Whenever the medical classics mention "Hou Bi" (Sore Throat/Throat Obstruction), it refers to a condition where breathing through the throat is obstructed, speech is impossible, and the Qi of Heaven (Yang) is blocked. When "Yan Tong" (Pharyngeal Pain) or "Ai Tong" (Oropharyngeal Pain) are mentioned, it refers to the inability to swallow saliva and food, indicating that the Qi of Earth (Yin) is blocked. When "Hou Bi accompanied by Yan Ai Tong" is mentioned, it means both the throat and pharynx are diseased, and both Heaven and Earth Qi are blocked. Generally, those suffering from Hou Bi must also have Yan Ai Tong; however, those with Yan Ai Tong cannot necessarily have Hou Bi. Currently, all prescriptions for diseases affecting both the throat and pharynx are included in the Hou Bi section.
If Hou Bi is accompanied by aversion to cold, and the Cun pulse (Inch position) is smaller and weaker than the Guan (Bar) and Chi (Cubit) positions, these are all exterior syndromes. It is appropriate to use Gan Jie Tang (Licorice and Platycodon Decoction) or Ban Xia Gui Zhi Gan Cao Tang (Pinellia, Cinnamon Twig, and Licorice Decoction), dispersing according to the specific patterns of cold and heat. If even water and fluids cannot be swallowed, use Jie Du Xiong Huang Wan (Detoxifying Realgar Pill), four or five pills, ground in extremely sour vinegar and administered into the mouth. After thick phlegm is vomited, administer other medications. Intermittently holding one shell of fresh ginger juice in the mouth yields miraculous effects.
Note: Cases of Hou Bi with aversion to cold are all due to cold restraining heat, with cold pathogen closed on the exterior and heat郁 (stagnant) within. Ginger juice disperses the exterior cold, allowing the internal heat to extend and the disease to heal. One must strictly avoid using acid-cold agents like Dan Fan (Chalcanthite) to drop into the throat, as this will cause Yang Qi to become stagnant and unable to extend; nor should one use cold purgatives like Xiao Shi (Niter) and Da Huang (Rhubarb) to purge downwards, as this will cause Yang Qi to sink into the interior, leading to immediate disaster.
Han He stated: When the Cun pulse is weaker and smaller than the Guan pulse, it is appropriate to eliminate Yin pathogens and assist Yang Qi. Yuan (Li Dongyuan) stated: Insufficiency in both Cun pulses indicates insufficiency of Yang Qi; therefore, exterior-releasing medicines are used to lift the Qi and ascend to assist Yang. This method can also be used if all three pulses are small and weak.
When Hou Bi occurs in villages and the symptoms are similar among patients, it belongs to the pathogenic factors of seasonal epidemics and climate cycles. Treatment must first focus on dispersing the exterior. It is also strictly forbidden to use acid drops or cold purgatives, as this will trap the pathogen inside, preventing its expulsion. There are two types of such diseases.
The first type belongs to Fire. The classics state: "The arrival of Shaoyang leads to Hou Bi." It also states: "When Shaoyang governs the year, during the third Qi period, when hot summer heat arrives, people suffer from Hou Bi." Treatment should use Zhang Zhongjing's Jie Geng Tang (Platycodon Decoction). If the face is red with spots, it belongs to Yang Du (Yang Toxin), and sweating methods from various Yang Du prescriptions should be used.
The second type belongs to Dampness. The classics state: "Excess of Taiyin causes internal stagnation of fire leading to Hou Bi." It also states: "When Taiyin governs below, excessive dampness causes swelling of the throat and Hou Bi." Treatment should use Ban Xia Gui Zhi Gan Cao Tang from *Huo Ren Shu* (The Book That Revives People). If the face is blue-black, it belongs to Yin Du (Yin Toxin), and sweating methods from various Yin Du prescriptions should be used.
[Zhang Zhongjing] Jie Geng Tang: Treats sore throat caused by exogenous heat in Shang Han (Cold Damage). (Prescription see Shang Han Sore Throat. Hai Zang's Gan Jie Tang for sore throat added with Niu Bang Zi [Arctium Fruit] and Zhu Ru [Bamboo Shavings] is excellent.)
