The Fuyang pulse is slightly fine and wiry. According to medical principles, abdominal fullness should occur. If there is no abdominal fullness, there will certainly be difficulty in defecation and pain in the hypochondriac regions on both sides. This is caused by deficiency-cold pathogenic factors ascending from below. It should be treated with warm herbs.
The Fuyang pulse corresponds to the Stomach meridian. A slightly fine and wiry quality represents a Yin pattern. When Yin pathogenic factors affect Yang positions (the Spleen and Stomach belong to Earth, where Yin and Yang intersect), the Spleen and Stomach are compromised, leading to abdominal fullness. If abdominal fullness does not occur, the Yin pathogen must attack laterally towards the hypochondrium and downwardly obstruct the anal opening, resulting in difficult defecation and pain in the bilateral hypochondriac regions. However, this cold pathogen does not enter from the exterior but arises internally from the lower part and ascends; thus, the disease originates from within. This is what is meant by "when the Kidney is deficient, cold moves within." Therefore, dispersing methods should not be used, but rather warming and tonifying methods.
When a patient presents with abdominal fullness, if palpation elicits no pain, it indicates a Deficiency pattern; if palpation elicits pain, it indicates an Excess pattern. The Excess pattern can be treated with purgative methods. If the tongue coating is yellow and purgatives have not yet been administered, the yellow coating will disappear after purgation.
Abdominal fullness that occasionally subsides and then returns to its original state indicates a Cold pattern; warm herbs should be administered.
Abdominal fullness that persists without relief indicates an Excess pattern. Fullness that subsides temporarily and then returns occurs because the cold qi in the abdomen dissipates temporarily upon encountering Yang qi and re-aggregates upon encountering Yin qi. This is also due to cold arising from within; hence, warm herbs should be administered.
If a patient has a sallow complexion, dry skin without thirst, cold-excess in the chest, and unremitting diarrhea, the prognosis is poor, indicating death.
A sallow complexion indicates Spleen deficiency and deterioration of Qi and Blood. Dryness without thirst occurs because Qi fails to reach the surface and Middle Jiao lacks Yang qi. With exhaustion of Qi and decline of Yang, the Middle Jiao Earth energy is already ruined. Furthermore, with cold binding above and Zang organs collapsing below, what reliance is there for unblocking or astringing? Thus, death ensues.
A wiry pulse at the Cunkou position leads to tension and pain in the hypochondrium and aversion to cold.
A wiry pulse at the Cunkou position is also a sign of Yin pathogens affecting Yang positions. Hence, there is tension and pain in the hypochondrium, and the cold is acquired from the exterior. This differs from the bilateral hypochondriac pain caused by a wiry Fuyang pulse. The former is accompanied by difficult defecation, whereas this case features aversion to cold.
Patients suffering from internal cold often yawn. If a patient has clear nasal discharge, fever, a normal facial color, and frequent sneezing: when Yang qi attempts to ascend but is pulled down by Yin, yawning occurs; when Yin qi attempts to enter but is repelled by Yang, sneezing occurs. In cases of internal cold, Yang qi is suppressed, leading to frequent yawning. Clear nasal discharge, fever with a harmonious complexion indicate that the pathogen cannot linger, leading to frequent sneezing.
In cases of internal cold accompanied by diarrhea, it is due to interior deficiency. The inability to sneeze despite the urge indicates cold in the abdomen.
Internal cold with diarrhea occurs because the interior Qi is inherently weak, lacking defense, allowing pathogens to directly invade the Zang organs. The inability to sneeze indicates that Zheng Qi is pressured by the pathogen; it cannot expel the pathogen nor accept it, so Yang qi attempts to move but stops, and the pathogen attempts to leave but remains.
In thin individuals, periumbilical pain indicates wind-cold invasion causing stagnation of Grain Qi. If purgatives are incorrectly used, Qi will counterflow upward. If it does not counterflow, epigastric stuffiness will occur.
