Su Wen states: "When a girl reaches seven years of age, her Kidney Qi becomes abundant, her teeth change, and her hair grows; at fourteen (two sevens), Tian Gui (the subtle essence promoting reproductive development) arrives, the Ren Mai (Conception Vessel) becomes unobstructed, and the Chong Mai (Penetrating Vessel) becomes full. Her menstruation occurs on time, thus she can conceive." At forty-nine (seven sevens), "the Ren Mai is deficient, the Chong Mai declines, Tian Gui is exhausted, the pathway of menstruation is blocked, thus her body deteriorates and she can no longer bear children." For men, "at eight years old, his Kidney Qi is solid, his hair grows, and his teeth change; at sixteen (two eights), his Kidney Qi is abundant, Tian Gui arrives, his essence overflows and is discharged. When Yin and Yang harmonize, he can reproduce." By sixty-four (eight eights), "his teeth and hair fall out... all five Zang organs decline, sinews and bones become loose and weak, Tian Gui is exhausted. Thus his sideburns turn white, his body feels heavy, his gait is unsteady, and he can no longer reproduce."
The Qi and Blood of the human body vary in deficiency, excess, cold, and heat, which can only be known through pulse diagnosis. To speak solely of herbs while ignoring the pulse is absurd. The pulse should not be too rapid (Shu, indicating Heat); it should not be too slow (Chi, indicating Cold); it should not be excessively forceful and Excess (Shi). An Excess pulse often indicates that righteous Qi is deficient and pathogenic Fire has invaded, creating an appearance of strength; the treatment method should be to dissipate stagnation to eliminate the pathogen, and only after the pathogen is gone can righteous Qi be tonified. It should not be too weak and Deficient (Xu), which indicates Qi and Blood deficiency; the treatment method should be to tonify Qi and Blood. Furthermore, women often have more Qi than Blood, with disharmony of Cold and Heat and irregular menstruation. All these conditions require pulse diagnosis and flexible treatment. It is essential that the pulses of both husband and wife are peaceful and forceful, intercourse occurs at appropriate times, and medications are not used recklessly, so that offspring may be conceived.
Someone asked: "Huang Qin (Scutellariae Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma) are the sacred herbs for calming the fetus. Are these two herbs absolutely indispensable during pregnancy?" I replied: "Not necessarily. The fetus relies on the Spleen for nourishment, just as a bell hangs from a beam. If the main pillars are not firm, the beam will bend. Therefore, to calm the fetus, one must first secure the two Kidneys. If the Kidneys are warm and harmonious, the Spleen will generate vibrant Qi. Why must one rely on Bai Zhu and Huang Qin to calm the fetus? Generally, if there is Heat in the abdomen causing fetal unrest, cooling herbs are indeed used; if there is Cold in the abdomen causing fetal unrest, warming herbs are necessary. This is the standard method. However, one fails to realize that the two Kidneys contain the source of Water and Fire and are the root of the Chong and Ren vessels. They are crucially connected to the fetal origin, far beyond the capability of Bai Zhu and Huang Qin to calm. If fetal unrest is due to lack of Water in the Kidneys, Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) is used to strengthen Water; if it is due to lack of Fire in the Kidneys, Ba Wei Di Huang Wan (Eight-Ingredient Pill with Rehmannia) is used to benefit Fire. Therefore, regulating menstruation should involve Du Zhong (Eucommiae Cortex), Xu Duan (Dipsaci Radix), E Jiao (Colla Corii Asini), Ai Ye (Artemisiae Argyi Folium), Dang Gui (Angelicae Sinensis Radix), and Wu Wei Zi (Schisandrae Fructus), incorporated into Liu Wei or Ba Wei Decoctions as a quick method. In summary, this principle is consistent throughout. This insight is not covered in other books, which is why I specifically highlight it."
[Ba Wei Tang (Eight-Ingredient Decoction)] consists of Shu Di Huang (Rehmanniae Radix Preparata), Shan Yu Rou (Corni Fructus), Shan Yao (Dioscoreae Rhizoma), Mu Dan Pi (Moutan Cortex), Fu Ling (Poria), Ze Xie (Alismatis Rhizoma), Fu Zi (Aconiti Lateralis Radix Preparata), and Rou Gui (Cinnamomi Cortex).
In women pregnant for two or three months, the Cun (Inch) pulses of both hands are floating and large, the Guan (Bar) pulses are slippery, and the Chi (Cubit) pulses are slightly rapid. This is the pulse of pregnancy. If a lump appears first in the left breast, it is a boy; if in the right breast, it is a girl. If the left Chi pulse is slippery, large, and rapid, it is a boy; if the right Chi pulse is slippery, large, and rapid, it is a girl. If both Chi pulses are surging, it indicates twins. If all six pulses are surging and large but the body does not feel hot, it is pregnancy. Additionally, if the Chi pulses vary in size (large or small), depth (deep or superficial), or rhythm (moving or stopping), differing between day and night, this indicates a "Ghost Fetus" (abnormal pregnancy or pseudocyesis). If the pulse comes like chaotic raindrops, suddenly disappearing and returning after a long time, this is also a Ghost Fetus. One must observe continuously for three or four days to confirm.