[Anonymous] Jing Jie Tang: Treats swelling and pain of the throat and pharynx, loss of voice, and severe pain upon swallowing.
Jing Jie (Schizonepetae Herba, half tael) Jie Geng (Platycodonis Radix, two taels) Gan Cao (Glycyrrhizae Radix et Rhizoma, one tael)
Crush into powder. Take four qian per dose, with one cup of water and three slices of ginger. Decoct until six fen remains, strain, and take warm.
[Huo Ren Shu] Ban Xia Gui Zhi Gan Cao Tang: Treats sudden invasion of cold causing sore throat. (Prescription see Shang Han Sore Throat.)
[Anonymous] For sudden Hou Bi with inability to speak. Use Xiao Xu Ming Tang (Minor Continuation of Life Decoction) with seven Xing Ren (Prunus Armeniaca Seed) decocted. Very effective. (Xu Ming Tang prescription see Wind Stroke Section.)
[Han He] Wu Xiang San (Five Fragrances Powder): Treats swelling and pain of throat/pharynx, blockage by toxic qi.

Grind into powder, mix evenly with She Xiang. Take two qian per dose, with one cup of water, at any time.
[Ben Cao] For acute wind-stroke Hou Bi nearing death. Bai Jiang Can (Bombyx Batryticatus) ground into powder, mixed with fresh ginger juice and taken. The throat heals immediately. (Dan Xi said: Jiang Can possesses Fire but has Earth and Wood qualities; aging gives it Metal Qi making it stiff. Treating Hou Bi utilizes its clearing transformation Qi within Fire to regulate Ministerial Fire and dissipate turbid, rebellious, stagnant disorders.)
[Chen] Fu Zi (Aconiti Lateralis Radix Preparata) peeled, processed until cracked, coated with honey and roasted until honey penetrates. Hold in mouth without swallowing the juice. Main treatment for Hou Bi.
[Han He] Ru Sheng Sheng Jin Ding (Like-Sage Gold Ingot): Treats acute closure of throat, swelling and pain of cheeks/jaw. Also for Dan E (Single Tonsil), Jie Hou (Goiter-like swelling), Chong She (Double Tongue), Mu She (Wooden Tongue).

Grind into powder, knead with fresh green onion juice into ingots. Before taking, rinse mouth with fresh well water. Chew five to seven fresh Bo He leaves, then chew one ingot thoroughly with them, and swallow with well water.
For severe cases, take three doses consecutively, and place one ingot on the affected area; the disease will subside with the medicine.
[Luo] Zheng Nan Shuai Bu Lin Ji Dai suffered from sore throat and diarrhea. (Prescription see Exterior Heat Interior Cold Section.)
In the spring of Wuchen year of Hongwu era, many villagers suffered from Hou Bi. This was due to the final Qi of the previous year and the initial Qi of the current year, which were evils of Double Fire. I repeatedly used Gan Jie Tang, adding Huang Lian (Coptidis Rhizoma), Ban Xia (Pinelliae Rhizoma), Jiang Can (Bombyx Batryticatus), and Niu Bang Zi Gen [Arctium Root] to disperse. For those with deficiency, add Ren Shen (Ginseng Radix et Rhizoma), [missing character], and Dang Gui (Angelicae Sinensis Radix). If fluids cannot enter, first use Jie Du Xiong Huang Wan, ground in vinegar and poured into the throat. After phlegm exits, pour ginger juice, then use the aforementioned medicines. All yield miraculous results. If treated previously with acid-cold drops like Dan Fan, they are incurable because the pathogen is trapped inside and cannot exit.
[Luo] Jie Du Xiong Huang Wan: Treats Chan Hou Feng (Coiling Throat Wind) and acute Hou Bi, sudden collapse, and locked jaw.