Thin individuals have weak viscera and Qi, making them susceptible to wind-cold invasion. Once invaded, Grain Qi stagnates, causing periumbilical pain that resembles interior Excess but is actually Deficiency-Cold. Warm herbs should be used to assist Spleen Qi transport. Incorrect use of purgatives expels grain but leaves wind-cold behind, further weakening Zheng Qi and allowing the pathogen to dominate, potentially attacking upwards or occupying the center.
For abdominal fullness with fever for ten days, floating and rapid pulse, and normal appetite, Houpo Qiwu Tang (Magnolia Bark Seven-Ingredient Decoction) is indicated.
Abdominal fullness indicates interior Excess. Fever with floating and rapid pulse indicates exterior pathogens. Normal appetite suggests the Stomach Qi is intact; treatment should seize this opportunity to attack. Zhi Shi (Immature Bitter Orange), Hou Po (Magnolia Bark), and Da Huang (Rhubarb) attack the interior. Gui Zhi (Cinnamon Twig) and Sheng Jiang (Fresh Ginger) attack the exterior. Gan Cao (Licorice) and Da Zao (Jujube) stabilize Zang Qi and harmonize the herbs during simultaneous interior-exterior treatment.
**Houpo Qiwu Tang Formula**
Hou Po (8 liang), Gan Cao, Da Huang (each 3 liang), Da Zao (10 pieces), Zhi Shi (5 pieces), Gui Zhi (2 liang), Sheng Jiang (5 liang).
Boil these seven ingredients in 1 dou of water to obtain 4 sheng. Take 8 ge warm, three times daily. Add Ban Xia (5 ge) if vomiting occurs. Remove Da Huang if diarrhea occurs. Increase Sheng Jiang to 8 liang if cold is prominent.
For abdominal cold qi, loud borborygmus, severe pain, chest-hypochondriac reverse fullness, and vomiting, Fuzi Jimi Tang (Aconite and Rice Decoction) is indicated.
Turbid Yin qi from the Lower Jiao not only dominates the Yin parts but also rebels into Yang positions. Due to Spleen-Stomach deficiency, the defenses are collapsed. Fu Zi (Prepared Aconite) assists Yang to drive out Yin; Ban Xia (Pinellia) descends rebellion and stops vomiting. Jing Mi (Rice), Gan Cao, and Da Zao strengthen the Earth to consolidate and restrain Yin qi.
**Fuzi Jimi Tang Formula**
Fu Zi (1 piece, processed), Ban Xia, Jing Mi (each 0.5 sheng), Gan Cao (1 liang), Da Zao (10 pieces).
Boil these five ingredients in 8 sheng of water until the rice is cooked. Strain. Take 1 sheng warm, three times daily.
For pain accompanied by constipation, Houpo Sanwu Tang (Magnolia Bark Three-Ingredient Decoction) is indicated.
Pain with constipation indicates failure of the Six Fu organs' Qi movement. Although composed of the same ingredients as Xiao Chengqi Tang, Xiao Chengqi Tang focuses on purging excess, using Da Huang as the sovereign herb. Houpo Sanwu Tang focuses on moving Qi, using Hou Po as the sovereign herb.
**Houpo Sanwu Tang Formula**
Hou Po (8 liang), Da Huang (4 liang), Zhi Shi (5 pieces).
Boil these three ingredients in 1.2 dou of water. First decoct Hou Po and Zhi Shi to obtain 5 sheng. Add Da Huang and decoct to obtain 3 sheng. Take 1 sheng warm. Stop when bowel movement is achieved.
Palpation revealing fullness and pain in the epigastrium indicates Excess. Purging is indicated; Da Chaihu Tang (Major Bupleurum Decoction) is appropriate.
Fullness and pain upon palpation indicate有形 (material) excess pathogens. While purging is indicated for Excess, the location of epigastric fullness/pain is higher than abdominal fullness/pain. Therefore, Da Chengqi Tang is unsuitable; Da Chaihu Tang is preferred. Da Chengqi Tang treats pure interior Excess, whereas Da Chaihu Tang also unblocks Yang obstruction.
**Da Chaihu Tang Formula**
Chai Hu (8 liang), Huang Qin, Shao Yao (each 3 liang), Ban Xia (0.5 sheng), Zhi Shi (4 pieces), Da Huang (2 liang), Da Zao (12 pieces), Sheng Jiang (5 liang).
