(Organized by Feng Xianxun: Wang Weichuan's Experience in Applying Qi-Tonifying and Stasis-Resolving Method, *Journal of Traditional Chinese Medicine*, 4112, 1981)
[Commentary] Lupus Erythematosus is a type of collagen disease, treated internally in Western medicine. Clinically divided into localized discoid and systemic types. Traditional Chinese Medicine generally believes its pathogenesis is mostly related to "Yin Deficiency".
Case 1 is Discoid Lupus Erythematosus, known in TCM as "Ya Dan Chuang" (Crow-Bitten Sore), traditionally considered difficult to treat. This case's syndrome differentiation belongs to Disharmony between Liver and Spleen, Qi stagnation and Blood stasis. Xiao Yao San (Free and Easy Wanderer Powder) was modified to soothe the Liver, harmonize the Spleen, invigorate Blood, and resolve stasis. According to Dr. Zhu's experience, if lupus erythema is obvious, add Ling Xiao Hua (Campsis Grandiflora Flos) and Ji Guan Hua (Celosia Cristata Flos); if skin ecchymosis exists, add Xian He Cao (Agrimoniae Herba) and Ou Jie Tan (Charred Node of Nelumbinis Rhizomatis); if joint pain occurs, add Wu She (Zaocys) Qin Jiao (Gentianae Macrophyllae Radix), Sang Ji Sheng; if night sweats occur, add Huang Qi Pi (Pericarpium Astragali) and Nuo Dao Gen (Rhizoma Imperatae). Modifications of herbs reflect detailed syndrome differentiation and careful consideration of medication.
Case 2 presents circular erythema on both malar regions with severe systemic symptoms, indicating Spleen-Kidney Dual Deficiency, Blood stasis obstructing collaterals,郁 (stagnation) transforming into heat. Treatment focuses on tonifying Qi and resolving stasis, strengthening Spleen and tonifying Kidney, while clearing stasis-heat. Prescription 1 features simultaneous application of warming, clearing, tonifying, and reducing methods. The use of insect drugs and the herbal medicine Nuo Mi Cao enhances detoxification, swelling reduction, unblocking stasis, and activating collaterals. The seven herbs in Prescription 2 are also distinctive; for example, Shi Da Nian (Shi Jian Chuan) clears heat and detoxifies, invigorates Blood and relieves pain; Ge Shan Qiao strengthens Stomach, resolves accumulation, detoxifies, and reduces swelling; Ku Qiao Tou strengthens Spleen and promotes dampness drainage... The treatment strategy aims to break through difficulties one by one in complex and critical conditions, turning danger into safety. Formulating and prescribing reflects Mr. Wang's unique experience and courage.

[Commentary] Ning Zhi Yi is caused by Wind-Heat-Toxic Evil invading the Black Cornea (Hei Jing), causing eye pain, photophobia, flowing tears, and yellow corneal opacity resembling congealed fat, hence the name. Case 1 syndrome belongs to Accumulated Heat in Liver and Lung, compounded by Wind-Heat-Toxic Evil attacking upwards to the eye. Treatment focuses on dispelling Wind and clearing Heat, assisted by calming Liver and eliminating nebula. Effect achieved by modifying Hong Zhong Yi Fang (Empirical Formula). Dr. Wei's experience indicates Chi Shi Zhi promotes ulcer healing, Shi Jue Ming eliminates nebula obstruction, so they are frequently added when encountering corneal nebula. Case 2 Feng Lun Ning Zhi Yi arising around, hands and feet cold, pulse fine almost imperceptible, surprisingly treated with Dang Gui Si Ni Tang (Cinnamon Twig Decoction with Angelica) type prescriptions, benefiting Blood, restoring pulse, warming meridians, and dispersing Cold, achieving effect. Comparing the two cases, deficiency/excess and cold/heat treatments are completely different.
