Case 2 Xiao XX, male, 40 years old. Frequently experiences pain in the epigastrium centered on the heart area, chest and diaphragm fullness and distension, dizziness and vertigo, tinnitus, palpitations, spontaneous sweating, insomnia, shortness of breath with insufficient respiration, aversion to cold on the back, and a bright white complexion. Pulse examination reveals a deficient, wiry, and forceless pulse; tongue is pale red. He has experienced massive hematemesis several times, black loose stools, and extreme fatigue. This is due to severe damage to the stomach collaterals, with the spleen losing its function of controlling blood. The upward overflow is not yet complete, leaking into the intestines. The treatment principle should be to tonify Qi and secure blood, following the intention of Jingyue's Dabu Yuan Decoction (Great Tonification of the Origin Decoction). Roasted Astragali Radix (Huangqi) 10g, Western Codonopsis Radix (Dangshen) 10g, Huai Yam Rhizome (Shanyao) 10g, Fried Ziziphi Spinosae Semen (Suanzaoren) 10g, Prepared Rehmanniae Radix (Shudihuang) 10g, Northern Schisandrae Fructus (Wuweizi) 3g, Angelicae Sinensis Radix Corpus (Dangguishen) 10g, Northern Lycii Fructus (Gouqi) 10g, Cloud Poria Cocos (Fushen) 7g, Corni Fructus (Shanzhuyu) ?g, Roasted Polygalae Radix (Yuanzhi) 3g, Broad Tangerine Peel (Chenpi) 3g, Roasted Glycyrrhizae Radix et Rhizoma (Gancao) 5g. Follow-up visit: After taking 10 doses consecutively, bleeding stopped, sweating ceased, he could sleep peacefully at night, gastric appetite increased, and stools turned yellow, further benefiting the Heart and Spleen. According to the above prescription, remove Rehmanniae Radix and Schisandrae Fructus, add steamed Atractylodis Rhizoma (Baizhu) 7g, and Longan Aril (Longyanrou) 10g. After taking more than 10 doses, stomach pain completely stopped, complexion became rosy and lustrous, and spirit recovered. (From "Medical Cases of Famous Modern Doctors", Li Congfu Medical Cases) [Commentary]: Hematemesis usually results from bleeding in the stomach, vomiting out through the mouth. If blood is vomited, it is named hematemesis. In clinical practice, it is often difficult to strictly distinguish between hematemesis and hematemesis (vomiting blood). This syndrome is mostly caused by accumulated heat in the stomach or liver depression transforming into fire damaging the Yang collaterals. Due to large amounts of blood loss, conditions such as collapse of blood and Qi, and deficiency of both Qi and Blood can also be seen.
Case 1 Syndrome belongs to Liver-Spleen disharmony and steaming damp-heat, causing hematemesis due to blazing heat in the stomach, with constipation and black stools. Treatment involves calming the Liver and harmonizing the Stomach, searching for and clearing damp-heat. Mr. Kong believes: "Excessive Liver Yang suppresses Stomach Qi... Little do they know that although the rupture is in the Stomach, its source is in the Liver. If one only treats the Stomach without treating the Liver, it is abandoning the root for the branches, how can one expect recovery?"
Case 2 Massive blood loss multiple times leading to blood collapse. Treatment involves tonifying Qi to secure blood, and further benefiting the Heart and Spleen to conclude the treatment. Generally, in the late stage of blood disorders, if there is Liver and Spleen blood deficiency, Heart Nutrient Yin malnourishment, and no deficient fire flaming upward, the method of tonifying Qi and securing collapse can be applied.