Decoct these eight ingredients in 1.2 dou of water to obtain 6 sheng. Strain and simmer again. Take 1 sheng warm, three times daily.
Abdominal fullness that does not decrease, or decreases insignificantly, requires purging; Da Chengqi Tang (Major Order the Qi Decoction) is indicated.
"Decreases insignificantly" means any reduction is negligible, emphasizing extreme fullness. Major purging is required. Although the previous three formulas differ in urgency, their purgative nature is consistent, illustrating "fullness in the middle should be drained from within."
**Da Chengqi Tang Formula** (See Spasm Disease Chapter)
For severe cold pain in the heart and chest, vomiting preventing food intake, abdominal fullness, skin protrusions resembling heads and feet, and unbearable tenderness, Da Jianzhong Tang (Major Construct the Middle Decoction) is indicated.
Cold pain in the heart/abdomen with vomiting and anorexia indicates excessive Yin-cold and powerless Middle Earth. Skin protrusions resembling heads and feet and extreme tenderness indicate Yin-cold coagulation and intestinal worms agitated by it. Da Jianzhong Tang greatly tonifies Middle Jiao Yang to overcome rebellious Yin. Shu Jiao (Sichuan Pepper) and Gan Jiang (Dried Ginger) warm the stomach and expel worms; Ren Shen (Ginseng) and Yi Tang (Maltose) calm the middle and boost Qi.
**Da Jianzhong Tang Formula**
Shu Jiao (2 he, stir-fried to remove oil), Gan Jiang (4 liang), Ren Shen (1 liang).
Decoct these three ingredients in 4 sheng of water to obtain 2 sheng. Strain. Add 1 sheng of Yi Tang. Simmer gently to obtain 2 sheng. Divide into two warm doses. After about one meal's time, drink 2 sheng of rice gruel, then take the next dose. Maintain a diet of rice gruel for one day and keep warm.
For unilateral hypochondriac pain with fever and a tight-wiry pulse, indicating Cold, treat with warming purgatives; Da Huang Fu Zi Xi Xin Tang (Rhubarb, Aconite, and Asarum Decoction) is indicated.
Unilateral hypochondriac pain with a tight-wiry pulse indicates Yin-cold aggregation in one area. Fever here results from Yang suppression. Warming is needed to resolve cold; purging is needed to remove accumulation. Thus, warming purgatives are indicated. Professor Cheng stated: Da Huang is bitter and cold, mobile and unretentive; combined with the great heat of Fu Zi and Xi Xin, its cold nature disperses while its mobilizing property remains.
**Da Huang Fu Zi Xi Xin Tang Formula**
Da Huang (3 liang), Fu Zi (3 pieces), Xi Xin (2 liang).
Decoct these three ingredients in 5 sheng of water to obtain 2 sheng. Divide into three warm doses. For robust patients, decoct to 2.5 sheng. Take one dose every four to five li of walking time.
For cold qi causing reversal cold (Jue Ni), Chi Wan (Red Pill) is indicated.
Reversal cold due to cold qi involves Lower Jiao Yin-cold rebelling upward. Fu Ling (Poria) and Ban Xia descend rebellion; Wu Tou (Aconite) and Xi Xin disperse cold. Zhu Sha (Cinnabar), being heavy and red, enters to break Yin and eliminate rebellion.
**Chi Wan Formula**
Wu Tou (2 liang, processed), Fu Ling (4 liang), Xi Xin (1 liang), Ban Xia (4 liang).
Powder these four ingredients. Mix with Zhu Sha for color. Form pills with honey, size of hemp seeds. Take 3 pills with wine before meals, twice daily, once at night. Increase dosage gradually if ineffective, stopping when effective.
Abdominal fullness with a wiry-tight pulse: Wiry indicates Wei Qi stagnation causing aversion to cold; Tight indicates anorexia. Struggle between Pathogen and Zheng Qi constitutes Han Shan (Cold Colic). Han Shan causes periumbilical pain, sweating of clear fluid (Bai Jin), cold extremities, and a deep-tight pulse. Da Wu Tou Jian (Major Aconite Decoction) is indicated.