(II) Hua Yi Bai Xian (Flower-like Corneal Opacity)
Case 1 Male, 40 years old, Outpatient No.: 168428, Initial Visit Date: July 8, 1958. Left eye onset, recurring intermittently for over a year. Diagnosed as Dendritic Corneal Ulcer upon examination; treated with various Western medicines without success. Symptoms: Left eye Qi Lun (Qi Wheel/Sclera) shows slight red vessels, Feng Lun (Wind Wheel/Cornea) Ning Zhi invading Tong Shen (Pupil), tearing and stabbing pain, pulse wiry. Syndrome Differentiation: Caused by Liver Wind Qi Stagnation; when Clear Yang Qi does not ascend, internal stagnation fails to resolve, manifesting as Hua Yi Bai Xian. Treatment Principle: Dispel Wind-Heat and harmonize Qi and Blood, ascend Clear Yang and open various stagnations. Prescription: Nen Gou Teng (Uncariae Ramulus cum Uncis) 9g, Chan Yi 3g, Dang Gui 9g, Chuan Xiong 3g, Chao Bai Shao 9g, Zhi Xiang Fu (Cyperi Rhizoma) 12g. Case 2 Bao XX, Female, 11 years old, Outpatient No.: 126626. Initial Visit Date: January 26, 1966. Right eye started congesting, gritty pain, photophobia, tearing 30 days ago. Diagnosed as "Right Eye Dendritic Corneal Ulcer" by Tiao Hospital, given symptomatic treatment and mydriasis. Patient suffered same lesion in right eye five years ago. After iodine tincture cauterization, corneal scar remained. Examination: Right eye visual acuity 0.1+1, near vision Jaeger chart 2-3, gray-white deep dendritic infiltration near pupil at 7-8 o'clock position on right cornea, diameter approx 2mm, fluorescein staining positive, old thin nebula above. Pulse fine, tongue body red, coating thin white. Syndrome Differentiation: Excessive Fire in Liver and Lung, external contraction of Pathogenic Evil, internal and external pathogens combining, attacking upwards to Eye Orifice. (Right Eye Hua Yi Bai Xian; Right Eye Bing Xia Yi/Ice-Glint Nebula) Treatment Principle: Dispel Wind and Clear Heat, invigorate Blood and nourish Yin, eliminate nebula and brighten eyes. Prescription:
(1) Hong Zhong Yi Zhang Fang: Sheng Di Huang 12g, Chi Shao 10g, Meng Hua 6g, Bai Zhi 10g, Shi Jue Ming 15g (decoct first), Chi Shi Zhi 10g (wrapped and decocted), Chao Chuan Xiong 3g, Xia Ku Cao 6g, Zhi Gan Cao 5g. Decoct in water, 5 doses.
(2) Xi Huang San one bottle for instillation into affected eye, three times daily. January 31

Stick to original prescription modifications: Chao Chuan Xiong 3g, Sheng Di Huang 12g, Chi Shao 10g, Meng Hua 6g, Bai Zhi 10g, Sang Pi (Mori Cortex) 15g, Mu Tong 5g, Zhi Cao 6g, Di Gu Pi (Lycii Cortex) 10g. Decoct in water, 5 doses. Continue Xi Huang San instillation. February 5
(Final Visit): Eye pain disappeared, slight photophobia and tearing. Pulse fine and rapid, coating thin white, tongue body pale red. Change to dispelling Wind, clearing Liver, eliminating nebula, and brightening eyes method. Long Dan Cao (Gentianae Radix) 6g, Lian Qiao 10g, Xia Ku Cao 6g, Cao Jue Ming (Cassiae Semen) 10g, Meng Hua 10g, Jing Jie (Schizonepetae Spica) 5g, Bai Ju Hua 6g. Decoct in water, 12 doses. Continue Xi Huang San instillation. After taking medicine, above symptoms disappeared, corneal ulcer healed. (From "Selected Clinical Experience of Wei Wengui in Ophthalmology")
[Commentary] Hua Yi Bai Xian refers to white corneal opacity resembling flower petals or tree branches, presenting with photophobia, excessive tearing, friction pain, etc. Dendritic corneal ulcer falls within this syndrome category. Case 1 caused by Liver Wind Qi Stagnation. Treatment dispels Wind-Heat, harmonizes Qi and Blood, ascends Clear Yang, opens stagnations; few herbs but potent effect, good results. Case 2 syndrome belongs to Excessive Fire in Liver and Lung, external contraction of Pathogenic Evil, internal and external pathogens combining, attacking upwards to Eye Orifice. Used Wei's Hong Zhong Yi Zhang Fang, addressing both root and branch, mild drug properties. This formula is an empirical formula for treating various keratitis and corneal ulcers.