Xiong Huang (Realgar) Yu Jin (Curcumae Radix) (each one fen) Ba Dou (Crotonis Fructus, peeled/oiled, fourteen seeds)
Grind into fine powder, make pills with vinegar paste the size of mung beans. Swallow seven pills with tea liquor. Vomiting out tough phlegm leads to immediate revival. If not vomited, take another dose. For those appearing dead, if the heart is still warm and pills cannot be swallowed, pry open the mouth and force down; there is no case that cannot be cured. For children with fright-heat and phlegm obstruction, adjust dosage to two or three pills based on age/size.
(Another method: Use three Xiong Huang pills, ground in vinegar and administered; very effective, phlegm exits immediately and recovery follows.)
[Da Guan Ben Cao] For Chan Hou Feng. One piece of Xiong Huang, grind with fresh well water, administer urgently to induce vomiting. Recovery follows.
[Dan Xi] Chan Hou Feng belongs to Phlegm-Heat. It is appropriate to use a goose feather dipped in Tung Oil to induce vomiting. Another method: Use Ye Gan (Belamcandae Rhizoma) with reverse-flowing water to induce vomiting.
[World Transmission] Yuan Zhi (Polygalae Radix) de-heart, ground into powder. Take half qian, mixed with half cup of water. Hold a bamboo tube in mouth. Expectorates phlegm/saliva very quickly. Treats Hou Bi miraculously.
Wu Xi Jiao Gao (Black Rhinoceros Horn Paste): Treats swelling/pain of throat/pharynx, and all conditions including Jie Hou, Lan Hou (Rotting Throat), Dun Shi (Hidden Corpse), Chan Hou, Bi Hou, Ji Hou (Acute Throat), Fei Si Ru Hou (Flying Thread in Throat), Chong She, Mu She.
Zao Jiao (Gleditsiae Sinensis Fructus) (two strips, seeds crushed, soaked in three sheng water for one hour, squeezed for juice, strained, boiled in clay vessel into paste) Hao Jiu (Good Wine, one he) Yan Xiao (Saltpeter) Bai Cao Shuang (Soot from hundred herbs, ground, one qian, mixed evenly with Zao Jiao paste to thicken) Ren Shen (Ginseng Radix et Rhizoma, one qian, powdered) Peng Sha (Borax) Bai Shuang Mei (White Frost Plum) (each a little, ground and mixed into paste)
Mix all above medicines. Dip goose feather slightly into throat. Continue until all tough phlegm is expelled. Then chew two cun of Gan Cao, swallow juice and saliva. If Mu She (Wooden Tongue), first rub tongue cold with coarse cloth dipped in water, then rub with ginger slice, then apply medicine.
[Shan Ju Fang] Chan Hou Feng. Rub Zao Jiao in water and pour down. Vomiting leads to cure.
[Dou Men Fang] For Chan Hou Feng/Hou Bi with inability to eat/drink nearing death. Use one stem of Fan Hun Cao (Antennariae Herba), wash clean, insert into throat. Extracting cold phlegm leads to cure, miraculous. Further, swallow Ma Ya Xiao (Natrii Sulfas) dissolved in saliva to eradicate the root. Fan Hun Cao is also known as Zi Wan (Adenophorae Radix).
[World Transmission] For sore throat. Use Tu Wu Yao (Arecae Semen Preparatum? Note: Text says 'Ai Zhang Gen' - Cinnamomi Ramulus?), i.e., Ai Zhang Gen, decoct two cups of vinegar to one cup. Hold in mouth then swallow. Vomiting phlegm/saliva indicates cure.
[Du] For Chan Hou Feng, constrained Qi not flowing. Use She Tui (Bovis Colla Preparata) roasted yellow and Dang Gui (Angelicae Sinensis Radix) in equal parts, ground into powder. Take one qian bi with warm wine. Vomiting leads to cure.

For throat/pharynx pain with locked jaw: Remove shell from Ba Dou (Crotonis Fructus), press oil onto paper with paper. Make this paper into a wick, light it, blow out flame. Smoke into nose. Immediately saliva flows from mouth/nose, jaw opens.