Wiry and tight pulses are Yin patterns. Wiry Yin arises internally; Tight Yin arises externally. Wiry pulse blocking Wei Qi causes aversion to cold (Yin blocking exterior Yang). Tight pulse causing anorexia indicates Yin blocking Stomach Yang. With decline of both Wei and Stomach Yang, and intensification of external and internal cold, Yin rebels upward unchecked, and Yang lies suppressed below. This struggle is Han Shan. Periumbilical pain, clear sweat, cold extremities, and deep-tight pulse are definitive signs of Han Shan. Bai Jin refers to clear, non-salty sweat (deficiency sweat); some texts read "spontaneous sweat," which is also acceptable. Da Wu Tou Jian is extremely pungent and hot, a potent formula to restore Yang and scatter Yin. It must not be taken multiple times in one day.
**Da Wu Tou Jian Formula**
Wu Tou (5 large pieces, boiled, skin removed, not chopped).
Decoct in 3 sheng of water to obtain 1 sheng. Strain. Add 2 sheng of honey. Boil until water vapor evaporates to obtain 2 sheng. Robust patients take 7 ge; weak patients take 5 ge. If ineffective, administer the next day. Do not take multiple times in one day.
For Han Shan with abdominal pain and hypochondriac pain with interior tension, Dang Gui Sheng Jiang Yang Rou Tang (Angelica, Ginger, and Mutton Decoction) is indicated.
This treats Cold excess with Blood deficiency. Blood deficiency fails to nourish vessels; Cold excess causes vessel contraction. Hence, abdominal/hypochondriac pain and interior tension. Dang Gui and Sheng Jiang warm Blood and scatter Cold; Yang Rou (Mutton) tonifies deficiency and benefits Blood.
**Dang Gui Sheng Jiang Yang Rou Tang Formula**
Dang Gui (3 liang), Sheng Jiang (5 liang), Yang Rou (1 jin).
Decoct in 8 sheng of water to obtain 3 sheng. Take 7 ge warm, three times daily. If Cold is severe, increase Sheng Jiang to 1 jin. If pain is severe with vomiting, add Chen Pi (Tangerine Peel) 2 liang and Bai Zhu (Atractylodes) 1 liang. Add 5 sheng of water if increasing ginger. Decoct to obtain 3.2 sheng.
For Han Shan with abdominal pain, cold limbs, numbness, body pain, unresponsive to moxibustion/acupuncture/herbs, Di Dang Wu Tou Gui Zhi Tang (Aconite and Cinnamon Twig Decoction) is indicated.
Abdominal pain and cold limbs indicate Yang extinction in the interior. Numbness or body pain indicates Yang obstruction in the exterior. This is Cold damaging both interior and exterior, requiring simultaneous treatment. Wu Tou warms the interior; Gui Zhi releases the exterior. Xu stated: Failure of previous treatments implies conflicting attacks on interior/exterior. "Knowing" (Zhi) refers to feeling like drunkenness, indicating Ying-Wei warming and Qi victory. Vomiting indicates Yin pathogen rejected by Yang and expelled upward, constituting "hitting the disease."
**Wu Tou Gui Zhi Tang Formula**
Wu Tou.
Decoct Wu Tou in 2 sheng of water to reduce by half. Strain. Mix with 5 he of Gui Zhi Tang to obtain 1 sheng. Initial dose: 2 ge. If ineffective, take 3 ge. If still ineffective, increase to 5 ge. Drunken sensation or vomiting indicates efficacy.
If a pulse is rapid and tight turning wiry, like a bowstring, unyielding to pressure: Rapid-wiry pulse indicates need to purge Cold. Tight-large-slow pulse indicates epigastric hardness. Large-tight pulse indicates Yang containing Yin; purging is indicated.
Rapid is Yang; Tight/Wiry is Yin. Their appearance together suggests cold-heat intermingling. Regarding Han Shan, the rapid rate follows the wiry quality, representing Yin condensing on Yang, not Yang generating heat. Regarding Wind-Malaria, the wiry quality follows the rapid rate, representing Wind emerging from Heat, not Yin generating Cold. This clarifies the text. Thus, rapid-wiry pulse requires purging Cold. Tight-slow and large-tight are similar. Large is Yang but here indicates Yin Excess ("Yang containing Yin"), warranting purging.