(III) Tong Shen Gan Que (Irregular Pupil / Atrophy of Iris)
Case 1 Xu XX, Male, 28 years old, Initial Visit Date: April 25, 1942. Bilateral Tong Shen Gan Que, hidden yellow fluid (hypopyon), blurred vision and eye pain, tongue ulceration and mouth sores, red patches on legs, genital erosion. Sick for five years, rising and falling, frequent recurrent episodes. Pulse wiry and rapid, tongue coating yellow and greasy. Syndrome caused by Damp-Heat flowing and depositing, disease located in Heart, Spleen, Liver, Kidney. *Su Wen* states: "All pain and sores belong to the Heart," "All dampness, swelling, and fullness belong to the Spleen." Heart governs Blood, Spleen governs Flesh. Blood Heat combined with internal Dampness, Damp-Heat merging, Heat-Toxin transmitting downwards, entering Liver Meridian generating Yin sores, entering Spleen Meridian generating flesh ulcers, entering Heart Meridian causing tongue ulceration, entering Kidney Meridian causing Tong Shen Gan Que. Disease infiltrates ceaselessly, destroying skin and flesh, consequences worrying. Long Dan Cao, Chao Zhi Zi, Huang Qin, Chai Hu, Che Qian Zi, Mu Tong, Dang Gui Wei (Tail of Angelicae Sinensis Radix), Cang Zhu, Mi Ren (Coix Seed), Sheng Di, Gan Cao, Zhu Ye. After 2 doses, modify original prescription and take continuously for 10 doses, redness and swelling receded, yellow fluid disappeared, vision restored, all pain and sore itching removed. However, after two months, eye disease recurred. Follow-up visit: Previous treatment, redness receded, yellow fluid disappeared, vision clarified. But less than two months later, old disease returned, plus chills and vexatious fever, chest and abdomen discomfort, pulse soggy, tongue coating turbid and greasy: Also caused by steaming Damp-Heat and turbid Evil ascending. Treatment: Promote dampness drainage and transform turbidity, clear Heat and detoxify. Fu Ling, Cang Zhu, Zhu Ling, Ze Xie, Huang Qin, Chao Zhi Zi, Mi Ren, Hua Shi (Magnesii Silicas), Gan Lu Xiao Du Dan (Sweet Dew Detoxication Powder - wrapped and decocted). After 3 doses, then take continuously for 5 doses, eye disease cured again.

Case 2 Zhang XX, Male, 28 years old, Initial Visit Date: June 12, 1959. Left eye sick for ten days, eye red, blurred vision, eye pain, radiating to head distending pain. Diagnosed by Western medicine as Left Acute Iridocyclitis (Visual Acuity 0.2). Patient ruddy complexion, dry mouth craving drink, red tongue, yellow coating, rapid pulse. Syndrome belongs to Tong Shen Gan Que, Blood penetrating Tong Shen. Chong Wei Zi (Leonuri Fructus), Chi Shao, Yuan Shen (Scrophulariae Radix), Mai Dong (Ophiopogonis Radix), Zhi Mu, Sheng Shi Gao (Gypsum Fibrosum), Huang Qin, Sheng Gan Cao, Wu Ling Zhi (Trogopterori Faeces). After 2 doses, pain reduced, left eye redness and swelling decreased, anterior chamber hemorrhage disappeared, turbidity reduced. Remove Shi Gao, Wu Ling Zhi, Gan Cao from original prescription, add Di Gu Pi. After another 3 doses, left eye redness and swelling disappeared, vision increased to 0.5. Continue original prescription for 6 doses, left eye congestion disappeared, vision increased to 1.0.

[Commentary] Tong Shen Gan Que is often a sequelae of severe Ning Zhi Yi. Tong Shen loses perfect roundness, edges like sawteeth or plum blossom shape, hence the name.
Case 1 besides Tong Shen Gan Que, also presents Damp-Heat flowing and depositing syndromes, disease in Heart, Spleen, Liver, Kidney. Treatment uses modified Long Dan Xie Gan Tang (Gentian Liver-Draining Decoction), effective after 10 doses, but old disease recurred a month later with worse severity. This is due to steaming Damp-Heat and turbid Evil ascending. Treatment focuses on promoting dampness drainage, transforming turbidity, clearing Heat, and detoxifying, emphasizing "expelling Evil", Evil expelled, Zheng (Healthy Qi) safe, thus cured.
Case 2 not only Tong Shen Gan Que, but also Blood Penetrating Tong Shen. Treatment starts from the whole body, focusing on internal treatment. Due to Liver Meridian Blood Heat, Blood stasis blocking collaterals, treatment uses moving and invigorating Blood, and must cool Blood and clear Heat to treat the root.
