This formula utilizes the principle of "same qi attracting same qi," using Yin to counteract Yin, guiding the pathogenic factor to exit through the yin orifices from which it originally entered. Since during sexual intercourse and seminal emission, heat pathogens must enter via the yin aperture, as the proverb says: "From where it comes, there let it go." Other herbs such as Shengdi Huang (Rehmanniae Radix), Maidong (Ophiopogonis Radix), Zhimu (Anemarrhenae Rhizoma), Huangbai (Phellodendri Cortex), Xijiao (Rhinocerotis Cornu), Huanglian (Coptidis Rhizoma), Huashi (Talcum) can also be adopted. [Chai Hu (Bupleuri Radix) may also be added.]
Puji Xiaodu Yin (Revised Edition): In the fourth month of Taihe Year 2, many people suffered from epidemics. Initially, they felt aversion to cold, high fever, and heaviness of the body. Subsequently, severe swelling occurred on the head and face, eyes could not be opened, upper asthma and throat obstruction developed, with a dry tongue and mouth. Commonly known as "Big Head Cold," various wind-dispelling drugs were ineffective. Li Dongyuan stated: "The upper half of the body is governed by Heaven's Qi. When pathogenic heat lodges between the Heart and Lungs, it attacks upward to the head and eyes, causing swelling." This formula treats this condition, saving many lives.
Lian Qiao (Forsythiae Fructus), Huang Lian (Coptidis Rhizoma), Huang Qin (Scutellariae Radix) [clears and purges Heart and Lung fire], Xuan Shen (Scrophulariae Radix) [treats rootless fire], Qing Dai (Indici Folium) [disperses stagnant fire, stops heat-induced irritability], Bo He (Menthae Haplocalycis Herba), Jing Jie (Schizonepetae Spica) [disperses wind-heat, clears head and eyes], Niu Bang Zi (Arctii Fructus) [disperses wind-heat, reduces edema], Gan Ju (Chrysanthemi Flos) [treats head and eye pain/swelling], Gan Cao (Glycyrrhizae Radix et Rhizoma), Jie Geng (Platycodonis Radix) [acts as a carrier agent to prevent rapid downward action], Chai Hu (Bupleuri Radix) [as an ascending drug, aiming to reach the upper part], Ju Hong (Citri Reticulatae Pericarpium) [regulates Qi to open congestion], Ren Shen (Ginseng Radix et Rhizoma) [supports healthy Qi to expel pathogenic factors; do not add if there is no deficiency]. If constipation occurs, add Da Huang (Rhei Radix et Rhizoma) stir-fried with wine [drains according to excess, a method akin to removing firewood from under the cauldron]. Grind all into fine powder. Use half mixed with decoction, sipping frequently. For diseases located in the upper jiao, do not disdain small doses taken frequently. Use the other half to make honey pills, hold in the mouth to dissolve before lying down, allowing the medicinal properties to ascend.
This formula primarily focuses on dispersing pathogenic factors, reducing fever, and detoxifying. Generally, treatment methods should avoid harsh purgation; if forced, pathogenic factors will not be expelled, but rather healthy Qi will be damaged. If there is heat accumulation in the bowels, one may use Da Huang soaked in wine or Xuan Ming Fen (Mirabilite) for mild purgation. If the patient is weak with a faint pulse, consider adding Ren Shen (Ginseng Radix et Rhizoma) 3-5 fen. If this condition starts at the nose and forehead with redness and swelling of the face and eyes, it belongs to the Yangming channel; add Shi Gao (Gypsum Fibrosum) if thirsty. If it starts around the ears and eyes, front, back, top, and bottom, with redness and swelling of the temporal region, it belongs to the Shaoyang channel; double the dosage of Chai Hu (Bupleuri Radix) and Hua Fen (Trichosanthis Radix). If it starts on the crown, extending to the vertex, it belongs to the Taiyang channel; add Qiang Huo (Notopterygii Rhizoma et Radix). If all three Yang channels are affected, with combined swelling of the head, face, anterior and posterior ears/nose, and the head is large as a jar, add Qiang Huo (Notopterygii Rhizoma et Radix) and Ge Gen (Puerariae Lobatae Radix), and double Chai Hu (Bupleuri Radix) for treatment. Externally apply Qingliang Jiuku San (Cooling and Saving from Suffering Powder).
Qingliang Jiuku San (Revised Edition): Huang Lian (Coptidis Rhizoma), Huang Bai (Phellodendri Cortex), Da Huang (Rhei Radix et Rhizoma), Mang Xiao (Mirabilite), Xiong Huang (Realgar), Qing Dai (Indici Folium) [equal parts, ground into powder]. Ba Jiao Gen (Musa Basjoo Radix) [crushed for juice] mixed with honey, applied to the swollen area.