If a woman's menstruation has stopped and one wishes to verify pregnancy, she may take Tan Tai Wan (Fetus-Exploring Pill). If there is slight movement in the abdomen, it is pregnancy.
[Tan Tai Wan] consists of Chuan Xiong (Ligustici Chuanxiong Rhizoma). Grind into powder, and take with thick decoction of Ai Ye (Artemisiae Argyi Folium).
If a pregnant woman has a deep, fine pulse accompanied by abdominal pain and immobility of the fetus, this is due to the accumulation of pathogenic factors and water retention. Treatment depends on whether the lower abdomen is cold or warm, using herbs to regulate Qi and invigorate Blood.
If a woman's menstruation has been absent for two or three months, her body feels ill, and all six pulses are surging and large, this is the pulse of pregnancy. Her spirit remains normal, she craves salty and sour foods, dislikes the smell of food, or desires only one specific item. She may vomit phlegm-water, feel bloated, experience alternating chills and fever. This is called "E Zu" (Nausea of Pregnancy). Commonly known as "Bing Er" (Sickness of the Child), characterized by nausea and aversion to food. Do not treat this as a simple Cold or Heat disease. Instead, regulate Qi and resolve phlegm, administering Bao Sheng Tang (Life-Preserving Decoction) with added Ding Xiang (Caryophylli Flos) and Sheng Jiang (Zingiberis Rhizoma Recens).
[Bao Sheng Tang] consists of Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling (Poria), Gan Cao (Glycyrrhizae Radix et Rhizoma), Chen Pi (Citri Reticulatae Pericarpium), Hou Po (Magnoliae Officinalis Cortex), Sheng Jiang (Zingiberis Rhizoma Recens), and Ding Xiang (Caryophylli Flos).
"E Zu" often occurs around the third month of pregnancy, when Ministerial Fire transforms the fetus. Vigorous Fire consumes Qi and rushes upward to the stomach, causing immediate vomiting after eating. Since Shaoyin Kidney Water nourishes the fetus, Shaoyang Fire becomes even more intense. First use Xiao Yao San (Free and Easy Wanderer Powder) to stop vomiting, then use Qing Gan Zi Shen Tang (Clear Liver and Nourish Kidney Decoction) with added Du Zhong (Eucommiae Cortex) and Xu Duan (Dipsaci Radix). For severe cases, add Huang Lian (Coptidis Rhizoma) and Wu Zhu Yu (Evodiae Fructus).
[Qing Gan Zi Shen Tang] consists of Shu Di Huang (Rehmanniae Radix Preparata), Shan Yu Rou (Corni Fructus), Shan Yao (Dioscoreae Rhizoma), Chai Hu (Bupleuri Radix), Mu Dan Pi (Moutan Cortex), Ze Xie (Alismatis Rhizoma), Fu Ling (Poria), and Bai Shao (Paeoniae Radix Alba).
For "E Zu" vomiting, use Zi Su Yin (Perilla Decoction) with added Fu Ling (Poria), Ban Xia (Pinelliae Rhizoma), Zhi Shi (Aurantii Fructus Immaturus), and Cao Guo (Tsaoko Fructus). This is caused by rebellious Qi, retained phlegm, and food. If there is chest oppression, dizziness, vomiting, aversion to cold, and sweating, use Si Qi Tang (Four Words Decoction) or Ban Xia Fu Ling Tang (Pinellia and Poria Decoction). If there is retching, inability to eat, heart deficiency, and irritability, use Shen Ju San (Ginseng and Tangerine Peel Powder). If there is Stomach Cold with retching and abdominal distension, add Ding Xiang (Caryophylli Flos) and Huo Xiang (Pogostemonis Herba).
[Zi Su Yin] consists of Zi Su Ye (Perillae Folium), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Gan Cao (Glycyrrhizae Radix et Rhizoma), Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Bai Shao (Paeoniae Radix Alba), Ren Shen (Ginseng Radix), and Da Fu Pi (Arecae Pericarpium).
[Ban Xia Fu Ling Tang] consists of Ban Xia (Pinelliae Rhizoma), Fu Ling (Poria), Zi Su Ye (Perillae Folium), Huo Xiang (Pogostemonis Herba), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Ju Hong (Citri Reticulatae Pericarpium Rubrum), and Gan Cao (Glycyrrhizae Radix et Rhizoma).
