Heat caused by phlegm accumulation presents with a string-slippery pulse (Xian-Hua Mai, String-like and Slippery Pulse) and symptoms of chest and diaphragm oppression/blockage, along with pain in the back. The *Huoren Shu* states that phlegm in the Zhongwan area causes aversion to cold and fever, resembling Taiyang disease (Exterior Syndrome), but differs in that there is no headache or stiffness of the neck.
Heat caused by blood stasis presents with a choppy pulse (Se Mai, Choppy Pulse). The patient may rinse their mouth with water but does not wish to swallow it; their feet are invariably cold (Jue Leng), and the lower abdomen is invariably tight and knotted. This condition should not be treated with cold herbs nor dispersed with acrid herbs; only by unblocking the blood will the heat cease.
Danggui Chengqi Tang (Angelica Decoction for Descending Excess)
Danggui (Radix Angelicae Sinensis), Dahuang (Radix et Rhizoma Rhei) (each 4 qian); Mangxiao (Natrii Sulfas), Gancao (Radix Glycyrrhizae Uralensis) (each 2 qian).
Chop the herbs, add ginger, and decoct. Another version excludes Mangxiao and includes Shaoyao (Radix Paeoniae Alba), named Liangqing Yinzi (Cooling Clearing Decoction).
Bone-steaming heat occurs when heat lurks within while steaming outward. Symptoms include heat in the skin and muscles, night sweats, sallow emaciation, and foul breath persisting over time. This is caused by latent heat in the bone marrow and obstruction of Ying (Nutrient) and Wei (Defensive) Qi circulation. It is common among young men and women.
Maijian San (Wheat Decoction Powder) (See Formula in the Consumption Chapter.)

Grind into powder. Take 3 qian per dose with one cup of child's urine, one dark plum, five spoons of pig gallbladder, one pig spinal cord, half a qian of leek root, and one cup of water. Decoct until reduced to 70%, strain, and take warm at any time. The original formula contains Qianhu (Radix Peucedani), but lacks Ren Shen, Huang Qin, Gancao, Danggui, and Shaoyao. I likely added these from Chaihu Yinzi to prepare for tonifying deficiency and draining heat. Qianhu was removed to avoid excessive dispersion.
Food stagnation heat typically manifests as evening fever, aversion to the smell of food, frequent belching of rotten smells, with a slippery or excess pulse. As stated in the *Huoren Shu*, food injury causes headache, rapid pulse, and fever, but the left Renying pulse is peaceful, and the body is not painful. Alcohol toxicity heat presents with a rapid pulse and red urine. The Classics state: "Alcohol qi combats grain qi, causing intense heat in the center, so heat pervades the body, resulting in internal heat and red urine."