If Hou Bi does not have aversion to cold, and the Cun pulse is large, slippery, and solid compared to Guan/Chi, these belong to interior excess syndromes. It is appropriate to use cold medicines like Xiao Shi (Niter) and Qing Dai (Indigo Naturalis) to descend, or acid agents like Bai Fan (Alumen) to astringe. Han Zhihe said: When Cun pulse is larger than Guan/Chi, it is appropriate to eliminate Yang and assist Yin. Dongyuan (Li Dongyuan) said: Solid pulses in both Cun indicate Yin excess and Yang deficiency; purging downwards leads to cure. Therefore, I often use this method for acute Hou Bi, effective as drum beating. If all three pulses are solid, this method can also be used.
[Anonymous] Yu Yao Chi (Jade Key): Treats Wind-Heat Hou Bi and Chan Hou Feng.
Yan Xiao (Saltpeter, one and a half taels) Peng Sha (Borax, half tael) Nao Zi (Camphora, one character) Bai Jiang Can (Bombyx Batryticatus, one fen)

[Shan Ju Fang] For fractured throat Qi not flowing: Slowly pour cold water down.
*Nei Jing* has four acupuncture/moxibustion methods for Hou Bi. Now, selecting points along meridians passing through throat/pharynx verified for efficacy, and points for other diseases interfering causing Hou Bi, covering the Six Meridians.
First, select Hand and Foot Yangming. Classics say: "Hou Bi with inability to speak, select Foot Yangming. Able to speak, select Hand Yangming." Also: "Stomach Foot Yangming Meridian goes from Da Ying anterior to Ren Ying, attaches along throat. Diseases produced: Neck swelling, Hou Bi. Select based on excess/deficiency, heat/cold, sinking/collapsing." Also: "Foot Yangming branch is named Feng Long, eight cun from ankle, diverges to Taiyin. Disease: Qi rebellion leads to sudden Hou Bi and aphonia. Select the divergent point." Also: "Large Intestine Hand Yangming Meridian diseases include Hou Bi. Select based on excess/deficiency, heat/cold, sinking/collapsing."
Second, select Hand Shaoyang. Classics say: "San Jiao Hand Shaoyang Meridian exits Que Pen, ascends neck behind ear. Moving disease: Ai swelling, Hou Bi. Select based on excess/deficiency, heat/cold, sinking/collapsing." Also: "Pathogen enters Hand Shaoy Luo, causing Hou Bi. Prick nail bed of middle finger/ring finger, distance from corner like leek leaf. Strong people recover instantly, elderly shortly. Left treat right, right treat left."
Third, two meridians pass through throat/pharynx. Classics say: "Liver Foot Jueyin Meridian follows behind throat. Therefore, pain behind throat, select here, repeatedly verified." Also: "Kidney Foot Shaoyin Meridian passes through Liver/Diaphragm, follows throat. Dou Hanqing said: The accurate point is Zhao Hai (KI6) treating throat blockage."
Fourth, one case of other diseases interfering causing Hou Bi. Classics say: "Heart Cough symptoms: Throat feels stuck like a lump, severe cases throat swelling/Hou Bi. Select Heart Shu point, namely Da Ling (PC7)."
[Sun Simiao] Wen Lu Gong had throat swelling one day. Hanlin Throat Department treated for three days, worsening. Emperor summoned Sun Simiao. Sun said: "The illness came from a brush used by the Chancellor to write judgments. Remove brush head, dip handle in water, apply medicine into throat, will heal." Sun pricked randomly. Chancellor fainted, unconscious. Attendants shocked/sweated. Sun laughed: "Not that I cannot save him, but soon he will vomit over a sheng of pus/blood, and in ten days will recover." Meeting Emperor, Emperor pleased: "Sun calls good doctors, very skilled." I once treated a man with Hou Bi, pricked Tai Xi (KI3) extracting half cup of dark blood, cured. Thus, Hou Bi is due to evil blood not dispersing. Generally treating this disease: Acute cases must first disperse. If dispersion fails, then remove phlegm. If phlegm removal fails, then remove dirty blood.
[Zhai Xuan Fang] Treat Hou Bi: Feng Long (ST40) Yong Quan (KI1) Guan Chong (SI1) (Severe cases: Hide small three-edged needle in pen tip, pretend to drop medicine on throat, then prick out purple blood. Instantly cured.)