(Thirteen) Hematochezia
Case 1 Zhang X, male, 50 years old. Blood in stool, occurring intermittently for four or five years. Recent attacks are severe, with fresh red blood and large volume, five to six times a day. Anal prolapse, dizziness, dark vision, shortness of breath, palpitations, food lacks taste, pale complexion, body fatigue, mental exhaustion, faint and forceless pulse, lasting over two months. This condition is rectal and anal bleeding, possibly caused by the development of internal hemorrhoids. It is due to inherent physical deficiency, uneven circulation of Qi and Blood, stagnation and heat in the Stomach and Intestines, frequent dry and hard stools, inability to excrete fecal toxins, forcing blood downward, stasis accumulating in the intestines. Once filled, it flows down upon defecation. Temporary bleeding occurs; shortly after stopping, stasis forms again and pours out, cycling endlessly, resembling fistula formation, hence the intermittent occurrence over years. Without addressing both root and branch, there is still fear of unnoticed recurrence. Urgent treatment with hemostatic, heat-clearing, and Middle-Jiao Qi-tonifying agents is required. Bie Zhi Ren (Ginseng) 6g, decocted thickly, taken in two divided doses mixed in; Roasted Astragali Radix (Huangqi) 18g, Bleached Atractylodis Rhizoma (Baizhu) 9g, Hang White Peony Root (Baishao) 9g (stir-fried with Bupleuri Radix 4.5g), Black Cimicifugae Rhizoma (Shengma) 3g, Black Saposhnikoviae Radix (Jingsui) 6g, Fried Sanguisorbae Radix (Diyu) 9g, Fried Sophorae Flos (Huaimi) 9g, Charred Tangerine Peel (Pitan) 6g, Angelicae Sinensis Radix Corpus (Dangguishen) 9g, Gardenia Fruit (Zhizi) 6g, Fried Aurantii Fructus (Zhiqiao) 6g, Donkey-hide Gelatin (Ejiao) 9g (dissolved separately, taken in two divided doses mixed in); Roasted Glycyrrhizae Radix et Rhizoma (Gancao) 4.5g. Second visit: After taking 3 doses, bleeding stopped, bowel movements returned to normal, once daily. Dizziness, palpitations, shortness of breath, dark vision, and snow-white pallor remain. Immediate regulation and tonification are needed; continue regular intake to prevent recurrence. Jilin Ginseng 9g, decocted thickly, taken in two divided doses mixed in; Wild Atractylodis Rhizoma (Baizhu) 9g, Cloud Poria Cocos (Fuling/Fushen) 9g each, Corni Fructus (Shanzhuyu) 9g (fried), Longan Aril (Longyanrou) 15g, Angelicae Sinensis Radix Corpus (Dangguishen) 9g, Large Prepared Rehmanniae Radix (Shudihuang) 9g, Huai Yam Rhizome (Shanyao) 15g, Roasted Astragali Radix (Mianhuangqi) 24g, Roasted Polygalae Radix (Yuanzhi) 9g, Aucklandiae Radix (Muxiang) 3g, Deer Antler Glue (Lujiaojiao) 9g (dissolved separately, taken in two divided doses mixed in); Schisandrae Fructus (Wuweizi) 9g (crushed), Roasted Glycyrrhizae Radix et Rhizoma (Gancao) 3g. (Organized by Shi Zhimo et al.: Shi Jinmo Medical Cases, *Chinese Journal of Traditional Chinese Medicine* 5:328, 1958)
Case 2 Wang XX, male, 21 years old, unmarried. One year ago, the patient suffered from stomach pain and hematochezia, diagnosed and treated at XX Hospital. Condition improved but was not completely cured. Five days ago, sudden onset of stomach disease with massive hematochezia. Despite treatment, bleeding did not stop. Extreme mental fatigue led him to seek treatment at our hospital. Current symptoms: Pale complexion, lackluster lips and nails, mental fatigue and limb weariness, listlessness, poor appetite, palpitations, insomnia, vertigo, dry mouth at night, mild or occasional slight epigastric pain, black tarry stools, strong positive fecal occult blood test (++++), pale tongue, dry white coating, wiry, large, and forceless pulse. Differentiation: Frequent hematochezia indicates Qi and Blood deficiency. This prolonged bleeding has caused even greater depletion. Observing pale face and lips, fatigue, vertigo, and palpitations, these are purely deficiency signs. The large, forceless pulse and black stools indicate Qi and Blood deficiency failing to control blood. Hematochezia with mild stomach pain suggests less pathogen and more deficiency. Treatment Method: Tonify Qi and nourish Blood, nourish Blood and stop bleeding. Prescription: Shenqi Siwu Tang modified. Codonopsis Radix (Dangshen) 24g, Astragali Radix (Huangqi) 15g, Raw and Prepared Rehmanniae Radix (Sheng/Shu Dihuang) 12g each, Hang White Peony Root (Baishao) 9g, Angelicae Sinensis Radix Corpus (Dangguishen) 9g, Platycladi Cacumen (Cebaiye) 15g, Moutan Cortex (Danpi) 6g, Donkey-hide Gelatin (Ejiao) 9g (taken with medicine), Fried Sanguisorbae Radix (Diyutan) 9g, Sour Jujube Seed (Suanzaoren) 9g, Roasted Glycyrrhizae Radix et Rhizoma (Gancao) 4.5g. Second visit: After taking 2 doses, visible hematochezia disappeared, stool color turned yellow, fecal occult blood test negative, spirit improved, symptoms significantly reduced. Plan to continue original prescription for 2 more doses (one dose daily). Third visit: 20 days later, the patient reported that after taking the second-visit medicine, all symptoms