**Supplementary Formulas**
**Waitai Wutou Tang**: Treats Cold Colic. Severe abdominal绞痛 (colicky pain), thief-wind invading five Zang, rigidity preventing turning, periodic attacks, genital retraction, cold extremities. (Identical to Da Wu Tou Jian).
**Waitai Chaihu Guizhi Tang**: Treats sudden heart-abdominal pain.
Chai Hu (4 liang), Huang Qin, Ren Shen, Shao Yao, Gui Zhi, Sheng Jiang (each 1.5 liang), Gan Cao (1 liang), Ban Xia (1.5 he), Da Zao (6 pieces).
Decoct in 6 sheng of water to obtain 3 sheng. Take 1 sheng warm, three times daily.
**Waitai Zouma Tang**: Treats sudden heart pain, abdominal distension, constipation.
Ba Dou (2 pieces, peeled/cored, roasted), Xing Ren (2 pieces).
Wrap in silk, crush. Infuse in 2 he hot water, squeeze juice, drink. Should cause purging. Adjust for age/strength. Treats Fei Shi Gui Ji diseases.
Q: How to distinguish Su Shi (Retained Food)?
A: Cunkou pulse floating and large, pressing reveals roughness (Se); Chi position is also slightly rough (Se). This indicates retained food. Da Chengqi Tang is indicated. Rapid-slippery pulse indicates Excess; retained food; purging cures it. Da Chengqi Tang is indicated. Diarrhea with anorexia indicates retained food; purging is indicated. Da Chengqi Tang is indicated.
Floating-large Cunkou pulse indicates abundant Grain Qi. Excess grain damages Spleen. Roughness indicates Spleen injury and stagnation, impairing Qi-Blood flow. Rough Chi pulse indicates Middle Qi blockage preventing essence transmission downward. Retained food requires Da Chengqi Tang to purge. Rapid-slippery and floating pulses all indicate Excess/Abundance of Grain Qi. Excess warrants purging. Excess grain damages Spleen (water/grain separation); stagnation damages Stomach (aversion to food odor). Diarrhea with anorexia confirms retained food needing purging. Spleen/Stomach transform water/grain and move fluids/Qi; they must not stop. Stagnation leads to cessation of transformation and Qi movement, ending life. Da Chengqi Tang must rapidly remove stagnant grain to restore Qi flow and transformation. Mere digestion aids would consume Zheng Qi before removing the pathogen, offering no benefit.
**Da Chengqi Tang Formula** (See Spasm Disease Chapter)
Retained food in the Upper Wan (upper stomach) requires emesis; Gua Di San (Thistle Powder) is indicated.
Retained food in the Lower Wan requires purging; food in the Upper Wan requires emesis. The Classic states: "Where it is high, induce vomiting."
**Gua Di San Formula**
Gua Di (1 fen, roasted yellow), Chi Xiao Dou (3 fen, boiled).
Powder these two ingredients. Decoct Xiang Chi (Fermented Soybean) 7 he in water to obtain juice. Mix with powder (1 qian bi). Take warm. If no vomiting, slightly increase dose. Stop when vigorous vomiting occurs.
Pulse tight like a turning rope, irregular, indicates retained food. Pulse tight with headache and wind-cold indicates retained food in the abdomen not digesting.
"Tight like a turning rope" indicates tightness combined with slipperiness, unlike wind-cold tightness which is rigid and wiry. Cold binding is tight and fixed; food-induced tightness fluctuates between tight and slippery, like turning a rope, hence "irregular." "Tight pulse with headache/wind-cold" does not mean simultaneous retained food and wind-cold. It means retained food stagnation generates Qi rising to cause headache, mimicking wind-cold, but is actually food accumulation resembling Taiyang disease. Zhang Zhongjing warns against misdiagnosing this as exterior syndrome and inducing sweat, highlighting that abdominal retained food causes such symptoms.
