Pulse: A normal pulse that is wiry and large indicates deficiency due to overwork. A pulse that is large but forceless suggests declining Yang qi, which is easy to support; a pulse that is rapid but forceless indicates difficult-to-remove Yin fire. Weakness at the Cun position (distal) indicates upper damage; floating and large indicates internal depletion. If both Cun and Chi positions are micro, it signifies a body of five labors. Blood deficiency presents as a soggy pulse on the left; Qi timidity is pushed back on the right; if pulses on both sides are micro, there is no surplus of blood and Qi. In labor consumption (Lao Zhai), a pulse that is rapid or choppy and fine, accompanied by tidal sweating, coughing, hemoptysis, and emaciation, leads to death.
The *Neijing* states: Prolonged viewing damages blood; prolonged lying down damages Qi; prolonged sitting damages flesh; prolonged standing damages bones; prolonged walking damages sinews. As for the fire of the seven emotions and five hearts flying away, and excessive lust in male-female sexual desires, these are all causes of deficiency and damage. The *Jiyao* states: Diseases of deficiency and damage arise from feeling cold and heat due to weakness. Feeling cold damages Yang; Yang deficiency leads to Yin excess. Damage proceeding from top to bottom: first damage affects the Lung, causing skin contraction and hair loss; second damage affects the Heart, causing blood vessels to be deficient and unable to nourish the Zang-Fu organs, leading to amenorrhea in women; third damage affects the Stomach, where food cannot transform into muscle and flesh. Treatment should use acrid, sweet, and bland substances; if damage exceeds the Stomach, it is incurable. Feeling heat damages Yin; Yin deficiency leads to Yang excess. Damage proceeding from bottom to top: first damage affects the Kidney, causing bone flaccidity inability to rise from bed; second damage affects the Liver, causing sinew laxity inability to hold oneself; third damage affects the Spleen, where food cannot be digested. Treatment should use bitter, sour, and salty substances; if damage exceeds the Spleen, it is incurable. It also says: Damage to Heart and Lung causes weary complexion; damage to Kidney and Liver causes withered form. The *Nanjing* states: The method for treating damage: damage to the Lung benefits its Qi; damage to the Heart tonifies its Ying blood; damage to the Spleen regulates diet and adapts to cold and warmth; damage to the Liver soothes its center; damage to the Kidney benefits its Essence. These are the general principles of etiology and treatment for deficiency and damage.

Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) treats middle Qi insufficiency, or mistaken use of draining drugs causing limb fatigue, dry mouth and fever, tasteless food; or irregular diet and fatigue causing body heat, pulse large and forceless; or headache, aversion to cold, and spontaneous sweating; or high-pitched breathing and panting, body heat and vexation, pulse micro, fine, weak and soft, spontaneous sweating, body fatigue and little food intake; or middle Qi deficiency failing to control blood; or malaria and dysentery due to diet and fatigue, unhealed due to Spleen-Stomach deficiency; or Yuan Qi deficiency with wind-cold invasion, unable to tolerate releasing exterior methods, use this instead; or after intercourse followed by fatigue and cold invasion; or after fatigue and cold invasion followed by intercourse, urgently add Fuzi (Aconiti Lateralis Radix Praeparata). I believe the human body relies primarily on the Spleen and Stomach. When Spleen-Stomach Qi is solid, the Lung receives nourishment; when Lung Qi is abundant, Water is generated. Water rising allows Fire to descend; when Water and Fire interact, Heaven and Earth achieve harmony. When Spleen-Stomach is deficient, the four Zang organs lack vital energy. Thus, Mr. Dongyuan wrote treatises on "Spleen and Stomach" and "Internal and External Injury," earnestly emphasizing protecting the Spleen and Stomach as the root. His Buzhong Yiqi Tang is listed first among formulas. Its original intent is clear. Hence, "tonifying the Kidney is not as good as tonifying the Spleen." The symptoms mentioned above are briefly listed; others should be inferred similarly. The subtleties of this formula are noted below.