disappeared and bleeding stopped. He went home and stopped medication. Two days ago, due to diet, he noticed black, loose, unformed stools yesterday, but spirit was good, appetite slightly decreased, dry mouth without desire to drink, empty irritability and insomnia, red tongue edges, white coating, wiry-large left pulse, weak right pulse. Considering pulse and symptoms, this is Blood deficiency with Liver Heat and Qi deficiency with Spleen Cold. Treatment: Nourish Blood and clear Heat, tonify Qi and warm the Middle, following Huangtu Tang modified. Raw Rehmanniae Radix (Shengdi) 15g, Hang White Peony Root (Baishao) 9g, Black Sanguisorbae Radix (Diyu) 9g, Codonopsis Radix (Dangshen) 15g, Prepared Zingiberis Rhizoma (Paojiangtan) 3g, Donkey-hide Gelatin (Ejiao) 9g (taken with medicine), Longmen Earth (Fulonggan) 12g (wrapped in lotus leaf and decocted), Roasted Licorice (Gancao) 3g. One dose daily, taken for 3 consecutive days. After medication, stool color turned yellow and formed; take 2 more doses, stool normalizes, bleeding disappears, fecal occult blood test negative. Advised to pay attention to daily life and dietary regulation, continue medication to consolidate efficacy and prevent recurrence. (Xiao Xi: Case of Gastric Hemorrhage Treatment, *New Chinese Medicine* 2:18, 1978) [Commentary]: Any bleeding before or after defecation, or simple bleeding alone, is collectively called hematochezia. *Jingui Yaolue* distinguishes between distant blood and proximate blood. *Linzheng Yaojue* states: "Clear and bright red blood is Changfeng (Intestinal Wind); turbid and dark blood is Zangdu (Visceral Toxin)." Hematochezia is mostly caused by Spleen deficiency failing to control blood or damp-heat pouring downward into the Large Intestine, damaging the Yin collaterals.
Case 1: Long course of disease, bright red blood in large volume, righteous Qi deficiency and pathogenic excess. Urgent treatment with hemostasis, heat-clearing, and Middle-Jiao Qi tonification, addressing both root and branch, supporting righteousness and dispelling pathogens simultaneously. After bleeding stops, if righteous Qi has not recovered, continuing with regulatory and tonic agents is relatively safe.
Case 2: Syndrome belongs to distant blood. Due to excessive blood loss causing dual deficiency of Qi and Blood, treatment with dual tonification of Qi and Blood aims to achieve the effect of Qi充盈 securing blood and nourishing Blood stopping bleeding, which is quite appropriate.
(Fourteen) Accumulated Blood Syndrome
Case 1 Female, under 20 years old, rather emaciated. Went out to buy goods, suddenly frightened, upon return went mad, fighting people randomly, immense strength. ... Ill for seven or eight days, madness remains unchanged. ... Menstruation absent for two months. ... Pulse deep and tight, lower abdomen seems distended. ... This is Accumulated Blood Syndrome, purging will cure it. Prescribed Taohé Chengqi Tang. Peach Kernel (Taoren) 30g, Raw Rhubarb (Shengjun) 15g, Mirabilite (Mangxiao) 6g, Roasted Glycyrrhizae Radix et Rhizoma (Gancao) 6g, Cinnamon Twig (Guizhi) 6g, Immature Bitter Orange (Zhishi) 9g. Next day asked, learned that after taking, much black blood was passed, madness stopped, body not tired, able to drink porridge, shy and avoiding people when seeing them. Then wrote a follow-up formula for him, no further visits. (Cao Yingfu *Experience Records of Classical Formulas*)
Case 2 Male. Lower abdominal fullness and pain, clear and long urination, unable to see objects. Argument confirms Accumulated Blood, but secretly doubted it. Tentatively gave Taohé Chengqi Tang. After taking, black stool passed shortly, but symptoms remained. Gave two more doses, increased amount, illness still same, more surprised. Thought: If this is not Accumulated Blood, taking purgatives for three doses should have become a bad disease; if indeed this syndrome, why no slight improvement? Must be light medicine and heavy disease. Student Zhang Cicong stood by side, said: How about Didang Wan? I said: Examining the syndrome, light medicine cannot cure, decided to purge with Didang Tang. After taking, black stool accompanied by old blood came down together. Advanced one more dose, patient could lie quietly on bed, abdominal fullness flat, pain relieved. Knew medicine hit the disease, still used previous formula reducing amount: Blister Beetle (Maochong) 6g, Leech (Shuizhi) 4.5g, Peach Kernel (Taoren) 15g, Rhubarb (Chuanjun) 15g. Later reduced to Blister Beetle and Leech 1.2g each, Peach Kernel and Rhubarb 4.5g each. Recovered under Zhang Cicong's regulation. Later asked patients, it was due to labor carrying heavy loads causing blood coagulation forming Accumulated Blood Syndrome. (Cao Yingfu *Experience Records of Classical Formulas*) [Commentary]: Accumulated Blood Syndrome is the syndrome appearing from "Taiyang disease unresolved, heat binding in Bladder" in *Shanghan Lun*. Internal heat stagnating in Lower Jiao Bladder, heat flourishing and blood stasis, the person is like mad.