[Shen Ju San] consists of Ren Shen (Ginseng Radix), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma), Mai Dong (Ophiopogonis Tuber), Chi Fu Ling (Red Poria), Zhu Ru (Bambusae Caulis in Taenia), Hou Po (Magnoliae Officinalis Cortex), and Sheng Jiang (Zingiberis Rhizoma Recens).
If there is rebellious phlegm, vomiting of sour fluid, aversion to food smells, dizziness, blurred vision, and excessive sleepiness, use Xuan Fu Ban Xia Tang (Inula and Pinellia Decoction). If vomiting is due to food stagnation, use Zhi Suo Er Chen Tang (Immature Orange and Amomum Two-Chen Decoction).
[Xuan Fu Ban Xia Tang] consists of Xuan Fu Hua (Inulae Flos), Ban Xia (Pinelliae Rhizoma), Fu Ling (Poria), Gan Cao (Glycyrrhizae Radix et Rhizoma), Ren Shen (Ginseng Radix), Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Xi Xin (Asari Radix et Rhizoma), and Gan Jiang (Zingiberis Rhizoma).
[Zhi Suo Er Chen Tang] consists of Zhi Ke (Aurantii Fructus Immaturus), Sha Ren (Amomi Fructus), Ban Xia (Pinelliae Rhizoma), Fu Ling (Poria), Chen Pi (Citri Reticulatae Pericarpium), and Gan Cao (Glycyrrhizae Radix et Rhizoma).
For vomiting and food selectivity due to phlegm and Qi stagnation in the Middle Jiao, use Er Chen Tang (Two-Chen Decoction) with added Bai Zhu (Atractylodis Macrocephalae Rhizoma), Huo Xiang (Pogostemonis Herba), and Sha Ren (Amomi Fructus). If the pulse is weak and vomiting persists despite medication, one must regulate Blood and return it to its source. Ancient texts say, "With pregnancy, vomiting occurs." Use Er Si Tang (Two-Four Decoction) removing Di Huang (Rehmanniae Radix), adding Ding Xiang (Caryophylli Flos), Zhi Ke (Aurantii Fructus Immaturus), and Jie Geng (Platycodonis Radix). If there is oppression, fullness, aversion to food, vomiting, fetal unrest, and lower back/abdominal pain, use An Tai Yin (Calming Fetus Decoction).
If contracting Shang Han (Cold Damage) with irritability, fever, headache, fetal unrest, and vomiting preventing eating, use Ren Shen Bai Zhu San (Ginseng and Atractylodes Powder).
If Stomach Deficiency causes rebellious Qi, vomiting clear fluid, and inability to eat for several days, use Shen Shu San (Ginseng and Atractylodes Powder) with added Sha Ren (Amomi Fructus) and Zhu Ru (Bambusae Caulis in Taenia). For nausea, add Gan Jiang (Zingiberis Rhizoma). If the chest and diaphragm feel constricted, add Zhi Shi (Aurantii Fructus Immaturus).
If vomiting and dizziness occur with a very wiry Left pulse, this is triggered by anger. Avoid Ren Shen (Ginseng Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma). Regulating Qi is paramount; focus on regulating Qi to calm the fetus.
If Stomach Deficiency causes vomiting and inability to eat, crush Sha Ren (Amomi Fructus), soak in Ginger Juice briefly, stir-fry until purple, brew in boiling water, add a little Ginger Juice, and sip. Zi Su Tang (Perilla Decoction) is also suitable.
[Ren Shen Bai Zhu San] consists of Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Gan Cao (Glycyrrhizae Radix et Rhizoma), Mai Dong (Ophiopogonis Tuber), Chi Fu Ling (Red Poria), Zhu Ru (Bambusae Caulis in Taenia), Ban Xia (Pinelliae Rhizoma), Chuan Xiong (Ligustici Chuanxiong Rhizoma), and Qian Hu (Peucedani Radix).
[Shen Shu San] consists of Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Gan Cao (Glycyrrhizae Radix et Rhizoma), Su Geng (Perillae Caulis), Huo Xiang (Pogostemonis Herba), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Ding Xiang (Caryophylli Flos), Wu Mei (Mume Fructus), and Sheng Jiang (Zingiberis Rhizoma Recens).
The rhyme for Leakage Fetus in Pregnancy states: "Blood flows like menstruation; severe leakage dries the fetus, killing it. It harms the mother, causing worry; hasten to send divine pills to save her."
Leakage Fetus is caused by accidentally consuming items that disturb the fetus, toxic heat damaging the gestational sac, sexual exhaustion leading to shock, or excessive consumption of heating herbs drying the fetus. Mild cases have mild leakage; severe cases have severe leakage. If not treated urgently, blood depletion leads to death. There are two methods to calm the fetus: if the mother's illness disturbs the fetus, treat the mother alone, and the fetus will calm itself; if fetal unrest causes the mother's illness, calm the fetus alone, and the mother's illness will heal.