Yang syndromes presenting with Yang pulses are favorable (shun); presenting with Yin pulses is unfavorable (ni) [because Qi, Blood, Essence, and Fluids were deficient before illness onset. Those with innate Yin pulses should be examined carefully]. After sweating, if the body cools and the pulse becomes calm, it is auspicious. If after sweating or purging, high fever returns with a restless pulse [pathogenic factor is too strong, not subdued by medicine], it is incurable; if accompanied by asthma/shortness of breath, death is certain. Fever with a restless pulse [True Yin exhausted, pathogenic factor prevails], delirious speech [Kidney Qi exhausted], inability to eat [Stomach Qi exhausted], seeing these three signs means death even if symptoms temporarily subside. Pulses both Yin and Yang are deficient, fever does not stop [Healthy Qi deficient, Pathogenic factor excessive], death. Heat disease lasting seven or eight days, pulse minute/small [Healthy Qi deficient], blood in urine and dry mouth [True Yin injured], death. Severe deficiency in warm disease [Original Yuan Qi deficient, unable to resist pathogenic factors], death. Heat disease with unknown pain location [pain exists but location unclear], deafness, dry mouth, inability to self-control [body heavy, unable to support itself], when Yang wins heat is severe, when Yin wins cold is significant, heat in marrow, death, incurable. Heat disease with spasms [limb convulsions, opisthotonos, teeth grinding, all signs of spasm caused by extreme heat injuring Yin and Blood], death. Heat disease with blurred vision [Essence and Qi of Zang-Fu organs exhausted], fever not stopping, death. Heat disease with rotting tongue base [Spleen and Kidney meridians connect to the tongue base, Yin of both organs exhausted], fever not stopping, death. Delirium, stiff tongue, hard tongue, black tongue [Heat severe, fluids exhausted], death. Yang syndrome with delirium, body should be hot, pulse should be surging and large, but instead hands and feet are cold, pulse deep, thin, and faint [Pathogenic factor excessive, Healthy Qi defeated], death. Fixed gaze [Kidney Water about to exhaust], delirium [Heart heat causing spirit confusion], asthma and fullness [Qi escaping from above], death. Diarrhea [Qi escaping from below], also death. Fixed gaze with head shaking, body like smoked color, Heart exhaustion death. Fixed gaze with wild speech, enuresis unaware, Kidney exhaustion death. Black ring around mouth, cold sweat turning yellow, Spleen exhaustion death. Sweating with wet hair, continuous asthma, Lung exhaustion death. Lips and corners of mouth turning blue, hand and foot tremors, Liver exhaustion death. Chest binding symptoms complete, with irritability, death [Misguided purging exhausted fluids, solitary Yang has no attachment and desires to escape]. Vomiting then irritability [Upper and lower attacking each other, Middle Qi severed], cold hands and feet [Spleen and Stomach Qi exhausted], death. Diarrhea with fever, cold limbs, restlessness unable to lie down [Yin and Yang both exhausted], death. Diarrhea with fever, continuous sweating [Excessive sweating leads to loss of Yang], death. Diarrhea, cold limbs, inability to eat, suddenly able to eat again [This is the exposure of Stomach Yang, a sign of the lamp dying but flaring up again], named "Chu Zhong" (Elimination of Middle), death [Meaning Stomach Yang is completely eliminated; sometimes it belongs to recovery of Stomach Qi, judgment should be made based on form and Qi]. Diarrhea with aversion to cold, curled body, cold hands and feet [Yin excessive, no Yang], death [In heat diseases, overuse of cold/cool drugs may cause this occasionally]. Diarrhea with severe abdominal fullness [Spleen Qi failed], death. Diarrhea stops but dizziness occurs; frequent spontaneous vertigo, death [This is Yin exhausted, diarrhea stops, solitary Yang has no attachment and floats upward]. Cold sweat like oil, beads not flowing [Absolute sweat exiting], death. Grasping clothes, touching bed, hands picking at empty air, death [Sometimes due to heat causing spirit confusion, rescue should refer to pulse and symptoms]. Eyes chaotic without spirit, eyes lacking luster, death [Loss of spirit means death]. Mouth open like a fish, breath not returning [Kidney cannot grasp Qi and return it to origin], death. Bad breath that cannot be approached [Internal heat extreme, Zang-Fu organs injured], death. Tongue surface red-purple, dry and shiny [Heart Fire self-burning], death. Tongue curled and scrotum contracted [Jueyin Qi exhausted], death. Skin dry and rough, large muscles wasted away [Taiyin Qi exhausted], death.