Case 1 is an Accumulated Blood madness case. Patient menstruation absent for two months, pulse deep and tight, lower abdomen seems distended, triggered by fright, so no external pathogen known. "Only if lower abdomen is urgent and knotted, can it be attacked, suitable for Taohé Chengqi Tang." Added Zhishi to *Shanghan Lun* original formula to eliminate fullness and break knots, assisting passage power. After medicine, much black blood passed, madness stopped, body not tired. Although using heavy medicine to break stasis and purge, body still not tired, because patient's weakness was not severe, and medicine hit the disease appropriately. Even if righteous Qi is too deficient, or medicine dosage too high, causing fatigue after purging, do not panic; can still use tonic agents to regulate.
Case 2 is male Accumulated Blood. Accumulated Blood syndrome is mostly seen in women, rare in men. Because it is rare, syndrome identification must be precise; otherwise, pondering attack methods, hesitating, can delay disease mechanism. Initially used Taohé Chengqi Tang but disease heavy medicine light, later purged with Didang Tang then recovered. Case described in detail, worth pondering.
(Fifteen) Palpitation and Trembling Palpitation

(Ouyang Qi: Experience in Treating Neurosis, *Zhejiang Journal of Traditional Chinese Medicine* 5:11, 1964) [Commentary]: Palpitation and Trembling Palpitation refer to the syndrome of self-perceived heart restlessness and rapid heartbeat, often accompanied by panic and inability to control oneself. There is still a difference in severity between the two. Palpitation is mostly caused by external factors, such as sudden fright or anger, generally milder syndrome, good overall condition; Trembling Palpitation is mostly caused by internal factors, no external fright but heart feels full, triggered by slight labor, slightly heavier syndrome, poorer overall condition. Palpitation for a long time can develop into Trembling Palpitation. Many suffer from this disease clinically. But neurosis as called by Western medicine is more common, accounting for a certain proportion of cardiovascular diseases.
Case 1 Palpitation illness, upper limbs slightly tremble, belongs to emotional disease, Heart has no master, Liver Wind wants to move, tongue red little coating slightly dry, pulse fine and rapid, internal heat injuring Yin cause. Treatment: Calm Heart and subdue Liver, clear Heat and nourish Yin. Because spirit is vague, fearing Phlegm-Heat internal closure, slightly added Tangerine Peel (Juhong), Bamboo Shavings (Zhuru), Fritillariae Bulbus (Chuanbeimu), Curcumae Radix (Yujin) and other resolving Phlegm and opening penetration agents to prevent trouble before it happens.
Case 2 Syndrome belongs to Heart Yang not stirring, Water-Fluid reversing upward, thus seeing Qi rushing up, borborygmus, palpitations worsening with movement etc. Formula uses Linggui Zhugan Tang modified, stimulating Heart Yang, tonifying Qi and moving Water, suppressing reversal, few medicinal ingredients but specific therapeutic effect.
Case 3 Belongs to Trembling Palpitation range. Due to excessive sweating injuring Heart Yin, thus seeing spirit timid and trembling palpitation unrest. Also due to improper regulation, overusing Qi tonifying and Blood nourishing,反而 helping the villain. Disease although originating from damage, not Heart-Spleen dual deficiency syndrome, actually Phlegm Turbidity is the problem. Turbid Phlegm not gone, Clear Yang not ascending, then difficult to recover quickly. Thus with Wendan Tang modified, soothe depression and descend Phlegm. Wait until Phlegm gone and turbidity cleared, symptoms show spirit deficient and house empty image, continue using regulating and tonifying Heart-Spleen agents to solidify root.
