates Heart extinction. Protruding Shenque point indicates Spleen extinction. Flat Quepen point indicates Lung extinction. Flat Yongquan point indicates Kidney extinction. Cessation of food intake indicates death, as Stomach qi is exhausted. Fluid leaking from the thighs indicates death, as Spleen is injured and ruptured. (From *Huibu*)
[Medication for Edema]
Main formula: Siling San (Four-Poria Powder) added with Cangzhu (Atractylodis Rhizoma), Muxiang (Aucklandiae Radix), Chenpi (Citri Reticulatae Pericarpium), Houpo (Magnoliae Officinalis Cortex), Zhike (Aurantii Fructus), Jiangpi (Zingiberis Pericarpium). [Appendix: Lung Distension with Body Swelling]
The Lung governs skin and hair. Wind pathogen enters the Lung, failing to diffuse and unblock. Lung distension and leaf elevation prevent regulating water passages and transmitting downward to the Bladder, also causing swelling. Symptoms: eyelids swell first, initial panting and inability to lie flat, lower abdomen unaffected. Qingfei Tingli Wan is main.
